Impaired warm and cold perception thresholds in leprosy skin lesions.

The aim of the present study was to determine the frequency of alteration in warm perception thresholds (WPT), cold perception thresholds (CPT) and the warm and cold perception interval (WCPI) in leprosy-suspected skin lesions, and to determine if these tests could assist in the diagnosis of leprosy. Tests were conducted using a thermal sensory analyser TSA-2001 (Medoc Ltd, Israel) and the method of levels. A cross-sectional study of 112 patients presenting leprosy-suspected skin lesions ('patch'), with no clinical evidence of peripheral nerve damage, was conducted. Leprosy diagnosis was based

Enzyme replacement therapy and intraepidermal innervation density in Fabry disease.

We prospectively evaluated the effect of enzyme replacement therapy (ERT) on the intraepidermal nerve fiber density (IENFD) and thermal threshold in patients with Fabry disease, an X-linked disorder associated with a painful small-fiber neuropathy and decreased linear IENFD in a length-dependent pattern. Twenty-five hemizygous male patients with Fabry disease were enrolled in a 6-month, randomized, placebo-controlled ERT trial of 0.2 mg/kg of alpha-galactosidase A (agalsidase-alfa) every 2 weeks followed by an additional 12 months of open-label ERT for both populations. IENFD and thermal thres

Mechanosensory and thermosensory changes across the border of impaired sensitivity to pinprick after mandibular nerve injury.

PURPOSE: The study goal was to determine how sensory function varies across the border of impaired sensitivity to pinprick in patients with mandibular nerve injuries. PATIENTS AND METHODS: Borders of decreased sensitivity to pinprick were mapped in 15 patients who reported altered sensation. Four mechanoreceptive, 2 thermoreceptive, and 2 thermonociceptive functions were studied at 5 sites separated by 0.6 cm across the border. The tests were repeated to evaluate day-to-day consistency in the pattern of variation for each sensory measure. RESULTS: The estimates of sensory function were not f

Evaluating physiological strain during cold exposure using a new cold strain index.

A cold strain index (CSI) based on core (T(core)) and mean skin temperatures (T(sk)) and capable of indicating cold strain in real time and analyzing existing databases has been developed. This index rates cold strain on a universal scale of 0-10 and is as follows: CSI = 6.67(T(core t) - T(core 0)). (35 - T(core 0))(-1) + 3.33(T(sk (t)) - T(sk 0)). (20 - T(sk 0))(-1), where T(core 0) and T(sk 0) are initial measurements and T(core t) and T(sk t) are simultaneous measurements taken at any time t; when T(core t) > T(core 0), then T(core t) - T(core 0) = 0. CSI was applied to three databases. The

The effect of peripheral glycerol on trigeminal neuropathic pain examined by quantitative assessment of abnormal pain and sensory perception.

In nine patients with trigeminal neuropathic pain after nerve injury, we examined prospectively the effect of peripheral glycerol neurolysis on abnormal pain and sensory perception. In the painful facial skin area of these patients, we found increased temperature and tactile thresholds and the presence of abnormal temporal summation of pain. In seven patients, neuropathic pain was peripheral and disappeared after application of local anaesthesia at or proximal to the site of nerve injury. Neuropathic pain was central in two patients, and unresponsive to local anaesthesia applied proximal to th

Sensory abnormalities in consecutive, unselected patients with central post-stroke pain.

This study examined the sensory abnormalities in an unselected, consecutive group of patients with central post-stroke pain (CPSP) surviving more than 1 year after stroke. The sensory examination included clinical examination and quantitative measures with detection and pain thresholds to heat and cold stimuli, argon laser, von Frey hair and determination of stimulus-response function in the 10-45 degrees C range. Sensory examination was in 11 identified CPSP patients (5 female, 6 male; aged 43-80 years) carried out in the painful area using the contralateral homologue area as reference. Pain

Somesthetic sensitivity in young and elderly humans.

Absolute thresholds were measured on 27 young (ages 19 to 31) and 21 elderly (ages 55 to 84) humans to six modes of cutaneous stimulation (single ramp-and-hold skin indentations--tactile, vibration at 40 and 250 Hz, temperature increases and decreases, and noxious heat) at two sites, the thenar eminence and the plantar foot. Comparisons of the elderly and young groups showed that elderly persons were significantly, p less than or equal to .001, less sensitive than young individuals to mechanical stimuli (tactile and vibration) at both sites. No significant differences were found in thresholds

The antinociceptive effects of stimulating the pretectal nucleus of the rat.

Changes in the tail-flick latency to noxious heat were studied following electrical stimulation of the dorso-medial thalamus of the rat. Brief (15 sec), low intensity (35 microA) stimulation of the anterior pretectal nucleus caused no escape behavior or motor deficits but increased tail-flick latency for more than 45 min. Responses to non-noxious stimuli were enhanced but the animals were not hyperactive. The anterior pretectal nucleus does not receive retinal or accessory visual inputs like other parts of the pretectal complex but is known to receive axons from somatosensory cortex and projec

TRPV1 activity and substance P release are required for corneal cold nociception.

