[Statistical study of aqueous flare in rhegmatogenous retinal detachment].

Inflammation of the anterior chamber in rhegmatogenous retinal detachment was studied in 338 eyes with a laser flare cell meter. The mean flare value, 32.84 +/- 81.39 pc/msec, was significantly higher (p < 0.0001) than the 5.01 +/- 2.05 pc/msec of the control group of 133 normal eyes. Clinical findings of retinal detachment were grouped into 11 factors and each factor was divided into 34 details. The following details were proven by t-test to cause significant elevation of the flare value: (1) duration of more than 3 months, (2) hypotony of less than 9 mmHg, (3) aphakia, (4) retinal breaks in

Enhanced visualization of vitreoretinal interface by laser biomicroscopy.

BACKGROUND: The use of slit-lamp biomicroscopy provides valuable information for the diagnosis and management of vitreoretinal disorders. However, intense backscatter from the fundus often precludes the visualization of fine structures in the vitreoretinal interface. METHODS: Laser biomicroscopy is a new method designed to improve the visualization of fine vitreoretinal structures at the macula. This method was applied to eyes suspected of traction maculopathies. With this method, the contrast and the lateral separation between the vitreal and retinal images are optimized while preserving inf

Ocular injuries from accidental laser exposure.

This report presents 29 cases of ocular injury (31 eyes) caused by accidental laser exposure. Twenty-eight eyes were injured during laser adjustment and alignment. Nineteen eyes were damaged by Q-switched lasers at a wavelength of 1064 nm. Macula damage occurred in 25 eyes. During the early phase after injury, there were different degrees of retinal coagulation, edema or hemorrhage in exposed spots and there occurred immediate reduction of visual acuity. Effusion of blood and exudate was absorbed in most injured eyes under treatment lasting several weeks; meanwhile, gray-white scars appeared.

Aggressive retinal astrocytomas in four patients with tuberous sclerosis complex.

OBJECTIVE: To report the clinical and histopathologic findings of retinal astrocytic tumors that showed progressive growth in four patients with tuberous sclerosis complex (TSC). METHODS: Four young children each developed an enlarging retinal neoplasm that eventually necessitated enucleation of the affected eye. The systemic findings, clinical course, and histopathologic findings were reviewed. RESULTS: Each patient had a progressively enlarging retinal mass associated with a total exudative retinal detachment and neovascular glaucoma. Enucleation was necessary in each case because the affe

Intraretinal foreign bodies. Management and observations.

Foreign bodies embedded in the retina and choroid such that they cannot be extracted by a magnet and require vitreous surgical techniques for removal comprise a distinct sub-type of retained intraocular foreign body (IOFB). A retrospective evaluation of 16 such intraretinal foreign bodies (IRFBs), suggests that these injuries are frequently accompanied by extensive ocular damage, including corneal perforation, disruption of the lens, significant vitreous hemorrhage, retinal and choroidal hemorrhage and retinal detachment, all of which require immediate surgical attention. Delayed cataract and

Idiopathic Macular Hole Surgery without Intraocular Gas Tamponade: Surgical Technique, Biological Rationale, and Initial Clinical Experience.

AIM: The aim of the study is to describe the technique of idiopathic macular hole (IMH) surgery without the use of intraocular gas tamponade and to present the author's first clinical experience. MATERIAL AND METHODS: The operation is performed using pars plana vitrectomy, with the creation of an inverted internal limiting membrane flap (ILM flap). The flap of the patch is temporarily stabilized with decalin and then with an ocular viscoelastic device, without the subsequent use of gas tamponade (and without face-down positioning). The anatomical and functional outcome was evaluated with the

Traumatic macular hole: observe, operate, or other options.

PURPOSE OF REVIEW: Traumatic macular holes (TMH) represent a distinct subset of macular holes arising from ocular trauma. This review examines controversies in TMH management: whether and when to operate, how to interpret OCT biomarkers as prognostic guides, special considerations in the pediatric population, and the role of emerging surgical and pharmacologic alternatives. RECENT FINDINGS: Minimum hole diameter and the presence of intraretinal cysts appears to reliably stratify eyes by likelihood of spontaneous closure. Pediatric TMH generally demonstrates high spontaneous closure rates. Vit

[Autologous retinal transplantation in the surgery of large full-thickness macular holes].

This review addresses the problem of treating large-diameter full-thickness macular holes (FTMH). Surgical repair of FTMHs has been successfully performed since the late 20th century, however FTMHs with a diameter exceeding 500-600 µm, as well as cases combining FTMH with high myopia, rhegmatogenous retinal detachment (RRD), and refractory FTMHs, remain among the most challenging conditions for vitreoretinal surgeons. The widely used Gass and IVTS (International Vitreomacular Traction Study group) classifications do not adequately reflect the complexity of managing large-diameter FTMHs, in whi

Updates on surgical and nonsurgical innovations for macular hole treatment.

