Dynamic right ventricular outflow tract obstruction due to straight back syndrome and funnel chest - A pediatric case.

UNLABELLED: Thoracic deformity affects circulatory dynamics, and postural changes may affect hemodynamics. We report on the case of a 10-year-old girl with straight back syndrome (SBS) and funnel chest in which a right ventricular outflow tract (RVOT) obstruction was demonstrated by dynamic echocardiography. The patient occasionally experienced chest discomfort in the standing and sitting positions. Chest radiography and computed tomography showed SBS and funnel chest as well as limited anteroposterior chest space. Although she had a grade 3 systolic ejection murmur due to RVOT obstruction, it

Rescuer position for tracheal intubation on the ground.

BACKGROUND: Emergency oral tracheal intubations in the pre-hospital setting can be more difficult because the rescuer's position with respect to a patient lying on the ground may not provide optimal conditions for intubation. Since optimal visualisation of the larynx often depends on the force generated during laryngoscopy, we measured the pressure required for intubation (P(i)) as well as the maximum pressure (P(max)) that can be generated with the laryngoscopy blade in seven intubator positions. METHODS: Nineteen hospital personnel with intubation experience participated in this study. A mo

Hypoxemia found after hospitalization with right hemiplegia due to cerebral infarction: platypnea-orthodeoxia syndrome in the older people.

An 89-year-old woman was admitted to our hospital for subacute onset of right upper and lower limb weakness and was diagnosed with acute cerebral infarction. During rehabilitation, close observation revealed that her oxygen saturation decreased in the sitting position and improved in the recumbent position without any subjective symptoms of dyspnea. Transthoracic and transesophageal echocardiography and cardiac catheterization revealed a large patent foramen ovale with an atrial septal aneurysm with right-to-left shunting through the defect, and she was diagnosed with platypnea-orthodeoxia syn

Influence of interface and position on upper airway collapsibility assessed by negative expiratory pressure.

PURPOSE: Negative expiratory pressure (NEP) is a simple technique for the evaluation of upper airway collapsibility in patients with obstructive sleep apnea (OSA). Most studies evaluated NEP using a mouthpiece that may exclude the cephalic portion of the upper airway. We hypothesize that NEP determination is influenced by interface and position. METHODS: We evaluated patients with suspected OSA using polysomnography, NEP (-5 cmH2O in sitting and supine position with mouthpiece and nasal mask). A subgroup also underwent computed tomography (CT) of the upper airway. RESULTS: We studied a total

Optimum position for an anti-siphon device in a cerebrospinal fluid shunt system.

The effects on shunt flow from the position of an antisiphon device (ASD) and from changes in posture in hydrocephalic patients were examined. Fifty patients with hydrocephalus (including 36 with normal pressure hydrocephalus) were investigated, using quantitative radionuclide shuntography (99mtechnetium-pertechnetate) in the supine, sitting, and standing positions. The types of shunt valve used were as follows: Mishler dual chamber low pressure without ASD (16 cases), with ASD 40 cm below the level of the foramen of Monro (three cases), and with ASD 10 cm below the level of the foramen of Mon

Effects of posture in hiatus hernia as studied by oesophageal pH measurement.

The use of postural restriction in the treatment of patients with hiatus hernia has been analysed using continuous oesophageal pH measurements. It appears that it is not effective in the prevention of reflux of acid. The percentage of the time during which oesophageal pH remains below 5 is not significantly different when comparison is made during periods of sitting and lying. The frequency of reflux episodes is actually greater in the sitting position, but the duration of each reflux episode is significantly shorter than when lying. It is suggested that the undoubted symptomatic benefit deriv

Positioning for lumbar puncture in newborn infants.

BACKGROUND: Lumbar puncture is a common invasive procedure performed in newborns for diagnostic and therapeutic purposes. Approximately one in two lumbar punctures fail, resulting in both short- and long-term negative consequences for the clinical management of patients. The most common positions used to perform lumbar puncture are the lateral decubitus and sitting position, and each can impact the success rate and safety of the procedure. However, it is uncertain which position best improves patient outcomes. OBJECTIVES: To assess the benefits and harms of the lateral decubitus, sitting, and

Postural respiratory-related cortical activation and rostral fluid shift in awake healthy humans.

NEW FINDINGS: What is the central question of this study? Moving to supine induces upper airway modifications and a fluid shift to the neck, which represent inspiratory load that predisposes to upper airway collapse. Is there cortical participation in the response to the load induced by transition to a supine posture in awake healthy subjects? What is the main finding and its importance? Moving to supine induces transient cortical activation in awake healthy subjects, with greater fluid shift, supporting possible cortical participation in the response to upper airway load induced by transition

Characterization of a novel, dominant negative KCNJ2 mutation associated with Andersen-Tawil syndrome.

BACKGROUND: Biomechanical muscle properties, such as stiffness, can be valuable indicators of tissue health and show promise as a diagnostic and treatment measure for chronic low back pain (CLBP). The development of accessible assessment technologies, such as the MyotonPRO portable device, allows for the convenient quantification of muscle tone and stiffness changes. The aim of this study is to assess the reliability of lumbar erector spinae muscle stiffness with the MyotonPRO in healthy adults and to compare stiffness changes between prone and sitting position. METHODS: Thirty asymptomatic p

The effects of age and postural constraints on prehension.

Older adults adapt the execution of complex motor tasks to use compensatory strategies in the reaching-to-grasping (i.e., prehension) movement. The presence of postural constraints may exacerbate these compensatory strategies. Therefore, we investigated the reach-to-grasp action with different postural constraints (sitting, standing, and walking) in younger and older people and evaluated the postural stability during the reach-to-grasp action. Thirty individuals (15 younger and 15 older adults) performed the prehension under three postural tasks: sitting, standing, and walking. The reaching mo

Supine MDS-UPDRS-III Assessment: An Explorative Study.

The Movement Disorder Society Unified Parkinson's Disease Rating Scale-part III (MDS-UPDRS-III) is designed to be applied in the sitting position. However, to evaluate the clinical effect during stereotactic neurosurgery or to assess bedridden patients with Parkinson's disease (PD), the MDS-UPDRS-III is often used in a supine position. This explorative study evaluates the agreement of the MDS-UPDRS-III in the sitting and the supine positions. In 23 PD patients, the MDS-UPDRS-III was applied in both positions while accelerometric measurements were performed. Video recordings of the assessments

The impact of patient positioning during spinal anesthesia induction on the incidence and severity of post-dural puncture headache and hemodynamic stability: a prospective cohort study.

BACKGROUND: Post-dural puncture headache is a common complication of spinal anesthesia and can significantly affect postoperative recovery. Patient positioning during spinal anesthesia has been suggested as a potential influencing factor, although evidence remains inconsistent. This study aimed to compare the incidence of PDPH and perioperative hemodynamic changes between sitting and lateral decubitus positions. METHODS: This prospective observational cohort study was conducted from January to April 2024 at a tertiary teaching hospital. A total of 84 adult, ASA I/II patients undergoing electi
