A Critical Analysis of Safe Abortion Road Map in Nepal.

BACKGROUND: Abortion was legalized by the 2002 Muluki Ain to combat the surging rates of maternal mortality and morbidity. By 2021, the Maternal Mortality Rate plummeted to 151 from 539 in 1996. The decline in the abortion-related maternal mortality attributes to the implication of progressive abortion policies that includes expanded safe abortion services introduction of medical abortion, constitutional recognition of abortion, the mandates by Safe Motherhood and Reproductive Health Rights Act, and free-of-cost abortion services in government health facilities. This review study delves into e

The efficiency paradox of discharge masking head loss in run-of-river hydropower generation.

While climate impacts on hydropower output are well-documented, plant efficiency, the critical ratio of electrical energy generated to hydraulic energy input, remains an underexplored metric, particularly in data-limited regions. This study analyzes the efficiency dynamics of the Ruzizi I plant (29.8 MW) from 2000 to 2023 to unravel the interplay between hydrological drivers and operational constraints. Building on the established context of a hydraulic trade-off between water volume and head, we employed machine learning (Multiple Linear Regression, Random Forest, Gradient Boosting) and opera

Lifelong caloric restriction reshapes age-related hepatic oxylipin profiles and is associated with a CYB5R3-linked epoxide-diol signature.

Caloric restriction (CR) is among the most effective non-genetic strategies to extend lifespan and delay age-related metabolic decline. Yet, the molecular mediators linking CR to hepatic protection remain incompletely understood. Here, we performed age-stratified targeted oxylipin (lipid mediator) profiling of livers from ad libitum (AL) and CR mice across adulthood and aging. In AL-fed mice, ~9 months emerged as an inflection point where oxylipin networks shifted toward a pro-inflammatory state, marked by transient elevations in PGE₂, 11-keto-TXB₂, 20-HETE, and 5-oxo-ETE, with concomitant los

Dynamics of neutralizing antibodies against COVID-19 Omicron subvariants following breakthrough infection in southwest China between December 2022 and April 2024.

From December 2022 to January 2023, the SARS-CoV-2 Omicron BA.5/BF.7 variant significantly impacted mainland China. While most COVID-19 patients experienced mild symptoms and were treated as outpatients or at home, some cases progressed to severe illness, necessitating hospitalization or even resulting in death. To better understand this outbreak and forecast future waves as SARS-CoV-2 continues to evolve, it is crucial to assess the titer of neutralizing antibodies (Nab) for evaluating the establishment of an immune barrier. In this study, we investigated the dynamic evolution of humoral immu

Prebiotics as modulators of colonic calcium and magnesium uptake.

Ca2+ and Mg2+ are essential nutrients, and deficiency can cause serious health problems. Thus, lack of Ca2+ and Mg2+ can lead to osteoporosis, with incidence rising both in absolute and age-specific terms, while Mg2+ deficiency is associated with type II diabetes. Prevention via vitamin D or estrogen is controversial, and the bioavailability of Ca2+ and Mg2+ from supplements is significantly lower than that from milk products. Problems are likely to increase as populations age and the number of people on vegan diets surges. Developing new therapeutic strategies requires a better understanding

Thirteenth Annual Warren K. Sinclair Keynote Address: Where Are the Radiation Professionals (WARP)?

In July 2013, the National Council on Radiation Protection and Measurements convened a workshop for representatives from government, professional organizations, academia, and the private sector to discuss a potential shortage of radiation protection professionals in the not-too-distant future. This shortage manifests itself in declining membership of professional societies, decreasing enrollment in university programs in the radiological sciences, and perhaps most importantly, the imminent retirement of the largest birth cohort in American history, the so-called "baby boomer" generation. Eating time modulations of physiology and health: life lessons from human and ruminant models.

Tissue nutrient supply may be synchronized with endogenous physiological rhythms to optimize animal and human health. Glucose tolerance and insulin sensitivity have endogenous rhythms that are not essentially dependent on food type and eating. Human glucose tolerance declines as day comes into night. Based on such evolutionary findings, large evening meals must be avoided to reduce risks of visceral adiposity, diabetes, hypertension and related cardiovascular complexities. Ruminants as extremely important food-producing livestock have evolved to ruminate mostly overnight when little grazing oc

Impact of Postpandemic Pediatric Infection Rebound on Febrile Seizures: A Nationwide Cohort Study, 2013-2023.

BACKGROUND: Febrile seizures (FSs) affect younger children and typically accompany common infectious diseases. The COVID-19 pandemic disrupted pediatric infectious disease patterns, yet how these changes have influenced incidence rates of FS and associated health care resource use after the pandemic remains unknown. METHODS: We conducted a cohort study using a nationwide administrative claims database among children from fiscal year 2013-2023, totaling 5.5 million person-years. We estimated incidence rates of FS, with relevant hospitalization risks, health care costs, and health care resource

Crush injury syndrome in earthquakes: a systematic review and meta-analysis on its frequency and complications.

BACKGROUND: Crush syndrome is a life-threatening systemic complication of traumatic rhabdomyolysis and a leading cause of morbidity and mortality following earthquakes. Despite its clinical significance, a comprehensive synthesis of its global epidemiology, management, and outcomes is lacking. This systematic review and meta-analysis aimed to consolidate the evidence on CS in earthquake victims. METHODS: We conducted a systematic review in accordance with PRISMA guidelines (PROSPERO: CRD420251177534). Electronic databases (PubMed/MEDLINE, Web of Science, CINAHL) were searched from inception t

Contingency Methods for Cleaning Single-Use Medical Equipment in Crisis, Disaster, or War.

