Reduction of right ventricular pacing in patients with sinus node dysfunction using an enhanced search AV algorithm.

BACKGROUND: Dual chamber pacing typically results in a high percentage of ventricular pacing. A number of studies have been conducted suggesting detrimental effects of ventricular desynchronization produced by long-term RV pacing. Pacemaker algorithms that extend the AV interval to uncover intrinsic AV conduction have been utilized to reduce ventricular pacing. These algorithms are often limited to AV intervals below 250 ms limiting the ventricular pacing reduction. We hypothesized that by allowing AV intervals to extend beyond 300 ms, a marked reduction in RV pacing can be achieved. METHODS:

Right ventricular myocardial diastolic dysfunction in different kinds of cardiac hypertrophy: analysis by pulsed Doppler tissue imaging.

BACKGROUND: Right ventricular (RV) chamber involvement has been demonstrated in hypertrophic cardiomyopathy (HCM) as well as in hypertensive left ventricular hypertrophy (LVH) but little is known about RV myocardial dysfunction occurring in these two pathologies. The aim of this study was to compare Doppler tissue imaging (DTI) of the right ventricle in HCM and LVH in relation to DTI of the left ventricle and Doppler standard of the RV and left ventricular (LV) inflow. METHODS: Thirty controls, 20 hypertensives with LVH, and 23 patients with HCM involving the interventricular septum underwent

Common femoral vein Doppler as an adjunct for serial assessment of right ventricular dysfunction after lung transplantation.

Right ventricular dysfunction complicates the early course after lung transplantation, particularly when primary graft dysfunction elevates pulmonary vascular resistance. Bedside assessment is challenging: transthoracic echocardiographic windows are often limited after clamshell thoracotomy, and pulmonary artery catheters are typically removed within days. Common femoral vein (CFV) Doppler offers a noninvasive, position-independent means of detecting venous congestion via peripheral venous waveform pulsatility. We describe a bedside protocol integrating serial CFV Doppler with echocardiography

MnTBAP attenuates pulmonary hypertension-induced right ventricular dysfunction and remodeling by modulating the CaMKIIδ-SERCA2a signaling pathway.

A life-threatening condition, pulmonary arterial hypertension (PAH) is typified by the progressive pathological remodeling of pulmonary blood vessels, which progresses to right heart failure (RHF). The pathophysiological mechanisms underlying the progression from PAH to RHF remain unclear. Manganese (III) tetrakis (4-benzoic acid) porphyrin chloride (MnTBAP) is a synthetic manganese-based superoxide dismutase (SOD) mimetic with anti-inflammatory and antioxidant properties, attenuating PAH. This study investigated whether MnTBAP directly affects RHF in rats with monocrotaline (MCT)-induced PAH.

Distribution and prognostic implications of right and left ventricular systolic dysfunction in wild-type transthyretin amyloid cardiomyopathy.

We aimed to characterize the prevalence and severity of left ventricular (LV) and right ventricular (RV) systolic dysfunction and their prognostic implications in wild-type transthyretin amyloid cardiomyopathy (ATTRwt-CM). We enrolled 256 consecutive contemporary ATTRwt-CM patients. LV systolic function was assessed by LV ejection fraction (LVEF), global longitudinal strain (LV-GLS), and stroke volume index (SVi), while RV systolic function was assessed using Tricuspid Annular Plane Systolic Excursion (TAPSE). At ATTRwt-CM diagnosis time, median LVEF was 47.5% (Q1-Q3: 40.7%-54.3%). Preserved L

The Impact of Insulin Resistance on Lung Volume Through Right Ventricular Dysfunction in Diabetic Patients-Literature Review.

Insulin resistance (IR), a core component in the development of type 2 diabetes mellitus (T2DM), is increasingly recognized for its role in cardiovascular and pulmonary complications. This review explores the relationship between IR, right ventricular dysfunction (RVD), and decreased lung volume in patients with T2DM. Emerging evidence suggests that IR contributes to early structural and functional alterations in the right ventricle, independent of overt cardiovascular disease. The mechanisms involved include oxidative stress, inflammation, dyslipidemia, and obesity-factors commonly found in m

Beyond the left ventricle: Right ventricular dysfunction as a critical determinant in type 1 diabetes-related cardiomyopathy.

Diabetic cardiomyopathy (DCM) has long been considered as a left ventricular (LV) disease with diastolic dysfunction preceding systolic dysfunction in diabetes. However, it is increasingly recognized that the right ventricle (RV) is also affected by diabetes and may be independently responsible for adverse outcomes in diabetic patients with or without LV failure. Yu et al conducted a 30-week longitudinal evaluation of biventricular function and pathology in OVE26 diabetic mice and revealed early diastolic dysfunction preceding systolic decline, suggesting that early LV diastolic impairment pre

Identification of Biomarkers for Right Ventricular Dysfunction in Idiopathic Dilated Cardiomyopathy Via Urinary Proteomics and Machine Learning.

BACKGROUND: Right ventricular dysfunction (RVD) is a common complication of idiopathic dilated cardiomyopathy linked to poor outcomes. However, reliable noninvasive biomarkers for RVD remain lacking. This study aimed to identify urinary proteomic markers using mass spectrometry and machine learning. METHODS: In this prospective cohort, patients with idiopathic dilated cardiomyopathy were classified by cardiac magnetic resonance imaging into groups with RVD (RV ejection fraction <45%) and without RVD groups. Baseline urine samples were profiled by data-independent acquisition mass spectrometry

Multimodal Characterization of High-risk PH-HFpEF phenogroup with Right Ventricular Dysfunction: Vascular Mechanics and Myocardial Transcriptomics.

