Comparing the Outcomes of Suture Anchor Repair and Rein-Type Capsular Suture for Triangular Fibrocartilage Complex Foveal Tears With a Minimum 2-Year Follow-Up.

PURPOSE: Recent biomechanical studies have highlighted the importance of foveal reinsertion when repairing triangular fibrocartilage complex (TFCC) injury with foveal tears. However, clinical studies comparing different repair techniques are scarce. We compared the clinical outcomes of suture anchor repair and rein-type capsular suture in patients with TFCC palmer 1B foveal tears with a minimum of 2-year follow-up. METHODS: This was a single-surgeon, single-center, retrospective, comparative study. We included patients who underwent TFCC repair surgery due to a foveal tear from December 2013

Distal radioulnar joint kinematics before surgery and 12 months following open foveal reinsertion of the triangular fibrocartilage complex: comparison with the contralateral non-injured joint.

BACKGROUND AND PURPOSE: Foveal triangular fibrocartilage complex (TFCC) lesion may cause distal radioulnar joint (DRUJ) instability. Dynamic radiostereometry (dRSA) has been validated for objective measurement of DRUJ kinematics. We evaluated DRUJ kinematics by dRSA before surgery and 12 months following open foveal reinsertion of the TFCC in comparison with contralateral non-injured DRUJs. PATIENTS AND METHODS: In a prospective cohort study, 21 patients (11 men) of mean age 34 years (22-50) with arthroscopically confirmed foveal TFCC lesion were evaluated preoperatively, and at 6 and 12 mont

Injury to the dorsal sensory branch of the ulnar nerve in the arthroscopic repair of ulnar-sided triangular fibrocartilage tears using an inside-out technique: a cadaver study.

This anatomic study of the commonly described inside-out Tuohy needle technique was performed to better define the course of needle passage relative to the anatomic structures in this region including the dorsal sensory branch of the ulnar nerve (DBUN) and extensor carpi ulnaris (ECU) tendon. Ten fresh-frozen cadaver specimens had arthroscopic-guided passage of a Tuohy needle through the triangular fibrocartilage (TFC). Dissection of the ulnar side of the wrist was performed and various measurements were recorded. The average minimum distance between suture A (the suture closest to the nerve)

Role of Direct MRI Arthrography by Combined Distal Radioulnar and Radiocarpal Compartment Contrast Injection Over Conventional MRI in Detection and Differentiation of Triangular Fibrocartilage Complex Injuries.

BACKGROUND: Wrist pain is a prevalent and diagnostically complex issue, often stemming from subtle injuries to the triangular fibrocartilage complex (TFCC). Due to the small size and intricate structure of the wrist ligaments, conventional MRI may not reliably detect all TFCC tears, especially peripheral or partial injuries. Direct magnetic resonance arthrography (MRA) has emerged as a superior imaging modality by improving contrast resolution and visualizing joint integrity more precisely. OBJECTIVE: To evaluate the diagnostic value of direct MR arthrography using combined radiocarpal (RC) a

Outcomes of Below-Elbow Versus Above-Elbow Immobilization in the Management of a Partial Triangular Fibrocartilage Complex Tear Without Distal Radio-Ulnar Joint Disruption: A Retrospective Study.

Background The triangular fibrocartilage complex (TFCC) is a crucial biomechanical structure that supports the distal radioulnar joint (DRUJ), forearm rotation, and load transfer along the ulnar aspect of the wrist. Conservative treatment is the preferred approach for managing stable TFCC injuries, but the techniques of immobilization remain a topic of debate. This study aims to evaluate whether above-elbow immobilization provides better pain relief, functional recovery, and fewer complications than below-elbow immobilization in the conservative treatment of partial TFCC tears without DRUJ dis

Editorial Commentary: Symptomatic Recurrent Instability of the Distal Radioulnar Joint After Triangular Fibrocartilage Complex Repair in Patients With Ulnar-Sided Wrist Pain May Be Related to Generalized Laxity.

An unstable distal radioulnar joint can cause ulnar-sided wrist pain. Stability of the distal radioulnar joint, which varies among individuals, is dependent on the bony structure of the sigmoid notch and the surrounding soft tissues, such as the interosseous membrane, the extensor carpi ulnaris tendon, the pronator quadratus, and the deep radioulnar ligaments. In particular, avulsions of the deep radioulnar ligaments lead to instability and disturbed wrist proprioception. In patients with ulnar-sided pain, a distiction must always be made between clinical instability and generalized laxity. Tr

Designing a 20 mm incision to protect the dorsal branch of the ulnar nerve during arthroscopic repair of triangular fibrocartilage complex injuries: Cadaver study and preliminary clinical results.

The ulnar-sided approach for arthroscopic peripheral triangular fibrocartilage complex (TFCC) repair may be associated with injury to the dorsal branch of the ulnar nerve (DBUN). The goal of this study was to develop a small incision to help minimize DBUN injury. Ten cadaveric upper limbs were used to measure the anatomic parameters of the DBUN. Based on these measured anatomical relationships, a 20 mm longitudinal incision with the ulnar styloid process as the midpoint was designed to explore and protect the DBUN. Three additional cadaveric upper limbs were used to test the feasibility of thi

Comparison of the clinical and functional outcomes of two immobilisation protocols after arthroscopic peripheral triangular fibrocartilage complex (TFCC) repair in adults: a single-centre, double-blinded randomised controlled trial protocol.

INTRODUCTION: Injury to triangular fibrocartilage complex (TFCC) is a common cause of ulnar-sided wrist pain, of which peripheral TFCC tears are amenable to repair. The surgical approaches to treat TFCC tears are well-established, with arthroscopic or arthroscopic-assisted repair as the preferred method. However, the postoperative rehabilitation protocols significantly vary across different studies, ranging from 2 to 9 weeks, often without sufficient justification. METHODS AND ANALYSIS: This research is designed to conduct a randomised controlled trial at a single centre with double-blinding

Editorial Commentary: In Cases of Concurrent Triangular Fibrocartilage Complex Tears and Ulnar Styloid Process Fracture Nonunions, We May Neither Need to Excise nor Repair the Ulnar Styloid Fracture Nonunion Fragment.

The ulnar-sided wrist contains multiple potential pain generators that may present in isolation. Occasionally, however, wrist trauma results in multiple concurrent and overlapping injuries that make diagnosis and treatment of these conditions challenging. Deep/foveal tears of the triangular fibrocartilage complex (TFCC) may occur in the setting of nonunited ulnar styloid process fractures. Treatment of these injuries has historically included open TFCC repair with fixation or excision of the ulnar styloid fracture nonunion fragment; however, recent literature suggests that addressing the ulnar
