Study protocol for a comparative diagnostic accuracy study of bedside tests used to detect arterial disease in diabetes: TEsting for Arterial disease in Diabetes (TrEAD) study.

INTRODUCTION: In the UK, over 7000 amputations are performed each year because of diabetes. Up to 80% of these are preceded by a foot ulcer and could therefore be prevented with improvements in ulcer care. Peripheral arterial disease is an important risk factor for the development of diabetic foot ulceration. However, its diagnosis in diabetes is challenging due to the presence of neuropathy and arterial calcification. Commonly used bedside tests either have low sensitivities or little supporting evidence to justify their use. Duplex ultrasound (DUS) has good correlation to angiography finding

Fat grafting and platelet-rich plasma for the treatment of diabetic foot ulcers: A feasibility-randomised controlled trial.

Chronic, nonhealing diabetic foot ulcers (DFU) are increasing in prevalence and are often unresponsive to conventional therapy. Adipose tissue, containing adipose-derived stem cells, and platelet rich plasma (PRP) are regenerative therapies rich in growth factors which may provide a solution to chronic wound healing. This study aimed to assess the feasibility of conducting a definitive randomised controlled trial (RCT) to investigate the efficacy of these therapies for the treatment of DFU. This was a single centre, feasibility, three-arm, parallel group RCT. Eligible DFU patients were randomi

[The diabetic foot].

Problems of the diabetic foot are frequent. The magnitude of the clinical picture and morbidity mirrors the severity and complexity of the underlying pathobiology. The three pathogenetic mechanism involved are ischemia, neuropathy and infection. Seldom do these mechanisms work in isolation, rather most foot problems result from a complex interplay among all three. The clinical picture of the diabetic foot reaches from the neuropathic deformity with diminished or absent sensation of pain to limited gangrene or superficial ulcer. The polymicrobial infection leads to extensive tissue destruction

Corticosteroid Injection Practices of Australian Podiatrists for Common Foot and Ankle Conditions.

INTRODUCTION: Corticosteroid injections are commonly used to manage foot and ankle conditions; however, there is limited evidence guiding corticosteroid selection, dosing, imaging guidance and injection techniques. This study aimed to describe current corticosteroid injection practices among Australian podiatrists and podiatric surgeons. METHODS: A descriptive cross-sectional survey was distributed via REDCap to Australian podiatrists and podiatric surgeons endorsed to prescribe scheduled medicines. The questions were developed using an iterative process and focused on demographic and profess

Strategies to reduce severe diabetic foot infections and complications during epidemics (STRIDE).

AIMS: Patients with diabetes, including those with foot complications, are at highest risk for severe outcomes during the COVID-19 pandemic. Diabetic foot ulcers (DFU) present additional challenges given their superimposed risk for severe infections and amputations. The main objectives were to develop a triage algorithm to effectively risk-stratify all DFUs for potential complications, complying with social distancing regulations, preserving personal protective equipment, and to assess feasibility of virtual care for DFU. METHODS: Longitudinal study during the COVID-19 pandemic performed at a

Nutritionally induced peripheral neuropathies.

The authors have presented a discussion of some of the more common nutritional deficiency syndromes. A discussion of the etiology, clinical manifestation, diagnosis, and prognosis has been provided. Special attention has been given to the relationship between nutritional deficiency and the development of peripheral neuropathies. When patients suffering from peripheral neuropathy present, the possibility of a nutritional deficiency should be considered. The diagnosis of a nutritional deficiency neuropathy is rarely established. This may not merely be the result of a low incidence of the problem

Congenital metatarsus adductus: clinical evaluation and treatment.

Congenital metatarsus adductus, a deformity at the tarsometatarsal joints in which the metatarsals are deviated internally in relation to transverse plane, predisposes to "pigeon-toed" gait. Treatment is aimed at achieving normally balanced foot by overcorrection at the deformity sites. The most common problem after treatment is pronation deformity, which can be avoided by carefully positioning the heel at the time of casting. The sooner conservative measures are instituted, the more easily and rapidly the deformity will be corrected. If conservative casting does not result in improvement, the

Risk factors for infection following ingrowing toenail surgery: a retrospective cohort study.

BACKGROUND: Ingrowing toenails are a common and painful condition often requiring surgical management. Practitioners who perform surgery on ingrowing toenails include orthopaedic surgeons, general practitioners, podiatrists and podiatric surgeons. There has been limited investigation into the specific surgical approaches used by Australian podiatric surgeons for ingrowing toenails, or the associated infection rates for these procedures. The aim of this study was to assess the frequency and type of ingrowing toenail surgery performed by podiatric surgeons, and identify risk factors for post-ope

[Integrated podiatrist care for prostate cancer patients treated with docetaxel: Feasibility and results].

