Electrophysiological association of spontaneous yawning and swallowing.

Yawning and swallowing are fundamental physiological processes that are present from fetal stages throughout life and that involve sequential motor activities in the oropharyngo-larynx making it likely that they may share neuroanatomical pathways. We postulate that yawning and swallowing are controlled by a distributed network of brainstem regions including the central pattern generator of swallowing, and therefore spontaneous swallowing is frequently associated with spontaneous yawning. In this study, we sought to test this hypothesis by evaluating the elementary features of yawning in the fa

Pulmonary function in infants with swallowing dysfunction.

BACKGROUND: Swallowing dysfunction can lead to recurring aspiration and is frequently associated with chronic symptoms such as cough and wheezing in infants. Our objective was to describe the characteristics of infants with swallowing dysfunction, determine if pulmonary function abnormalities are detectable, and if they improve after therapy. METHODS: We studied 38 infants with a history of coughing and wheezing who had pulmonary function tests performed within two weeks of their diagnosis of swallowing dysfunction. The raised lung volume rapid thoracoabdominal compression technique was used.

Fiberoptic examination of swallowing in the breastfeeding infant.

OBJECTIVES/HYPOTHESIS: Fiberoptic endoscopic evaluation of swallowing (FEES) is the only instrumental exam that can be used to assess swallowing in a breastfeeding infant. There is minimal data on its efficacy and safety in this patient population. Analyze the common indications for, findings of, and safety of FEES exam in breastfeeding infants. STUDY DESIGN: Case series. METHODS: Consecutive 10 months old or younger breastfed infants undergoing a FEES exam between June 1, 2012, and August 1, 2014, at a university-based, tertiary care urban children's hospital were studied. Common indication

Occurrences of yawn and swallow are temporally related.

Yawning is a stereotyped motor behavior characterized by deep inhalation and associated dilation of the respiratory tract, pronounced jaw opening, and facial grimacing. The frequency of spontaneous yawning varies over the diurnal cycle, peaking after waking and before sleep. Yawning can also be elicited by seeing or hearing another yawn, or by thinking about yawning, a phenomenon known as "contagious yawning". Yawning is mediated by a distributed network of brainstem and supratentorial brain regions, the components of which are shared with other airway behaviors including respiration, swallowi

Effect of swallowing exercises in independent elderly.

The aim of this study was to investigate the effects of swallowing exercises on oral function in independent elderly patients visiting the Department of Prosthodontics at Tokyo Dental College Chiba Hospital. Twenty-nine patients aged over 65 years (15 men and 14 women; mean age, 75.0 years) in whom treatment was completed at this college were enrolled in the study. The patients were instructed on how to perform swallowing exercises monthly (including at baseline) for the first 2 months from the start of the study and asked to perform those exercises at least once daily. At the beginning and en

Oesophageal rupture: a tough diagnosis to swallow.

An elderly gentleman presented with acute dyspnoea and right-sided pleuritic chest pain. Two-weeks previously an oesophageal stent had been inserted for dysphagia secondary to oesophageal carcinoma. With low PaO(2), a neutrophilic leucocytosis, raised inflammatory markers and a right-sided pleural effusion, antibiotics were prescribed for pneumonia. Computed tomographic pulmonary angiogram (CTPA) ruled out pulmonary embolus. The patient rallied transiently and his effusion improved. His respiratory distress returned 14 days later. A chest x-ray revealed a right-sided hydropneumothorax, and a c

Swallowing disturbance questionnaire for detecting dysphagia.

OBJECTIVES: To assess the accuracy of the swallowing disturbance questionnaire (SDQ) that had originally been designed and validated for detecting swallowing problems among patients with Parkinson's disease and was now applied for identifying patients with dysphagia associated with various other etiologies. STUDY DESIGN: Prospective. METHODS: One-hundred patients with the complaint of swallowing disturbances who underwent a full swallowing survey at the Tel-Aviv Voice and Swallowing Disorder Clinic participated. They all filled in the SDQ. The collected data included patient characteristics,

A swallowed sewing needle migrating to the liver.

