Functional results after extensor indicis proprius to extensor pollicis longus tendon transfer for ruptures associated with distal radius fractures.

Rupture of the extensor pollicis longus (EPL) tendon is a recognised complication after distal radius fracture (DRF) that leads to loss of active thumb extension. Transfer of the extensor indicis proprius (EIP) tendon to the EPL is one of the most common reconstructive procedures, but prospective outcome data are limited. This study evaluated functional and patient-reported outcomes after EIP-to-EPL tendon transfer exclusively in patients with EPL rupture secondary to DRF. Thirty consecutive patients (26 women, 4 men; median age 65 years, range 17-80) underwent EIP-to-EPL tendon transfer betwe

Effects of occupational therapy on hand function in patients with systemic sclerosis: a randomized controlled trial.

Hand involvement is one of the most disabling features of systemic sclerosis (SSc). Although occupational therapy is frequently employed in rheumatic conditions, its particular effects in SSc have not been studied in detail yet. This study aimed to evaluate the impact of a structured three-week occupational therapy program on hand function in SSc. This randomized controlled trial involved adult patients with SSc. Participants were randomly allocated to an intervention group that received a three-week occupational therapy program or a control group that received standard rheumatic care. Evaluat

[Is a dynamic or stable technique needed for metacarpophalangeal joints of the hand in ulnar nerve palsy?].

INTRODUCTION: It seems interesting to choose between the dynamic and the static techniques for the rehabilitation of the hand suffering from an ulnar nerve palsy on the basis of the grasp (F1) and pinch (F2) strengths which are important in this rehabilitation. METHODS: From june 1997 to december 2001, 30 ulnar palsies all post traumatic with non complicated claw deformities have been collected and distributed in three groups of ten patients. In the group A only the static techniques have been used on the MP joints of the fourth and fifth digits and the thumb; in the group B only the dynamic

Pick-up test: an indication for opponensplasty in patients with severe carpal tunnel syndrome.

PURPOSE: To use a pick-up test to assess thenar motor deficit and results of opponensplasty. METHODS: Eight consecutive patients with idiopathic severe carpal tunnel syndrome and severe thenar motor deficit were identified. All were females aged 39 to 60 (mean, 53) years and right-hand dominant. The severity of thenar motor deficit was assessed using the pick-up test. Failure to pick up a coin with the thumb and index finger only in a normal pulp-to-pulp pincer grip was an indication for an opponensplasty (in addition to open carpal tunnel release) to hasten recovery of thumb abduction. Design and experimental testing of a force-augmenting exoskeleton for the human hand.

BACKGROUND: Many older Americans suffer from long-term upper limb dysfunction, decreased grip strength, and/or a reduced ability to hold objects due to injuries and a variety of age-related illnesses. The objective of this study was to design and build a five-fingered powered assistive exoskeleton for the human hand, and to validate its ability to augment the gripping and pinching efforts of the wearer and assist in performing ADLs. METHODS: The exoskeleton device was designed using CAD software and 3-D printed in ABS. Each finger's movement efforts were individually monitored by a force sens

A Pulsed Electrical Joint Stimulator for the Treatment of Osteoarthritis of the Hand and Wrist.

The hand is commonly affected by osteoarthritis (OA). The development and progression of OA are believed to involve inflammation, even in the early stages of the disease. Inflammatory and proinflammatory cytokines have also been shown to be elevated in the flexor tenosynovium of idiopathic carpal tunnel syndrome (CTS). A large percentage of patients with hand OA also have a concomitant CTS. This study evaluated the results of a pulsed electrical joint stimulator in patients who had hand OA with or without CTS. Pain, tenderness, and swelling; grip strength and pinch force; and Patient and Physi

Prevalence and clinical-radiological features of hand osteoarthritis in individuals with knee osteoarthritis.

Hand osteoarthritis (HOA) frequently coexists with knee osteoarthritis (KOA), yet its clinical and radiographic burden in ageing adults with KOA remains under-recognised. To determine the prevalence and joint-specific distribution of clinical, radiographic, and symptomatic HOA in individuals with KOA; to describe associated functional characteristics; and to assess whether HOA severity is related to KOA severity. A descriptive cross-sectional study was conducted among adults ≥ 40 years with tibiofemoral KOA (KL grades 1-4). HOA was evaluated using American College of Rheumatology criteria, han

Trapeziometacarpal Arthrodesis Technique and Outcomes using a Nitinol Compression Staple.

Trapeziometacarpal (TMC) arthrodesis is a reliable procedure to treat individuals with trapeziometacarpal osteoarthritis (TMC OA) who have failed conservative treatment. Arthrodesis has classically been deemed as a suitable treatment for young, active males, particularly in the post-traumatic setting, who desire maximum strength. There are numerous arthrodesis techniques described in the literature, however, reported high nonunion rates may deter surgeons from treating TMC OA with arthrodesis.
