The Journal of the American Academy of Psychoanalysis1991eng
Because personal affluence and an alloplastic adjustment tend to go together, psychiatric treatment encounters special problems in trying to help the very rich. These patients frequently show a strong sense of entitlement and a denial of any psychological problem. This creates special treatment problems. They feel entitled to the "very best," which includes the "best doctor." If their psychological problems cannot be magically erased they will switch psychiatrists until they find the "right one." Part of this is a tendency to externalize their emotional problems. Problems are thought to be cau

The Psychodynamic Life Narrative is a psychotherapeutic maneuver used during the first few sessions of a consultation with a patient in a crisis situation. By establishing a powerful bond between physician and patient, it offers the possibility of a rapid relief of dysphoric symptoms. In some situations, it may be useful in mobilizing a recalcitrant patient to accept psychotherapy or, in the case of the physically ill, to accept diagnostic procedures and treatments previously refused. It is most effective in patients whose general adaptation has been stable and whose psychological homeostasis

This study investigates the development of concepts of psychosis in the Jewish Hospital in Warsaw, within the context of social and historical processes to which the hospital was the subject and a broader scope of European concepts of psychosis. In the years 1898-1909, the first chief physician of the psychiatric ward, Adam Wizel, focused mainly on hysteria. The interest in psychoses was initiated by Maurycy Bornsztajn, who started to promote psychoanalytic ideas. The second decade of the functioning of the Jewish Hospital's psychiatric ward was marked by issues concerning the classification o

This chapter repositions ideals away from their role as defensive structures restraining aggressive and lustful drives (as traditionally viewed) toward their place in shaping creativity and love. We select and mold our particular ideals in providing meaning and in this manner help to create those selfobjects needed to resolve or soothe our needs. This creative process may include "reshaping" of the available object to represent the "idealized other." From this perspective, Kohut's view of idealization and the idealized parental imago will be considered, including my own notion of a one-and-a-h

Something that happened to one of the authors recently led them to re?ect upon what the analyst's falling ill may represent and the problems it may give rise to in the analytic relationship. Such an eventuality injects a massive dose of the analyst's personal reality into the analytic space, thereby allowing the patient a glimpse of images of vulnerability, frailty and loss, and mobilizing emotions, fantasies and defences in both the analyst and the patient. The authors' survey of the literature ranges between two different theoretical perspectives intrapsychic and inter-subjective that, in th

The author points out that all Freud's publications and lectures from the period 1894-1896 had the aim of establishing an aetiological theory of the neuroses and psychoses. Freud found that the clinical pictures covered by the theory had a sexual aetiology and were distinguishable from each other by specific mechanisms. Numerous references are presented to show that what the author calls Freud's early clinical theory had its roots in contemporary neuropathology and psychiatry, and the prevailing views in these disciplines are discussed. He was able to build on the work of predecessors such as

In this paper I suggest that the analysis of perversion necessarily involves the elaboration and analysis of a perverse transference-countertransference. Both analyst and analysand contribute to and participate in the perverse transference-countertransference which intersubjective construction is powerfully shaped by the perverse structure of the patient's unconscious internal object world. In the fragment of an analysis that is presented, I illustrate the way in which the analyst makes use of his experience in (of) the transference-countertransference in gaining understanding of the perverse

E-A.C. are avoided in our practice and literature. However, they often blur the distinction between transferential illusion and reality, more so in training analyses. E-A.C. excite the patient's curiosity, exhibitionism and 'primal scene' fantasies with their concomitant anxieties. Such fears lead patients to phobically avoid analysing these encounters, whose effects analysts may also dread. However, calling attention to the avoidance may promote significant analytic developments. Illusion on the psychoanalytic stage is a regressive force which brings out significant affects and internalized r

This paper explores the origins and development of the "contemplative psychotherapy" project in the United States, emerging around psychoanalyst Edward Podvoll and the intellectual environment at Naropa University during the 1970s and 1980s. It situates Podvoll's work within the broader dialogue between Buddhism and Western psychotherapy, highlighting how his contributions shed light on this dynamic and complex encounter. The paper begins by outlining Podvoll's psychiatric and psychoanalytic background, then examines the establishment of contemplative psychotherapy at Naropa, and finally discu