CONTEXT: Swallowing foreign bodies is a common problem in children. Although most objects pass through the gastrointestinal tract with no untoward effect; long, sharppointed, or slender objects can perforate the gut. Migration of a swallowed object to the liver is extremely rare and very few cases have been reported in the literature up to now. The aim of this study is to draw attention to this subject once again by contributing a case report of a child with hepatic migration of a swallowed sewing needle. CASE REPORT: A 16-year-old girl presented to the emergency room of pediatrics department

[Swallowing disorders in the elderly: an underestimated problem].

Swallowing disorders are frequently observed in the elderly and often accompany an organic pathology. Such disorders may quickly constitute a threat for life: inhalation syndrome, pulmonary infection, undernutrition, dehydration. Diagnosing swallowing disorders is difficult and relies on anamnesis, clinical examination combined with paraclinical investigations. However, there is little place for medicinal or surgical treatment, their management being integral and multidisciplinary.

The Swallowing Centre: rationale for a multidisciplinary management.

The need for professional management of dysphagic patients is growing. The scenario of patient care settings spans from the acute ward to chronic care facilities or home, requiring a health care network able to integrate hospital and community resources and optimise human and instrumental resources. This is also valid for Swallowing Centres, where admission, management, treatment and follow-up of discharged patients are a priority. The complexity of symptoms and the specificity of the underlying disease require a multidisciplinary approach to the patient. The coordinator of the Swallowing Cent

[Swallowing phobia: symptoms, diagnosis and treatment].

INTRODUCTION: Choking phobia (or swallowing phobia) is characterized by a fear of swallowing foods, liquids or pills, sometimes after an episode of choking on food. METHODS: Forty-one case reports on swallowing phobia from 1978 to 2005 were studied. Clinical and therapeutic variables of the disorder were studied. RESULTS: It appears to occur more often in females (two-thirds of the cases) and has a high comorbidity with anxiety disorders (panic disorder, 41 %; obsessive conditions, 22 %, and separation anxiety, 15 %). Life-events and eating traumatic antecedents are frequently present (44% a

Anal pain secondary to swallowed bone.

SummarySwallowed foreign bodies can be a source of morbidity or rarely mortality since they can impact in the pharynx and the oesophagus and cause discomfort or even perforate to cause mediastinitis. Most (80% to 90%)1 foreign bodies including swallowed bone pass into the stomach, proceed through the intestines and are passed in stool without problems. Swallowed bone has been documented to cause intestinal perforation2, enterovesical fistula3 and perianal abscesses4. Two cases of swallowed bones, which passed through the alimentary tract to finally cause anal pain and anal fistula, are present

[Strategy for safe nutrition in swallowing disorders].

Swallowing deficits may affect the ability of patients to feed properly and sufficiently, therefore inducing malnutrition. In order to provide effective and durable treatment, a team of experts should evaluate the swallowing function of each individual patient. Treatment protocols must be carefully analyzed, and adapted to progression of the disease. We refer paradigmatically to patients with Huntington disease, although indications may be conveniently extended to other patients with impaired transit of food.

Interrater reliability of videofluoroscopic swallow evaluation.

The past two decades have brought an enormous widening of interest in and knowledge about swallowing disorders. The most frequently used technique for swallow evaluation is X-ray videofluoroscopy. Most interventions are based on this examination. Only a few studies assessing interobserver reliability of videofluoroscopy have been published. The aim of our study was to assess the interobserver reliability of videofluoroscopy for swallow evaluation. Fifty-one consecutive dysphagic patients referred for videofluoroscopy were entered into the study regardless of their underlying disorder. The firs

[Dietetic management of patients with impaired swallowing].

Swallowing deficits may be present in patients with neurological, otolaryngoiatric or internal medicine diseases. These deficits impair their feeding capability, thus inducing malnutrition. Diet therapy in these conditions involves specific instructions for properly choosing, handling and eating food, in order to allow patients to preserve their autonomy and safety while eating, thus preventing malnutrition. We refer paradigmatically to patients with Huntington disease, which is the main neurologic cause of swallowing deficits, although these guidelines may be conveniently extended to other pa