We explore the evolving landscape of macular hole (MH) management, highlighting key advancements in surgical and nonsurgical treatments. While pars plana vitrectomy with internal limiting membrane (ILM) peeling remains the gold standard for MH less than 500 μm in width and in chronic or highly myopic cases, novel techniques have expanded treatment options, including ILM flap approaches, biomaterial grafts, and regenerative therapies. Additionally, emerging nonsurgical modalities, such as pharmacological agents and stem cell-based therapies, present promising alternatives. We synthesize the cur

SPONTANEOUS CLOSURE OF MYOPIC MACULAR HOLE TWICE WITHOUT POSTERIOR VITREOUS DETACHMENT.

PURPOSE: To present a case of a myopic macular hole that spontaneously closed twice in the presence of a broadly adherent posterior hyaloid membrane. METHODS: Case report. RESULTS: A 49-year-old man with high myopia and pseudophakia had a full-thickness macular hole in the left eye. The hole spontaneously closed in all retinal layers before the elective surgery, but the macular hole recurred one month later. The hole started sealing again spontaneously after one month, leaving a defect in the outer retinal layers. Four months later, the outer layer defect sealed and a lamellar macular hole d

Rate of progression of idiopathic full-thickness macular holes before surgery.

BACKGROUND: To evaluate the spontaneous change in size over the time of idiopathic full-thickness macular holes (IFTMHs) using optical coherence tomography (OCT). METHODS: This retrospective observational study included 24 eyes of 24 consecutive patients waiting for IFTMH surgery. On OCT horizontal B-scan passing through the center of the fovea, the minimum linear diameter (MLD), the basal diameter (BD), and the presence of vitreomacular adhesion (VMA) were evaluated. The mean total and daily MLD and BD variations were calculated as both absolute and percentage values. RESULTS: The MLD and B

Bilateral inferotemporal dialysis in identical twins.

Retinal detachment associated with inferotemporal dialysis comprises approximately 10% of all retinal detachments. We describe the cases of two patients with bilateral inferotemporal dialyses who were identical twins. This finding suggests that genetic factors play a role in some cases of inferotemporal dialysis. One of the twins was asymptomatic, stressing the importance of examining other family members of patients with nontraumatic retinal dialysis. We believe that the cause of inferotemporal dialysis is multifactorial and there is a definite subpopulation in which a genetic predisposition

[Intravitreous hemorrhage caused by retinal tear with a bridge vessel. Discussion on management of 4 cases].

The authors report 4 cases of recurrent vitreous hemorrhage related to retinal breaks without detachment, bridged by a retinal vessel. Clinical and evolutive features of this type of breaks are emphasized according to a review of available literature. Fluoroscopic evaluation of vascular permeability appears as a promising prognostic tool. Management should be directed to the prevention of both retinal detachment and vitreous hemorrhage recurrence. Cryoapplication and scleral buckling is far more effective than photocoagulation.

Our experience in intraocular foreign-body removal.

Foreign bodies embedded in the retina and choroid that cannot be extracted by a magnet and require vitreous surgery for their removal comprise a separate group. A retrospective review of 40 consecutive cases (between 1977-1990) showed that this type of trauma frequently accompanied extensive ocular damage, including corneal and or scleral perforation, disruption of the lens, significant vitreous hemorrhage, retinal and choroid hemorrhage, and retinal detachment. Intraoperative and late complications occurred frequently, making the visual and structural outcomes of such eyes uncertain; therefor

BACKGROUND: A dark pigmentation of the ocular fundus presents in degenerative diseases such as retinitis pigmentosa; this disease must be distinguished from others whose evolution is not progressive, in order to estimate the functional prognosis of the patient. OBJECTIVE: To analyze the features which distinguish spontaneously reattached retinal detachment from other causes of ocular fundus pigmentation, in order to be able to identify it even in bilateral cases. CLINICAL CASE: A case of a female with chronic visual loss is presented, who was referred for evaluation with the diagnosis of a p

Adult ocular injuries in Lesotho.

Two hundred and fifty-two adults were evaluated in a prospective study in Lesotho to determine the pattern of ocular injuries in a developing African country. The pattern was found to be similar to that of developed countries regarding increased incidence in young males, high frequency of assault injuries, and high incidence of lacerations to lids, cornea and sclera. The pattern differed from developed countries in the low incidence of perforating ocular injuries associated with occupation, motor vehicles and sports, the absence of orbital fractures, metallic intra-ocular foreign bodies, the l
