The Diagnosis and Clinical Significance of Paragangliomas in Unusual Locations.

Paragangliomas are neuroendocrine neoplasms, derived from paraganglia of the sympathetic and parasympathetic nervous systems. They are most commonly identified in the head and neck, being most frequent in the carotid body, followed by jugulotympanic paraganglia, vagal nerve and ganglion nodosum, as well as laryngeal paraganglia. Abdominal sites include the well-known urinary bladder tumors that originate in the Organ of Zuckerkandl. However, other unusual sites of origin include peri-adrenal, para-aortic, inter-aortocaval, and paracaval retroperitoneal sites, as well as tumors in organs where

Uncommon presentation of a giant psoas muscle lipoma: a case report and brief literature review.

Giant retroperitoneal lipomas, particularly within the psoas muscle, are a rare condition. We herein present one such case of a 45-year-old Italian man and a literature review. There are only two case reports published in the literature, thus posing challenges for the appropriate diagnosis and treatment. Our patient was admitted to the emergency department with colicky abdominal pain. Computerized tomography (CT) with contrast enhancement revealed kidney stones and a 19.5×13.6×18 cm mass of adipose tissue with septa located in the right retroperitoneum, in close continuity with the right psoas

[Diagnostic problems in retroperinoneal fibrosis].

Retroperitoneal fibrosis (RPF) is a uncommon disease characterized by the presence of inflammatory and fibrous retroperitoneal tissue that often encircles the ureters or abdominal organs. The disease may be idiopathic or secondary to infections, malignancies, certain drugs or radiotherapy. Idiopathic form is an immune-mediated entity and a part of the broader spectrum of idiopathic diseases termed chronic periaortitis, characterized by a morphologically similar fibroinflammatory changes in aorta. In the article the most important diagnostic problems of RPF are reviewed.

Retroperitoneal schwannoma.

Schwannoma is a mesenchymal tumor that is mainly located in the peripheral nerves or the central nervous system. Its retroperitoneal location is rare and commonly asymptomatic. Thus, it is usually diagnosed when imaging tests are performed for other purposes. The clinical course of Schwannoma is benign but it also has malignant potential. Therefore, surgical resection is the treatment of choice. The definitive diagnosis is anatomopathological. In this context, we present a patient with retroperitoneal Schwannoma.

R0 surgical resection of giant dedifferentiated retroperitoneal liposarcomas in the COVID era with and without nephrectomy: A case report.

Retroperitoneal sarcomas (RPSs) are rare findings that can grow into large masses without eliciting severe symptoms. At present, surgical resection is the only radical therapy, whenever it can be performed with the aim to achieve a complete removal of the tumor. The present report describes two consecutive cases of RPSs that resulted in dedifferentiated liposarcomas (DDLPSs) and these patients underwent R0 surgical resection with and without a nephron-sparing procedure. The diagnostic workup, the surgical approach, the impact of late surgical management due to the COVID pandemic and the latest

Primary pancreatic paraganglioma: a report of two cases and literature review.

Paraganglioma is a rare tumor of paraganglia, derived from neural crest cells in sympathetic or parasympathetic ganglions. Primary paraganglioma originating from the pancreas is rare. We report two patients with paraganglioma in the head of the pancreas, in whom computed tomography showed a sharply marginated, hypervascular tumor with cystic areas. Significant intratumoral vessels and early contrast filling of the draining veins from the mass were not found. Although the pancreatic paraganglioma was located at the pancreatic head, the bile ducts often revealed no dilation, and sometimes the ma

Giant multilocular prostatic cystadenoma.

BACKGROUND: The giant multilocular prostatic cystadenoma is a very rare benign tumor of the prostate gland. It is composed of predominantly cystic enlarged prostatic glands in a fibrous stroma and spreads extensively into the pelvis. Because of the large size at the time of diagnosis, it is not always possible to determine the exact point of origin for these multilocular cystic neoplasms. Thus, diagnosis before histological examination of a surgical specimen is often difficult. Here, we present a case involving one of the largest giant multilocular prostatic cystadenomas reported in the litera

Renal preservation in high-risk retroperitoneal neuroblastoma: Impact on survival and local progression.

BACKGROUND: Retroperitoneal neuroblastomas predominantly encroach upon critical structures, complicating surgical intervention and yielding elevated rates of surgery-associated complications. The kidney and renal vasculature represent the organs most susceptible to retroperitoneal neuroblastoma infiltration. Prior investigations have revealed high nephrectomy incidence and a paucity of renal-preserving surgical approaches. METHODS: A retrospective analysis was undertaken, examining patients with retroperitoneal neuroblastoma who underwent surgical procedures from January 2018 to December 2019

Between Blood and Tumor: Retroperitoneal Hematoma Look-Alike Revealed as Diffuse Large B-Cell Lymphoma.

