Piloting and validation of a novel self-administered online cognitive screening tool in normal older persons: the Cognitive Function Test.

BACKGROUND: Subtle cognitive changes have been described that may predate the onset of clinically recognizable Alzheimer's disease (AD) and may reflect pathological changes in the brain that are detectable up to 10 years before the onset of AD. Early screening for cognitive status can have benefits in terms of early management and prevention strategies for cognitive decline. METHOD: A novel computerized cognitive screening tool, the Cognitive Function Test (CFT), was compared with established paper tests of episodic memory, executive function and processing speed, domains previously shown to

Assessing executive and community functioning among homeless persons with substance use disorders using the executive function performance test.

This study investigates the utility of an occupational therapy assessment of executive and performance functioning and indicators of community adjustment among adults with histories of homelessness and substance abuse. A sample of 60 individuals living in a supported housing programme was interviewed about community living experiences and was assessed using the Executive Function Performance Test (EFPT). A descriptive and correlational study design was used. Measurable levels of cognitive and task impairment were documented, in older adults, non-US citizens and those diagnosed with cognitive i

Stability of the dexamethasone suppression test in borderline personality disorder with and without comorbid PTSD: a one-year follow-up study.

Alterations in hypothalamic-pituitary-adrenal axis feedback regulation have been repeatedly reported in patients with borderline personality disorder (BPD). Due to the cross-sectional design of these studies, little is known about the longitudinal course of HPA axis functioning. In a sample of 13 patients with BPD, the dexamethasone suppression test (DST) has been used in a one-year follow-up study. There were no changes of cortisol concentrations before or after dexamethasone intake between baseline and follow-up examination. Patients with comorbid posttraumatic stress disorder (PTSD) showed

Prospective comparison of the tuberculin skin test and 2 whole-blood interferon-gamma release assays in persons with suspected tuberculosis.

BACKGROUND: Interferon-gamma release assays (IGRAs) are attractive alternatives to the tuberculin skin test (TST) for detecting Mycobacterium tuberculosis infection. However, the inability to definitively confirm the presence of most M. tuberculosis infections hampers assessment of IGRA accuracy. Although IGRAs are primarily indicated for the detection of latent tuberculosis infection, we sought to determine the sensitivity of the TST and 2 whole-blood IGRAs (QuantiFERON-TB assay [QFT] and QuantiFERON-TB Gold assay [QFT-G]) in situations in which infection is confirmed by recovery of M. tuberc

Testing the Multilevel Personality-to-Purpose (MPP) framework: An experimental evaluation of an online self-help workbook for promoting purpose in life.

Grounded in the Multilevel Personality-to-Purpose Framework, a six-chapter online self-help workbook was developed to promote purpose in life (purpose) and evaluated in this study. Adult participants (N = 235) recruited from Prolific.ac were randomly assigned to either (a) an experimental condition (completing the workbook in 2 weeks; n = 118) or (b) a waitlist-control condition (n = 117). Assessments were administered on three occasions: preintervention (Time 1), postintervention (Time 2), and 4-week follow-up (Time 3). Linear mixed models partially supported the study hypothesis, indicating

Primary care versus sleep specialist pathways for obstructive sleep apnea with home testing: access, adherence, and treatment personalization.

PURPOSE: Although expanding obstructive sleep apnea (OSA) management capabilities has been a longstanding goal, patients report higher positive airway pressure (PAP) therapy compliance when cared for by specialists at centers accredited by the American Academy of Sleep Medicine (AASM) [1]. At Mayo Clinic, primary care providers (PCPs) are now authorized to order home sleep apnea testing (HSAT) and subsequent therapy, eliminating the need for consultation with Sleep Medicine. This study evaluates whether patients diagnosed with OSA through primary and specialty care pathways experience comparab

Real-Time Dynamic Adaptive Test Assembly for Personalized Measurement of Cognitive Abilities Under Continuous Item Bank Growth.

The accurate measurement of individual cognitive abilities in educational settings increasingly relies on personalized assessment systems that adapt to the learner's current standing on the measured ability. These systems generate and use assessment items continuously and in real-time, bypassing the field-test cycle that traditional calibration requires. Item banks therefore contain a growing share of items whose parameters must be predicted first, and each test form must be assembled in real time to match the learner's current ability level and content needs for effective personalized instruc

Design, implementation, and pilot testing of electronic health record deprescribing tools for persons living with dementia in U.S. primary care.

BACKGROUND AND OBJECTIVES: Electronic health record (EHR) tools are widely used to influence prescribing behaviors. However, the application of EHR tools to deprescribing in older adults, particularly persons living with dementia, is understudied, despite the potential to ameliorate cognitive decline by targeting medications like benzodiazepines and anticholinergics. We explored the design and implementation of EHR-based tools for deprescribing using a multi-phase mixed-methods approach. RESEARCH DESIGN AND METHODS: Within a large healthcare system, we first conducted semi-structured qualitat

Reduced reactivity to fear conditioning and pain tests in persons involved in violent video gaming is influenced by adverse childhood experiences.

Video gaming, including violent video gaming, has become very common and lockdown measures of the COVID-19 pandemic even increased the prevalence rates. In this study, we examined if violent video gaming is associated with more adverse childhood experiences (ACE) and if it impairs pain processing and fear conditioning. We tested three groups of participants (violent video gamers, nonviolent video gamers, and non-gamers) and examined fear conditioning as well as pain perception during functional magnetic resonance imaging (fMRI). Violent video gamers displayed significantly higher pain threshol

Validity of the 30-Second Sit-to-Stand test as a measure of lower extremity function in persons with multiple sclerosis: Preliminary evidence.

BACKGROUND: The 30-Second Sit-To-Stand (30SSTS) is a quick, inexpensive, safe, and widely used clinical measure of lower extremity function. To date, there is limited evidence regarding the use of 30SSTS in multiple sclerosis (MS). The purpose of this study was to examine the construct validity of the 30SSTS test in persons with MS compared with non-MS healthy controls. METHODS: Twenty ambulatory persons with MS and twenty age- and sex-matched healthy controls completed the 30SSTS, Timed 25-Foot Walk (T25FW), Timed Up and Go (TUG), Six-Minute Walk (6MW), and Godin Leisure-Time Exercise Questi

Demand-Resource Profiles and Job Satisfaction in the Healthcare Sector: A Person-Centered Examination Using Bayesian Informative Hypothesis Testing.

Work characteristics may independently and jointly affect well-being, so that whether job demands deplete or energize employees depends on the resources available in the job. However, contradictory results on their joint effects have emerged so far in the literature. We argue that these inconsistencies can be partially explained by two arguments in the contemporary literature in the field. First, most studies in the job design domain are based on classic variable-centered methodologies which, although informative, are not well suited to investigate complex patterns of interactions among multip
