Elastic, viscous, and mass load effects on poststroke muscle recruitment and co-contraction during reaching: a pilot study.

BACKGROUND: Resistive exercise after stroke can improve strength (force-generating capacity) without increasing spasticity (velocity-dependent hypertonicity). However, the effect of resistive load type on muscle activation and co-contraction after stroke is not clear. OBJECTIVE: The purpose of this study was to determine the effect of load type (elastic, viscous, or mass) on muscle activation and co-contraction during resisted forward reaching in the paretic and nonparetic arms after stroke. DESIGN: This investigation was a single-session, mixed repeated-measures pilot study. METHODS: Twent

Electromyography in craniomaxillofacial trauma.

BACKGROUND AND PURPOSE: Electromyography is used in conjunction with clinical diagnosis to determine the presence and extent of craniofacial injuries; it is also an aid in prognosis of recovery. This article reviews the neural pathophysiology following trauma and the basic principles of electrodiagnostic testing; such understanding helps to determine indications for electrodiagnostic testing. METHODS AND MATERIALS: Electrodiagnostic techniques can detect and differentiate the degree of injury. There are three major categories of nerve injury--neurapraxia, axonotmesis, and neurotmesis. In neur

Post-section recruitment of epileptiform discharges in electrocorticography during callosotomy in 48 patients with Lennox-Gastaut syndrome.

We aimed to clarify the relationships between post-sectional recruitment of epileptiform discharges (PRED) seen in electrocorticograms (ECoG) during callosotomy in patients with Lennox-Gastaut syndrome (LGS) and postoperative seizure outcome. We retrospectively analyzed ECoG obtained during subtotal callosotomy (6 cm) in 48 patients with LGS (32 males, 16 females; age range, 1-20 years; mean age, 7.6 years) from July 1993 to November 1996 and compared recorded findings with postoperative seizure outcome. At analysis, all patients had been followed postoperatively for more than 10 years. Of 48

Graph-Theoretical Signature from Neural and Vascular Signals Reveals Spinal Cord Stimulation Frequency-Specific Brain Network in Disorders of Consciousness Patients.

Introdution: Spinal cord stimulation (SCS) has emerged as a promising neuromodulatory intervention for patients with disorders of consciousness (DoC). However, the identification of optimal stimulation frequencies remains a subject of ongoing debate. Although previous electroencephalography (EEG) and functional near-infrared spectroscopy (fNIRS) studies have suggested the therapeutic efficacy of 5- and 70-Hz, respectively, the integrative neurovascular mechanisms and frequency-specific network dynamics underlying these effects remain to be elucidated. Objective and Impact Statement: This study

Analysis of the neural mechanisms of social anxiety based on EEG features and machine learning and construction of a diagnostic model.

Social anxiety is a common psychological problem, and its accurate diagnosis and investigation of underlying neurophysiological mechanisms are of significant importance. This study aims to explore the neuroelectrophysiological characteristics and diagnostic value of social anxiety by integrating event-related potentials (ERP), event-related spectral perturbation (ERSP), and machine learning methods. A total of 128 participants with varying levels of social anxiety completed an emotional face Go/No-Go task, during which electroencephalographic (EEG) data were collected for ERP and time-frequenc

Beyond snapshots: A dynamic framework for continuous motor unit tracking to enhance neurophysiological assessments.

Tracking individual motor units (MUs) across multiple testing visits is critical for understanding neuromuscular adaptations and disease progression. However, existing methods fail to account reliably for spatiotemporal variations in motor unit action potentials (MUAPs) and lack validation for longitudinal tracking. Here, we present a robust computational framework for continuous MU tracking that integrates automatic estimation of electrode grid displacements and compensation for MUAP shape variations. Our algorithm demonstrated superior tracking accuracy, capturing 63% and 75% of MUs in the t

Map-based spatial perspective taking reveals frontal late negativity loss and posterior delta gain in mild cognitive impairment.

