Employing innovation to enhance the safety and reliability of restorative surgical techniques for patients with familial adenomatous polyposis at a national referral centre.

INTRODUCTION: Restorative proctocolectomy (RPC) and total colectomy with ileorectal anastomosis (TC-IRA) are traditional surgical options for individuals with familial adenomatous polyposis (FAP). Re-appraisal and modification to these techniques, such as near-total colectomy with ileo-distal sigmoid anastomosis (NT-IDSA) and RPC with robotic intracorporeal single-stapled anastomosis (RPC-RiSSA), have been implemented in recent years. This study aimed to evaluate the early postoperative outcomes associated with novel techniques employed in a single centre for restorative surgery in patients wi

Ileo-Anal Pouch Anastomosis and New Remedial Approaches for Ulcerative Colitis: A Review Article.

Ulcerative colitis (UC) causes diffuse friability and superficial wall degeneration that is accompanied by bleeding. UC, now recognized as a global illness, affects millions of people globally. The most effective treatment for UC is Ileal Pouch Anal Anastomosis (IPAA). Many medical experts and patients favor proctocolectomy with IPAA because it improves bowel function and allows feces to pass via the anus. Considering the most recent research, systematic reviews, statistical analysis, and recommendations, a selective literature search was carried out. The database used was PubMed. The current

Prophylactic surgery for patients with longstanding ulcerative colitis. Which option? Histopathological and clinical implications.

Patients with longstanding chronic ulcerative proctocolitis are at risk to develop colorectal cancer Conflicting views as regards surveillance, the indications for surgery and type of preventive procedure exist. For permanent prevention of cancer development complete removal of all potential malignant colorectal mucosa has to be done. Panprocto-colectomy with a conventional ileostomy or continent ileostomy removing all colorectal mucosa should therefore eliminate further risks of colorectal cancer. Colectomy and ileorectal anastomosis is a controversial issue. While many surgeons today are rel

Robotic ileal pouch-anal anastomosis after proctectomy: a feasible and promising approach? Short-term outcomes and long-term functional results from a two-center retrospective study.

Minimally invasive ileal pouch-anal anastomosis (IPAA) is the standard restorative procedure for ulcerative colitis (UC) requiring surgery. The clinical impact of the robotic approach remains uncertain. This study aimed to compare perioperative and long-term functional outcomes following robotic versus laparoscopic IPAA in two high-volume tertiary centers. This bicenter retrospective study included consecutive patients who underwent restorative proctocolectomy (RPC-IPAA) or restorative proctectomy (RP-IPAA) with IPAA for UC between 2012 and 2024. Patients were classified into robotic or laparo

The fate of the rectum in ulcerative colitis at index surgery and beyond-a contemporary cohort.

PURPOSE: Proctectomy is frequently deferred at index colectomy for ulcerative colitis due to acuity or immunosuppressive treatments. The retained rectum remains symptomatic in over 50% with associated cancer risk. Management options include index or delayed proctectomy with or without restoration of continuity or surveillance. Comparative studies of perioperative outcomes and reasons for retaining the rectum are lacking. METHODS: This 13-year retrospective cohort assesses the fate of the rectum in 168 ulcerative colitis patients by analysing index proctectomy, staged proctectomy and retained

Surgical management of the colorectum in FAP: tailored approaches for optimal outcomes.

Familial adenomatous polyposis (FAP) is an inherited condition that predisposes individuals to colorectal cancer without preventive treatment. Surgical management typically involves restorative proctocolectomy with an ileal pouch anal anastomosis or colectomy with ileorectal anastomosis. Complete removal of the large intestine and rectum with a permanent stoma may also be required in selected cases. This narrative review highlights decision-making in FAP regarding the timing, extent, and modality of large bowel surgery. Key considerations include the extent of polyps, cancer risk in the remain

Role of probiotics in the management of pouchitis.

The concept that the gut microbiota plays a major role in the development of pouchitis in ulcerative colitis patients after restorative proctocolectomy with ileal pouch-anal anastomosis, is widely accepted and supported by a widespread use of some antibiotics (metronidazole and/or ciprofloxacin, rifaximin) to treat this condition. A possible, safer therapeutic alternative is represented by probiotics. To date the controlled studies addressing the possible efficacy of probiotic agents in the treatment of active pouchitis are few and inconclusive. By contrast the prophylactic use of a probiotic

Successful radiofrequency ablation of an anterior abdominal wall desmoid in familial adenomatous polyposis.

BACKGROUND: Desmoid tumours are an important cause of mortality in familial adenomatous polyposis (FAP). There are few effective treatment strategies. This report describes the use of radiofrequency ablation to debulk and palliate an abdominal wall desmoid tumour in FAP. METHODS: A 22 year old woman with FAP developed a large abdominal wall desmoid tumour after restorative proctocolectomy. The tumour was treated with 16 separate radiofrequency ablations. The follow up was 36 months from the first ablation. RESULTS: The procedure was well tolerated with minor complications; mild superficial c

Colectomy for acute colitis: is it safe to close the rectal stump?

We report 62 operations for acute colonic inflammatory bowel disease in which the rectal stump was closed. Operative findings were of severe colitis in 46, toxic megacolon in 8 and faecal peritonitis in 8 patients. Histology showed ulcerative colitis in 48, Crohn's disease in 9 and indeterminate colitis in 5 patients. Clinical evidence of stump leakage occurred in only one of 53 patients with a long rectal stump in contrast to 3 of 9 patients who had a short rectal stump. Leaving a very short stump also led to difficulty at subsequent proctectomy in 3 patients and at restorative proctocolectom

[Sexual function after ileo-anal anastomosis].

