Describing and monitoring quality in value-based palliative and end-of-life care at a population level in Wales: achievements and future direction.

Improving and monitoring quality of palliative and end-of-life care is well documented within current and historic literature. However, there is a paucity of evidence around national data-informed approaches to ensure healthcare services are delivering high-quality, equitable care that meets the needs of the population within the last year of life. Wales has a legal 'Duty of Quality' obligation and has taken on the challenge to measure and monitor quality within the healthcare system at a population level within the last year of life, creating a dashboard using nationally held routine data. Top Ten Tips Palliative Care Clinicians Should Know About Caring for Chinese American Patients.

The Chinese American population is one of the fastest-growing communities in the United States, composed of ∼5.4 million people, and represents ∼5.5% of overseas Chinese populations. With an expected exponential population rise, Chinese American patients who experience serious illness or approach end-of-life (EOL) may find their cultural values influencing the medical care they receive. Palliative care clinicians must recognize diverse cultural beliefs and preferences of Chinese American patients and their families. In this study, we provide 10 cultural pearls to guide the provision of palliat

Impact of a multifaceted strategy in end-of-life care in a tertiary hospital: A quasi-experimental study.

OBJECTIVE: To evaluate the impact of a multifaceted strategy for quality end-of-life care in a tertiary public hospital in Brazil. METHODOLOGY: The study design was quasi-experimental. The multifaceted strategy was applied between January and June 2017, and involved training the healthcare team in end-of-life discussions, the creation and documentation of advance directives, and consultation with the team specialized in palliative care. The periods analyzed were the pre-test period (Time 1, July 2015 to June 2016) and the post-test period (Time 2, July 2017 to June 2018). RESULTS: Time 1 inv

Pharmacological Interventions for Anxiety and Depression in Terminal Illness: A Scoping Review of Randomized Controlled Trials.

Anxiety and depression are highly prevalent among patients with chronic and progressive illnesses. At the terminal stage, these symptoms are closely linked with existential distress and can worsen quality of life. A scoping review of randomized controlled trials (RCTs) reporting on pharmacological interventions for anxiety and depression in terminal illnesses was conducted. MEDLINE, Embase, and CENTRAL databases were searched from inception to September 2024. Two reviewers independently extracted data on study design, population, interventions, and outcomes. Thirteen RCTs involving 621 partici

Relational-temporal dignity at the end of life: Ethical foundations for the DiRePal model in the algorithmic age.

OBJECTIVES: The increasing presence of artificial intelligence (AI), electronic patient-reported outcomes (ePROMs), and digital infrastructures in palliative care is transforming how clinical encounters are organized and how suffering is interpreted. These technological shifts heighten the risk of relational compression and a reduction of dignity to measurable outputs. This paper proposes the DiRePal model (Relational-Temporal Dignity in Palliative Care) as a philosophical framework to re-examine dignity beyond coherent narrative identity or autonomy-centered ethics, emphasizing relational pre

General practitioners and general practitioner registrars' experiences and learning from caring for dying patients: a scoping review.

BACKGROUND: The general practitioners' (GP) workforce is in crisis, with 22% of GPs feeling so stressed by the pressures of general practice they cannot cope. Patient death is the greatest stressor in medical practice and has an impact on the personal stress and well-being of doctors. Providing good holistic care for dying patients in the community keeps patients in their preferred place of care and reduces unnecessary costly interventions and hospital admissions. It is crucial to explore the experiences and learning of GP and general practitioner registrars' (GPR)-who represent the future pri

The principle of double effect in palliative care: a systematic review of ethical, legal, and clinical perspectives.

BACKGROUND: The Principle of Double Effect (PDE) has historically been used to justify medical interventions intended to relieve suffering, even when such treatments may carry a foreseeable risk of hastening death. In palliative care, the doctrine is particularly relevant to opioid administration and palliative sedation. However, its relevance and practical importance remain debated in contemporary clinical and ethical discourse. OBJECTIVE: This systematic review aims to examine how the Principle of Double Effect is interpreted and applied in palliative care literature, with a focus on ethica

Quality Measures for End-of-Life in Children With Heart Disease: A Modified Delphi Approach.

CONTEXT: How to define high quality end-of-life (EOL) care for children with advanced heart disease (AHD) is unknown. OBJECTIVES: We sought to develop expert-panel endorsed quality measures (QM) for EOL for pediatric AHD. METHODS: The Modified Delphi panel methodology was used to assess potential QM. Proposed QMs were derived from EOL QM in pediatric oncology, previously conducted bereaved parent interviews, and advisory board discussions. Panelists were nominated by professional organizations, research collaboratives, and national experts. The panel included 13 experts from pediatric cardio

Assessment and support of people experiencing problematic substance use at the end of life.

There is limited evidence relating to end of life care for people with a history of problematic substance use, despite a growing need for treatment related to an older opiate-using population and increased premature mortality associated with alcohol-related liver disease. This article examines several significant challenges for nurses in supporting people who have engaged in problematic substance use, often resulting in comorbidities such as kidney damage, respiratory conditions and/or chronic pain. These challenges include ensuring accurate assessment and screening and how to openly communica

Palliative Obstinacy: When Care Becomes a Constraint at the End of Life.

