The authors report a case of xanthogranulomatous pyelonephritis presenting with nephrocutaneous fistula. This case illustrates all of the typical features of this disease: late diagnosis, non-functioning affected kidney, staghorn calculi, urinary tract anomaly, perinephritis with fistulization. The authors review the diagnostic and therapeutic modalities of xanthogranulomatous pyelonephritis and discuss the other aetiologies of nephrocutaneous fistula. OBJECTIVES: Simple nephrectomy is performed for a benign pathology that does not require the excision of either the adrenal gland or any adenopathies. When it is carried out in cases of stone disease, however, it is frequently not a "simple" technique owing to the presence of significant inflammation and infection.
METHODS: Ninety-six simple laparoscopic nephrectomies performed because of stone disease between 2006 and 2015 were retrospectively studied. A descriptive statistical analysis was performed, as well as an evaluation of the associated complications.

Replacement lipomatosis is an abnormal fatty proliferation of the renal sinus, hilus, perirenal and, sometimes, periureteral spaces. Our experience with 18 cases of replacement lipomatosis is reported: 6 patients had the massive form, one of them bilateral, 5 patients had associated xanthogranulomatous pyelonephritis and 7 had an initial focal form. A case of longstanding chronic pyelonephritis associated with severe recurrent nephrolithiasis followed by excessive lipomatosis, ureteritis cystica and xanthogranulomatous pyelonephritis is presented. In addition to the definition and morphologic description of lipomatosis its differential diagnosis is discussed. This also applies to the pathogenesis of these diseases. The special role of lipomatosis in the perpetuation of a circulus vitiosus is stressed. In special cases it seems to be justified to speak of clinical malignant lipomatosis.

We present a case of nephrobronchial fistula in a middle age immunocompetent woman who complained cough and weight loss, with underlying asymptomatic nephrolithiasis. Xanthogranulomatous pyelonephritis (XGP) is a rare form of chronic pyelonephritis. With the review of the literature, we reported two cases of XPN with psoas muscle abscesses caused by Staphylococci aereus in one of the patient and Serratia mascerentes in the other. Both of the patients had renal calculus. We performed nephrectomy with psoas abscess drainage and started appropriate antibiotics, but one of the patients died of septic shock. Other patient is free of symptoms at the end of 5 years follow-up.

Five different benign causes of ureteropelvic junction obstruction in adults without prior obstructive history are presented: aortic aneurysm, renal cyst, xanthogranulomatous pyelonephritis, eosinophilic ureteritis, and a crossing blood vessel. Although uncommon, these etiologies warrant consideration when an adult patient presents with ureteropelvic junction obstruction without a prior history of obstruction.