BACKGROUND: Pulmonary hypertension due to heart failure with preserved ejection fraction (PH-HFpEF) is a highly heterogeneous disease associated with right ventricular (RV) failure and adverse outcomes. Current diagnostic tools inadequately characterize pulmonary vascular disease and RV dysfunction, limiting treatment precision. We hypothesized that deep phenotyping-including invasive hemodynamics, advanced imaging, and myocardial transcriptomics-would identify high-risk PH-HFpEF phenotypes with distinct clinical, physiologic, and molecular characteristics. METHODS: 42 PH-HFpEF participants (

Right ventricular dysfunction, late gadolinium enhancement, and female gender predict poor outcome in patients with dilated cardiomyopathy.

AIMS: Dilated cardiomyopathy (DCM) shows a variable disease course and is associated with significant morbidity and mortality. So far, left ventricular function (LVF) is the major determinant for risk stratification. However, since it has shown to be a poor guide to individual outcome, we studied the prognostic value of cardiovascular magnetic resonance imaging (CMR) parameters, late gadolinium enhancement (LGE) and epicardial adipose tissue (EAT). METHODS AND RESULTS: 140 patients with DCM underwent late gadolinium enhancement (LGE) CMR. During a median follow-up of 3 years, 22 patients (16%

Right ventricular dysfunction, but not tricuspid regurgitation, is associated with outcome late after left heart valve procedure.

BACKGROUND: Significant tricuspid regurgitation (TR) late after left heart valve procedure is frequent and associated with increased morbidity. Surgical correction carries a significant mortality risk, whereas the impact of TR on survival in these patients is unclear. OBJECTIVES: This study sought to assess the impact of significant TR late after left heart valve procedure. METHODS: A total of 539 consecutive patients with previous left heart valve procedure (time interval from valve procedure to enrollment 50 ± 30 months) were prospectively followed for 53 ± 15 months. RESULTS: Significant

Assessment of right ventricular endocardial dysfunction in mild-to-moderate mitral stenosis patients using velocity vector imaging.

PURPOSE: Right ventricular (RV) function plays an important role in the development of clinical symptoms, exercise capacity, prognosis, and survival in patients with mitral stenosis (MS). The purpose of this study was to evaluate global and regional RV systolic functions using a novel technique, VVI, in mild-to-moderate MS patients without clinical symptoms of heart failure. METHODS: The study population consisted of 60 patients (mean age 51.7 ± 11.6 years; 85% female) with isolated rheumatic mitral valve stenosis and 40 age- and sex-matched control subjects (mean age 49.1 ± 10.5 years; 76.7%

Right ventricular dysfunction and B-type natriuretic peptide in asymptomatic patients after repair for tetralogy of Fallot.

Early detection of right ventricular (RV) dysfunction is essential in the assessment of patients with repaired tetralogy of Fallot (TOF). This study aimed to assess latent RV dysfunction in asymptomatic patients with TOF and to determine the predictive value of B-type natriuretic peptide (BNP). Pressure-volume loops were recorded for 16 young patients (New York Heart Association class 1 or Ross class 0; median age, 14.2 years) using the conductance catheter technique. All the patients had RV dilation secondary to pulmonary regurgitation after surgical repair of TOF. Indexes of RV function were

Right ventricular dysfunction after tetralogy repair in a pediatric patient with successful ECMO support--a case report.

Right ventricular dysfunction after surgical repair of tetralogy is a serious life threatening complication. Although pulmonary vasodilators and inotropes can be used for cardiac support in tetralogy repair, these drugs may be ineffective in some patients. To date extracorporeal membrane oxygenation (ECMO) support may be used effectively in these patients when medical therapy fails. We report a case with successful ECMO support in right ventricular dysfunction and pulmonary hypertension after total surgical correction of Fallot's tetralogy. ECMO circuit was instituted in the operating room due

Abnormal Venous Flow in Pregnant Women with Mild Right Ventricular Dysfunction in Repaired Tetralogy of Fallot: A Clinical Model for Organ Dysfunction in Preeclampsia.

Background: Pregnant women with congenital heart disease carry a high risk of complications, especially when cardiac function is suboptimal. Increasing evidence suggests that impaired right ventricular (RV) function has a negative effect on placental function, possibly through venous congestion. We report a case series of hepatic and renal venous flow patterns in pregnant women with right ventricular dysfunction after repaired Tetralogy of Fallot (ToF), relative to those observed in normal pregnancy and preeclampsia. Methods: At 20-24 weeks pregnancy, RV function was measured by echocardiograp

Hepatic elastography as a noninvasive marker of right ventricular dysfunction and prognosis in precapillary pulmonary hypertension: A pilot study.

BACKGROUND: Right ventricular (RV) dysfunction is a major determinant of outcomes in precapillary pulmonary hypertension (PH). Hepatic elastography may reflect systemic venous congestion and right-sided heart dysfunction, but its role in precapillary PH remains unclear. OBJECTIVES: To evaluate standard and central hepatic elastography (SHE and CHE) as noninvasive markers of right heart dysfunction and to explore their prognostic relevance in precapillary PH. METHODS: In this prospective study, 26 patients with precapillary PH underwent echocardiography and hepatic elastography. Associations
