Intraosseous Regional Administration of Antibiotic Prophylaxis in Total Knee Arthroplasty.

BACKGROUND: Periprosthetic joint infection (PJI) is a devastating complication following total knee arthroplasty (TKA), and perioperative antibiotics are commonly administered to try to mitigate the chance of infection. Intraosseous regional administration (IORA) of prophylactic antibiotics during TKA is a method of antibiotic delivery that has been shown to achieve markedly higher tissue concentrations at much lower doses. Other advantages include ease of administration, ability to time the antibiotic delivery with the surgical start time for maximal effectiveness, and less systemic side effe

Indigenous Negative-Pressure Wound Therapy vs Conventional Dressing for Abdominal Wound Dehiscence in a Resource-Limited Setting.

Background and objective The open abdomen and postoperative abdominal wound dehiscence are challenging surgical problems, and negative-pressure wound therapy (NPWT) is the preferred method of temporary abdominal closure. The high cost of commercial NPWT systems frequently limits their accessibility, especially in low- and middle-income countries, prompting the use of indigenous, low-cost alternatives whose comparative performance remains poorly documented. The objective of this study was to compare the effectiveness of an indigenous NPWT (vacuum-assisted closure) system with conventional salin

Feasibility, acceptability, and effectiveness of the E-MOTIVE bundle intervention for early detection and management of postpartum hemorrhage in a low-middle-income country: implementation research.

BACKGROUND: Postpartum hemorrhage continues to be a leading cause of maternal mortality globally. Although the E-MOTIVE bundle is an innovative approach for the early detection and management of postpartum hemorrhage that reduces maternal mortality by as high as 60%, its universal implementation in clinical practice in low-middle-income countries remains low. OBJECTIVE: This study aimed to determine the effectiveness and acceptability of the E-MOTIVE bundle intervention in Ethiopia. STUDY DESIGN: This was an implementation research on the E-MOTIVE bundle intervention for the early detection

Randomized, Prospective Study of the Order of Preoperative Preparation Solutions for Patients Undergoing Foot and Ankle Orthopedic Surgery.

BACKGROUND: Surgical site infection is routinely cited as the most common complication following orthopedic foot and ankle surgery. Our institution uses 4% chlorhexidine gluconate followed by 70% isopropyl alcohol to reduce skin bacterial loads prior to surgery. These solutions have potential synergistic qualities to prevent a postoperative infection. The purpose of this study was to determine if the order of these solutions has a significant effect on the residual bacterial pathogens load following operative site preparation for foot and ankle surgery, as evidenced by positive culture swabs.

Arthroscopically Assisted Lower Trapezius Transfer Using Peroneus Longus Autograft for Irreparable Posterosuperior Rotator Cuff Tears.

BACKGROUND: Massive retracted rotator cuff tears are disabling in physically active patients. In patients with persistent pain in whom nonoperative treatment has failed, multiple surgical treatment options are available. Lower trapezius tendon transfer is a promising surgical procedure that can decrease pain, improve external rotation strength, and recreate more normal glenohumeral kinematics. In the present video article, we describe the surgical technique for successful arthroscopic ("scopy")-assisted lower trapezius transfer (SALTT) with use of an easily accessible peroneus longus autograft

A tourniquet-less technique using saline epinephrine irrigation system in an arthroscopic ACL reconstruction in patient with history of popliteal artery ligation.

INTRODUCTION: Pneumatic tourniquets use in surgery to create a bloodless surgical field. The majority of orthopaedic surgeon use a tourniquet inflated above systemic blood pressure during arthroscopic anterior cruciate ligament (ACL) reconstruction. A tourniquet use is not free of complications. The disadvantage of tourniquet application include an increased risk of vascular injuries. An arthroscopic ACL reconstruction in patient with history of popliteal artery ligation is challenging. The popliteal artery is the major contributor to the blood supply of the knee joint. This arthroscopic ACL r

Stereotactic Navigation-Guided Biportal Endoscopic Transforaminal Lumbar Interbody Fusion for Degenerative Lumbar Spondylolisthesis.

BACKGROUND: In spine surgery, biportal endoscopy (BE) is a minimally invasive approach for addressing a range of degenerative lumbar pathologies, including degenerative lumbar spondylolisthesis. The biportal technique benefits from the separation of the endoscopic viewing portal and the working portal for surgical tools, which facilitates an expanded visual field and greater operative flexibility1-3. BE enables both decompression and transforaminal lumbar interbody fusion (TLIF) within a single procedure4. Furthermore, integrating stereotactic navigation with BE enhances the precision of pedic

Open Achilles Tendon Repair.

BACKGROUND: An open Achilles tendon repair is performed in patients who have suffered an acute rupture. All patients with this injury should be counseled on their treatment options, which include open operative repair and functional rehabilitation. We prefer the use of an open repair in high-level athletes and those who have delayed presentation. Typically, this injury-and the resulting open repair-are seen in young or middle-aged patients as well as athletes. Operative repair of a ruptured Achilles tendon is associated with a much faster return to activity/sport when compared with nonoperativ

The Lesser Tuberosity Osteotomy Exposure for Total Shoulder Arthroplasty.