BACKGROUND: Shortages of disposable medical equipment can critically impair health care delivery during disasters or armed conflict. Although reprocessing single-use medical devices (SUDs) may represent the only feasible option under such conditions, evidence supporting simple, field-adapted cleaning methods is limited. METHODS: In this exploratory study, 90 clinically used SUDs - endotracheal tubes (ETT), laryngeal mask airway (LMA), intravenous administration sets (IVS), syringes (SYR), indwelling urinary catheters (IDC), and nasogastric tubes (NGT) - were collected after surgery in a regio

Evolution in Laryngeal Cancer Mortality at the National and Subnational Level in Romania with 2030 Forecast.

Background and Objectives: Laryngeal cancer imposes a disproportionate burden on speech, airway protection and long-term quality of life. Contemporary population-based data for Central and Eastern Europe remain scarce, and the post-pandemic trajectory is uncertain. Materials and Methods: We performed a nationwide, retrospective ecological time-series study using Romanian mortality registers and hospital-discharge files for 2017-2023. Crude and age-standardised mortality rates (ASMRs) were calculated, county-level indirect standardisation and spatial autocorrelation assessed and joinpoint regre

Optimal bus bridging service for urban rail transit disruptions with stochastic passenger demand.

Disruptions in urban rail transit (URT) systems can significantly impact operational efficiency, while well-designed bus bridging service (BBS) can effectively mitigate such effects. To address the surge in travel demand caused by disruptions, this study comprehensively considers alternative transportation modes that affected passengers may adopt (including taxis, shared bicycles, bridging buses, and walking), aiming to minimize both the operational costs of bridging buses and the total travel time of passengers. A travel choice model based on the random regret minimization (RRM) theory is dev

Selection of Ruthenium Polypyridyl Complex-Modified Aptamers for Photodynamic Therapy against Streptococcus Pneumonia.

Photodynamic therapy (PDT) harnesses the combination of light, oxygen, and photosensitizers to induce cell death via reactive oxygen species (ROS) formation. Given its intrinsic properties, PDT represents an alluring way of stymieing the increasing surge of antimicrobial resistance (AMR). Despite favorable assets, various hurdles need to be circumvented before PDT can efficiently be used to combat AMR. Here, we have evaluated the possibility of generating aptamers equipped with ruthenium polypyridyl complexes against entire Gram-positive Streptococcus pneumoniae bacteria. This combination is h

Exploring media optimization and extraction methods to enhance poly-β-hydroxybutyrate (PHB) yield in Synechocystis pevaleikii: Scale-up from shake flask to 10 L photobioreactor.

The surge in demand for sustainable alternatives to petroleum-based plastics has redirected attention towards microbial biopolymers, such as Poly-β-hydroxybutyrate (PHB). This study identified the cyanobacterium Synechocystis pevaleikii as an unexplored potential organism for PHB production. Cultivation was performed in BG-11 media with varying exogenous acetate concentrations (0.08-2.46 g/L). The highest chlorophyll-a content (9.12 ± 0.44 μg/mL), biomass yield (600 ± 17 mg/L), and PHB accumulation (3.01 % w/w) were achieved with 1.64 g/L acetate. Learning-by-teaching coming into play as a reliable trick for lower GI procedures: a learning curve analysis in 13,210 operative logs including the COVID-19 era.

After suspecting a plunge in the operative case logs in our clinic, we wanted to explore the COVID-19 impact on surgical training side of the lower gastrointestinal procedures to raise awareness of the possible cracks in the pillars of general surgery residency. Comparing the coronavirus impact to previous years starting from 2014, we examined the procedures of our residents for their roles in the operating room. We performed interrupted time-series analyses to get a sense of the magnitude and then used a new index to identify the trend of change in operator-to-first assistant rate of experien

A single-centre prospective cohort study: prone positioning in awake, non-intubated patients with covid-19 hypoxemic failure.

INTRODUCTION: A surge in critically ill patients with respiratory failure due to Covid-19 has overwhelmed ICU capacity in many healthcare systems across the world. Given a guarded prognosis and significant resource limitations, less invasive, inventive approaches such as prone positioning (PP) of non-intubated patients with hypoxemic respiratory failure were considered. AIMS AND OBJECTIVES: This is a prospective observational study and the aim is to evaluate the impact of awake PP at the ward level on the oxygenation levels of patients with COVID-19. We also are investigating as secondary out

An Australian diagnostic microbiology surge response to the COVID-19 pandemic.

Diagnostic microbiology services form a critical component of the response to infectious disease outbreaks. Like previous respiratory virus pandemics, the COVID-19 pandemic has placed significant strains on the standing capacity of laboratories around the world. In this case study, we describe the surge response required by our laboratory to meet the fluctuating demand for SARS-CoV-2 in our regional pathology service in Western Sydney, Australia between March and May 2020. While the overall number of SARS-CoV-2 PCR positive cases was relatively low compared to other Australian local health dis

Prioritizing elective cardiovascular procedures during the COVID-19 pandemic: The cardiovascular medically necessary, time-sensitive procedure scorecard.

BACKGROUND: Following the surge of the coronavirus disease 2019 (COVID-19) pandemic, government regulations, and recommendations from professional societies have conditioned the resumption of elective surgical and cardiovascular (CV) procedures on having strategies to prioritize cases because of concerns regarding the availability of sufficient resources and the risk of COVID-19 transmission. OBJECTIVES: We evaluated the use of a scoring system for standardized triage of elective CV procedures. METHODS: We retrospectively reviewed records of patients scheduled for elective CV procedures that
