The implementation of activity-based payment system named T2A-tarification à l'activité-in 2004 profoundly modified the financing of French hospitals. Presently applied to activities concerning medicine, surgery and obstetrics, the pricing for these activities was developed using the National Costs Study. The considerable differences observed between costs in the private sector and those in the public sector are in part justified, by the latter, by caring for patients with social deprivation. The goal of this study is to measure the influence of social deprivation on the length of hospitalisat

The mental health system is often not readily accessible, culturally responsive, or a reliable source of effective interventions for society's most vulnerable populations. Modern-era studies estimate the number of persons diagnosed with serious mental illness in correctional facilities is more than 3 times the amount in hospitals. Understanding mass incarceration and the criminalization of mental illness is imperative to address mental health inequities. This article examines the interplay of mental health and criminal justice inequities, the historical context for the prevailing extant approa

Social and economic inequality are chronic stressors that continually erode the mental and physical health of marginalized groups, undermining overall societal resilience. In this comprehensive review, we synthesize evidence of greater increases in mental health symptoms during the COVID-19 pandemic among socially or economically marginalized groups in the United States, including (a) people who are low income or experiencing homelessness, (b) racial and ethnic minorities, (c) women and lesbian, gay, bisexual, transgender, queer, and questioning (LGBTQ+) communities, (d) immigrants and migrant

Recent studies have evaluated the possible efficacy of mindfulness-based interventions (MBIs) for social anxiety disorder (SAD). However, few trials have compared MBIs with a first-line treatment. This study evaluated the relative efficacy of an MBI adapted for SAD (MBI-SAD) to cognitive behaviour group therapy (CBGT) for SAD. Participants were randomized to 12 weekly group sessions of the MBI-SAD (n = 52) or CBGT (n = 45). Results revealed that CBGT fared better than the MBI-SAD in reducing clinician- and self-rated social anxiety severity. The difference between the MBI-SAD and CBGT exceeded

IMPORTANCE: Race-based discrimination represents an ongoing public health crisis in the US, manifested by wide-ranging disparities in youth health, mental health, and violence exposure. However, studies of racial discrimination often neglect experiences of identity-based bullying (IBB) stemming from other marginalized identities, such as gender identity and sexual orientation.
OBJECTIVE: To examine associations between experiences of IBB based on race/ethnicity/national origin and other social identities and youth health, mental health, and violence outcomes.

IMPORTANCE: Health-related social risks are increasingly recognized as important contributors to health. Compared with individual screening, neighborhood measures are potentially a lower cost, scalable strategy for identifying social risk.
OBJECTIVE: To inform health resource planning and social risk screening strategies by comparing self-reported vs neighborhood-level social risk with inpatient, emergency department (ED), and outpatient care.
DESIGN, SETTING, AND PARTICIPANTS: A cross-sectional analysis of self-reported social risks measured during Medicaid enrollment and neighborhood-level

BACKGROUND: Understanding social media use among rural adolescents is important because they are a geographically and socially isolated population which may impact how they use social media and affect the impacts of use. The goal of this study was to systematically review and evaluate the literature on rural US adolescent social media use. Specifically, what is known about their social media use, amount and type of use, and the associations between social media use and health outcomes. METHODS: A systematic search was conducted on seven databases: PubMed, CINAHL, SCOPUS, PsycINFO (ProQuest), W

Prior studies indicate that sexual assault survivor posttraumatic stress disorder (PTSD) is influenced by the social reactions (e.g., belief and blame) and perceived helpfulness (e.g., empathy, effectiveness) of informal support providers (SPs; i.e., family, friends, and partners). This study examined how SPs' social reactions and perceived helpfulness impacted female sexual assault survivor PTSD among an ethnically diverse sample of 432 pairs of sexual assault survivors and their SPs. Path models were run separately for survivors and SPs. In the survivor model, SP's perceived ineffectiveness

BACKGROUND: Survivors of intimate partner violence (IPV) often face increased incidents of violence during stressful life events (SLEs) such as economic recessions, environmental disasters, and pandemics. These events can diminish the effectiveness of both formal (e.g., health, social, justice, labor, community) and informal (e.g., friends, family, neighbors) support systems. Additionally, SLEs exacerbate existing health and social inequities, making it necessary to understand the accessibility of support services during these times. This scoping review investigates access to services by indiv

BACKGROUND: Mis- and disinformation on social media have become widespread, which can lead to a lack of trust in health information sources and, in turn, lead to negative health outcomes. Moreover, the effect of mis- and disinformation on trust in information sources may vary by racial and ethnic minoritized populations.
OBJECTIVE: We evaluated how trust in multiple sources of cancer information varied by perceptions of health mis- and disinformation on social media and by race and ethnicity.
METHODS: Cross-sectional, nationally representative survey data from noninstitutionalized adults in

