Endovascular and cerebral bypass therapies are rarely used in children. The authors describe the treatment of a partially coiled giant distal vertebral artery (VA)-posterior inferior cerebellar artery (PICA) aneurysm in a child. They performed a side-to-side PICA-PICA anastomosis followed by endovascular VA aneurysm deconstruction with PICA preservation. A healthy 11-year-old boy developed progressive holocephalic headaches over the course of 2 months. Magnetic resonance imaging and magnetic resonance angiography revealed a large right PICA aneurysm causing brainstem compression. In November 2

BACKGROUND: Ruptured aneurysms of the intracranial vertebral artery (VA) or posterior inferior cerebellar artery (PICA) are challenging to treat as they are often dissecting aneurysms necessitating direct sacrifice of the diseased segment, which is thought to carry high morbidity due to brainstem and cerebellar stroke. However, relatively few studies evaluating outcomes following VA or proximal PICA sacrifice exist. We sought to determine the efficacy and outcomes of endovascular VA/PICA sacrifice.
METHODS: A retrospective series of ruptured VA/PICA aneurysms treated by endovascular sacrifice

BACKGROUND: Posterior fossa arteriovenous malformations (AVMs) are associated with increased risk of rupture and severe consequences from such rupture. The hemorrhagic risk of prenidal aneurysms (anr) on the posterior inferior cerebellar artery (PICA) may exceed that of the AVM in posterior fossa AVMs fed by PICA (PICA-AVM).
OBJECTIVE: To characterize the relative risks of aneurysm and AVM hemorrhage in patients with posterior fossa AVMs.
METHODS: We retrospectively reviewed patients diagnosed with AVM. Patients with posterior fossa AVMs were divided into 3 groups: PICA-AVM with prenidal ane

BACKGROUND AND IMPORTANCE: The surgical management of posterior inferior cerebellar artery (PICA) aneurysms often poses a challenge because of tight surgical corridors, proximity to the lower cranial nerves, and the presence of perforators supplying the medulla. This report describes an excision and double reimplantation bypass for treatment of a dolichoectatic PICA aneurysm found on familial screening in a young man with a family history of aneurysmal subarachnoid hemorrhage.
CLINICAL PRESENTATION: The patient underwent a right far-lateral craniotomy with excision of the dolichoectatic right

OBJECTIVE AND IMPORTANCE: We describe a very rare case involving a ruptured intracranial aneurysm at the distal posteroinferior cerebellar artery (PICA) branching from the extracranial vertebral artery.
CLINICAL PRESENTATION: A 53-year-old woman experienced the sudden onset of a severe occipital headache and vomiting. Computed tomographic scanning revealed subarachnoid and intraventricular hemorrhage. Cerebral angiography of the left vertebral artery demonstrated the left PICA branching from the extracranial segment of the extracranial vertebral artery at the level of C2; a saccular aneurysm

BACKGROUND: Bow hunter's syndrome is a rare cause of vertebrobasilar insufficiency arising from mechanical compression of the vertebral artery (VA) during rotation of the head. Surgical treatment usually involves direct decompression of the VA at the site of compression. We describe what is to our knowledge the first reported case of a posterior inferior cerebellar artery (PICA)-to-PICA in situ bypass for treatment of Bow hunter's-type ischemia in a patient with a VA dissection.
CASE DESCRIPTION: The patient was a 41-year-old man who developed disabling symptoms of vertebrobasilar insufficien

BACKGROUND: True posterior inferior cerebellar artery (PICA) aneurysms outside the vertebral artery-PICA region are rare, with approximately 30 cases reported in just a few papers; no treatment paradigm has been advocated. The objective of this study was to present detailed clinical features and outcomes for several treatments for true PICA aneurysms and suggest an algorithm for treatment strategies.
METHODS: We retrospectively analyzed outcomes of patients treated for PICA aneurysms with microsurgical and endovascular treatments. We also investigated the influence of several factors on the m

BACKGROUND: The authors present a rare case of a ruptured aneurysm at the choroidal branch of the posterior inferior cerebellar artery (PICA).
CASE DESCRIPTION: A 77-year-old female was admitted to our institute because of sudden onset of severe headache and vomiting. Radiologic examination revealed intraventricular hemorrhage caused by rupture of the aneurysm at the choroidal branch of the PICA. The fusiform aneurysm was resected after ligation via a midline suboccipital approach.
CONCLUSIONS: The conclusions drawn from this experience and a review of the literature include the following: (

The incidence of posterior fossa aneurysm is reported to be 4-15% of all intracranial aneurysms in large series. Most aneurysms of the posterior inferior cerebellar artery (PICA) arise at the origin of the PICA. However, aneurysms of the distal part of the PICA have only rarely been described. This report presents three cases of distal PICA aneurysm with subarachnoid hemorrhage. Two aneurysms were located in the superior retrotonsillar segment of the PICA in two cases, and another was located in the tonsillohemispheric branch in one case. Obliteration of these aneurysms was comparatively easil