As a protective mechanism, the cornea is sensitive to noxious stimuli. Here, we show that in mice, a high proportion of corneal TRPM8+ cold-sensing fibers express the heat-sensitive TRPV1 channel. Despite its insensitivity to cold, TRPV1 enhances membrane potential changes and electrical firing of TRPM8+ neurons in response to cold stimulation. This elevated neuronal excitability leads to augmented ocular cold nociception in mice. In a model of dry eye disease, the expression of TRPV1 in TRPM8+ cold-sensing fibers is increased, and results in severe cold allodynia. Overexpression of TRPV1 in T

Evidence of small-fiber neuropathy in neurofibromatosis type 1.

INTRODUCTION: Large-fiber neuropathy is rare in neurofibromatosis type 1, but small-fiber neuropathy has not been studied. METHODS: Patients with neurofibromatosis type 1 underwent nerve conduction studies for large-fiber assessment. Small-fiber tests included quantitative thermal thresholds, laser Doppler flare imaging, intraepidermal nerve fiber density, and corneal nerve fiber length. RESULTS: Of the 52 patients enrolled, 31 (60%) were female and the mean age was 33.0 ± 12.3 years. Four (8%) patients had abnormal nerve conduction studies. Small-fiber tests were frequently abnormal: therma

[Quantitative Sensory Testing in the facial area: a review].

Quantitative Sensory Testing is an established method to evaluate somatosensory function. In the facial area, the procedures depend on the localisation of disorders and the modalities of interest. The test stimuli are of thermal or mechanical nature (touch, pain, vibration, or pressure stimuli). According to the protocol of the German Neuropathic Pain Network, comprehensive information on the function of afferent nerves can be generated in the facial area as well. Standard values have been obtained for the cheek and intraoral mucosa. For various orofacial pain conditions, studies concerning th

Thermal and vibratory thresholds after liposuction in patients with Dercum's disease.

Dercum's disease is characterised by pronounced pain in the adipose tissue and a number of associated symptoms and is, in most patients, accompanied by obesity. Postoperative sensory change after liposuction is a well-known side effect, and probably caused by mechanical trauma to the nerves. The aim of this investigation was to find out whether the thermal and sensory sensation changed after liposuction in patients with Dercum's disease. The thermal and vibratory thresholds were examined preoperatively, and three and 12 months after, liposuction in 39 affected patients. There were only small d

Quantitative sensory testing in trigeminal nerve damage assessment.

Evaluating sensory nerve damage is a challenging and often frustrating process. Diagnosis and follow-up is usually based on the patient's history and gross physical evaluation in addition to simple sensory tests such as brushing or pin prick. Based on evidence accumulated from clinical and animal experiments, quantitative sensory testing (QST) has emerged as a useful tool in the assessment of sensory nerve damage. QST has demonstrated diagnostic capabilities in temporomandibular disorders, burning mouth syndrome, oral malignancies, numb chin syndrome, posttraumatic pain, and whiplash injuries,

Recovery of function in adjacent nerve roots after surgery for lumbar disc herniation: use of quantitative sensory testing in the exploration of different populations of nerve fibers.

Results from experimental and clinical studies indicate that adjacent nerve roots may be affected in sciatica because of lumbar disc hemiation. This may be caused by proinflammatory mediators in the epidural space being transported into nerve roots at the same or neighboring lumbar segments. The aim of the present study was to investigate the recovery of function of sensory nerve fibers in the adjacent noncompressed nerve roots. Thirty-nine patients undergoing microdiscectomy for monoradiculopathy were investigated with quantitative sensory testing immediately before surgery, and at 6 weeks, 4

Preliminary study of low-level laser for treatment of long-standing sensory aberrations in the inferior alveolar nerve.

PURPOSE: The incidence of inferior alveolar nerve (IAN) damage during removal of third molar teeth has been reported to be as high as 5.5% and up to 100% during sagittal split osteotomy. Sensory aberrations in the IAN persisting for longer than 6 months leave some degree of permanent disability. The purpose of this double-blind, clinical trial was to examine the effects of low-level laser (LLL) treatment using a GaAIAs laser (820 nm, Rønvig, Denmark) on touch and temperature sensory perception after a long-standing postsurgical IAN injury. PATIENTS AND METHODS: Thirteen patients were divided

Air velocity influences thermoregulation and endurance exercise capacity in the heat.

This study examined the effects of variations in air velocity on time to exhaustion and thermoregulatory and perceptual responses to exercise in a hot environment. Eight male volunteers completed stationary cycle exercise trials at 70% peak oxygen uptake until exhaustion in an environmental chamber maintained at 30 °C and 50% relative humidity. Four air velocity conditions, 30, 20, 10, and 0 km/h, were tested, and the headwind was directed at the frontal aspect of the subject by 2 industrial fans, with blade diameters of 1 m and 0.5 m, set in series and positioned 3 m from the subject's chest.

Tactile cues significantly modulate the perception of sweat-induced skin wetness independently of the level of physical skin wetness.

Humans sense the wetness of a wet surface through the somatosensory integration of thermal and tactile inputs generated by the interaction between skin and moisture. However, little is known on how wetness is sensed when moisture is produced via sweating. We tested the hypothesis that, in the absence of skin cooling, intermittent tactile cues, as coded by low-threshold skin mechanoreceptors, modulate the perception of sweat-induced skin wetness, independently of the level of physical wetness. Ten males (22 yr old) performed an incremental exercise protocol during two trials designed to induce
