[Laparoscopic resection of a gastric giant cavernous hemangioma in a child].

BACKGROUND: Gastric hemangioma rarely appears during the pediatric age, with only thirteen cases in the medical literature. Its manifestations are digestive bleeding and intestinal obstruction. The bleeding can be severe and for such reasons it is usually necessary to apply surgical procedures. Up until now every case had been treated with resection and reconstruction through open surgery. We present the case of a patient treated successfully through a laparoscopic approach. CLINICAL CASE: 7 year old boy presenting hematemesis and hypovolemic shock, after stabilizing the patient a digestive e

[Preoperative care and surgical techniques in the varicose disease of the lower extremities complicated by trophic ulcers].

The principles of preoperative management of patients with varicosity complicated by trophic ulcers are discussed. The main type of treatment is surgery including subfascial ligation of the perforating veins of the thigh, leg, and foot+phlebectomy after Troyanov-Trendelenburg-Bebcock-Narath. The author suggests or modifies approaches to the perforating veins. The basis of preoperative management is formed of measures causing a general effect on the organism (lysozymo--and immunoglobulinotherapy, stimulation of regeneration by means of zinc sulphate, transfusion of albumins, etc.), local treatm

Novel Application of an Ultrasonic Bone Aspirator for Endoscopic Modified Medial Maxillectomy.

BACKGROUND: Endoscopic modified medial maxillectomy (EMMM) is a surgical technique developed to approach maxillary sinus lesions, such as papilloma and postoperative maxillary cyst, while preserving the postoperative nasal morphology and nasal function. In this technique, a diamond burr is used to remove the bone, which may damage adjacent soft tissue. We developed EMMM using an ultrasonic bone aspirator (UBA) instead of a conventional diamond burr. The purpose of this study was to clarify the effectiveness of the UBA in EMMM in comparison to the conventional diamond burr technique in terms of

Urorectal fistulae following the treatment of prostate cancer.

UNLABELLED: Study Type--Therapy (case series) Level of Evidence 4. What's known on the subject? and What does the study add? It is known that urorectal fistulae is a serious but rare complication of the treatment of carcinoma of the prostate. This study adds a distinction between post-surgical fistulate and post-irradiation fistulae. Essentially most post-surgical fistulae are simple and relatively easily dealt with: the expectation is that the patient will return to normality; whereas post-irradiation fistulate are by their nature complex and much more difficult to treat with a much more unce

Dissection with harmonic scalpel versus cold instruments in parotid surgery.

BACKGROUND: The harmonic scalpel (HS) has been used successfully in several head and neck surgical procedures. Some authors highlighted its advantages in reducing operative time, blood loss, and damages to surrounding tissue. In our study, we compared the results obtained during parotidectomy using the HS with the traditional approach to determine the benefits of the HS. METHODS: 130 patients with benign parotid tumors were enrolled and randomized into two groups for this prospective study. 63 patients underwent HS parotidectomy, and 67 patients received a parotidectomy using cold instruments

Laparoscopic Left Lateral Segmentectomy for Symptomatic Hepatic Cysts: A Case Series.

Large hepatic cysts can cause abdominal pain, pressure symptoms, or liver dysfunction. Although laparoscopic fenestration is the standard surgical approach, recurrence remains a concern. As laparoscopic hepatectomy techniques have advanced, we have adopted laparoscopic left lateral segmentectomy as a curative treatment for symptomatic cysts located in the left lateral segment. Between 2018 and 2023, 4 patients underwent laparoscopic left lateral segmentectomy for symptomatic hepatic cysts at our institution. All procedures were performed using five ports. Cystic fluid was aspirated as much as

Neuroendoscopy in the Surgical Management of Lateral and Third Ventricular Tumors: Looking Beyond Microneurosurgery.

BACKGROUND: Intraventricular tumors pose a surgical challenge because of the difficulty in reaching their deep location through safe corridors and their adherence or proximity to vital neurovascular structures. Although microneurosurgery is the mainstay of surgical management, neuroendoscopy aided by adjuncts, namely, navigation and ultrasonic aspirators, has made a great contribution to improving surgical results. OBJECTIVE: This article reviews the experience of a neurosurgical unit with endoscopic procedures for intraventricular tumors. The current indications, benefits, and complications

The Reliability of the Ultrasonic Bone Scalpel in Cervical Spondylotic Myelopathy: A Comparative Study of 46 Patients.

BACKGROUND: In patients with cervical spondylotic myelopathy (CSM), laminectomy is usually performed with a Kerrison rongeur or a high speed drill (HSD).The HSD, which is most often selected for laminectomy, may cause complications such as duratomy, thermal and mechanical neural injuries. With an ultrasonic bone scalpel (UBS), a less traumatic laminectomy can be performed in a shorter time. The aim of this study was to compare the results of laminectomies using HSD and UBS. METHODS: Evaluation was made in 46 patients who were operated on for CSM. Cervical laminectomy was performed on 23 patie

Annular pancreas associated with pancreatolithiasis: a case report.

