General physicians still fail to take travel histories.

BACKGROUND: Global travel continues to facilitate the spread of infectious diseases, as demonstrated by the West African Ebola epidemic, COVID-19 and mpox outbreaks. Accurate travel histories are essential for formulation of appropriate differential diagnoses, avoiding delays in diagnosis, treatment and implementation of relevant infection prevention and control measures. We repeated a 2011 study that had revealed very poor recording of travel histories in two hospitals in the UK. METHODS: A retrospective review of the medical notes on admission of sequential patients referred to acute medica

Emerging Role of Emergency Physicians in Antimicrobial Stewardship.

The emergency department (ED) is increasingly recognized as a critical site for antimicrobial stewardship, where most initial antibiotic decisions occur, and emergency physicians (EPs), previously regarded as primarily focused on empiric treatment, are now emerging as leaders. Policy milestones, including accreditation standards and national guidelines, have catalyzed this transition, yet physician roles remain heterogeneous, and evidence on long-term outcomes is limited. Drawing on English-language literature, including intervention studies, descriptive reports, and policy documents, this edi

Substance use among Indian Physicians: A narrative review.

BACKGROUND: Substance use disorders (SUDs) are a global public health problem with more adverse impacts among the population at risk. Healthcare workers, including physicians, are at increased risk of substance use problems in developing countries, including India, due to prolonged working hours, financial stress due to poor pay during early career, and poor work-life balance. AIM: We conducted a narrative review to assess the rate and pattern of substance use among Indian physicians. METHODS AND RESULTS: As per limited prevalence studies among Indian physicians since the year 2000, tobacco

Role of biomarkers in antimicrobial stewardship: physicians' perspectives.

Biomarkers are playing an increasingly important role in antimicrobial stewardship. Their applications have included use in algorithms that evaluate suspected bacterial infections or provide guidance on when to start or stop antibiotic therapy, or when therapy should be repeated over a short period (6-12 h). Diseases in which biomarkers are used as complementary tools to determine the initiation of antibiotics include sepsis, lower respiratory tract infection (LRTI), COVID-19, acute heart failure, infectious endocarditis, acute coronary syndrome, and acute pancreatitis. In addition, cut-off va

Evaluating Stigmatization Toward Mental Illnesses Among Resident Physicians.

Background  Individuals with mental illness are stigmatized by peers in societal, workplace, and healthcare settings. The stigmatization individuals experience from healthcare providers is well documented and pervasive, often being a detriment to the quality of healthcare these individuals receive. Objective Recognizing and addressing stigmatization toward individuals with mental illnesses is imperative during residency training in Graduate Medical Education (GME) programs and throughout professional practice. We hope that this cross-sectional study will cultivate mindfulness and improve the h

Postpartum complications following neuraxial anaesthesia for obstetric physicians.

Neuraxial analgesia and anaesthesia are widely accepted and well-tolerated modes of delivery analgesia, being employed in up to 76% of vaginal deliveries and 94% of caesarean deliveries in the United States.1 A cause of considerable concern for postpartum women, their family and caring health professionals is the occurrence of unexplained postpartum complications, not only for management in the index pregnancy, but the uncertain risk of recurrence in future pregnancies. Complications of neuraxial blocks may impact significantly on the ability of mothers to care for and bond with their newborn.

Functional constipation in children: What physicians should know.

Functional constipation (FC) is considered the most common functional gastrointestinal disorder in children with a pooled global prevalence of 14.4% (95% confidence interval: 11.2-17.6) when diagnosed based on the Rome IV criteria. Its pathophysiological mechanisms are thought be multifactorial and complicated, resulting in difficult management. Currently, the most effective medication, when used in parallel with toilet training, is osmotic laxatives. Children's adherence to medication and parental concern regarding long-term laxative use are the main contributors to treatment failure. Recentl

Leukemia: What Primary Care Physicians Need to Know.

Leukemia is caused by an abnormal proliferation of hematopoietic stem cells in the bone marrow. The four general subtypes are acute lymphoblastic, acute myelogenous, chronic lymphocytic, and chronic myelogenous. Acute lymphoblastic leukemia primarily occurs in children, whereas the other subtypes are more common in adults. Risk factors include certain chemical and ionizing radiation exposures and genetic disorders. Common symptoms include fever, fatigue, weight loss, joint pain, and easy bruising or bleeding. Diagnosis is confirmed with bone marrow biopsy or peripheral blood smear. Hematology-

Elevating Health Disparities Education Among Trainees and Physicians.

