The initial resuscitation of septic shock.

Septic shock is the most severe form of sepsis, characterized by (a) persistent hypotension despite fluid resuscitation and (b) the presence of tissue hypoperfusion. Delays in the diagnosis and initiation of treatment of septic shock is associated with increasing risk for mortality. Early and effective fluid resuscitation and vasopressor administration play a crucial role in maintaining tissue perfusion in septic shock patients. A low diastolic arterial pressure (DAP) correlates with severity of arteriolar vasodilation, compromises left ventricle oxygen supply and can be used for identifying s

The ABCDs of ResUS - Resuscitation Ultrasound.

Point-of-care ultrasound is ubiquitous in the hand of the clinician in many specialties, especially in the evaluation of the patient who is in shock or cardiac arrest. Two of the more common protocols are ACES (Abdominal and Cardiothoracic Evaluation with Sonography) and RUSH (Rapid Ultrasound for Shock and Hypotension). The purpose of this editorial is to introduce a new protocol that builds on many of the previous ones but is more comprehensive and easier to use. The ABCDs of ResUS - Resuscitation Ultrasound - is a systematic head to toe approach to facilitate the workup of the unstable pati

Challenges and possibilities in forward resuscitation.

The environmental and logistical constraints of the prehospital setting make it a challenging place for the treatment of trauma patients. This is perhaps more pronounced in the management of battlefield casualties before extraction to definitive care. In seeking solutions, interest has been renewed in implementing damage control resuscitation principles in the prehospital setting, a concept termed remote damage control resuscitation. These developments, while improving conflict survival rates, are not exclusive to the military environment, with similar situations existing in the civilian setti

Resuscitation at birth in neonates with meningomyelocele.

AIMS: Compare the need for neonatal resuscitation procedures between newborn infants with and without meningomyelocele (MMC). RESULTS: This retrospective case-control study included 94 neonates with MMC, defined as open spinal dysraphism with exposure of nervous tissue, and 94 controls without malformations, paired with MMC infants by gender, mode of delivery, gestational age and time of birth. Infants were born at a university hospital in São Paulo, Brazil, from 2001 to 2010. After adjusting for perinatal variables (prenatal care, maternal hypertension, birth during the day shift, cephalic p

Fluid resuscitation in the trauma patient.

Trauma is responsible for 25% of all emergency department visits causing a healthcare burden of more than $400 billion a year. Timely and appropriate fluid resuscitation may mean the difference between survival and death. Controversy surrounds the best method of fluid resuscitation for optimum recovery. This article will evaluate current research regarding the various types of fluid resuscitation in the trauma population. Colloid and crystalloid fluid will be compared describing the risks and benefits of each.

Cardiopulmonary resuscitation with vasopressin in a dog.

That endogenous vasopressin levels in successfully resuscitated human patients were significantly higher than in patients who died pointed to the possible benefit of administering vasopressin during cardiopulmonary resuscitation (CPR). Several CPR studies in pigs showed that vasopressin improved blood flow to vital organs, cerebral oxygen delivery, resuscitability and neurological outcome when compared with epinephrine. In a small clinical study, vasopressin significantly improved short-term survival when compared with epinephrine indicating its potential as an alternative pressor to epinephri

The role of oxygen in neonatal resuscitation.

New knowledge has accumulated in recent years making it prudent to ask questions regarding current oxygenation policies and guidelines. Because new-born resuscitation affects so many individuals, and because resuscitation procedures may have dramatic consequences on infant and child health, intensified discussion and research in this field are not only necessary but are a requirement. In particular, there is a lack of data on infants born before term. It is difficult to give absolute recommendations on which oxygen concentration should be applied for newborn resuscitation; however, it seems th

Resurrection of steroids for sepsis resuscitation.

Corticosteroids were proposed to treat patients with severe sepsis as early as 1940. A summary of all available randomized controlled trials performed between 1966 and 1993 was provided in two systematic review that recommended to abandon the use of high dose coricosteroids to treat patients with severe infection. Nonetheless, a doubt still persist regarding the efficacy of a strategy of replacement therapy in cathecolamines-dependent shock. This strategy relies mainly on the concept that septic shock may be complicated by 1) an occult adrenal insufficiency, 2) a glucocorticoid peripheral resi

Role of naloxone in newborn resuscitation.

OBJECTIVE: Because of questions about the basis for the use of naloxone in resuscitation of the newborn, we wished to evaluate the use of naloxone at our institution and an affiliated hospital. METHODOLOGY: Evaluation of the actual use of naloxone at a university hospital and a community hospital: we document naloxone use by daily survey for a month in one; in the other, we perform a retrospective record review of 1 year's use. RESULTS: The university hospital had 240 births during February, 1998. Naloxone was given twice: once, 7 minutes before delivery to a woman at term who had received o

An argument for colloid resuscitation for shock.

A focused review of the physiologic mechanisms of colloid and crystalloid fluid resuscitations for acute critical illness is presented. This review suggests that postresuscitation plasma volume, cardiac output, left ventricular mechanical performance, and global and microcirculatory O2 supplies are more favorable with colloid therapy. Conversely, crystalloid may adversely affect microcirculatory blood flow and resultant O2 supply and use by ischemic tissues in shock. Poor relief of global and regional hypoxia may persist in critically ill patients after resuscitation with crystalloid.

Cardiopulmonary resuscitation of the newborn. An update.

The abrupt transition from intrauterine to extrauterine life represents a series of profound physiologic changes. This process puts the baby at risk for asphyxia. At birth, the newborn is, therefore, more frequently in need of resuscitation than at any other age. This article reviews the rationale for the sequence and process of neonatal resuscitation, emphasizing recent changes in recommendations.

[Drug evaluation in neuroanesthesia and resuscitation].

The evaluation of a drug used in neuroanesthesia is based upon controlled trials. The main principles of such trials are not different from trials of other drugs but some characteristics should be emphasized. The first phase of the development of a drug is an evaluation in healthy volunteers of its tolerance and kinetics, which must combine rapid and short action without residual effect. The second phase is a study of the tolerance and kinetics but also of its efficacy because only patients are involved. Action upon cerebral circulation is of great importance. During the third phase, trials ha

[Anesthesia and resuscitation in surgery of pheochromocytoma].

Anesthetic technique in surgery for pheochromocytoma presents well recognized difficulties. Major complications (hypertensive crises, rhythm disturbances, collapse, pulmonary edema and hypoglycemia) can often be avoided by a good preoperative examination and full per-operative monitoring. The choice of drugs during anesthesia and per-operative resuscitation are discussed in this article together with particular situations such as pheochromocytoma in pregnancy or the per-operative discovery of a previously unrecognized pheochromocytoma.

[Therapeutic recommendations for the resuscitation of children].

The general management and steps of cardiopulmonary cerebral resuscitation are the same for infants, children and adults: Airway management; Breathing, Cardiac compressions and Drugs to restart circulation and maintain cerebral and myocardial oxygen supply. However, priorities and techniques differ somewhat because of variations in size, physiology and cause of circulatory arrest. In pharmacological support there are several new aspects: The immediate correction of acidosis is not necessary in CPR and iatrogenic alkalosis has deleterious effects. Beta-receptor stimulators and calcium should no
