[Acute complications during middle ear surgery: part 2: Accidents in classical stapes surgery and their solutions].

Stapes surgery is remarkably different from regular middle ear surgery. The inner ear space has to be opened and the risk of postoperative deterioration of hearing and deafness is much higher. Peculiarities involve submerged parts of the foot plate into the vestibulum, the overhanging facial nerve with or without bony covering, bleeding from the mucosa, and problems involving the chorda tympani. Labyrinthine reactions during or post surgery are common. Important is the question of whether or when the piston should be removed. Rare cases such as a gusher with a wide connection between perilymph

[Pressure relations between endocranial and intracochlear fluid spaces in patients with inner ear diseases].

Intracranial pressure is transmitted to the perilymph of the cochlea via the cochlear aquaeduct and via Reissner's membrane to the endolymph. The "Tympanic Membrane Displacement Analyser (TDA)" is a new device that may be a useful non-invasive method to show intracranial and intracochlear pressure changes indirectly measured by tympanic membrane displacement during stapedial reflex contraction. The TDA was utilised in 20 normal persons and in 29 patients with unilateral diseases of the inner ear. No significant differences in the tympanic membrane displacement were found between patients with

Local Long-Term Inner Ear Drug Delivery in Normal Hearing Guinea Pig-An Animal Model to Develop Preventive Treatment for Noise-Induced Hearing Loss.

Noise-induced hearing loss (NIHL) is one of the leading causes of sensorineural hearing loss with global importance. The current treatment of choice for patients with hearing problems is a hearing aid or a cochlear implant. However, there is currently no treatment to restore physiological hearing. The development of preventive drugs is currently the focus of hearing research. In order to test the efficacy of a drug, the active ingredient has to be applied at reliable concentrations over a period of time. Osmotic minipumps can provide local drug delivery into the perilymph. Combined with a coch

Comparison of noninvasive evaluation of endolymphatic hydrops in Meniere's disease and endolymphatic space in healthy volunteers using magnetic resonance imaging.

CONCLUSIONS: The comparison of noninvasive standard evaluation of endolymphatic hydrops in Meniere's disease and the endolymphatic space in healthy volunteers using magnetic resonance imaging (MRI) evaluation should be carried out first. OBJECTIVE: To compare the standard evaluation of endolymphatic hydrops in Meniere's disease and the endolymphatic space in healthy volunteers in the cochlea and the vestibule in the same age group by applying noninvasive intratympanic gadolinium (Gd) perfusion through the eustachian tube and three-dimensional fluid-attenuated inversion recovery magnetic reson

The diagnostic performance of cochlear endolymphatic hydrops and perilymphatic enhancement in stratifying Ménière's disease probabilities: A meta-analysis of semi-quantitative MRI-based grading systems.

BACKGROUND: The diagnosis of Meniere's Disease (MD) presents significant challenges due to its complex symptomatology and the absence of definitive biomarkers. Advancements in MRI technology have spotlighted endolymphatic hydrops (EH) as a key pathological marker, necessitating a reevaluation of its diagnostic utility amidst the need for standardized and validated MRI-based grading scales. METHODS: Our meta-analysis scrutinized the diagnostic efficacy of semi-quantitative MRI-based cochlear endolymphatic hydrops (EH) and perilymphatic enhancement (PLE) grading systems in delineating clinicall

Recent advances in local drug delivery to the inner ear.

Inner ear diseases are not adequately treated by systemic drug administration mainly because of the blood-perilymph barrier that reduces exchanges between plasma and inner ear fluids. Local drug delivery methods including intratympanic and intracochlear administrations are currently developed to treat inner ear disorders more efficiently. Intratympanic administration is minimally invasive but relies on diffusion through middle ear barriers for drug entry into the cochlea, whereas intracochlear administration offers direct access to the colchlea but is rather invasive. A wide range of drug deli

The effects of pressure on cochlear microphonics in experimentally induced hydropic ears in the guinea pig.

The effect of perilymphatic pressure on the cochlear microphonics (CM) threshold was examined in guinea pigs with experimentally induced hydrops. When a pressure of 300 mm H2O was applied to the scala tympani of the basal turn, the 500 Hz CM threshold shifts in ears during the 1st week following obliteration of the endolymphatic sac and duct were significantly smaller when compared with shifts in normal ears. Larger threshold shifts were then regained at 4-12 weeks after surgery. When acetazolamide was administered to hydropic animals, the effect of the increase of perilymphatic pressure on th

Perilymphatic ATP plays a critical role in modulating cochlear function to protect from loud sound induced hearing loss.