Health disparities disproportionately affect patients in racial and ethnic minority groups, and these disparities are linked to economic, environmental, and social disadvantage. It is widely known that health disparities impact patients with allergic and immunologic conditions, yet universal and comprehensive training in health disparities is lacking. More robust educational opportunities are needed to fully equip trainees with tools to recognize and develop effective strategies to reduce the burden of health disparities. Also, there are no universal standards or requirements for professional

Diagnostic Accuracy of Skin Cancer by Family Physicians.

BACKGROUND: Skin cancer is the most common type of cancer worldwide. Family physicians (FPs) need to differentiate between nonmalignant and malignant skin conditions, but the diagnostic accuracy of FPs has never been studied in primary care. AIM: To assess the accuracy of skin cancer diagnoses by FPs. Our secondary aim was to analyze the number of patients with premalignant lesions and examine the diversity of skin-related questions in Dutch primary care. METHOD: This study is a retrospective cohort of all new skin-related health questions between January 1, 2018, and July 1, 2018, in a Dutc

What family physicians should know about interventional radiology?

Interventional radiology (IR) is a rapidly evolving specialty. The minimally invasive nature of the procedures has led to a paradigm shift in treating many disease processes from conventional surgery to interventional techniques that have become the first choice and the preferred method for the treatment of peripheral vascular disease, many aortic pathologies, and venous diseases. Interventional oncology procedures have become widely available for the treatment of solid hepatic and renal tumors. This includes palliative techniques for many advanced malignancies, and fluid drainage that is excl

Should Wellness Be a Core Competency for Physicians?

There has been a recent rise in calls for action around wellness and physician health. In medical education, wellness has been proposed as a physician competency. In this article, the authors review the history of the "wellness as a competency" concept within U.S. and Canadian residency programs and medical schools. Drawing from literature on the discourses of wellness and competence in medical education, they argue that operationalizing wellness as a physician competency holds profound implications for curricula, admissions, evaluation, and licensure. While many definitions of "wellness" and

Practice Predictors of Buprenorphine Prescribing by Family Physicians.

INTRODUCTION: Both opioid use disorder and mortality for opioid overdoses are increasing. Family physicians (FPs) can treat opioid use disorder if they are waivered to prescribe buprenorphine. Our objective was to determine personal, practice, and community characteristics associated with FPs prescribing buprenorphine. METHODS: We used data from the 2017 and 2018 American Board of Family Medicine examination registration questionnaire. The questionnaire asked about current prescribing of buprenorphine, as well as about practice size, organization, and location. Logistic regression was used to

Neuropsychological impairment associated with substance use by physicians.

Physicians and other healthcare professionals are at increased risk for the development of substance use disorders when compared to the general population. Substance use is associated with impairment in neuropsychological functioning, which may impact physicians' ability to practice with reasonable skill and safety. This review describes common neurocognitive deficits observed following use of various substance classes, including stimulants, benzodiazepines, cannabis, alcohol, and opioids. It also reviews the neurocognitive impact of pharmaceutical treatments for opioid use disorder. Clinical

Kansas Family Physicians Perceptions of Parental Vaccination Hesitancy.

INTRODUCTION: In the past few decades, patients expressing the idea that vaccines are unsafe or unneeded have been experienced increasingly by physicians and other healthcare providers. Discussions with patients regarding their reasons for vaccine refusals are important, as it may provide information that can be utilized in an intervention to increase vaccination rates and combat the spread of diseases that are making a resurgence in the United States. The main objective of this study was to explore the perceptions of family physicians as to why parents in Kansas may be vaccine hesitant. METH

Addressing Depression, Burnout, and Suicide in Oncology Physicians.

The best practice of oncology relies heavily on a mentally and physically healthy oncology clinician workforce. Historically, the mental health of physicians and clinicians has largely been ignored, perhaps in the spirit of a collective collusion to maintain the illusion of Oslerian equanimity. With exceedingly high and unacceptable rates of burnout and suicide in the practice of medicine and oncology in particular, a tacit disavowal of the problem is no longer acceptable. The practice of oncology presents several unique work-related issues that challenge the mental health of its clinicians an

Scleroderma renal crisis: a review for emergency physicians.

Scleroderma renal crisis (SRC) remains a high-risk clinical presentation, and many patients require emergency department (ED) management for complications and stabilization. This narrative review provides an evidence-based summary of the current data for the emergency medicine evaluation and management of SRC. While SRC remains a rare clinical presentation, surveillance data suggest an overall incidence between 4 and 6% of patients with scleroderma. The diagnostic criteria for SRC include a new onset blood pressure > 150/85 mm Hg OR increase ≥ 20 mm Hg from baseline systolic blood pressure, al