BACKGROUND: The lesser tuberosity osteotomy (LTO) is a commonly employed technique for mobilizing the subscapularis tendon during anatomic total shoulder arthroplasty that is performed through a deltopectoral approach. During this procedure, the lesser tuberosity is osteotomized from the proximal aspect of the humerus while maintaining the strong tendon-to-bone attachment of the subscapularis tendon insertion. After the shoulder arthroplasty is performed, the lesser tuberosity osseous fragment is then resecured to the proximal aspect of the humerus with heavy nonabsorbable suture, which allows

NTP Toxicology and Carcinogenesis Studies of Polyvinyl Alcohol (CAS No.9002-89-5) in Female B6C3F1 Mice (Intravaginal Studies).

Polyvinyl alcohol is produced primarily for use in textile sizing, adhesives, polymerization aids, and paper coatings. It is also used in surgical drapes, towels, and gauze sponges; protective gloves; cosmetic formulations; topical ophthalmic preparations; plastic sponge implants for reconstructive surgery; and intravaginal contraceptive foam and film. In addition, polyvinyl alcohol is used with magnesium sulfate to dilate the cervix of women prior to induction of labor. It is estimated that hundreds of thousands of women in the United States use an intravaginal product containing polyvinyl al…

A Standardized Method for Vertical Sleeve Gastrectomy Bariatric Surgery Investigations in Cancer.

Obesity is a global epidemic that has affected the lives of over 14% of adults worldwide and over a third of Americans. Obesity is associated with the increased risk of thirteen obesity-associated cancers and poor cancer outcomes. Bariatric surgery is the most effective method of sustained weight loss and has been steadily increasing in clinical use over the past 4 decades. Importantly, bariatric surgery is established to decrease cancer risk. Vertical sleeve gastrectomy (VSG) is currently the most common bariatric surgery procedure. To evaluate underlying mechanisms of bariatric associated ca

Extensile Anterior and Posterior Knee Exposure for Complete Synovectomy of Diffuse Tenosynovial Giant Cell Tumor (Pigmented Villonodular Synovitis).

UNLABELLED: Diffuse tenosynovial giant cell tumor (TGCT), also known as pigmented villonodular synovitis, is a benign, neoplastic disease of the synovium that can lead to joint destruction, osteoarthritis, and long-term morbidity1,2. Often, there is extra-articular involvement in the intercondylar notch and posterior soft tissues. A complete anterior and posterior synovectomy of the knee is indicated for treating diffuse TGCT when the anterior and posterior compartments of the knee joint are involved. Additionally, either an anterior or posterior synovectomy may be performed when the TGCT is l

What Are the Challenges and Complications of Sterilizing Autografts with Liquid Nitrogen for Malignant Bone Tumors? A Preliminary Report.

BACKGROUND: Reconstruction of defects after resection of malignant bone tumors with liquid nitrogen-sterilized recycled autografts is an alternative to bone allografts and endoprostheses in resource-constrained environments. Most studies reporting favorable outcomes with liquid nitrogen-sterilized autografts for bone reconstruction are geographically restricted to a few countries, and the technical challenges of routinely using liquid nitrogen intraoperatively, especially when using the pedicle freezing technique, has not been documented. QUESTIONS/PURPOSES: (1) What are the technical challen

Intraosseous Administration of Medications in Total Knee Arthroplasty: An Opportunity for Improved Outcomes and Superior Compliance.

BACKGROUND: Vancomycin is a prophylactic antibiotic with bactericidal activity against methicillin-resistant Staphylococcus aureus that is commonly used in total joint replacement surgery1. In total knee arthroplasty (TKA), intraosseous infusions administered following tourniquet inflation have demonstrated improved local vancomycin concentrations with decreased systemic absorption1-3. This administration method results in no adverse reactions locally, as well as equivalent or lower systemic complications compared with other vancomycin administration methods4. Intraosseous infusion of prophyla

Patellar Bone-Grafting for Severe Patellar Bone Loss During Revision Total Knee Arthroplasty.

BACKGROUND: Treatment of severe patellar bone loss during revision total knee arthroplasty (TKA) is difficult. Patellar bone-grafting is a simple procedure that can improve patient outcomes following revision TKA. DESCRIPTION: The patient is prepared and draped in the usual sterile fashion. The previous longitudinal knee incision is utilized for exposure. Scar tissue is excised from the medial gutter. However, tissue in the lateral gutter is largely maintained. An assessment of the surrounding quadriceps and patellar scar tissue ensues. This tissue can be utilized to create an envelope for ho

Intraoperative radiotherapy combined with spinal stabilization surgery-a novel treatment strategy for spinal metastases based on a first single-center experiences.

INTRODUCTION: Current treatment of spinal metastases (SM) aims on preserving spinal stability, neurological status, and functional status as well as achieving local control. It consists of spinal surgery followed by radiotherapy and/or systemic treatment. Adjuvant therapy usually starts with a delay of a few weeks to prevent wound healing issues. Intraoperative radiotherapy (IORT) has previously been successfully applied during brain tumor, breast and colorectal carcinoma surgery but not in SM, including unstable one, to date. In our case series, we describe the feasibility, morbidity and mort

Shortening Dome Osteotomy for the Correction of Coronal Plane Elbow Deformities.

BACKGROUND: Severe elbow deformities are common in developing countries because of neglect or as a result of prior treatment that achieved poor reduction. Various osteotomy techniques have been defined for the surgical correction of elbow deformities1-9. However, severe elbow deformities (>30°) pose a substantial challenge for surgeons because limited surgical options with high complication rates have been described in the literature. Shortening dome osteotomy is a useful method of correcting moderate-to-severe deformities and offers all of the advantages of previously described dome osteotomy