PURPOSE: To demonstrate that occipital artery (OA)-p1 posterior inferior cerebellar artery (PICA) bypass can be an alternative for complex posterior circulation aneurysms.
METHODS: A far-lateral approach to craniotomy was performed on 20 cadaveric specimens, and the OA was obtained 'in-line.' Its length, diameter, and the number of p1/p2 and p3 segmental perforators were determined, and the relationship between the caudal loop and cerebellar tonsil position was also assessed. The distance between the PICA's origin and the cranial nerve XI (CN XI), the buffer length above the CN XI after disse

Nonsaccular vertebral artery aneurysms involving the posterior inferior cerebellar artery (PICA) are rare. Treatment is considered a significant challenge because of their angiographic and anatomical features, especially in high-riding PICA. Therefore, meticulous preoperative angiographic and anatomical evaluation is necessary. Moreover, consideration of the distance between the cerebellar skull base and caudal loop of the PICA is important. We present two cases of occipital artery-high-riding PICA bypass and discuss important preoperative technical considerations.

We experienced a case of subarachnoid hemorrhage due to a rupture of a vertebral artery dissecting aneurysm (VADA) involving the posterior inferior cerebellar artery (PICA) treated with a novel endovascular technique, which used a balloon catheter to protect the PICA origin via the contralateral vertebral artery. We performed coil embolization of the VADA and proximal parent artery occlusion via the ipsilateral vertebral artery under protection of the PICA. Using a balloon catheter instead of a stent, dense packing was achieved while preserving the PICA and avoiding hemorrhagic complications w

Double origin of the posterior inferior cerebellar artery (PICA) is an infrequent developmental anomaly predisposing an individual to intracranial aneurysm formation. Fenestration of the PICA is extremely rare, and furthermore, fenestration of the double origin of the PICA in association with an intracranial aneurysm is unique. We present the first reported case of fenestration of the double origin of the PICA associated with a dissecting aneurysm of the contralateral vertebral artery (VA) that healed spontaneously after medical observation.

OBJECTIVE: Our surgical results were reviewed to clarify the cause of glossopharyngeal neuralgia (GPN) and the effects of the microvascular decompression (MVD) procedure.
METHODS: Fourteen cases of idiopathic GPN were operated on through the transcondylar fossa (supracondylar transjugular tubercle) approach. Their clinical data and operative records were retrospectively reviewed.
RESULTS: In every case, vascular compression on the glossopharyngeal nerve was found and MVD was performed without any major complications. In 13 of the 14 cases the neuralgia completely disappeared postoperatively.

A 74-year-old patient was diagnosed with a subarachnoid hemorrhage suspected from a dissecting aneurysm located at the lateral medullary segment of the posterior inferior cerebellar artery (PICA). Because perforators to the medulla arose both proximal and distal to the dissecting segment, revascularization for distal flow was essential. However, several previously reported methods for anastomosis, such as an occipital artery-PICA bypass or resection with PICA end-to-end anastomosis could not be used. Ultimately, we performed an in situ side-to-side anastomosis of the proximal loop of the PICA

Posterior inferior cerebellar artery (PICA) occlusion (with or without vertebral artery occlusion) may result in severe disability and even mortality. Patients who underwent mechanical thrombectomy (MT) for PICA occlusion (with or without vertebral artery occlusion) between 2021 and 2023 were included in the study. All five patients who underwent MT were male, aged between 45 and 68 years, with National Institute of Health Stroke Scale scores ranging from 3 to 10. The successful recanalization rate in our cases with PICA occlusion was 60%. As the MT technique, an isolated stent retriever was u

The authors report an unusual surgical treatment for an aneurysm on the left peripheral posterior inferior cerebellar artery (PICA). The computed tomographic scan showed subarachnoid haemorrhage and a haematoma in the left cerebellar region close to the fourth ventricle. The 3D CT angiography demonstrated an aneurysm on the left peripheral PICA which was confirmed on four-vessel angiography. The aneurysm was approached through a posterior suboccipital craniotomy and the PICA was reconstructed with multiple clips. The literature concerning peripheral PICA aneurysms and their treatment is review

Dissecting aneurysms occur when blood extrudes into the wall of a vessel. Posterior circulation dissections are recognised as an important cause of cerbral infarction and subarachnoid haemorrhage(SAH), however posterior inferior cerebellar artery (PICA) aneurysmal dissections are rare. A 49-year-old man who presented with SAH was found to have a left PICA dissection on cerebral angiography. The lesion was treated with surgical clipping proximal to the dissection and a distal PICA to PICA anastomosis. The pathology, diagnosis, presentation and treatment of these difficult lesions is discussed.