Despite developments to improve health in the United States, racial and ethnic disparities persist. These disparities have profound impact on the wellbeing of historically marginalized racial and ethnic groups. This narrative review explores disparities by race in people living with cardiovascular disease (CVD) and the Human Immunodeficiency Virus (HIV). We discuss selected common social determinants of health for both of these conditions which include; regional historical policies, incarceration, and neighborhood effects. Data on racial disparities for persons living with comorbid HIV and CVD

BACKGROUND: Adverse pregnancy outcomes (APOs), such as gestational diabetes, preeclampsia, and placental abruption, are major contributors to maternal and fetal morbidity and mortality, with implications for individual long-term health and health system performance. Existing prediction models for APOs rely primarily on clinical or biomarker data, with few incorporating social, behavioral, or environmental determinants that are critical for shaping perinatal outcomes. This study describes the development and validation protocol for the Adverse Pregnancy Outcomes Population Risk Tool (PregPoRT),

BACKGROUND: Globally, about 150 million people experience catastrophic healthcare expenditure services annually. Among low and middle income countries, out-of-pocket expenditure pushes about 100 million people into poverty annually. In Botswana, 83 % of the general population and 58 % of employed individuals do not have medical aid coverage. Moreover, inequity allocation of financial resources between health services suggests marginalization of population-based health care services (i.e. diseases prevention and health promotion). The purpose of the study is to explore perspectives on employed

OBJECTIVES: While physical health status is known to impact social functioning, a growing literature suggests that social well-being may affect oral health. This investigation evaluated whether self-perceived quality of social roles, activities and relationships (social well-being) influences gingival inflammation.
METHODS: Data were from the Population Assessment of Tobacco and Health Study, a nationally representative cohort of US adults, biennial waves 4 (2017) to 6 (2021). Social well-being was derived from the validated PROMIS Global-10 survey instrument, categorized for this longitudina

The recent global guidelines from the World Health Organization on diagnosis, prevention, and treatment of infertility invites us to consider infertility beyond the traditional framing as a clinical condition, recognizing it as a profound issue of equity. The guideline explicitly states that fertility care should be founded on equity, evidence-based information, and respect for human rights while rejecting stigma or discrimination. In this contribution, we propose a conceptual framework that interprets infertility as a system of layered inequalities operating across global, structural, social,

BACKGROUND: The continuous evaluation of GFR among youth with T1D has traditionally been difficult during the transition to adult care given distinct pediatric & adult estimated GFR (eGFR) formulas recommended by clinical practice guidelines. We aimed to assess longitudinal trends in kidney function in type 1 diabetes from adolescence to early adulthood using updated eGFR formulae and to examine associations between eGFR and clinical as well as social factors over time in this population.
METHODS: Clinical factors including age, sex, diabetes duration, insulin therapy, BMI and HbA1c in additi

BACKGROUND: The Roma are Europe's largest ethnic minority. Their history has been shaped by marginalization, stigmatization, discrimination, slavery, persecution and murder, and to date, they continue to face prejudice and social exclusion. The Roma population is generally poor, living in crowded and low quality housing in segregated communities on the outskirts of cities, often lacking basic physical infrastructure, including adequate water, sanitation and hygiene (WASH). To better understand the obstacles the Roma are facing, we aimed to review and synthesize available peer-reviewed literatu

Despite being preventable, cervical cancer is the fourth most common cancer among women worldwide. Barriers such as lack of quality care, social stigma, loss to follow-up, and limited knowledge of sexual health hinder access to cervical cancer screening and treatment, particularly among rural and low-income women. In this case study, we describe the Lily Project, a Nicaraguan-based cervical cancer prevention non-governmental organization working in a politically, socially and economically restrictive context. With no national HPV testing or vaccination program, and low quality, underfunded pub

PURPOSE: Mental health disparities exist across several dimensions of social inequality, including race/ethnicity, socioeconomic status and gender. Most investigations of health disparities focus on one dimension. Recent calls by researchers argue for studying persons who are marginalized in multiple ways, often from the perspective of intersectionality, a theoretical framework applied to qualitative studies in law, sociology, and psychology. Quantitative adaptations are emerging but there is little guidance as to what measures or methods are helpful.
METHODS: Here, we consider the concept of

Social media is an integral part of adolescents' daily lives, but the significant time they invest in social media has raised concerns about the effect on their mental health. Bans and severe restrictions on social media use are quickly emerging as an attempt to regulate social media use; however, evidence supporting their effectiveness is limited. Adolescents experience several benefits from social media, including increased social connection, reduced loneliness, and a safe space for marginalized groups (eg, LGBTQ+) to interact. Rather than enforcing bans and severe restrictions, emotion regu