Generalized, persistent, and free-floating anxiety was first described by Freud in 1894. The diagnostic term generalized anxiety disorder (GAD) was not in classification systems until the publication of the diagnostic and statistical manual for mental disorders, third edition (DSM-III) in 1980. Initially considered as a residual category to be used when no other diagnosis could be made, it is not accepted that GAD represents a distinct diagnostic category yet. Since 1980, revisions to the diagnostic criteria for GAD in the DSM-III-R, DSM-IV and DSM-5 classifications have slightly redefined thi

This paper, originally published in 1980, is a significant contribution to the study of psychological trauma and its treatment; particularly countertransference aspects of that treatment. Initially it was widely cited but then became little known, largely because of the inaccessibility of the original publication. The three major concepts in Symond's approach are: (1) self-hate and shame are the key dynamics in post traumatic distress; (2) ordinary professional attitudes of those who are supposed to help often intensify the traumatized person's self-hate and shame. Martin Symonds called this i

Freud's monograph on the analysis of Little Hans is examined from a perspective aimed at highlighting elements of current thinking that would be considered mutative from those originally emphasized at the time it was written, and with a specific focus on the relative importance of verbal versus nonverbal interventions.

The dynamic and developmental issues confronted by a generation of German psychoanalysts who were born between 1930 and 1945 are studied through a questionnaire and follow-up interviews. These individuals grew up in a society that, due to the collective guilt of its adults, either superimposed general self-idealization or self-acquittal with respect to those who were identified as perpetrators of this trauma, or would not address or process the trauma, at all. In addition, these psychoanalysts, in their responses, perceived their own training analysts, part of the older generation, to have unc

The author traces the history of free association, the "fundamental rule," through the Freud-Ferenczi relationship and controversy. The use of "activity," first proposed by Freud in 1910 with phobic and compulsive patients, was then championed by Ferenczi in the early twenties. The goal of activity was to enhance-or, more accurately, "to force"--the associations into the analysis. Subsequently, Ferenczi reversed himself, concluding that his analysis was re-creating the traumatic parental environment which originally caused the patient's neurosis. The far-reaching results of Ferenczi's change o

This paper deals with clinical material gathered from the long-term, psychoanalytically-oriented treatment of a patient with a shoe fetish. Genetic and dynamic aspects of this problem are emphasized. The combination of a dominating mother and a passive and absent father, the sharing of a bed with an older sister until age eleven, and the receipt of frequent enemas contributed directly to the development of the fetish, the most crucial determinant being the commulative traumatic effect of the enemas. During adolescence, the patient also resorted to transvestism to deal with the intensification

This article offers a critical reworking of ecological and social-ecological models as applied to intellectual and developmental disabilities, autism, and severe mental illness. Drawing on a qualitative synthesis of 18 studies, we show that ecological frameworks are predominantly used as descriptive mapping devices, that power and inequalities enter analyses in partial and fragmented ways, and that intersystemic connections often generate formally inclusive yet experientially exclusionary arrangements. To address these limitations, we propose an Integrated Ecological Model for Mental Disabilit

Neuropsychopharmacologia Hungarica : a Magyar Pszichofarmakologiai Egyesulet lapja = official journal of the Hungarian Association of Psychopharmacology2007eng
About 50 years of demolition work, it's time now for a return to the grand syntheses. Two of the great syntheses of the 19th century have now been shattered. Marxism lies in fragments. And psychoanalysis has largely drifted outside of psychiatry to find a new and doubtless temporary home in departments of literary studies. To be sure, the third of the great syntheses, Darwin's theory of evolution, remains intact. But otherwise, as far as the eye can see, there is rubble. The time for new attempts at synthesis is now nigh. After decades of pioneering work in the neurosciences, the fundamental i
