Current opinion in clinical nutrition and metabolic care2025eng
PURPOSE OF REVIEW: To review the impact of nutrition on critically ill older adults, focusing on the role of comorbidities such as malnutrition, sarcopenia, and frailty, as well as the importance of nutritional support for outcomes such as mortality, recovery, and long-term disability.
RECENT FINDINGS: Malnutrition is highly prevalent in the elderly and is strongly associated with adverse outcomes, including increased mortality, longer ICU stays, and higher rates of infection. In addition, frailty, multimorbidity, and cognitive decline significantly contribute to the vulnerability of older pa

KEY CLINICAL MESSAGE: Gastric outlet obstruction can be a dangerous complication of intragastric balloons, as it can result in severe metabolic alkalosis. As weight loss procedures and medical tourism become more popular, physicians should have a high index of suspicion for complications of invasive procedures, particularly in returning travelers.
ABSTRACT: Intragastric balloons for weight loss have decreased in frequency in the United States. However, they are still frequent in low- and middle-income countries. Severe complications occur in less than 3% of patients who undergo this procedure

Anorexia nervosa (AN) has a high mortality rate due to the widespread organ dysfunction caused by the underlying severe malnutrition. Malnutrition-induced hepatitis is common among individuals with AN especially as body mass index decreases, while acute liver failure and aplastic crisis related to coagulation disease and encephalopathy rarely occur in AN patients. The supervised increase of caloric intake can quickly improve the elevated aminotransferases caused by starvation and aplastic crisis. This current case report describes a 12-year-old adolescent girl who was admitted with a 3-month h

Anorexia Nervosa (AN) has always been prevalent in adolescents. During the COVID-19 pandemic, the eating disorder community across the world has noted a sharp increase in adolescent patients with AN and the severity of medical compromise. Rarely seen sequelae have become increasingly more common. This case report describes two previously healthy female adolescents with severe malnutrition due to AN admitted for nutritional stabilization who exhibited paranoid delusions responsive to thiamine and olanzapine. Though neither exhibit the full triad or all the Caine criteria for Wernicke's Encephal

This article presents current diagnostic conceptualisations of eating disorders, including new disorders such as binge eating disorder (BED) and avoidant/restrictive food intake disorder (ARFID). This is followed by contemporary findings in the epidemiology of eating disorders, their broad sociodemographic distribution and the increases in community prevalence. Advances and the current status of evidence-based treatment and outcomes for the main eating disorders, anorexia nervosa, bulimia nervosa and BED are discussed with focus on first-line psychological therapies. Deficits in knowledge and

Malnutrition is one of the main factors determining cachexia syndrome, which negatively impacts the quality of life and survival. In cancer patients, artificial nutrition is considered as an appropriate therapy when the impossibility of an adequate oral intake worsened nutritional and clinical conditions. This study aims to verify, in a home palliative care setting for cancer patients, if home artificial nutrition (HAN) supplies a patient's energy requirement, improving nutritional and performance status. A nutritional service team performed counseling at a patient's home and assessed nutritio

OBJECTIVE: To describe how primary care clinicians can detect an eating disorder and identify and manage the associated medical complications.
DESIGN: A review of literature from 1994 to 1999 identified by a MEDLINE search on epidemiology, diagnosis, and therapy of eating disorders, including anorexia nervosa and bulimia nervosa.
MEASUREMENTS AND MAIN RESULTS: Detection requires awareness of risk factors for, and symptoms and signs of, anorexia nervosa (e.g., participation in activities valuing thinness, family history of an eating disorder, amenorrhea, lanugo hair) and bulimia nervosa (e.g.

Diffuse reticulate purpura (DRP) is a rare, visually striking cutaneous manifestation of severe malnutrition and has been reported in a handful of adolescents with anorexia nervosa. We describe a case of DRP in a 13-year-old female with anorexia nervosa, marking one of the few documented cases in a female patient. She presented with a net-like, violaceous purpuric rash across her trunk and back following several months of significant weight loss, driven by restrictive eating and excessive exercise. Her examination otherwise revealed marked cachexia and bradycardia, without signs of trauma. Lab

BACKGROUND: Severe anorexia nervosa can result in life-threatening systemic complications. However, arterial rupture in the context of anorexia nervosa has not been reported.
CASE REPORT: A 31-year-old woman who had developed anorexia nervosa in her early teens and had extreme malnutrition with a body mass index of 8.5 kg/m2 was admitted to our hospital with impaired consciousness due to hypoglycemia. During the management of hypoglycemia and refeeding syndrome, the patient developed hematochezia and shock. Contrast-enhanced computed tomography revealed rupture of the right external iliac and

