Systematic review: outcomes and post-operative complications following colectomy for ulcerative colitis.

BACKGROUND: Colectomy for ulcerative colitis is associated with short- and long-term complications. Estimates of the frequency of such complications are variable and may have changed since the introduction of biological therapy. Understanding the true burden of surgical complications is important to clinicians in assessing risks and benefits of colectomy vs. continued medical therapy. AIM: To ascertain the outcomes of colectomy and ileal pouch surgery in patients with ulcerative colitis in the biologics era. METHODS: Embase, MEDLINE and The Cochrane Library were searched for studies (2002-20

Elective and emergent operative management of ulcerative colitis.

Surgical therapy of ulcerative colitis is effective, safe, and provides an improved quality of life in those whose disease cannot be managed medically. In the elective setting, widespread acceptance of restorative proctocolectomy has made surgical therapy an attractive option in the overall management of ulcerative colitis. Enthusiasm for this procedure should be tempered by the acknowledgment of the significant incidence of pouchitis in the long term, however. Proctocolectomy with ileostomy remains a good surgical option for patients who are unsuitable for restorative procedures. The standard

Long-Term Bowel Function and Fate of the Ileal Pouch After Restorative Proctocolectomy in Patients With Crohn's Disease: A Systematic Review With Meta-Analysis and Metaregression.

BACKGROUND AND AIMS: Debate exists on whether ileal pouch anal anastomosis [IPAA] can be safely offered to patients diagnosed with Crohn's disease [CD]. Our aim was to assess the outcome of IPAA for CD vs ulcerative colitis [UC]. METHODS: We used a PRISMA/MOOSE-compliant meta-analysis. Studies published between 1993 and 2018 were retrieved. Primary end points included complications. Secondary endpoints included functional outcome. The time of CD diagnosis was considered [intentional vs incidental IPAA]. RESULTS: Eleven studies comprising 6770 patients [CD = 352, UC = 6418] were included, wit

Indeterminate colitis: the real story.

PURPOSE: Up to one in five patients undergoing surgery for ulcerative colitis will have ambiguous histology, with features of both ulcerative colitis and Crohn's disease, and are categorized as having indeterminate colitis. We hypothesized that functional outcomes in indeterminate colitis patients undergoing ileal pouch-anal anastomosis are comparable with those of ulcerative colitis patients undergoing ileal pouch-anal anastomosis. METHODS: Physician-conducted interviews of 120 consecutive ileal pouch-anal anastomosis patients with a preoperative diagnosis of ulcerative colitis were reviewed

Is ileal pouch-anal anastomosis really the procedure of choice for patients with ulcerative colitis?

PURPOSE: Ileal pouch-anal anastomosis is widely claimed to have replaced total proctocolectomy with ileostomy as the "procedure of choice" for ulcerative colitis, largely on the basis of a perceived improved quality of life. There exists relatively little support for this assertion in the literature. Our aim was to determine if educated patients choosing total proctocolectomy with ileostomy have a similar quality of life as with ileal pouch-anal anastomosis. METHODS: All patients with ulcerative colitis referred to a single surgeon and deemed an appropriate surgical candidate were educated an

The lateral reservoir technique of ileal endorectal pull-through for ulcerative colitis and familial polyposis in children.

Total abdominal colectomy and endorectal ileal pull-through have been widely used in children as a definitive continence-preserving procedure for ulcerative colitis (UC) and familial polyposis (FP). Controversy exists in regards to the results achieved with the various techniques utilized to accomplish this procedure, including straight ileal pull-through, pull-through with balloon dilatation of the lower ileal segment, and the construction of a variety of ileal reservoirs: S-shaped, J-shaped, or lateral. We have utilized the two-stage lateral reservoir approach advocated by Fonkalsrud et al,

Does ethnicity impact the long-term outcome after restorative proctocolectomy with ileal pouch anal anastomosis?

BACKGROUND: Racial/ethnic disparities were reported to impact the phenotype of inflammatory bowel disease and its treatment. The impact of race/ethnicity on ileal pouch anal anastomosis (IPAA's) outcomes is unknown. Our aim was to investigate whether it impacts IPAA's outcomes. METHODS: A prospectively maintained IPAA database(1983-2022) was searched to identify patients with IPAA who had race/ethnicity data available regardless of diagnosis. The cohort was divided into 2 groups: Whites and Non-Whites. We analysed patients' demographics, postoperative complications, Kaplan-Meier pouch surviva

Probiotics in the management of Crohn's disease and ulcerative colitis.

Probiotics are nowadays frequently used by patients with inflammatory bowel disease, however literature data are conflicting related to their importance. In mild to moderate ulcerative colitis probiotics can be used effectively in induction and maintaining remission, and prevention of pouchitis. As the other side of the shield, there is not sufficient evidence to support the use of probiotics in daily clinical practice in Crohn's disease. The aim of the present review is to provide help for clinicians about the probiotic use in patients with inflammatory bowel disease. The comparison of litera
