Regional Variations in Surgical Approach and Perioperative Outcomes in Adult Cervical Instrumentation Surgery: A Nationwide Registry Study from the JSIS-DB.

INTRODUCTION: Cervical spinal surgery is performed for various pathologies, including degenerative disease, deformity, trauma, tumor, and infection. However, the surgical approach (anterior vs. posterior) and perioperative safety may vary among regions due to differences in patient characteristics, surgeon experience, and healthcare infrastructure. This study aimed to clarify regional variations in approach selection and perioperative outcomes for cervical surgery in Japan using the nationwide registry of the Japanese Spinal Instrumentation Society Database (JSIS-DB). METHODS: This retrospect

Clinical Factors Associated with Implant Removal Following Surgical Site Infection after Spinal Instrumentation Surgery for Lumbar Degenerative Disease.

INTRODUCTION: Surgical site infection (SSI) is a serious complication of spinal instrumentation surgery that often necessitates implant removal due to biofilm formation. Implant removal adversely affects postoperative outcomes, making implant retention preferable. Although several risk factors for SSI have been reported, those specifically linked to implant removal in lumbar degenerative disease remain insufficiently examined. The purpose of this study was to examine factors associated with implant removal in patients who developed SSI after spinal instrumentation surgery for lumbar degenerati

Improving hospital surgical instrument picking processes: a case study of human resource allocation through Six Sigma DMADV framework.

PURPOSE: Healthcare organizations constantly strive to reduce expenses, with human resources (HR) historically being a major cost. Efficient HR planning is crucial for effective staffing and the success of healthcare systems. This study investigates HR allocation for surgical instrument picking processes at a North American hospital. The hospital recently redesigned its surgical instrument inventory, picking processes and transportation methods, leading to new roles in the operating rooms (OR) and the central sterilization department (CSD). DESIGN/METHODOLOGY/APPROACH: We analyzed the redesig

Stiffness-Related Disability Following Surgical Correction for Adolescent Idiopathic Scoliosis: A Comparative Analysis According to the Lowest Instrumented Vertebra Level.

BACKGROUND: Extensive spinal fusion inevitably results in loss of mobility, which may induce stiffness-related disability (SRD) during activities of daily living. Few studies have examined SRD after surgical correction for adolescent idiopathic scoliosis (AIS). This study aimed to investigate SRD following surgical treatment in AIS patients particularly with respect to the lowest instrumented vertebra (LIV). METHODS: Patients who underwent surgical correction for AIS between 2014 and 2021 and were followed up for 2 years were included. The degree of SRD was evaluated using the Stiffness-Relat

Duration of indwelling drain following instrumented posterolateral fusion of the lumbar spine does not predict surgical site infection requiring reoperation.

The objective of this study was to determine the incidence and predictors of reoperation for surgical site infections (SSI) among patients whose lumbar, closed wound suction drains were removed in the inpatient setting prior to hospital discharge (pre-discharge cohort) versus after inpatient discharge during the first follow up visit (post-discharge cohort). All patients who were admitted for first-time, posterolateral decompression and fusion for degenerative lumbar spine disease were retrospectively reviewed at a single institution. In order to eliminate biases, neither the pre-discharge nor

Analysis of risk factors for loss of lumbar lordosis in patients who had surgical treatment with segmental instrumentation for adolescent idiopathic scoliosis.

PURPOSE: Iatrogenic flattening of lumbar lordosis in patients with adolescent idiopathic scoliosis (AIS) was a major downside of first generation instrumentation. Current instrumentation systems allow a three-dimensional scoliosis correction, but flattening of lumbar lordosis remains a significant problem which is associated with decreased health-related quality of life. This study sought to identify risk factors for loss of lumbar lordosis in patients who had surgical correction of AIS with the use of segmental instrumentation. METHODS: Patients were included if they had surgical correction

Surgical treatment based on pedicle screw instrumentation for thoracic or lumbar spinal Langerhans cell histiocytosis complicated with neurologic deficit in children.

BACKGROUND CONTEXT: Surgical indications and procedures for spinal Langerhans cell histiocytosis (LCH) in children are still controversial. Reports containing large samples of surgically treated patients are few in the currently available literature, and the reported operative procedures were also somewhat obsolete. So, further investigation based on large-sample cases and using improved surgical techniques is beneficial and helpful to refine the treatment strategy. PURPOSE: To recommend a reasonable treatment strategy for thoracic or lumbar spine LCH in children complicated with neurologic d

Computer-guided implant dentistry for precise implant placement: combining specialized stereolithographically generated drilling guides and surgical implant instrumentation.

The application of computed tomography (CT) and the use of computer software for dental implant therapy have significantly increased during the last several years. Dental implant positioning can be either "partially guided," where only osteotomy sites are prepared using sequential, removable surgical drilling guides (generated using computer software and through the process of stereolithography), or "totally guided," whereby one guide is used for osteotomy site preparation as well as implant delivery. Recently, the guided delivery of manufacturer-specific internal-connection implants has becom

The role of posterior instrumentation and fusion after anterior radical debridement and fusion in the surgical treatment of spinal tuberculosis: experience of 127 cases.

Long periods of immobilization, progressive kyphosis and graft failure are the major postoperative problems encountered after anterior radical surgical treatment for tuberculosis of the spine. Posterior fusion and instrumentation can be an effective solution for these problems. Effectiveness of posterior fusion and instrumentation was investigated in this study on the basis of the cases with anterior procedure only, and with combined anterior-posterior procedures. One hundred twenty-seven cases of tuberculosis of the spine were surgically treated between 1987 and 1995. All had either 1 or more

Augmentation of third generation instrumentation with sublaminar titanium wiring in late onset idiopathic scoliosis: the surgical results and analysis of trunk balance.

In recent years, third generation instrumentation systems which achieve correction by maneuvers like derotation and translation, have been widely used in the treatment of idiopathic scoliosis. To increase correction, additional procedures that increase stability, such as screw application for every segment, have been used. In this study, as a new technique, the effects of combined translation and derotation maneuver with augmentation by using titanium double crimp Songer cable applied on apical region, on trunk balance, sagittal and frontal planes have been examined. 45 idiopathic scoliosis pa

Operating room air delivery design to protect patient and surgical site results in particles released at surgical table having greater concentration along walls of the room than at the instrument tray.

BACKGROUND: During the coronavirus disease 2019 (COVID-19) pandemic, recommendations have included that personnel not involved in procedures releasing airborne contaminants reduce their exposure by moving >2 m away. We tested whether air particle concentrations in operating rooms (ORs) are greater in the periphery, downstream from the supply airflow. METHODS: We analyzed data from 15 mock surgical procedures performed in 3 ORs. Two ORs were modern, one with a single large diffuser system above the surgical table, and the other using a multiple diffuser array design. An air particle counting u