BACKGROUND: Enteral nutrition (EN) is an effective nutritional intervention for patients at risk of malnutrition or malnourished. However, complications such as gastrointestinal intolerance, hyperglycemia or refeeding syndrome can be triggered by EN.
AIM: To investigate the effects of a tube feeding formula (TFF) on patients' nutritional status, biochemical status, bowel habits and safety.
METHODOLOGY: Observational, prospective and multicenter study. Patients ≥ 18 years, undernourished or at nutritional risk, who were prescribed a high-calorie, high-protein, fiber-fortified TFF were include

Advanced HIV infection leads to profound immunosuppression and increased susceptibility to opportunistic infections. Resumption of antiretroviral therapy (ART) may result in immune reconstitution inflammatory syndrome (IRIS), which manifests as a worsening of an existing infection or the emergence of new ones. Disseminated mycobacteriosis caused by Mycobacterium avium complex (MAC) is one of the most severe opportunistic infections in the course of AIDS. The aim of this paper is to present the diagnostic and therapeutic difficulties in a AIDS patient who developed IRIS with generalized MAC inf

Anorexia nervosa (AN) is a severe, life-threatening psychiatric disorder often accompanied by significant physical health complications, requiring specialized treatment that is challenging to provide in a general medical setting. This case report presents the complex challenges encountered in managing a 49-year-old woman with severe AN of the purging subtype in a general medical ward setting. Admitted with a life-threatening low body mass index (BMI) of 7.9 and critical physical health issues, her case underscores the intricate relationship between the psychiatric and physical health aspects o

A 70-year-old woman presented dysphagia and postprandial vomiting, and weight loss of about 15 kg in one year. She was markedly emaciated with a body mass index of 12.4 kg/m2 and had difficulty in movement. Esophagogastroduodenoscopy and computed tomography revealed stenosis of the esophagogastric junction (EGJ) with no malignant findings. Additionally, based on the findings of the esophagogram and high-resolution manometry, the patient was diagnosed with esophageal achalasia. The patient also had an elevation in liver enzymes but was ruled out alcoholic, drug-induced, viral, or other hepatiti

Family-based therapy is the main evidence-based outpatient treatment for children and adolescents with eating disorders. In this context, nutritional education and family guidance can be carried out using different strategies, including calorie counting, exchange lists, and visual models and resources such as the Plate-by-Plate Approach®. The objectives of this study were to develop a visual resource adapted to the Mediterranean diet pattern and the eating and gastronomic habits of Spain, and to evaluate the energy and macro- and micronutrient contribution, as well as the methodology's nutriti

The nutrition of preterm infants remains contaminated by wrong beliefs that reflect inexactitudes and perpetuate old practices. In this narrative review, we report current evidence in preterm neonates and in preterm neonates undergoing surgery. Convictions that necrotizing enterocolitis is reduced by the delay in introducing enteral feeding, a slow advancement in enteral feeds, and the systematic control of residual gastric volumes, should be abandoned. On the contrary, these practices prolong the time to reach full enteral feeding. The length of parenteral nutrition should be as short as poss

Abnormalities of hematological and biochemical parameters are various and frequent during anorexia nervosa, and are mainly related to malnutrition, weight loss, and compensatory purgative behaviors. They are most often moderate and reversible through appropriate nutritional and weight rehabilitation, as well as well-conducted symptomatic treatment. Severe abnormalities are rarer, but are potentially serious or even fatal. Isolated moderate anemia and leukopenia are frequently noted, with thrombocytopenia being less frequent. Severe, bi-cytopenic, pancytopenic, and spinal cord injury are less c

Mosquitoes are one of the most important vectors of human disease. The ability of mosquitoes to transmit disease is dependent on the age structure of the population, as mosquitoes must survive long enough for the parasites to complete their development and infect another human. Age could have additional effects due to mortality rates and vector competence changing as mosquitoes senesce, but these are comparatively poorly understood. We have investigated these factors using the mosquito Aedes aegypti and the filarial nematode Brugia malayi. Rather than observing any effects of immune senescence

BACKGROUND/OBJECTIVE: Thyrotoxic periodic paralysis (TPP) is a rare condition causing weakness of the lower extremities associated with significant hypokalemia. Likewise, agranulocytosis due to methimazole use is a rare occurrence. We present the first documented case of concomitant TPP and agranulocytosis from methimazole use.
CASE REPORT: A 48-year-old woman presented with sore throat, fevers, odynophagia, and sudden-onset bilateral leg weakness. Methimazole had been started 10 weeks prior for a new diagnosis of Graves' disease. On admission, the patient was febrile, tachycardic, thyrotoxic