BACKGROUND: Extracellular adenosine triphosphate (ATP) signalling via purinergic receptors plays a key role in cochlear adaptation to loud sound. Traditionally, ATP activation of purinergic receptor P2X receptors has been proposed to induce a cation shunt, reducing the endolymphatic potential and the driving force for sound transduction. However, direct evidence for this protective mechanism remains limited. Here, we provide direct experimental evidence identifying a distinct, compartment-specific ATP signalling pathway that modulates cochlear function. METHODS: In mature Dunkin-Hartley guine

[Traumatic perilymphatic fistula. Report of a pediatric case].

Traumatic perilymphatic fistula is an unusual pathology. Generally caused by pencils, swabs, hair buckles, and matches. Among the most frequent symptoms, patients can present hearing loss and vertigo. Diagnosis requires a complete physical examination that includes otomicroscopy, audiometry and computed tomography of both boulders. Treatment depends on the patient's symptoms. In general, it is conservative at first, but may require surgery. We present a clinical case of a 6-year-old boy with perilymphatic fistula secondary to left ear trauma due to swab, which required surgical treatment.

CT and MRI findings in X-linked progressive deafness.

Congenital X-linked mixed deafness is a rare anomaly that has typical features and can be diagnosed on the basis of progressive mixed hearing loss and the typical imaging findings. Recognition of these findings may alter the course of treatment and perilymph gushing can be avoided. A 10-year-old male patient presented with a history of progressive hearing loss. Computed tomography of the temporal bones showed bulbous dilatation of the fundi of the internal auditory canals (IAC) and the absence of the bony plates separating the basal turn of the cochleas and IAC. Magnetic resonance imaging demo

Why treat sudden hearing loss.

Recent laboratory and clinical studies provide increasing evidence that idiopathic sudden hearing loss can be effectively treated. First, corticosteroid therapy has been shown to be efficacious in restoring hearing in patients with moderate hearing losses. Second, studies of cochlear blood flow and perilymphatic oxygen tension indicate that positive modifications in PO2 can be obtained with 5% CO2 and 95% O2. Future clinical trials are likely to show that improved oxygenation of the cochlea will have a positive effect upon hearing recovery. Third, patients with a clear history of perilymph fis

The effect of prolonged exposure to industrial noise on otosclerosis.

More than 3000 workers referred by the Workmen's Compensation Board of Ontario have been evaluated for pension for industrial hearing loss. They include the normal proportion of otosclerotics, many of whom have had surgical therapy. The operations have been done by a wide variety of surgeons in the province of Ontario, and the results are representative of those which will be obtained in a large section of the Canadian community. The results are significantly worse than generally quoted, and some of the deficiencies are attributable to occupation. These include perilymph fistula from excessive

Targeted inner ear delivery of gadolinium using microbubble-assisted ultrasound in an ovine model.

Current strategies for treating sensorineural hearing loss include auditory rehabilitation, cochlear implantation, and systemic or local drug administration. Transtympanic injection (TTI) allows local drug delivery to the middle ear, but drug diffusion through the round window membrane (RWM) into the inner ear (IE) remains inconsistent. Microbubble-assisted ultrasound (MB-assisted US) has emerged as promising modality to enhanced RWM permeability. While feasibility and safety have been demonstrated in small animal models, translational validation in large mammals is necessary. This study aimed

The Endolymph Signal in Non-contrast Enhanced 3D-real IR Image Differs between the Ears with and without Significant Endolymphatic Hydrops.

PURPOSE: To investigate whether significant differences exist in labyrinthine lymph fluid signal intensities on non-contrast-enhanced 3D real inversion recovery (3D-real IR) images between patients with and without significant endolymphatic hydrops (EH), potentially enabling the non-contrast detection of EH. METHODS: Thirty-nine patients suspected of having EH underwent 3D-real IR MRI before and 4 hours after intravenous administration of a single dose of gadobutrol. Signal intensities of the cerebrospinal fluid (CSF), perilymph, and endolymph were manually measured on pre-contrast images usi

MicroRNA Profiling as a Methodology to Diagnose Ménière's Disease: Potential Application of Machine Learning.

OBJECTIVE: Diagnosis and treatment of Ménière's disease remains a significant challenge because of our inability to understand what is occurring on a molecular level. MicroRNA (miRNA) perilymph profiling is a safe methodology and may serve as a "liquid biopsy" equivalent. We used machine learning (ML) to evaluate miRNA expression profiles of various inner ear pathologies to predict diagnosis of Ménière's disease. STUDY DESIGN: Prospective cohort study. SETTING: Tertiary academic hospital. SUBJECTS AND METHODS: Perilymph was collected during labyrinthectomy (Ménière's disease, n = 5), staped
