[Supraventricular paroxysmal reentry tachycardia. Empirical and guided therapy].

The empirical therapy of reentrant supraventricular tachycardias (A-V and junctional tachycardia) is based on a preliminary diagnosis through standard ECG to evaluate, whenever possible, the relationship between P wave and QRS. In order to distinguish atrial tachycardias from other types, we must employ vagal manoeuvres or drugs. Often we use methods of recording and stimulation such as Holter monitoring and transesophageal technique which can provide useful information about the electrophysiological mechanisms and therefore can better guide our choice of drugs. The decision of undertaking pha

Reappraising the role of class Ic antiarrhythmics in atrial fibrillation.

PURPOSE: The objective of the present systematic review was to compare the effectiveness and safety of class Ic agents for cardioversion of paroxysmal atrial fibrillation (AF), in patients with and without structural heart disease (SHD). METHODS: We focused on RCTs enrolling at least 50 adult patients with electrocardiogram-documented paroxysmal AF that compared either two pharmacological class Ic cardioversion agents (flecainide, propafenone), regardless of study design (parallel or crossover). We searched MEDLINE and the Cochrane Central Register of Controlled Trials. Initial search was per

[Pharmacology of modern anti-arrhythmia drugs in therapy of supraventricular tachycardia].

Prevention of recurrences of atrial fibrillation, slowing the ventricular rate during atrial fibrillation, and the acute management of atrioventricular junctional reentrant supraventricular tachycardia (paroxysmal supraventricular tachycardia) often require treatment with antiarrhythmic drugs. These drugs comprise a pharmacodynamically and pharmacokinetically heterogeneous group of agents whose individual properties determine correct use, contraindications and side effects. Stabilisation of sinus rhythm can be achieved with class IA and class IC sodium channel blocking drugs as well as with th

Agranulocytosis associated with aprindine and other antiarrhythmic drugs: an epidemiological approach.

The risk of agranulocytosis associated with anti-arrhythmic drugs has been assessed by studying previous drug exposure of all cases collected through a multicentre surveillance network in a defined geographical area during the period 1980-1988. One hundred and eighty-one patients with agranulocytosis (less than 500 granulocytes mm-3 at least in two different blood counts) were interviewed with a structured questionnaire. Eight cases attributable to anti-arrhythmic drugs were identified, all of them related to aprindine. Data on the consumption of several anti-arrhythmic drugs were identified,

Analysis of randomized controlled studies comparing ablation with antiarrhythmic drugs.

Catheter ablation plays a pivotal role in the management of atrial fibrillation (AF). Recent randomized controlled studies have shown that catheter ablation is superior to conventional antiarrhythmic medical treatment as first-line therapy in reducing symptoms, AF recurrences, and preventing progression of AF. If the examined antiarrhythmic medical regimens were given in therapeutic and adequate doses remains an open question. We performed a PubMed and Medline search for randomized trials comparing first-line antiarrhythmic drug therapy with catheter ablation of AF. Among the 442 patients rand

[Suicidal poisoning with antihypertensive drugs].

In the period from 1990 to 2002, 201 patients with suicidal antihypertensive drugs poisoning were treated, including 138 women and 63 men from 15 to 84 (mean 36) years old. The main causes of suicides were various kinds of depression (63%) as well as psychopathy and/or sociopathy (16%) and schizophrenia (10%). Twenty eight patients attempted repeatedly to commit suicide. Thirty six persons were poisoned by only antihypertensive drugs, in 165 remaining cases intoxications were mixed including antihypertensive and other different medications. beta-blockers (38.3%), calcium channel blockers (34.8

Electric cardioversion of atrial fibrillation resulting in pulmonary oedema in patient with apical hypertrophic cardiomyopathy.

The direct-current electric shock is considered to be safe treatment of arrhythmias and rarely leads to serious hemodynamic complications. A 62-year-old patient was admitted to the hospital due to a first symptomatic episode of atrial fibrillation. Patient was diagnosed with apical hypertrophic cardiomyopathy 20 years ago. Transoesophageal echocardiography was performed to exclude an atrial thrombus followed by electrical cardioversion with restoration of sinus rhythm. After 6 hours symptoms of pulmonary oedema developed. The patient's condition improved after furosemide administration. As the

[Abnormal liver function during therapy with amiodarone in patients with persistent atrial fibrillation].

The aim of this work was to analyze the prevalence of level function disorder in patients with atrial fibrillation taking constantly amiodarone. Two groups of patients were studied: in group 1 (n = 78) patients with persistent atrial fibrillation taking constantly amiodarone for rhythm control were included, group 2 (n = 67) consisted of patients with permanent atrial fibrillation taking propafenon, ethacizine and digoxin for rate control. In studied groups liver transaminase activity and data of liver echography after 90 +/- 8 days of treatment were estimated. In 51.58% patients of group 1 as

Pharmacologic therapy of atrial flutter.

