A novel Torque teno virus qPCR assay reveals longitudinal viral load variability among non-immunosuppressed subjects.

Torque teno virus (TTV) DNA is an emerging biomarker of the individual immune status with considerable potential for guiding immunosuppressive therapy. However, widespread clinical implementation has been hampered by the lack of standardized, high-throughput diagnostic assays and limited understanding of the physiological variability of TTV viral loads over time. Here, we developed and analytically validated a novel fully automated high-throughput quantitative PCR assay for standardized TTV-DNA measurement in routine diagnostics and large-scale clinical studies. The assay demonstrated high ana

Prevalence and viral load dynamics of PCV-2 and PCV-3 on large Hungarian swine farms.

Porcine Circovirus 3 (PCV-3) was first identified in 2016 and has since been detected in swine herds worldwide. The aim of this study was to survey the prevalence of PCV-2 and PCV-3 in large Hungarian swine herds. Between July 2020 and April 2022, a total of 1,408 blood samples were collected from 18 large herds and pooled sampling was applied by combining equal amounts from five individuals per pool. PCV-3 positive samples were detected in 94.4% (17/18) of the farms, with an overall apparent prevalence of 30.4% (89/293) in the tested pools. PCV-2 was found in 50.0% (9/18) of the farms, with 1

Dengue molecular dynamics: viral load and serotype shifts predict severity and death in Kolkata, Eastern India.

BACKGROUND: Dengue manifestations range from mild fever to fatal shock, driven by viral virulence and host responses. While Kolkata is hyperendemic, data linking specific serotypes and viral load to clinical outcomes remains limited. METHODS: This prospective observational study (January 2024-June 2025) analysed 120 dengue serology-reactive patients at a Kolkata teaching hospital. Viral nucleic acid was extracted for serotyping and quantification via the molecular method. Patients were stratified by severity using World Health Organization 2009 criteria. RESULTS: Dengue 2 serotype (DENV-2) w

Real-World Experience with Long-Acting Injectable Cabotegravir/Rilpivirine in HIV Patients with Unsuppressed Viral Load.

Long-acting injectable cabotegravir/rilpivirine (CAB/RPV-LA) is currently approved as a maintenance therapy for people with HIV (PWH) who are virologically suppressed. However, growing real-world evidence highlights its potential role in more complex viremic populations traditionally considered ineligible. We present a case series of eight PWH treated at two tertiary centers in Italy, all of whom faced persistent viremia, adherence difficulties, malabsorption syndromes, or psychosocial barriers. Following the switch to CAB/RPV-LA, all patients, despite heterogeneous clinical profiles and basel

Comparative Assessment of Viral Load Retention in Surgical and Fabric Masks Worn by COVID-19 Patients.

Face masks are widely recognized as a key intervention to limit SARS-CoV-2 transmission, yet the distribution and persistence of viral RNA across different mask regions and layers remain poorly understood. To address this, we analyzed 185 masks collected from 60 SARS-CoV-2-positive individuals in Rio de Janeiro between December 2020 and September 2022. Masks were sectioned into anatomical regions (nose, mouth, sides) and structural layers (inner, middle, outer), and viral RNA was quantified using RT-qPCR. Samples with the highest viral loads were selected for partial sequencing of the spike ge

Chromosome 1 variants associated with decreased HIV set-point viral load correlate with PRKAB2 expression changes.

A previous study investigated a genomic region on chromosome 1 associated with reduced human immunodeficiency virus type 1 (HIV) set-point viral load, implicating CHD1L as a novel HIV inhibitory factor. However, given that regulatory variants can influence expression of multiple nearby genes, further work is necessary to determine the impact of genetic variants on other genes in the region. This study evaluates the potential for genetic regulation of PRKAB2, a gene located upstream of CHD1L and encoding the β2 regulatory subunit of the AMPK complex, and for downstream impacts on HIV pathogenes

Determinants of viral load suppression among adolescents on antiretroviral therapy in Eswatini: a cross-sectional study.

BACKGROUND: The goal of antiretroviral therapy (ART) is to achieve a sustained HIV suppressed viral load. However, adolescents often present poor adherence to ART which is associated with lower rates of viral load suppression (VLS). The objective of this study was to determine the viral load suppression levels and the associated factors among adolescents living with HIV (ALHIV) and on ART in Eswatini. METHODS: We conducted cross-sectional analysis of data from 911 adolescents aged 10 to 19 living with HIV and on ART between the period January 2017 and September 2022. We collected data of demo

Varicella-Zoster Virus-Specific Cell-Mediated Immune Response Kinetics and Latent Viral Load Depending on Aging.

