Dorsal wedge calcaneal osteotomy as surgical treatment for insertional Achilles tendinopathy.

OBJECTIVE: The dorsal wedge calcaneal osteotomy aims to reduce the length of the calcaneus and elevate the insertion of the Achilles tendon, leading to a reduction of Achilles tension in its insertion. The purpose of this study is to assess the clinical and functional results of this osteotomy in patients with insertional Achilles tendinopathy and a high calcaneal inclination angle. MATERIAL AND METHODS: This is a retrospective study with 18 patients diagnosed with insertional Achilles tendinopathy, who were treated by dorsal wedge calcaneal osteotomy, excision of the wedge and fixation with

Hip abductor muscle activity during walking in individuals with gluteal tendinopathy.

The external hip adduction moment during walking is greater in individuals with gluteal tendinopathy (GT) than pain-free controls. Although this likely represents a greater demand on the hip abductor muscles implicated in GT, no study has investigated activation of these muscles in GT. For this purpose, fine wire electrodes were inserted into the segments of the gluteus minimus and medius muscles, and surface electrodes placed on the tensor fascia lata, upper gluteus maximus, and vastus lateralis muscles of eight individuals with, and eight without, GT. Participants underwent six walking trial

MMP-1 promoter genotype and haplotype association with posterior tibial tendinopathy.

PURPOSE: Posterior tibial tendon (PTT) is particularly vulnerable and its insufficiency is recognized as the main cause of adult acquired flatfoot. Some patients have a predisposition without clinically recognized cause, suggesting that individual characteristics play an important role in tendinopathy. The objective of the present study is to investigate the association of -519 (rs1144393) matrix metalloproteinase-1 (MMP-1) polymorphism and the -1607 (rs1799750) and -519 MMP-1 haplotypes and risk of PTT dysfunction. METHODS: The test group included 50 females who presented PTT dysfunction Gra

Transcriptional Adaptations to Muscle Loading in a Murine Model of Achilles Tendinopathy.

Achilles tendinopathy limits mobility and decreases quality of life. Physical therapy (eccentric muscle loading) improves tendon function; however, the underlying mechanisms are unknown. This study investigated the effect of load magnitude and treatment duration in a mouse Achilles tendinopathy model. We hypothesized that loading would upregulate signaling and metabolic transcriptional networks associated with improved tendon healing. Mice were randomly assigned to muscle loading groups (50 or 100% body weight (BW)) or age-matched injured/untreated (IU) and naïve control groups. Following indu

Overuse Injuries of the Hip and Pelvis: Apophysitis, Stress Fractures, and Tendinopathies.

Hip and pelvic overuse injuries are a frequent source of pain and lost playing time in athletes and may be challenging to diagnose because of overlapping symptoms and complex regional anatomy. Injury patterns vary according to skeletal maturity: in adolescents, open apophyses are particularly susceptible to traction-related apophysitis and avulsion injuries, whereas in skeletally mature individuals repetitive loading more often results in stress fractures and chronic tendinopathies. Clinical findings are often nonspecific, and misinterpretation may delay appropriate management or safe return t

Treatment of acute calcific tendinopathy of the hip using focused shockwave.

Calcific tendinopathy (CT) is a painful condition characterised by the formation of deposits of calcium hydroxyapatite within tendon insertions. This most often occurs in the shoulder but may occur at other sites such as the gluteus medius or minimus tendons. While extracorporeal shockwave treatment has been used in the management of shoulder CT, limited data exist for CT of the hip. Here we describe a man who developed sudden onset of right gluteal/posterior thigh pain. Imaging revealed prominent CT at the insertion of the gluteus maximus at the linea aspera. He was treated with focused shock

Targeting the IL-17A pathway for therapy in early-stage tendinopathy.

