Combined internal and external osteosynthesis a biological approach to the treatment of complex fractures of the proximal tibia.

Lateral plating was combined with external fixation to treat 18 complex, proximal, tibial fractures with severe soft tissue injury. All patients were followed up until functional restoration of the limb had been achieved. In 15 cases, bone healing was uneventful. One deep infection without knee joint arthritis, one delayed union and one malunion necessitated revision. Pin track complications were negligible and soft tissue breakdown was not observed. The short-term results regarding the functional outcome showed a painless condition and good function in all patients. The method of fixation des

A new antigen retrieval technique for human brain tissue.

Immunohistochemical staining of tissues is a powerful tool used to delineate the presence or absence of an antigen. During the last 30 years, antigen visualization in human brain tissue has been significantly limited by the masking effect of fixatives. In the present study, we have used a new method for antigen retrieval in formalin-fixed human brain tissue and examined the effectiveness of this protocol to reveal masked antigens in tissues with both short and long formalin fixation times. This new method, which is based on the use of citraconic acid, has not been previously utilized in brain

Comparative Evaluation of APAFIX and 10% Formalin for Human Histologic Tissue Preservation: A Pilot Study.

Formalin has long served as the standard fixative in histopathology; however, its limitations-such as antigen masking, nucleic acid degradation, and carcinogenicity-have prompted the search for safer alternatives. APAFIX, a formalin-free fixative, was developed to maintain diagnostic integrity while improving safety in routine tissue handling. This pilot study was conducted to compare the efficiency of the APAFIX solution with formalin in preserving human histologic tissue architecture and cellular morphology as assessed by routine Hematoxylin and Eosin (H and E) staining. Improvement and development of immersion fixatives for enhanced tissue preservation in the whole eyeball specimens.

The wall of eyeball limits the penetration of the common fixatives, raising inadequate fixation or degeneration of internal tissue and posing challenges for preparing thin sections of the whole eyeball. In addition, many formaldehyde-containing fixatives exhibit the strong cross-linking effect that can excessively mask antigenic epitopes, thereby hampering immunological analyses. This study tried to develop an available fixative that exerts high permeability to mouse whole eyeballs and applicability to immunostaining. Glyoxal is known to have milder cross-linking capacity compared to formaldeh

The effects of formalin fixation on the detection of apoptosis in human brain by in situ end-labelling of DNA.

The technique of DNA in situ end-labelling (ISEL) for the detection of apoptotic cells has recently become the method of choice. The incorporation of a labelled nucleotide to facilitate detection into the single-stranded region of DNA cleaved by endogenous nucleases has proved to be a sensitive and straightforward technique. Previous reports have applied the technique to the study of apoptotic cells in brain tissue, which is normally subjected to relatively long-term formalin fixation. In this study we have examined the effects of long-term formalin fixation on the ability to detect apoptosis

Multiacquisition Variable-Resonance Image Combination Magnetic Resonance Imaging to Study Detailed Bone Apposition and Fixation of Cementless Knee System Compared to Cemented Total Knee Replacements.

BACKGROUND: The ability to utilize magnetic resonance imaging (MRI) to assess bony fixation may allow a better understanding of implant design and longevity. A new cementless total knee arthroplasty (TKA) was introduced, and we hypothesized that this cementless system would show similar fixation compared to a cemented system as assessed by multispectral MRI. METHODS: Multiacquisition variable-resonance image combination selective MRI was performed in 20 patients implanted with a cementless TKA. A matched control group of 20 patients who had a cemented TKA was also evaluated. Each patellar, fe

Immunohistologic techniques for detecting the glycolipid Gb(3) in the mouse kidney and nervous system.

Shiga toxin-producing Escherichia coli causes hemolytic uremic syndrome, a constellation of disorders that includes kidney failure and central nervous system dysfunction. Shiga toxin binds the amphipathic, membrane-bound glycolipid globotriaosylceramide (Gb(3)) and uses it to enter host cells and ultimately cause cell death. Thus, cell types that express Gb(3) in target tissues should be recognized. The objective of this study was to determine whether immunohistologic detection of Gb(3) was affected by the method of tissue preparation. Tissue preparation included variations in fixation (immers

Is there a place for external fixation in humeral shaft fractures?

There is a good indication for unilateral axial dynamic external fixation in fractures of the humeral shaft when the fracture appears in the distal third or in cases of bilateral fractures. A non-union or a posttraumatic paralysis of the radial nerve may be indications for external fixation as well as fractures associated with multiple injuries. Further indications include osteitis, infected non-union and comminuted fracture. There is maximum protection of the soft tissue with this method of treatment. External fixation combines the advantages of conservative and operative treatment by influen

Handgun injuries with metacarpal and proximal phalangeal fractures: early definitive treatment.

Most gunshot injuries to the hand involve a combination of tissue types. The goal of this study is to report the results of early definitive treatment in extra-articular metacarpal and proximal phalangeal fractures due to low velocity gunshot wounds and to analyse their outcomes. A retrospective analysis of 51 metacarpal and 41 proximal phalangeal fractures of 76 patients due to low velocity gunshot wounds treated between January 2001 and December 2004 was carried out. We applied acute fixation in the first 24 hours. The patients were evaluated with total active motion scores, radiographic con

Splinting vs temporary external fixation in the initial treatment of ankle fracture-dislocations.