Atrial flutter is a relatively rare but nonetheless important arrhythmia. Its mechanism and anatomy have been defined as right atrial macroreentry. It responds to treatment with a variety of antiarrhythmic agents but, in general, drug efficacy for acute termination is low. The addition of pacing to drug therapy markedly improves the success rate for restoration of sinus rhythm. Useful antiarrhythmic agents include amiodarone, sotalol, disopyramide, flecainide, and propafenone, but definitive efficacy studies have not been performed. The risk of provoking 1:1 AV conduction and a marked increase

Direct intrauterine fetal treatment of fetal tachyarrhythmia with severe hydrops fetalis by antiarrhythmic drugs.

In cases of non-immune hydrops fetalis caused by tachyarrhythmias, the transplacental passage of antiarrhythmic drugs may be hampered. When this is proven by fetal blood sampling in cases of tachyarrhythmia refractory to transplacental treatment, additional administration of antiarrhythmic drugs into the fetus is necessary and seems to improve the results. Although injections of antiarrhythmic agents in fetal ascites are also highly effective, intravascular administration by sonographic guidance is to be preferred. Then, simultaneous measurements of fetal and maternal drug levels are possible

[A case of incessant junctional tachycardia in a female patient with aneurysm of the interauricular septum].

A permanent supraventricular tachycardia (SVT) was diagnosed in a 54-year-old hypertensive but cardiologically asymptomatic female patient, admitted to a surgery department for biliary lithiasis and hepatic echinococcosis. Heart rate was about 130 bpm and ECGs showed negative P waves in leads I, II, III, aVF, and precordial leads V2 to V6, being the RP' interval longer than P'R interval. Pharmacological intervention during Holter monitoring (20 hours) was instituted: following i.v. propranolol (4 mg), heart rate progressively decreased (to 112 bpm), mainly due to an increase in SVT RP' interva

1:1 atrial flutter induced by flecainide, whilst the patient was at rest.

Class Ic antiarrhythmic agents flecainide and propafenone are amongst the drugs most frequently prescribed to control atrial arrhythmias, in particular atrial fibrillation (AF). Despite being cited in some guidelines as a warning when using 1c antiarrhythmic agents, atrial flutter (AFl) with 1:1 atrioventricular conduction is rare in adults, with only small series reported in the literature, mainly including patients having 1:1 AFl during physical activity, and often associated with a predisposing factor, namely a dual AV nodal conduction pathway. We describe here a rare case of 1:1 AFl induce

[Treatment of atrial fibrillation in every days practice].

Atrial fibrillation is the most common arrhythmia in the adult. During recent years the therapeutic strategy has markedly changed. Some of these changes can be summarized as follows: Basis therapy includes betablockers and - in patients with structural heart disease - ACE-inhibitors and AT(1)-Blockers respectively. Class 1C-antiarrhythmic agents (flecainide or propafenon) should be restricted to patients with no or minimal left ventricular impairment. Amiodaron is the drug of choice in patients refractory to class 1C-agents and in those with already reduced left ventricular function. The "pill

How to enhance acute outcome of electrical cardioversion by drug therapy: importance of immediate reinitiation of atrial fibrillation.

INTRODUCTION: In almost 20% of patients with persistent atrial fibrillation (AF), electrical cardioversion is unsuccessful because of shock failure (i.e., no single sinus beat) or immediate reinitiation of AF (IRAF; recurrence within 2 min). Relative prevalence of shock failure and IRAF are not well known because data on outcome of electrical cardioversion therapy mostly do not distinguish shock failure from IRAF. METHODS AND RESULTS: In this review, the role of pretreatment with antiarrhythmic drugs on prevention of shock failure and total outcome of the cardioversion procedure are investiga

Foetal supraventricular tachycardia treated with sotalol.

This retrospective study (1991-95) presents our experience with sotalol in the treatment of 14 foetuses with supraventricular tachycardia (SVT). SVT was diagnosed in a structurally normal heart at a gestational age of 24-35 (median 28) weeks. In eight foetuses, hydrops was evident at presentation. In all patients pharmacological conversion with digoxin was tried before sotalol treatment was started. Sotalol was given orally to the mothers in a dose of 80-160 mg x 2. Cardioversion was obtained in 10 foetuses. In seven of these patients re-entry tachycardia and in five pre-excitation could be do

Behçet's disease and cardiac arrhythmia.

Only few cases of cardiac conduction disturbances and arrhythmias have been reported in Behçet's disease. We recently observed the case of a 16-year-old woman with Behçet's disease in whom cardiac arrhythmia became the main clinical symptom. This observation and a review of the literature led us to the conclusion that arrhythmia could represent the clinical manifestation of an underlying myocarditis due to Behçet's disease and can be regarded as a feature of cardiac involvement of the disease.

A Rare Case of Deglutition-Induced Atrial Fibrillation Secondary to Esophageal Dilation.

Atrial fibrillation (AF) is the most common arrhythmia in the world. However, deglutition-induced tachyarrhythmias are exceptionally rare. Diagnosis relies on a documented history, Holter monitoring and echocardiograms. The mechanism underlying deglutition-induced tachycardia remains uncertain, with leading hypotheses suggesting mechanical left atrial stimulation after esophageal distention or activation of the vagus nerve due to increased intra-esophageal pressure. Lifestyle changes, medications (e.g., beta-blockers and antiarrhythmics), and radiofrequency catheter ablation are viable treatme