Herpes zoster (HZ), an infection caused by varicella-zoster virus (VZV) reactivation, results from an age-related decline in VZV-specific cell-mediated immunity (CMI). Digital droplet PCR (ddPCR) precisely quantifies latent VZV DNA in the blood, a potential biomarker of subclinical viral reactivation and its association with HZ risk. This study assessed VZV-specific CMI, latent viral burden, and humoral immunity in healthy adults according to age. We prospectively enrolled healthy adults aged between 40 and 80 years between February and April 2024. VZV-specific CMI was quantified using interfe

Predictors of longitudinal viral load suppression among adults receiving first line antiretroviral therapy in Central Ethiopia.

HIV/AIDS continues to be a major global public health issue and a significant cause of death. While the WHO advocates for viral load testing as the primary approach to monitor treatment and detect antiretroviral therapy (ART) failure, the factors affecting viral load trends are frequently neglected. This study exclusively analyzes viral load trajectories, identifying predictors of virological suppression independent of adverse outcomes. This study aimed to assess predictors of longitudinal viral load suppression HIV patients on first-line ART in Central Ethiopia: a mixed-effects analysis. A re

Unravelling the acute respiratory infection landscape: virus type, viral load, health status and coinfection do matter.

INTRODUCTION: Acute respiratory infections (ARI) are the most common infections in the general population and are mainly caused by respiratory viruses. Detecting several viruses in a respiratory sample is common. To better understand these viral codetections and potential interferences, we tested for the presence of viruses and developed quantitative PCR (Polymerase Chain Reaction) for the viruses most prevalent in coinfections: human rhinovirus (HRV) and respiratory syncytial virus (RSV), and quantified their viral loads according to coinfections and health status, age, cellular abundance and

The viral load monitoring cascade in HIV treatment programmes in sub-Saharan Africa: a systematic review.

BACKGROUND: The United Nations' 95-95-95 (95% of people with HIV being aware of their diagnosis, 95% of those aware of their diagnosis being on treatment and 95% achieving viral suppression) target aims to reduce morbidity and mortality of HIV. However, with 60% of new HIV infections occurring in sub-Saharan Africa (SSA), achieving this target in the region is challenging. Viral load (VL) monitoring is the gold-standard approach of assessing treatment efficacy, and its implementation into national health systems is a global health priority if elimination of HIV as a public health threat is to

Clinical Significance of Elevated KSHV Viral Load in HIV-Related Kaposi's Sarcoma Patients in South Africa.

Kaposi's sarcoma (KS) is an AIDS-defining illness caused by Kaposi's sarcoma-associated herpesvirus (KSHV) predominantly in the context of HIV-related immune suppression. We aimed to explore the usefulness of KSHV DNA viral load (VL) measurement in predicting the severity, response to treatment and outcome of KS. We retrospectively assessed a cohort of KS patients (n = 94) receiving treatment at Groote Schuur Hospital, Cape Town, South Africa. Demographic and clinical data, KS staging and response to treatment were extracted from patient files, while long-term survival was ascertained from hos

Clinical characteristics and outcomes of Epstein-Barr virus viral load after allogeneic hematopoietic stem cell transplantation.

Epstein-Barr virus (EBV) reactivation can occur following allogenic hematopoietic stem cell transplantation (allo-HSCT). However, the clinical characteristics and outcomes of EBV-viral load are not well known. Thus, we retrospectively analyzed the clinical features and prognostic impact of the EBV viral load in 121 allo-HSCT recipients from our hospital. EBV DNA quantification was performed in whole blood after transplantation. Patients were grouped based on whether EBV DNA quantification reached > 1000 copies/mL during follow-up (N = 50) or not (N = 71). Patients with EBV > 1000 EBV copies/mL

Comprehensive profiling of pre-infection antibodies identifies HIV targets associated with viremic control and viral load.

BACKGROUND: High HIV viral load (VL) is associated with increased transmission risk and faster disease progression. HIV controllers achieve viral suppression without antiretroviral (ARV) treatment. We evaluated viremic control in a community-randomized trial with >48,000 participants. METHODS: A massively multiplexed antibody profiling system, VirScan, was used to quantify pre- and post-infection antibody reactivity to HIV peptides in 664 samples from 429 participants (13 controllers, 135 viremic non-controllers, 64 other non-controllers, 217 uninfected persons).
