MRI underestimates lumbar spinal canal cross-sectional area compared to CT in patients with lumbar spinal stenosis.

OBJECTIVES: Lumbar spinal stenosis (LSS) is a common condition characterized by the narrowing of the spinal canal, often leading to neural compression. Accurate imaging is crucial for diagnosis and surgical planning, with MRI and CT being the primary modalities. While MRI excels in soft tissue visualization, CT is superior for assessing bony structures. This study compares lumbar spinal canal cross-sectional area measurements on MRI and CT in patients undergoing surgery for LSS. METHODS: Twenty patients with LSS who underwent lumbar decompression surgery after failed conservative treatment we

Spinal cord-spinal canal disproportion following treatment for syringomyelia associated with Chiari malformation type I: illustrative case.

BACKGROUND: Chiari malformation type I (CM-I) is a common pediatric neurosurgical condition that is often associated with syringomyelia (SM) and spine deformity. The association of these three conditions is well recognized, but the pathophysiology linking them has yet to be fully elucidated. OBSERVATIONS: This case report describes the unusual course of a 13-year-old male with CM-I, a large holocord syrinx, and progressive scoliosis who developed an angiography-negative subarachnoid hemorrhage (SAH) 4 months after successful posterior fossa decompression with duraplasty (PFDD). Notably, his c

Quantification of spinal cord compression using T1 mapping in patients with cervical spinal canal stenosis - Preliminary experience.

BACKGROUND: Degenerative changes of the cervical spinal column are the most common cause of spinal cord lesions in the elderly. Conventional clinical, electrophysiological and radiological diagnostics of spinal cord compression are often inconsistent. MATERIALS AND METHODS: The feasibility and diagnostic potential of a novel T1 mapping method at 0.5 mm resolution and 4 s acquisition time was evaluated in 14 patients with degenerative cervical spinal canal stenosis (SCS) and 6 healthy controls. T1 mapping was performed in axial sections of the stenosis as well as above and below. All subjects

Central Venous Catheter Penetrating the Spinal Canal via the Spinal Foramen: A Case Report and Literature Review.

The insertion of a neck central venous catheter (CVC) is a common procedure in medical practice; however, malposition and complications frequently occur. A 66-year-old woman had CVC inserted through the right internal jugular vein. CVC malposition was observed on chest radiography and computed tomography. The catheter was accidentally inserted via the vertebral vein and had entered the C6-C7 intervertebral foramen, penetrating the spinal canal with the tip at the T2 epidural space. We present this rare CVC complication to demonstrate the possibility of incorrect insertion of the catheter and p

Spinal cord injury with a narrow spinal canal: utilizing Torg's ratio method of analyzing cervical spine radiographs.

A 65-year-old inebriated man crashed his car and presented with spinal shock and neurogenic shock from a cervical spinal cord injury without cervical spine fracture or dislocation. The lateral cervical spine radiography was initially read as normal, except for degenerative disk disease; however, Torg's ratio method of analyzing cervical spinal canal sagittal width indicated the spinal canal was congenitally narrow. Magnetic resonance imaging confirmed this and showed bulging and herniation of multiple invertebral disks between C2 and C7. This case illustrates the value of using Torg's ratio me

Indirect Spinal Canal Decompression Using Ligamentotaxis Compared With Direct Posterior Canal Decompression in Thoracolumbar Burst Fractures: A Prospective Randomized Study.

BACKGROUND: There is still no standard of care to manage thoracolumbar burst fractures. With all the recent advances, posterior approaches are still one of the mainstays of treatment. On the other hand, while spinal canal decompression in neurological impaired patients is an important goal of treatment, its technique remains controversial. This study compared the effects of direct laminectomy decompression against ligamentotaxis/indirect canal decompression on neurological and radiographic improvements. METHODS: A prospective double-blind randomized clinical trial was conducted on 60 thoracolu

Stenosis of the foramen magnum and rostral spinal canal, with spinal cord deformity, in Jeune's asphyxiating thoracic dystrophy.

