Resting-state functional magnetic resonance imaging reveals the effects of intravesical electrical stimulation on brain activity and functional connectivity in rats with detrusor underactivity.

INTRODUCTION: The central mechanisms of intravesical electrical stimulation (IVES) were explored in a rat model of detrusor underactivity (DU) induced by bilateral pelvic nerve crush (bPNC). Resting-state functional magnetic resonance imaging (rs-fMRI) was used to evaluate regional homogeneity (ReHo) and functional connectivity (FC). METHODS: Eighteen female Sprague-Dawley rats were randomly assigned to sham, bPNC, or IVES groups. The IVES group received 20-minute daily stimulations for 20 days after bPNC. Following treatment, rs-fMRI, cystometry, and immunofluorescence staining for ΔFosB (ne

A report on neurogenic bladder in COVID-19 vaccine-associated acute transverse myelitis.

INTRODUCTION: Acute transverse myelitis (ATM) is a rare neurological complication of Coronavirus disease (COVID-19) vaccines. Various vaccines have been linked to ATM, such as non-replicating viral vectors, ribonucleic acid, and inactivated vaccines. An ATM case is presented here involving the BNT162b2 vaccine leading to asymmetrical incomplete paraplegia and neurogenic bladder. CASE PRESENTATION: A 66-year-old male developed urinary retention one day after his second dose of the BNT162b2 vaccine, followed by rapidly progressing lower limb weakness. Clinical examination showed asymmetrical pa

Current concepts of the acontractile bladder.

The acontractile bladder (AcB) is a urodynamic-based diagnosis wherein the bladder is unable to demonstrate any contraction during a pressure flow study. Although it is often grouped with underactive bladder, it is a unique phenomenon and should be investigated independently. The purpose of the present review was to examine the current literature on AcB regarding its pathology, diagnosis, current management guidelines, and future developments. We performed a review of the PubMed database, classifying the evidence for AcB pathology, diagnosis, treatment, and potential future treatments. Over th

Null mutation in macrophage migration inhibitory factor prevents muscle cell loss and fibrosis in partial bladder outlet obstruction.

Idiopathic detrusor underactivity (DU) and detrusor decompensation which develops following partial bladder outlet obstruction (pBOO) are both associated with smooth muscle degeneration and fibrosis. Macrophage migration inhibitory factor (MIF), an important mediator of bladder inflammation, has been shown to promote fibroblast survival and muscle death in other tissues. We evaluated the hypothesis that MIF has similar actions in the bladder by studying detrusor responses to pBOO or sham surgery in anesthetized female mice rendered null for the mif gene (MIF KO) and in wild-type (WT) controls,

Surgical outcome of male patients with chronic central nervous system disorders and voiding dysfunction due to bladder outlet obstruction.

PURPOSE: Voiding dysfunction is common in patients with chronic central nervous system (CNS) diseases and has great impact on quality of life. Patients with chronic CNS disorders might have concomitant detrusor overactivity, detrusor underactivity, and voiding dysfunction. Although bladder outlet surgeries could relieve bladder outlet obstruction (BOO), patients might have persistent or exacerbated storage symptoms. This study investigated surgical outcome of patients with chronic CNS disorders after bladder outlet surgery. METHODS: A total of 63 male patients with cerebrovascular accident (C

Diagnosis of Urethral Resistance in Men: 70 Years of History.

BACKGROUND AND AIMS: The urethral resistance (UR) is one of the key outcomes of the urodynamic pressure-flow study (PFS) in men. Combined with a clinical meaningful cut-off point, urodynamic bladder outflow obstruction (BOO) can be determined. This characterization of BOO provides guidance in the management of lower urinary tract symptoms (LUTS), enabling the differentiation between BOO and detrusor underactivity. In this way, the UR can be objectified and monitored, providing insight into the expected results of a clinical intervention. The route from the raw urodynamic measurement towards th

Epidemiology and pathophysiology of neurogenic bladder after spinal cord injury.

Spinal cord injury (SCI) usually affects younger age groups with male preponderance. The most common traumatic cause is road traffic accident followed by sports accidents and gun-shot injuries. Infections and vascular events make up non-traumatic causes. There is regional variance in incidence and prevalence of SCI. Most systematic reviews have been undertaken from USA, Canada, and Australia with only few from Asia with resultant difficulty in estimation of worldwide figures. Overall, the incidence varies from 12 to more than 65 cases/million per year. The first peak is in young men between 15

Role of Bladder Functional Testing Prior to Surgeries for Benign Prostatic Obstruction.

PURPOSE OF REVIEW: There is no consensus on preoperative functional testing prior to surgeries for benign prostatic obstruction causing lower urinary tract symptoms (LUTS). RECENT FINDINGS: Surgical management offers definite benefits, but the results are not always satisfactory. The urodynamic study (UDS) is the gold standard for assessing bladder outlet obstruction (BOO) which is the best predictor of surgical success. Yet, it is not recommended by our urologic societies as standard testing prior to surgery. In this narrative review of the literature, we report recent findings and controver

Non-neurogenic female voiding dysfunction.

PURPOSE OF REVIEW: The pathophysiological mechanisms of female voiding phase dysfunction are poorly understood, and there are neither standard definitions nor guidelines for diagnosis and treatment. The aim of this review is to present up-to-date data and controversies associated with non-neurogenic female voiding dysfunction. RECENT FINDINGS: Conceptually, voiding phase dysfunction may have bladder or urethral causes. Bladder causes include detrusor contraction of inadequate magnitude or duration to effect bladder emptying (detrusor underactivity), or the absence of detrusor contraction (det

Urine biomarker could be a useful tool for differential diagnosis of a lower urinary tract dysfunction.

A precision diagnosis of lower urinary tract dysfunctions (LUTD) such as bladder outlet obstruction, detrusor overactivity (DO), interstitial cystitis/bladder pain syndrome (IC/BPS), dysfunctional voiding (DV), or detrusor underactivity (DU) needs invasive videourodynamic study. Exploring non-invasive tools to help screening LUTD is necessary for clinicians in their daily practice. This article reviews recently clinical studies of using urinary inflammatory proteins and oxidative stress biomarkers in the identification of specific LUTD among men and women with lower urinary tract symptoms (LUT