After restorative proctocolectomy, both men and women report an improved sexual life primarily because of a better general health. Sexual dysfunction has been reported by a few men who complain of lack of or retrograde ejaculation (2%) or impotence (3%). Sexual dysfunction, especially dyspareunia, can persist in as many as 20% of women after ileoanal anastomosis, but is less severe than preoperatively in most and is less common than after ileostomy, whether continent or not. Women with an ileal pouch-anal anastomosis can conceive, and have had successful deliveries, either vaginally or by cesa

Management of pouch neoplasia: consensus guidelines from the International Ileal Pouch Consortium.

Surveillance pouchoscopy is recommended for patients with restorative proctocolectomy with ileal pouch-anal anastomosis in ulcerative colitis or familial adenomatous polyposis, with the surveillance interval depending on the risk of neoplasia. Neoplasia in patients with ileal pouches mainly have a glandular source and less often are of squamous cell origin. Various grades of neoplasia can occur in the prepouch ileum, pouch body, rectal cuff, anal transition zone, anus, or perianal skin. The main treatment modalities are endoscopic polypectomy, endoscopic ablation, endoscopic mucosal resection,

A unique variant of afferent limb syndrome after ileal pouch-anal anastomosis: a case series and review of the literature.

BACKGROUND: Afferent limb syndrome is a relatively rare cause of small bowel obstruction after restorative total proctocolectomy with ileal pouch-anal anastomosis for patients with ulcerative colitis or familial adenomatous polyposis. DISCUSSION: This report describes three patients who developed recurrent small bowel obstruction after ileal pouch-anal anastomosis. The bowel obstruction was caused by torsion of the ileum at the inlet of the ileal J-pouch, which was thought to be a variant of afferent limb syndrome. This variant of afferent limb syndrome is characterized by a flexible afferent

Anatomical, physiological, and luminal adaptations occurring in an ileoanal pouch: opportunities for improving outcomes.

BACKGROUND: The ileal pouch represents a unique adaptation of the ileum to serve the dual roles of nutrient absorption, fecal storage, and evacuation in a new luminal microenvironment. However, the adaptation process produces early histological inflammation and therefore clues in the development of pouchitis may lie in how the ileal pouch adapts. AIMS: This narrative review explored anatomical and luminal ileal pouch adaptations, with a goal of designing clinical strategies that can strengthen the resilience of the pouch mucosa. METHODS: A comprehensive PubMed search was conducted, and findi

Managing Pouchitis: Where do Antibiotics End, and Advanced Therapies Begin?

PURPOSE OF REVIEW: Pouchitis is a common complication in patients with ulcerative colitis who undergo total colectomy with ileal pouch anal anastomosis that presents a challenge to clinicians. In the article, we review the management of patients with pouchitis based on best available evidence. RECENT FINDINGS: Pouchitis can be practically classified into acute intermittent pouchitis (AIP), chronic antibiotic dependent pouchitis (CADP) and chronic antibiotic refractory pouchitis (CARP) based on the frequency of pouchitis episodes and response to antibiotics. Antibiotics are effective for treat

An ileo-anal pouch doing the twist (with video).

A 37-year-old female patient had a past history of proctocolectomy for Crohn's disease, with ileal J-pouch-anal anastomosis. She was admitted for acute obstructive symptoms. CT scan revealed a 180 twisted ileo-anal anastomosis without signs of severe ischemia (Fig. 1and video), prompting urgent endoscopic treatment. Lower endoscopy showed a complete spiral of the J-pouch which was easily untwisted and passed. Aspiration followed by placement of a multiperforated Faucher tube allowed immediate symptoms relief. The tube was withdrawn 2 days later and the patient was discharged on the same evenin

Understanding fluorescence time curves during ileal pouch-anal anastomosis with or without vascular ligation.

BACKGROUND: Intraoperative indocyanine green fluorescence angiography (ICG-FA) may be of added value during pouch surgery, in particular after vascular ligations as lengthening maneuver. The aim was to determine quantitative perfusion parameters within the efferent/afferent loop and explore the impact of vascular ligation. Perfusion parameters were also compared in patients with and without anastomotic leakage (AL). METHODS: All consenting patients that underwent FA-guided ileal pouch-anal anastomosis (IPAA) between July 2020 and December 2021 were included. After intravenous bolus injection

Cost-effectiveness of preventive aspirin use and intensive downstaging polypectomy in patients with familial adenomatous polyposis: A microsimulation modeling study.

OBJECTIVE: Although there is increasing evidence to suggest the cost-effectiveness of aspirin use to prevent colorectal cancer (CRC) in the general population, no study has assessed cost-effectiveness in patients with familial adenomatous polyposis (FAP), who are at high risk of developing CRC. We examined the cost-effectiveness of preventive use of low-dose aspirin in FAP patients who had undergone polypectomy in comparison with current treatment practice. DESIGN: We developed a microsimulation model that simulates a hypothetical cohort of the Japanese population with FAP for 40 years. Three

Diagnostic and Management Considerations for the IPAA With Crohn's Disease-Like Features.

BACKGROUND: Patients with ulcerative colitis often develop medically refractory colonic inflammation or colorectal neoplasia, and approximately 10% to 15% of patients require surgery. The most common surgical procedure is a restorative proctocolectomy with IPAA. Even if the preoperative diagnosis is ulcerative colitis, approximately 10% of patients can develop inflammatory pouch conditions resembling a Crohn's disease phenotype. OBJECTIVE: This study aimed to review the diagnostic approach, prognosis, and management of IPAA with Crohn's disease-like features. DATA SOURCES: The data sources i