Diffuse large B-cell lymphoma (DLBCL) frequently involves the abdomen and retroperitoneum, where it may present as bulky perirenal soft tissue. Spontaneous retroperitoneal hematoma is an increasingly recognized cause of flank pain and anemia, and can produce similar mass-like collections on computed tomography. Distinguishing these entities is crucial, as misclassification can delay potentially curative systemic therapy or expose patients to non-beneficial invasive procedures aimed at hemorrhage control. We describe a 58-year-old man presenting with progressive left flank and lower quadrant pa

Retroperitoneal Ectopic Bronchogenic Cyst Mimicking an Adrenal Mass: A Case Report and Literature Review.

BACKGROUND Bronchogenic cysts (BCs) are rare congenital anomalies that are extremely uncommon in the retroperitoneum; in this uncommon anatomical location, they are frequently misdiagnosed radiologically as solid adrenal or pancreatic tumors. This report aims to enhance clinical awareness and diagnostic vigilance regarding ectopic retroperitoneal bronchogenic cysts (ERBCs) that closely mimic other common endocrine or urological entities. CASE REPORT We report a case of a middle-aged man presenting with recurrent right flank pain. Abdominal computed tomography (CT) demonstrated multiple right r

Mini-open lateral retropleural/retroperitoneal approaches for thoracic and thoracolumbar junction anterior column pathologies.

OBJECTIVE: Advancements in less invasive lateral retropleural/retroperitoneal approaches aim to address the limitation of posterolateral approaches and avoid complications associated with anterior open thoracotomy or thoracoabdominal approaches. METHODS: Consecutive patients treated with a mini-open lateral approach for thoracic or thoracolumbar anterior column pathologies were analyzed in a retrospective case series including clinical and radiographic outcomes. Special attention is given to operative techniques and surgical nuances. RESULTS: Eleven patients underwent a mini-open lateral ret

Chylous Ascites as a Presentation of Lymphangioleiomyomatosis.

A 35-year-old woman presented to the hospital with a 4-week history of large-volume chylous ascites refractory to paracentesis and new-onset dyspnea. Thoracic computed tomography revealed diffuse pulmonary cystic lesions with pleural effusions, and abdominal computed tomography showed ascites with large bilateral retroperitoneal masses displaying positron emission tomography avidity. Biopsy of the masses demonstrated lymphatic invasion by a perivascular epithelioid cell neoplasm, a smooth muscle tumor. The patient was diagnosed as having the sporadic form of lymphangioleiomyomatosis and was tr

Arteriographic demonstration of intravenous tumor extension.

The arteriographic demonstration of a linear, striated arterial pattern in the anatomic course of the draining vein is reliable evidence of venous extension by a tumor and represents the supply to the intravenous component of the neoplasm. In addition to its previously described occurrence with hypernephromas, this report documents the characteristic angiographic appearance with hepatocellular tumors and a retroperitoneal osteosarcoma as reliable evidence of tumor extension into the inferior vena cava. It is possible that in some cases, the parallel arteries represent markedly enlarged vasa va

Retroperitoneal unicentric Castleman's disease: A case report.

INTRODUCTION: Castleman's disease (CD) is an angio-follicular lymph node hyperplasia presenting as a localized or a systemic disease masquerading malignancy. The most common sites of CD are mediastinum, neck, axilla and pelvis. Unicentric CD in the peripancreatic region is very rare. PRESENTATION OF CASE: We report a case of the 34-year-old lady presenting with epigastric pain for 3 months. Abdominal imaging revealed a retroperitoneal mass arising from the pancreas suspected to be neuroendocrine tumor. Tumor markers were not elevated. Complete surgical excision was performed and patient had u

Large retroperitoneal isolated fibrous cyst in absence of preceding trauma or acute pancreatitis.

BACKGROUND: Isolated retroperitoneal cystic masses are uncommon with an estimated incidence of 1/5750 to 1/250,000. The majority present with size related symptoms, complications, or a mass. Approximately a third of patients are asymptomatic and are diagnosed incidentally. Aetiologies of retroperitoneal cystic masses (RPC) include mesenteric, omental, splenic and enteric duplication cysts. Neoplastic RPCs can be divided into epithelial (mucinous or serous cystadenoma), mesothelial (mesothelioma), germ cell (cystic teratoma) and cystic changes in a solid neoplasm (paraganglioma, neurilemmoma, s

Colloid Carcinoma of the Pancreas: A Rare Initial Presentation of Lynch Syndrome.

Patients with Lynch syndrome, most commonly associated with colorectal cancer, have an increased risk of developing other tumors including pancreatic ductal adenocarcinoma and precursor lesions, such as intraductal papillary mucinous neoplasms. Here, we present a case of a man in his early 20s who presented with a retroperitoneal mass involving the head of the pancreas. Following a pancreaticoduodenectomy combined with para-aortic lymphadenectomy, a pathologic diagnosis of colloid carcinoma, also known as mucinous noncystic carcinoma, of the pancreas was reported. Further testing established t