Spatial disorientation represents a clinically meaningful vulnerability during aging and is an early manifestation along the Alzheimer's disease continuum. Understanding how aging-related central nervous system (CNS) changes affect the neural computations required for spatial perspective taking (SPT) is essential for characterizing early neurodegenerative processes. In this study, older adults with mild cognitive impairment (MCI) and subjective cognitive decline (SCD) completed a simplified map-based SPT paradigm while electroencephalography (EEG) was recorded. Although behavioral accuracy was

Peripheral and central neuroinflammatory mechanisms regulating vagal sensory circuit processing and cough in lung disease.

BACKGROUND: The pulmonary system can be exposed to a diverse range of injurious insults which drive pathological events including inflammation, tissue damage and structural remodelling within the airways and lungs. SUMMARY: Patients with lung inflammation and injury often experience sensory neurological complications, suggesting that the pathological processes in the lung either impact or extend into the nervous system. KEY MESSAGES: Cough is a common presenting neurological symptom in patients with lung disease and there is growing evidence that neuroinflammation, either within the vagal ne

Paediatric electromyography in the modern world: a personal view.

Paediatric electromyography (EMG) is an invaluable diagnostic test for the investigation of neuromuscular disease, but its use is inconsistent between and within different countries. One perception is that the procedure is painful; however, in comparison with common investigations performed routinely in children, EMG is better tolerated. While some developments, such as those within clinical genetics, would appear to mark its demise, paradoxically the more genetic abnormalities that are discovered in conditions such as hereditary neuropathy, the more precise a delineation of the phenotype is r

The effects of prior cognitive control task exposure on responses to emotional tasks in healthy participants.

BACKGROUND: Recent advances have been made in the application of cognitive training strategies as interventions for mental disorders. One novel approach, cognitive control training (CCT), uses computer-based exercises to chronically increase prefrontal cortex recruitment. Activation of prefrontal control mechanisms have specifically been identified with attenuation of emotional responses. However, it is unclear whether recruitment of prefrontal resources alone is operative in this regard, or whether prefrontal control is important only in the role of explicit emotion regulation. This study exa

Task-specific recruitment of dorsal and ventral visual areas during tactile perception.

Many studies have found that visual cortical areas are active during tactile perception. Here we used positron emission tomographic (PET) scanning in normally sighted humans to show that extrastriate cortical regions are recruited in a task-specific manner during perceptual processing of tactile stimuli varying in two dimensions. Mental rotation of tactile Forms activated a focus around the anterior part of the left intraparietal sulcus. Since prior studies have reported activity nearby during mental rotation of visual stimuli, this focus appears to be associated with the dorsal visual (visuos

Recruitment of upper-limb motoneurons with epidural electrical stimulation of the cervical spinal cord.

Epidural electrical stimulation (EES) of lumbosacral sensorimotor circuits improves leg motor control in animals and humans with spinal cord injury (SCI). Upper-limb motor control involves similar circuits, located in the cervical spinal cord, suggesting that EES could also improve arm and hand movements after quadriplegia. However, the ability of cervical EES to selectively modulate specific upper-limb motor nuclei remains unclear. Here, we combined a computational model of the cervical spinal cord with experiments in macaque monkeys to explore the mechanisms of upper-limb motoneuron recruitm

Visual Evoked Potentials in Neuromyelitis Optica Spectrum Disorders.

BACKGROUND: Neuromyelitis optica spectrum disorders (NMOSDs) are rare antibody-mediated disorders of the central nervous system, with a predilection for the spinal cord and optic nerves. The clinical utility of evoked potential recordings (EPs) has already been established for multiple sclerosis, in particular, that of the abnormal visual evoked potentials (VEP), a key criterion in the McDonald diagnostic criteria for MS. However, there have been few reports on EPs in patients with NMOSD. AIM: The aim of our study was to assess the possible involvement of the optical pathway through VEP respo

Feasibility, safety, and preliminary outcomes of a school-based intervention combining paced-breathing HRV biofeedback and transcutaneous auricular vagus nerve stimulation in adolescents.

The vagus nerve is a major neurophysiological pathway linking the brain and body, and schools provide a practical setting for delivering stress-regulation programs. This uncontrolled pilot study assessed the feasibility, safety, acceptability, and preliminary outcomes of a school-based intervention combining paced-breathing heart rate variability (HRV) biofeedback with transcutaneous auricular vagus nerve stimulation (taVNS). Seventeen vocational education and training students aged 15-17 years enrolled in six weekly individual sessions of up to 30 min using emWave® biofeedback and Parasym™ ta

Acupuncture for chronic tinnitus: study protocol for a clinical trial with auditory brainstem response outcomes.