BACKGROUND: To compare the efficacy, functional outcome, and complication frequency of splinting and external fixation in the initial treatment of ankle fracture-dislocations. METHOD: Ankles with poor soft tissue conditions who underwent temporary stabilization due to using a splint or external fixator due to an ankle fracture-dislocation between 2012 and 2019 were retrospectively evaluated. Ankles were divided into two groups as the splint (n=69) and external fixator (n=48). The time between the injury to definitive surgery, reduction loss, operation time, functional outcome, pain, and soft-

[The periodontium in experimental bruxism].

Time course of morphologic changes in periodontal tissue was studied in 42 miniature pigs with experimental bruxism. An inflammation has developed in the marginal periodontium. At first the inflammatory infiltrate was situated under the epithelial fixation to the tooth involving destruction of the epithelial fixation and formation of pockets, inflammatory infiltrate, and granulation tissue, these destructive changes involving the lower sections of the periodontium. Signs of resorption were detectable in the dental alveolus osseous wall, cement, and dentin. The changes in the periodontal tissue

Is staged external fixation a valuable strategy for war injuries to the limbs?

UNLABELLED: High-energy weapons or blast injuries usually result in substantial tissue damage and are serious medical and public health problems. We report our experience with staged external fixation for war injuries to the extremities. Forty-seven patients with 64 high-energy limb fractures caused by war weapons were retrospectively reviewed. The fractures were associated with severe soft tissue damage. There were 14 Gustilo-Anderson Type IIIA fractures, 40 Type IIIB fractures, and 10 Type IIIC fractures. Soft tissue débridement followed by axial realignment of the fractured bones with immed

Clinical and Radiological Outcome of Complex Proximal Tibia Fractures Managed with Hybrid/Ilizarov Fixator with or without Minimal Internal Fixation.

BACKGROUND: Complex high grade proximal tibia fractures with associated extensive soft tissue injury pose a management challenge. The timing of surgery and fracture fixation options depend upon the extent of soft tissue damage. Post-operative complications such as wound breakdown, infection and infected non-union are common in such cases managed early with open reduction and internal fixation. Such fractures can be treated with primary closed reduction and Ilizarov/hybrid fixator application. MATERIALS AND METHODS: It is a retrospective cross sectional study conducted at two tertiary care mul

Percutaneous Treatment of Geriatric Distal Tibia and Fibula Fractures With a Poor Soft Tissue Envelope - A Case Series.

PURPOSE: Geriatric distal tibia and fibula fractures are typically treated with open reduction and internal fixation (ORIF) or prolonged cast immobilisation. These methods predispose to wound complications and delayed weight-bearing. Percutaneous fixation can offer a stable construct allowing for early weight-bearing while minimising wound complications. This case series details the management and outcomes of patients over 65 with distal tibia and fibula fractures and poor soft tissue envelope treated with percutaneous fixation. METHODS: A prospective observational study was conducted at two

Ring-and-flap: a novel and accessible alternative to fix-and-flap in Gustilo IIIB-C open tibia fractures (an institutional experience).

BACKGROUND: The fix-and-flap regime is resource intense and may not be readily available. As an alternative, the ring fixator serves as a versatile option for limb salvage, however it poses challenges to soft tissue reconstruction due to the limited working space around the rings and pins. We describe an institutional experience with a novel ring-and-flap regime utilising an open ring technique-which optimises the ring fixator construct for the flexibility of interval soft-tissue reconstruction. METHODS: A retrospective review of patients treated with a novel ring-and-flap regime over 2017-20

Open reduction and internal fixation of tibial pilon fractures.

Although it is evident that the fracture of the tibial plafond is a complex, often debilitating injury, its management is not clear. These injuries generally fall into one of two categories. The low-energy, rotational type of fracture has been shown to have excellent clinical and functional results with open reduction and internal fixation. The high-energy, compression type of fracture has had uniformly moderate results and historically high complication rates. Some authors think that bridging external fixation with or without limited internal fixation should be employed in high-energy fractur

Antegrade Kirschner Wire Transfixation of Interphalangeal and Metacarpophalangeal Joint in a Comminuted Thumb Proximal Phalangeal Fracture.

Comminuted proximal phalangeal fractures of the thumb are inherently unstable injuries requiring surgical fixation to provide stability and restore function. Current fixation methods include plate and screw fixation, which is associated with stiffness (in 64% of cases), extensor lag, and hardware complications requiring further operations. Moreover, intramedullary screws carry the risk of cartilage damage and fracture displacement during insertion. These modalities requiring extensive soft tissue dissection correlate with poor functional outcomes due to fibrosis and vascular disruption. A simp

Extra-articular proximal tibial fractures: nail or plate?

The surgical goals for treating proximal tibial fractures are to restore articular congruity, the mechanical axis, and knee motion while avoiding soft-tissue complications. The fracture pattern should be correctly identified and understood. For fractures with minimal intra-articular extension, fracture fixation with an intramedullary nail can decrease the risk of infection because it uses a small incision that is not placed directly over the injured soft tissue, and it provides better axial load sharing than a plate. Using the semi-extended technique, choosing the correct starting portal, inco

The multi-ligament ankle fracture: Epidemiology, key anatomical findings and fixation strategies in unstable open injuries.

BACKGROUND: Ankle fracture surgery has traditionally focussed on restoration of bony anatomy, with fixation of the malleoli and syndesmotic stabilisation where applicable. However, high energy open fracture-dislocations can also result in periosteal stripping of the stabilising capsuloligamentous attachments. As such, restoration of osseous anatomy alone may not result in sufficient articular stability. OBJECTIVES: In this series from a level 1 trauma centre, we report a subset of highly unstable open ankle fractures with combined capsuloligamentous injuries, in which restoration of osseous a