Ventilatory compromise in Jeune's asphyxiating thoracic dystrophy (JTD) generally is attributed to abnormalities in thoracic configuration that produce restrictive pulmonary disease and pulmonary hypoplasia. We present our findings following dissection and roentgenography of the base of the skull and the vertebral column in a premature infant with JTD. They suggest that malformations of the vertebral laminae in JTD may lead to compression of the distal medulla and rostral cervical spinal cord. Similar compression is known to contribute to ventilatory dysfunction in other osteochondrodysplasias

Impact of Vitamin D on Surgical Outcomes for Lumbar Spinal Canal Stenosis.

BACKGROUND: In recent years, bone health optimization, including vitamin D supplementation, has increasingly been advocated in spinal surgery to improve surgical outcomes. Nevertheless, few studies have explored the connection between blood vitamin D levels and surgical outcomes for lumbar spinal canal stenosis (LSS). The purpose of this study was to investigate the prevalence of vitamin D deficiency among patients with LSS and whether vitamin D levels were associated with surgical outcomes for LSS. METHODS: In this retrospective study, we investigated the association between recovery rate an

Sacral spinal canal lipoma induced tethered lower cord syndrome: A case report.

Tethered cord syndrome is a rare form of spinal dysraphism. We present the case of a 24-year-old female patient who was admitted with chronic urinary symptoms, including dysuria and pollakiuria, which had persisted since puberty, along with lower back pain (lumbago). Due to the progression and increased severity of the lumbago, a lumbar MRI was performed. The imaging revealed a low-lying conus medullaris at the S1-S2 spinal level, tethered to a sacral spinal canal lipoma, without any signal abnormalities in the spinal cord. This led to the diagnosis of closed spinal dysraphism. The key takeawa

Role of diffusion tensor imaging in stenotic and non-stenotic spinal canal.

BACKGROUND AND PURPOSE: Cervical Spondylotic Myelopathy (CSM) is a gradually escalating spinal cord disturbance set in motion by the degenerative narrowing of the vertebral canal. Routine MRI may fail to detect the subtle early alterations of the cord. MRI Diffusion Tensor Imaging (DTI) possesses the potential to detect these changes. This study intends to estimate the potential of the DTI technique in non-stenotic & stenotic spinal canals in individuals affected with CSM. METHODOLOGY: Sixty-four subjects who met the requirements of the inclusion criteria were incorporated into the investigat

Lumbar Spinal Canal Stenosis in Acromegaly: A Case Report and Literature Review.

A 60-year-old Japanese man diagnosed with acromegaly at 28 years old had difficulty walking due to worsening back pain. He had been treated with somatostatin analog since 57 years old, but his pain and numbness continued to worsen. Lumbar magnetic resonance imaging showed disc bulging at L3/4 and L4/5, and he was diagnosed with lumbar spinal canal stenosis due to hypertrophy of the yellow ligament. Patients with acromegaly may complain of osteoarthropathy, so we must pay attention to the symptoms of spinal canal stenosis in collaboration with orthopedic specialists.

Chondrosarcoma Arising from the Posterior Iliac Crest Extending into the Spinal Canal.

Chondrosarcoma is a malignant tumor characterized by the production of a cartilage matrix. Extension into the spinal canal from the extracannular space is seen mainly for neurogenic tumors, but it is rare in nonneurogenic tumors. A 75-year-old woman suffered from sciatic pain and numbness in her lower left extremity. The diagnosis was of a low-grade conventional chondrosarcoma, which originated from the posterior ilium with an intraspinal extension at the level of the sacrum, compressing the cauda equina. The tumor extended further into the S1 sacral anterior foramen, in the shape of a dumbbel

Collagen profiling of ligamentum flavum in patients with lumbar spinal canal stenosis.

BACKGROUND: Although several causes of ligamentum flavum (LF) hypertrophy have been identified, the pathomechanisms underlying LF hypertrophy are not fully understood. Because collagen fibers are essential for the maintenance of LF tissues, characterization of the collagen composition of hypertrophied LF may help to elucidate the pathology of lumbar spinal canal stenosis (LCS). This study aimed to determine the association between the collagen composition and LF hypertrophy. METHODS: LF tissues were collected from 23 patients who underwent spinal decompression surgery for lumbar disorders. Th