We present a case of annular pancreas associated with pancreatolithiasis. A 41 year-old Japanese man with epigastric pain was admitted to the surgical service at Miyazaki Medical College Hospital. Contrast duodenography revealed severe stenosis of the descending duodenum. Cholangiography showed a stenotic segment of the intrapancreatic common bile duct surrounded by calcifications. Computed tomography of the abdomen revealed calcifications in the posterior region of the pancreatic head. Percutaneous cannulation of the pancreatic ductal system, using ultrasonic guidance, demonstrated a slightly

Laparoscopic cholecystectomy: ultrasonic energy versus monopolar electrosurgical energy.

OBJECTIVE: Nowadays, laparoscopic cholecystectomy (LC) is undoubtedly considered the "gold standard" in the surgical treatment of symptomatic gallstones, gallbladder adenomas and acute cholecystitis. Among the alternative energy sources proposed (monopolar, bipolar electric scalpel, radiofrequency sealers) with the aim to dissect and/or seal, the ultrasonic energy has been frequently adopted, however without a widespread acceptance among surgeons for routine or emergency laparoscopic cholecystectomy. This study investigates the possible beneficial aspects of ultrasonic dissection and its effic

A valve replacement technique for heavily calcified mitral valve and annulus.

Severe mitral annular calcification is frequently encountered in patients with chronic renal failure requiring dialysis. The procedures are described for the implantation of mitral prostheses in two dialysis patients with mitral stenosis who exhibited heavily calcified mitral annuli. In both cases the calcifications extended around the entire mitral annulus as well as the valve leaflets, papillary muscle, atrial wall and ventricular myocardium, making it impossible to secure prosthetic valves to the mitral annuli. Before implantation, the calcification was partially debrided ultrasonically, an

Axillary dissection for breast cancer using electrocautery versus ultrasonic dissectors: A prospective randomized study.

BACKGROUND: The major morbidities of modified radical mastectomy both short- and long-term are sequelae of axillary dissection. Flap complications, prolonged seroma, need for axillary drainage, wound infection, lymphedema, shoulder stiffness, and paresthesia are major causes for morbidity after axillary dissection. Different techniques have been implemented to tackle these problems. Few of these include reducing the axillary dead space, using various forms of energy devices. AIMS: We have prospectively compared two energy sources, namely, ultrasonic dissector (UD) against the electrocautery d

[Post-traumatic rehabilitative treatment of hollow neck organs].

The attention of the reader is focused on the errors and shortcomings in rendering urgent emergency care for patients who underwent traumas of the neck hollow organs. The 30-year experience with 1000 patients managed by the authors convinced him of the necessity to consider more carefully the medical history of the patient, to use more frequently Killian's and Brunings's examinations, microlaryngoscopy, fibroendoscopy, x-rays. Restorative treatment becomes more effective under ultrasound and laser separation of the tissues, laryngochioido plexy, tracheopexy with establishment of the laryngeal

Below-knee amputation with an ultrasonic scalpel: evaluation of early postoperative clinical outcomes.

OBJECTIVE: Despite the development of microscopic reconstructive techniques for lower limb salvage, major limb amputation is still required for critical, unsalvageable lower leg wounds, with steadily increasing estimates of major limb amputations. In this study, the authors highlight a surgical technique for below-knee (BK) amputation using an ultrasonic scalpel, and evaluate its safety and effectiveness compared with the conventional method of using surgical blades. METHOD: A retrospective chart review was conducted at the Department of Plastic and Reconstructive Surgery, St. Vincent's Hospi

Case Report on Deep Brain Stimulation Rescue After Suboptimal MR-Guided Focused Ultrasound Thalamotomy for Essential Tremor: A Tractography-Based Investigation.

Essential tremor (ET) is the most prevalent movement disorder in adults, and can often be medically refractory, requiring surgical intervention. MRI-guided focused ultrasound (MRgFUS) is a less invasive procedure that uses ultrasonic waves to induce lesions in the ventralis intermedius nucleus (VIM) to treat refractory ET. As with all procedures for treating ET, optimal targeting during MRgFUS is essential for efficacy and durability. Various studies have reported cases of tremor recurrence following MRgFUS and long-term outcome data is limited to 3-4 years. We present a tractography-based inv

[A surgical technique of tricuspid valve repair for infective endocarditis using an autologous pericardium].

We report a surgically treated case of tricuspid valve endocarditis. A 33-year-old man was diagnosed with ventricular septal defect (VSD) and active infective endocarditis associated with severe tricuspid regurgitation. Ultrasonic echocardiography (UCG) showed vegetations attached to the tricuspid valve. His blood culture was positive for Streptococcus oralis. Although intravenous antibiotics therapy was effective, chest computed tomography( CT) revealed multiple septic pulmonary enboli in right lung and UCG showed severe tricuspid valve regurgitation. So we performed tricuspid valve repair by
