Preclinical evaluation and preliminary clinical study of 68Ga-NODAGA-NM-01 for PET imaging of PD-L1 expression.

BACKGROUND: Programmed cell death 1/programmed death ligand-1 (PD-L1)-based immune checkpoint blockade is an effective treatment approach for non-small-cell lung cancer (NSCLC). However, immunohistochemistry does not accurately or dynamically reflect PD-L1 expression owing to its spatiotemporal heterogeneity. Herein, we assessed the feasibility of using a 68Ga-labeled anti-PD-L1 nanobody, 68Ga-NODAGA-NM-01, for PET imaging of PD-L1. METHODS: Micro-PET/CT and biodistribution studies were performed on PD-L1-positive and -negative tumor-bearing mice. Additionally, a preliminary clinical study wa

Feasibility assessment of radiolabeled FAPI-04 for diagnostic and therapeutic use in rheumatoid arthritis.

OBJECTIVE: Fibroblast activation protein alpha (FAPα) plays a key role in cartilage degradation, inflammation, and bone erosion, particularly in rheumatoid arthritis (RA) where fibroblast-like synoviocytes in synovial tissue show elevated FAPα expression. This study explored radiolabeled FAP inhibitors for arthritis diagnosis and therapy. DESIGN: We used the radiotracer 68Ga-FAPI-04 for PET/CT imaging to predict collagen-induced arthritis (CIA) onset. Weekly scans quantified tracer uptake via SUV values, correlating results with disease scores and incidence. For therapeutic evaluation, 177Lu-

Physiological biodistribution of 68Ga-Pentixafor: PET/CT evaluation and implications for CXCR4 imaging.

PURPOSE: 68Ga-Pentixafor PET/CT is a novel imaging modality targeting the C-X-C chemokine receptor type 4 (CXCR4), which plays a crucial role in immune regulation, stem cell homing, and tumor progression. While its clinical use is expanding, comprehensive characterization of its physiological biodistribution remains limited. METHODS: This study retrospectively included 73 individuals who underwent 68Ga-Pentixafor PET/CT, comprising patients with various benign and malignant conditions, as well as healthy volunteers. Time-activity curves (TACs) were generated in two volunteers with dynamic and

Differential Detection of Hepatic Metastases on 68 Ga-DOTATATE PET/CT and 177 Lu-DOTATATE SPECT/CT.

68 Ga-DOTATATE PET/CT is indicated for selecting patients for peptide receptor radionuclide therapy (PRRT). Although highly sensitive, the detectability of smaller lesions, particularly in the liver, is lower. We present the case of a 58-year-old man with metastatic well-differentiated pancreatic neuroendocrine tumor whose MRI revealed progression of hepatic metastases. 68 Ga-DOTATATE PET/CT performed to determine eligibility for PRRT did not demonstrate DOTATATE-avid disease within the liver. 18 F-FDG PET/CT was also negative at the liver and the patient proceeded to 177 Lu-DOTATATE PRRT, whe

Evaluation of neutrophil labeling techniques using the chemotaxis radioassay.

Neutrophils isolated from human blood were labeled by various methods and exposed to a chemotactic gradient. The chemotactically functional cells that migrated into the gradient were isolated. The portion of radioactivity of the original cell suspension carried with the chemotactically responsive cells was related to the relative number of migrating cells as determined microscopically. Of the radionuclides used, P-32 diisopropylfluorophosphate (DFP), In-111 oxine, and Tc-99m sulfur colloid provided cell preparations with the highest relative portion of radioactivity confined to functionally in

Radiopharmaceuticals for the Diagnosis and Therapy of Neuroendocrine Differentiated Prostate Cancer.

Neuroendocrine differentiation of prostate cancer (PCa) is a relatively frequent event, generally understudied, that carries important prognostic information. It is the most frequently observed during the advanced stages of disease, when PCa has lost its sensitivity to androgen deprivation therapy or to chemotherapy, moderate to diffuse bone metastatic spread dominates the imaging scenario and it is responsible for painful clinical symptomatology. However, evidences indicate that neuroendocrine differentiation is a progressive phenomenon that starts at the very early part of the pathogenesis o

The evolving role of chemotherapy for muscle infiltrating bladder cancer.

In order to improve survival in a disease where the majority of deaths occur from metastases, the integration of systemic chemotherapy is crucial. Research efforts must continue to focus on refining case selection criteria, improving complete response proportions, and overcoming drug resistance. The blanket recommendation of a single therapeutic strategy such as radical surgery, chemotherapy, or radiation therapy to all patients is quickly becoming an outdated approach. Refinements in the understanding of the clinical, pathologic, and molecular features of urothelial tumors will ultimately imp

De Novo Design of a TROP-2-Targeting Peptide PET Tracer via AI-Guided Binding Interface Mimicry.

