Ferlins are multiple-C2-domain proteins involved in Ca2+-triggered membrane dynamics within the secretory, endocytic and lysosomal pathways. In bony vertebrates there are six ferlin genes encoding, in humans, dysferlin, otoferlin, myoferlin, Fer1L5 and 6 and the long noncoding RNA Fer1L4. Mutations in DYSF (dysferlin) can cause a range of muscle diseases with various clinical manifestations collectively known as dysferlinopathies, including limb-girdle muscular dystrophy type 2B (LGMD2B) and Miyoshi myopathy. A mutation in MYOF (myoferlin) was linked to a muscular dystrophy accompanied by card

A 40-year-old woman was referred to physical therapy with complaints of headaches. Examination raised suspicion of a "thunderclap headache," a condition characterized by sudden, intense headaches correlated with bleeding in and around the brain. The patient was referred to a neurologist, who ordered magnetic resonance angiography of the head and neck, which identified a partial dissection of the right vertebral artery. A subsequent computed tomography angiogram confirmed the dissection. J Orthop Sports Phys Ther 2020;50(6):344. doi:10.2519/jospt.2020.8858.

Vertebral fractures are one of the most common fractures associated with skeletal fragility and can cause as much morbidity as hip fractures. However, the epidemiology of vertebral fractures differs from that of osteoporotic fractures at other skeletal sites in important ways, largely because only one quarter to one-third of vertebral fractures are recognized clinically at the time of their occurrence and otherwise require lateral spine imaging to be recognized. This article first reviews the prevalence and incidence of clinical and radiographic vertebral fractures in populations across the gl

Vertebral fracture is a common clinical problem. Osteoporosis is the leading cause of non-traumatic vertebral fracture. Often, vertebral fractures are not clinically suspected due to nonspecific presentation and are overlooked during routine interpretation of radiologic investigations. Moreover, once detected, many a times the radiologist fails to convey to the clinician in a meaningful way. Hence, vertebral fractures are a constant cause of morbidity and mortality. Presence of vertebral fracture increases the chance of fracture in another vertebra and also increases the risk of subsequent hip

Incidental vertebral lesions on imaging of the spine are commonly encountered in clinical practice. Contributing factors include the aging population, the increasing prevalence of back pain, and increased usage of MR imaging. Additionally, refinements in CT and MR imaging have increased the number of demonstrable lesions. The management of incidental findings varies among practitioners and commonly depends more on practice style than on data or guidelines. In this article we review incidental findings within the vertebral column and review management of these lesions, based on available Class

Osteoarthritis is a common complication in the elderly and is often associated with osteophyte growth on vertebral bodies. The clinical presentation of vertebral osteophytes is related to anatomical structures adjacent to the spinal column. For instance, cervical osteophytes potentially involve the pharynx and esophagus, leading to dysphagic symptoms that may be accompanied by food aspiration, vocal fold paralysis and obstructive sleep apnea. In addition to anterior cervical osteophytes, posterior and uncinate process osteophytes may form, compressing the spinal cord and vertebral artery blood

Pyogenic osteomyelitis seldom affects the spine (2-7%). It is diagnosed by the isolation of a bacterial agent in the vertebral body, the intervertebral disks or from paravertebral or epidural abscesses. We report a retrospective study of ten patients who attended a medical clinic with this disease to emphasize its unusual presentation and difficult diagnosis. Lumbar and dorsal spine were the most common sites affected. Dorsolumbar pain and paraparesis were the most frequent symptoms. Staphylococcus aureus were isolated in eight patients, Escherichia coli in one and Haemophilus sp. in other Leu

Vertebral osteonecrosis classically presents with an intravertebral vacuum cleft phenomenon or a fluid-filled cleft on MR images. These clefts are usually found in older patients presenting with more severe fractures, more significant collapse and instability. Therefore, although considered for a long time as pathognomonic for vertebral osteonecrosis, vertebral clefts are now considered to represent fracture non-union. The double-line sign is classically described for osteonecrosis of long bones, but has been reported in one case of concurrent spinal cord and vertebral bone marrow radionecrosi