This paper develops the concept of "palliative obstinacy" to describe situations in which palliative care, rather than alleviating suffering in accordance with the patient's wishes and values, becomes a new form of clinical paternalism at the end of life. Drawing on a case narrated by Elyse Ona Singer and Norma Alicia Ordóñez-Vázquez of a patient with cancer requesting physician-assisted death, the paper argues that some models of palliative care unjustifiably pathologize the desire to die by treating it exclusively as a symptom of untreated suffering, depression, or inadequate care. Patterns for End-of-Life Care of Patients With Advanced Cancer: An Integrative Review.

BACKGROUND: The patterns of care for patients with advanced cancer in the last week of life are critical for ensuring quality and dignity during a vulnerable time. As patients approach end of life, the focus typically shifts from aggressive treatment to palliative care, emphasizing comfort and symptom management. The term "non-comfortoriented care" describes a set of medical interventions and treatments that prioritize prolonging life. This can lead to increased physical discomfort, emotional burden, or reduced quality of life. AREAS OF UNCERTAINTY: For those who die in a medical setting, end

Demystifying dying in end-of-life-care: A phenomenological perspective.

This article focuses on experiences of the process of dying. The empirical material consists of interviews with patients in palliative care and their significant others. The analysis draws attention to the lived experiences of the participants - embedded in time, identities, social relations, and everyday lives - and to how the possibility of sense-making is conditioned by the lifeworld. Guided by this analysis, the article concludes that professionals can offer to help patients and significant others, as end of life may entail multi-faceted suffering, but this needs to be understood as reorie

Integrating Palliative Care and Hematologic Malignancies: Bridging the Gaps for Our Patients and Their Caregivers.

Patients with hematologic malignancies (HMs) struggle with immense physical and psychological symptom burden, which negatively affect their quality of life (QOL) throughout the continuum of illness. These patients are often faced with substantial prognostic uncertainty as they navigate their illness course, which further complicates their medical decision making, especially at the end of life (EOL). Consequently, patients with HM often endure intensive medical care at the EOL, including frequent hospitalization and intensive care unit admissions, and they often die in the hospital. Our EOL hea

Reflections on an Evidence Review Process to Inform the Co-Design of a Toolkit for Supporting End-of-Life Care Planning With People With Intellectual Disabilities.

INTRODUCTION: There is growing recognition that healthcare inequalities faced by people with intellectual disabilities extend to their experiences at the end of life, resulting in calls for more inclusive research to help address these inequities. Our study aimed to address this through the co-design of a toolkit for supporting end-of-life care planning with people with intellectual disabilities. To inform the co-design process, we undertook an evidence review to identify existing tools, resources and approaches that were already being used in practice. METHODS: Our evidence review comprised

Anticoagulation at the end of life: whether, when, and how to treat.

Nearly 2 out of 3 patients with venous thromboembolism (VTE) and 1 out of 4 patients with atrial fibrillation (AF) will die within the year. Whether, when, and how to manage anticoagulation at the end of life requires many trade-offs. Patients and clinicians must balance symptom burden, greatly elevated bleeding and thrombosis risks, competing comorbidities and medications, and changing goals over time. This review uses cases of VTE and AF to present a framework for care that draws upon existing disease-specific data and cutting-edge palliative care science. It reviews strategies for the diffi

Thirst or dry mouth in dying patients?-A qualitative study of palliative care physicians' experiences.

INTRODUCTION: Thirst and dry mouth are common symptoms among patients at the end of life. In palliative care today, there is a focus on mouth care to alleviate thirst. There are no qualitative studies on thirst from a physician's experience, which is why this study is needed. PURPOSE: This study aimed to explore palliative care physicians' experiences and views of thirst in patients at the end of life. METHODS: A qualitative interview study with an inductive approach was carried out. Sixteen physicians working in specialised palliative care units in Sweden were included. The interviews were

"There is No Communication": A Qualitative Examination of Deaf Signers' Experiences With Advance Care Planning.

Objective(s): Deaf American Sign Language (ASL) users are subject to health care disparities resulting from communication and language barriers. Currently, few resources on advance care planning and end-of-life care exist in ASL. This study explores Deaf ASL users' perceptions and experiences with end-of-life care and advance care planning. Methods: Semi-structured 1:1 interviews with Deaf signers were translated, and transcribed into English by a bilingual researcher. Investigators inductively coded transcripts and identified themes of barriers, facilitators, knowledge, and sources of informa

Go Wish card game-exploring end-of-life wishes of patients in oncology palliative care: a qualitative study.

BACKGROUND: Among the methods that promote quality of life and care, discussing and remembering end-of-life (EOL) wishes for future care may contribute to decision-making about care and the promotion of a good death. Our aim was to investigate the most significant EOL desires among Brazilian cancer patients receiving palliative care (PC). METHODS: This was an exploratory, descriptive, and qualitative study conducted in a Palliative Care Oncology Unit. Fifteen patients played the Go Wish card game (GWCG), choosing and categorizing cards into themes as very important, more or less important, an