BACKGROUND: Docetaxel is frequently used for the treatment of metastatic prostate cancer patients. Nail toxicity is a commonly described side effect, but no precise recommendation exists concerning its management. We experimented the integration of a podiatrist in routine cancer care. METHODS: Patients having received docetaxel for a metastatic prostate cancer since the arrival of the podiatrist were studied. RESULTS: Fifty-six patients were included, half had docetaxel-induced nail toxicity and 18 were referred to the podiatrist. The integration of the podiatrist in routine care was feasibl

Wound healing and infection in nail matrix phenolization wounds. Does topical medication make a difference?

After nail matrix ablation using phenolization, a medicated wound dressing (10% povidone iodine), an amorphous hydrogel dressing (Intrasite Gel), and a control dressing (paraffin gauze) were evaluated. Forty-two participants, randomly divided into three dressing groups, were evaluated. Healing time did not differ between the 10% povidone iodine (33 days), amorphous hydrogel (33 days), and the control dressing (34 days). For all groups, the clinical infection rate was lower than in previous studies, and there was no clinical difference between groups (one infection in the povidone iodine and co

Human nail dust and precipitating antibodies to Trichophyton rubrum in chiropodists.

The toe-nail dust produced in chiropodial practice causes allergic hypersensitivity and Trichophyton rubrum is the most common fungal cause of nail dystrophy. Use of drills and burrs to reduce the thickness of hyperkeratotic nails generates dust and chiropodists develop precipitins to T. rubrum. A survey into the prevalence of these antibodies in 11.2% of state-registered chiropodists is described and 14% of the profession estimated to have antibodies to T. rubrum. In response to a questionnaire 49% stated that toe-nail dust troubled them; complaints of nasal and eye symptoms were 72 and 41% r

Leg ulcers in sickle cell disease: current patterns and practices.

Leg ulcers are a debilitating complication of patients with sickle cell disease, and their frequency in North America was reported to be 2.5% by the Cooperative Study of Sickle Cell Disease more than 20 years ago. We sought to determine if the frequency of leg ulcers in sickle cell patients in the United States had declined and to assess which treatments providers use most commonly. We sent an e-mail survey to health professionals belonging to the national Sickle Cell Adult Provider Network. Responses were obtained from 31 of them (26.0%). Most of them (96.0%) reported having some patients wit

Cost analysis of flap reconstruction for diabetic foot ulcers in the NHS: A single-centre study.

BACKGROUND: Diabetic foot disease costs NHS England up to £1 billion annually. Flap reconstruction can offer limb salvage in patients with diabetic foot ulcers (DFUs), but its cost-effectiveness remains unclear. This study evaluates the impact of flap reconstruction for DFU on direct secondary health costs in a single UK centre. METHODS: A retrospective cohort cost analysis was conducted for patients undergoing flap reconstruction for DFU that have failed conservative management between 2019 and 2024. Data on hospital admissions, surgery, outpatient care, podiatry, and Rapid Response and Inte

Comparative Outcomes of Percutaneous Needle Tenotomy Performed by a Podiatrist Versus an Orthopaedic Surgeon in the Management of Toe Ulcers.

BACKGROUND: Foot ulceration is a debilitating and often disabling complication of diabetes mellitus, with negative prognostic associations in terms of morbidity and mortality. Percutaneous needle tenotomy (PNT) is increasingly recognized as a safe, minimally invasive procedure for treating tendon-related deformities, including mechanical forefoot ulceration. This study evaluated clinical outcomes of needle flexor tenotomies performed by a podiatrist versus an orthopaedic surgeon. METHODS: This service evaluation reviewed consecutive adult patients with foot ulceration who received needle teno

Feasibility of a Low-Intensity, Technology-Based Intervention for Increasing Physical Activity in Adults at Risk for a Diabetic Foot Ulcer: A Mixed-Methods Study.

BACKGROUND: Among adults with diabetes, 19-34% will develop a diabetic foot ulcer (DFU), which increases amputation risk and health care costs, and worsens quality of life. Regular physical activity, when increased gradually, may help prevent DFUs. In this mixed-methods study, we examined the feasibility of a low-intensity, technology-based behavioral intervention to increase activity in adults at risk for DFUs. METHOD: Participants at risk for a DFU (n = 12; 66% female; mean age = 59.9 years) received four in-person exercise and behavioral counseling sessions over 2-3 weeks, supplemented wit