OBJECTIVE: Neurosurgical procedures such as proximal brachial plexus repair, scalenotomy, and direct isolation of the proximal vertebral artery require a good working knowledge of the triangle of the vertebral artery. This deep triangle of the neck is bound by the subclavian artery and the anterior scalene and longus cervicis muscles. In addition to the vertebral artery, many important structures are found in this area, such as the ganglionated sympathetic chain and certain cervical spinal nerves.
METHODS: Twenty formalin-fixed cadavers were used for this study. Dissection of this triangle wa

Vertebral body resection for locally advanced lung cancer can be performed with acceptable morbidity and mortality rates, and with improved long-term survival, when combined with chemotherapy and radiation. A consensus has not been reached on either the optimal extent of vertebral resection or the optimal treatment regimen. Should total vertebrectomies be the standard of care for all patients, even those with minimal spine involvement? Can the extended operative times and multiple incisions and anatomic limitations that place some of the mediastinal organs at risk be justified for potential im

BACKGROUND CONTEXT: With the aging of the population, painful osteoporotic compression fractures are becoming more common.
PURPOSE: To review the physiologic implications of these injuries as well as treatment options and outcomes, especially with reference to newer, percutaneous "augmentation" procedures, that is, vertebroplasty and kyphoplasty.
STUDY DESIGN/SETTING: A literature review.
METHODS: No direct, randomized studies comparing vertebroplasty, kyphoplasty and standard, nonoperative care are available.
RESULTS: The growing literature suggests a role for kyphoplasty and vertebroplas

Vertebral hemangiomas are the most common benign spinal tumors with an incidence range of 10-12%. Approximately 1-2% of the cases develop clinically significant symptoms causing the necessity for treatment. Based on our own results and a review of the literature we discuss the role of radiotherapy in the management of symptomatic vertebral hemangiomas. A total dose of 30.0 Gy given in five weekly fractions of 2.0 Gy has been proven as effective for the primary treatment and postsurgical irradiation for the prevention of a recurrence. In the literature review the results are summarized of 59 re

The segmental character of the vertebrate body wall is reflected by metamerically arranged tissues that are patterned during embryonic life as a consequence of somite formation, compartmentalization and differentiation. The somites bud off the paraxial mesoderm in a cranio-caudal sequence and are compartmentalized by local signals from adjacent structures. These signals may be mediated by diffusible substances such as Sonic hedgehog (Shh), Wnts and Bone morphogenetic protein (BMPs) or by cell-cell interactions via membrane-bound receptors and ligands such as Delta and Notch. Compartmentalizati

Degenerative vertebral dislocations (laterolisthesis) are responsible for progressive transverse intervertebral shift. These changes appear like a translation of the lateral edge of a vertebra compared to that of the underlying vertebra and are easily diagnosed. Vertebral dislocations can be secondary to active lumbar scoliosis or unilateral or asymmetrical degenerative spondylolisthesis. In every case, the appearance of laterolisthesis is reflected by the onset or aggravation of scoliosis which may become self-perpetuating. This lesion is secondary to osteoarthritic disruption of the integrit

Vertebral osteomyelitis is an unusual complication of pneumococcal infection. This report describes a patient who presented with back pain and lower extremity weakness after bacteremic pneumococcal pneumonia. Magnetic resonance imaging showed destruction of the L-4-L-5 vertebral bodies and thecal sac compression, and Streptococcus pneumoniae was isolated from an aspirate of the disk space. We found only 10 other cases of pneumococcal vertebral osteomyelitis in a review of the literature. In these 11 cases the presentations were usually subacute, and the onset of back pain was insidious. Elevat

Vertebral fracture is the most prevalent manifestation of osteoporosis in women, but there is very little information concerning vertebral fracture in men. These studies begin to determine the prevalence, radiographic character, and relationship to bone mineral density of vertebral deformity in men. A group of 144 white men aged 34-94 years (83% between 50 and 80 years) were studied. Thoracic and lumbar spine radiographs were obtained using standardized techniques, and morphometric measures of vertebrae (T6-L5) were obtained using a computerized digitization pad. Vertebral deformities (wedge,

Lymphosarcoma of the cauda equina in a domestic shorthair cat invaded the body of L7 and cranial portion of the sacrum. Clinical signs consisted of acute ambulatory paraparesis, tail paralysis, and dyschezia. Radiographically, there was evidence of bone lysis. Surgical exploration yielded a diagnosis, but did not induce improvement in the cat, which was subsequently euthanatized. Neurologic signs were similar to those associated with sacral nerve root avulsion injuries in cats.