BACKGROUND: Chronic tinnitus is a common clinical condition with no universally accepted effective treatment option. Increasing evidence suggests that dysfunction within the auditory pathway may contribute to tinnitus pathophysiology, and auditory brainstem response (ABR) has been proposed as a potential objective measure of auditory neurophysiological function. Acupuncture is widely used for tinnitus management; however, the relationship between acupuncture-associated symptom changes and objective electrophysiological alterations remains unclear. OBJECTIVE: This study aims to investigate dif

Targeted Interventions in Tourette's using Advanced Neuroimaging and Stimulation (TITANS): study protocol for a double-blind, randomised controlled trial of transcranial magnetic stimulation (TMS) to the supplementary motor area in children with Tourette's syndrome.

INTRODUCTION: Tourette's syndrome (TS) affects approximately 1% of children. This study will determine the efficacy and safety of paired comprehensive behavioural intervention for tics (CBIT) plus repetitive transcranial magnetic stimulation (rTMS) treatment in children with Tourette's syndrome. We hypothesise that CBIT and active rTMS to the supplementary motor area (SMA) will (1) decrease tic severity, and (2) be associated with changes indicative of enhanced neuroplasticity (eg, changes in in vivo metabolite concentrations and TMS neurophysiology measures). METHODS AND ANALYSIS: This study

Pain-motor integration in chronic pain: A neurophysiological study.

OBJECTIVE: Chronic pain may lead to functional changes in several brain regions, including the primary motor cortex (M1). Our neurophysiological study aimed to probe M1 plasticity, through a non-invasive transcranial magnetic stimulation protocol, in a cohort of patients with chronic pain. METHODS: Twenty patients with chronic pain (age ± SD: 62.9 ± 9.9) and 20 age- and sex-matched healthy controls (age ± SD: 59.6 ± 15.8) were recruited. Standardized scales were used for the evaluation of pain severity. Neurophysiological measures included laser-evoked potentials (LEPs) and motor-evoked poten

Freezing of gait and the effects of vibrotactile cueing in Parkinson's disease-a study protocol of a cross-sectional multimodal brain imaging approach in virtual walking scenarios.

BACKGROUND: Freezing of gait (FOG) is one of the most debilitating motor symptoms in Parkinson's disease (PD), affecting a substantial proportion of people with PD. Numerous hypothetical mechanisms exist, attempting to explain FOG. Current evidence originated from stationary measures in lying positions, limiting the understanding of neurophysiological changes elicited by FOG episodes during regular locomotion. Neurophysiological aspects of FOG, especially in response to vibrotactile cueing, have been explored only by a few studies. To address this gap, the current study aims to investigate FOG

Corticospinal control of respiratory muscles in chronic obstructive pulmonary disease.

Patients with chronic obstructive pulmonary disease (COPD) face an increased respiratory load and in consequence have an elevated respiratory drive. We used transcranial magnetic stimulation (TMS) to investigate associated changes in corticospinal excitability both at rest and during voluntary facilitation at different levels of inspiratory effort. Diaphragm and abdominal motor thresholds were significantly lower in COPD than healthy controls, but the quadriceps response was the same. In patients there was a significant increase in diaphragm response from rest during 20% inspiratory efforts bu

The cricothyroid muscle does not influence vocal fold position in laryngeal paralysis.

The status of the cricothyroid muscle, which is innervated by the superior laryngeal nerve, is believed to influence the vocal fold position in laryngeal paralysis. It is believed that isolated lesions of the recurrent laryngeal nerve generally result in the paralyzed vocal fold assuming a paramedian position but that with lesions of both the superior and recurrent laryngeal nerves, a more lateral (intermediate or cadaveric) vocal fold position can be expected. Twenty-six consecutive patients with unilateral vocal fold paralysis underwent transnasal fiberoptic laryngoscopy (TFL) and laryngeal