OBJECTIVES: Tendinopathy is a frequent clinical problem and represents an extraordinary health economic and socioeconomic burden with high unmet medical needs. Recent clinical evidence suggests blockade of interleukin 17A (IL-17A) for tendinopathy therapy. The present preclinical study elucidates the biological mechanisms of IL-17A pathway stimulation and blockade in tendinopathy. METHODS: We explored whether IL-17A and other IL-17 family members are differentially expressed in biopsies of healthy, early-stage and late-stage tendinopathic human rotator cuff tendons using RT-qPCR. IL-17 pathwa

Achilles Subtendons Stiffness Differ in People with and without Achilles Tendinopathy.

PURPOSE: Previous work showed altered mechanical properties of the Achilles tendon in the presence of tendinopathy, considering the Achilles tendon as a homogeneous structure with the gastrocnemius medialis (GM) subtendon representative of it. However, the Achilles tendon consists of three semi-independent structures: the GM, gastrocnemius lateralis (GL), and soleus (SOL) subtendons, each independently pulled by their respective muscle. The aim of this study was (i) to compare the mechanical properties of the different Achilles subtendons in humans in vivo by considering the force of each musc

Whipping or tearing? The biomechanics of Achilles tendinopathy in rearfoot strike runners.

BACKGROUND: Two biomechanical mechanisms for the development of Achilles tendinopathy in runners have been proposed: A whipping mechanism characterized by prolonged and excessive rearfoot eversion, and a tearing mechanism characterized by high eccentric plantar flexor forces. The purpose of this pilot study was to determine if runners with and without a history of Achilles tendinopathy exhibited gait biomechanics consistent with either of these mechanisms. METHODS: Seven male runners with previous or current Achilles tendinopathy and seven healthy male control runners were evaluated by three-

Impact of BMI on the Surgical Management of Chronic Insertional Achilles Tendinopathy.

Posterior heel pain secondary to insertional Achilles tendinopathy is a common condition that often times requires surgical management. Typically, this involves reflecting a portion of the Achilles tendon from its insertion to adequately debride devitalized or thickened tendon as well as any osseous prominence and then reattached into the calcaneus via suture anchors. Oftentimes, it is suggested that patients with an increased body mass index (BMI) have a higher risk of complications. However, there is no published evidence to support this claim. The present study is a retrospective review of

Clinical evaluation and management of calcific tendinopathy: an evidence-based review.

Calcific tendinopathy (CT) is an important musculoskeletal condition most commonly seen in the shoulder girdle, accounting for 10-42% of all shoulder pain. Despite the high prevalence within the shoulder region, CT has been demonstrated in many tendons throughout the axial and appendicular skeleton. Unlike degenerative tendinopathies, CT appears to be a self-limiting condition that affects otherwise-healthy tendon with deposition of calcium hydroxyapatite crystals between healthy tenocytes. In patients with functionally limiting symptoms or pain, the clinical course may be accelerated through

Insertional Achilles Tendinopathy: Analysis of 166 Procedures and Return to Activity.

The surgical procedure for Insertional Achilles tendinopathy (IAT) varies widely with no consensus as to approach and documenting return to activity (RTA). This study presents outcomes of surgery for IAT by a single surgeon, documenting activity level and return to activity. From January 2001 through January 2018, 166 procedures were reviewed. Surgery included debridement of the Achilles insertion, resection of the superior calcaneus and bursa, and re-attachment with suture anchors. There were 110 males and 56 females. The majority of patients in the entire cohort were runners (n = 54). The av

Minimally Invasive and Endoscopic Approach for the Treatment of Noninsertional Achilles Tendinopathy.

Minimally invasive treatment can offer an earlier recovery with less pain and scarring compared with traditional open surgeries. The goals of minimally invasive surgery are to debride degenerative tendon, stimulate healing, and, when appropriate, repair damaged tendon. Sclerotherapy and prolotherapy have been shown to reduce neovascularization and pain. Percutaneous stripping and endoscopic debridement are better options for diffuse tendinopathy. Plantaris release can be useful in diffuse disease in patients with primarily medial-sided Achilles pain. Overall, minimally invasive surgery provide