Trophoblast cell surface antigen 2 (TROP-2) is a transmembrane glycoprotein overexpressed across a range of epithelial malignancies, yet its clinical assessment still relies largely on invasive immunohistochemical analysis of biopsied tissues. In this study, we employed the PepMimic artificial intelligence platform to de novo design a TROP-2-targeting peptide, TR23, via binding interface mimicry, which was subsequently conjugated with DOTA and radiolabeled with 68Ga to yield the peptide-based PET probe [68Ga]Ga-DOTA-TR23. The tracer bound TROP-2 with nanomolar affinity (KD = 26.8 nM), showed s

Synthesis, preclinical evaluation, and clinical translation of [68Ga]Ga-Asp2-JR11, a SSTR2 antagonist for PET imaging of neuroendocrine neoplasms.

PURPOSE: Somatostatin receptor subtype 2 (SSTR2) is overexpressed in well-differentiated neuroendocrine neoplasms (NENs) and serves as a key target for positron emission tomography (PET) imaging. While SSTR2 agonists such as [68Ga]Ga-DOTA-TATE are widely used clinically, recent evidence suggests that antagonist radioligands can bind more receptor sites without inducing internalization, potentially offering superior imaging performance. Here, we report the synthesis, preclinical validation, and pilot clinical translation of [68Ga]Ga-Asp2-JR11, a novel SSTR2 antagonist radioligand featuring an -

Novel dimeric dual-modality FAP-targeted agents with favorable tumor retention for image-guided surgery: a preclinical study.

PURPOSE: Complete and minimally invasive cancer surgery remains challenging. Targeting the fibroblast activation protein (FAP) offers valuable opportunities for surgical planning, intraoperative guidance and improved resection outcomes. Herein, we developed the first dimeric, dual-modality FAP-targeted imaging agents and investigated the influence of different near-infrared cyanine-7 dyes on their final properties. METHODS: Four dual-modality ligands based on the Fusarinine C scaffold were synthesized. Their FAP specificity and retention were evaluated in cellular and xenograft tumor models.

Preclinical evaluation and first-in-human study of 68Ga- and Al18F-Labeled PSMA/FAP bispecific tracers for prostate cancer imaging.

PURPOSE: Prostate-specific membrane antigen (PSMA) expression may decrease during the progression of prostate cancer (PCa), compromising the efficacy of PSMA-targeted imaging. Fibroblast activation protein (FAP), a key component of the tumor microenvironment, offers a complementary target. This study aimed to develop and evaluate PSMA/FAP bispecific tracers with a biphenyl scaffold for improved PET detection outcomes of PCa. METHODS: The binding affinities of precursors and their corresponding non-radioactive standards were assessed using recombinant PSMA and FAP proteins. PET/CT imaging was

Preclinical evaluation of 68Ga- and 177Lu-Labeled AP-01 for the targeted treatment of prostate cancer with moderate PSMA expression.

PURPOSE: AG10 is a molecule that can bind to the thyroxine (T4) binding pocket of transthyretin (TTR). The reversible association between the hydrophilic peptides and TTR using AG10 could increase the circulation half-life (t1/2). We hypothesized that modifying PSMA-617 with AG10 could extend the t1/2 of PSMA-617 and mitigate the limitations caused by insufficient uptake. The aim of this study was to develop an optimal AG10-modified radiotherapeutic agent that could maximize both tumor uptake and absorbed dose, thereby enhancing therapeutic efficacy for the treatment of tumors, even with moder

Identification of Lymphatic and Hematogenous Routes of Rapidly Labeled Radioactive and Fluorescent Exosomes through Highly Sensitive Multimodal Imaging.

There has been considerable interest in the clinical use of exosomes as delivery vehicles for treatments as well as for promising diagnostic biomarkers, but the physiological distribution of exosomes must be further elucidated to validate their efficacy and safety. Here, we aimed to develop novel methods to monitor exosome biodistribution in vivo using positron emission tomography (PET) and optical imaging. Exosomes were isolated from cultured mouse breast cancer cells and labeled for PET and optical imaging. In mice, radiolabeled and fluorescently labeled exosomes were injected both via lymph

Preparation of ⁶⁸Ga-labelled DOTA-peptides using a manual labelling approach for small-animal PET imaging.

(68)Ga-DOTA-peptides are a promising PET radiotracers used in the detection of different tumours types due to their ability for binding specifically receptors overexpressed in these. Furthermore, (68)Ga can be produced by a (68)Ge/(68)Ga generator on site which is a very good alternative to cyclotron-based PET isotopes. Here, we describe a manual labelling approach for the synthesis of (68)Ga-labelled DOTA-peptides based on concentration and purification of the commercial (68)Ga/(68)Ga generator eluate using an anion exchange-cartridge. (68)Ga-DOTA-TATE was used to image a pheochromocytoma xen
