The clinical significance of bile duct sludge: is it different from bile duct stones?

BACKGROUND: Some patients with suspected common bile duct (CBD) stones are found to have sludge and no stones. Although sludge in the gallbladder is a precursor of gallbladder stones, the significance of bile duct sludge (BDS) is poorly defined. This study aimed to compare BDS with bile duct stones in terms of frequency, associated risk factors, and clinical outcome after endoscopic therapy. METHODS: The study enrolled 228 patients who underwent therapeutic endoscopic retrograde cholangiopancreatography (ERCP) for suspected choledocholithiasis. The patients were divided into two groups: patie

Endoscopic stenting in the management of biliary stones.

INTRODUCTION: Endoscopic sphincterotomy and stenting are established modes of treatment in the management of bile duct stones and cholangitis. It is the first choice of treatment when the patients are elderly and have other medical conditions rendering them unfit for surgery. AIMS: We studied the immediate and long-term outcome of endoscopic stenting in patients who underwent Endoscopic Retrograde Cholangiopancreatography (ERCP) for ductal stones. PATIENTS AND METHODS: From January 1990 to December 1992, 366 patients had ERCP done for biliary stone disease. Fifty-five of these patients recei

[Obstructive jaundice caused by residual and/or recurrent choledochal lithiasis: endoscopic and surgical treatment].

Results of surgical and endoscopic treatment in 79 patients (58 females and 21 males, age 40-89, mean 62), affected with jaundice, due to secondary choledochal lithiasis, are reported. Thirty-four patients (43%) underwent surgical operation: choledochotomy and T-tube 12, choledocho-jejunostomy 4, choledochoduodenostomy 3, papillosphincteroplasty 15. In 45 patients (57%) EST has been performed. Comparison of results has been obtained after 30 days and after 3-105 months (mean 49). Optimal results have been achieved in 94.9% of patients: 100% after surgery and 94.1% after EST. Morbidity and mort

Difficult biliary cannulation during ERCP: techniques and an evidence-informed practical algorithm.

Difficult biliary cannulation remains the pivotal technical barrier in therapeutic endoscopic retrograde cholangiopancreatography (ERCP) and is a major modifiable driver of post-ERCP pancreatitis (PEP). Contemporary practice has moved from prolonged standard attempts toward earlier recognition of difficult access, guidewire-based primary cannulation, targeted rescue techniques, and routine pancreatitis prophylaxis. This review summarizes current evidence and proposes a practical, evidence-informed algorithm for native papilla ERCP. Difficult cannulation is best recognized using objective proce

Clip Combined With Rubber Band vs Clip-Assisted Endoscopic Retrograde Cholangiopancreatography for Difficult Biliary Cannulation in Periampullary Diverticulum: A Propensity Score-Matched Analysis.

INTRODUCTION: Successful biliary cannulation is critical to the success of endoscopic retrograde cholangiopancreatography (ERCP). While various devices and auxiliary techniques have been developed to manage difficult cannulation in patients with periampullary diverticulum (PAD), however, comparative studies on different traction methods remain understudied. The aim of this study was to compare the safety and efficacy of clip-assisted ERCP (C-ERCP) vs clip combined with rubber band-assisted ERCP (CRB-ERCP) for difficult biliary cannulation in patients with PAD. METHODS: This study was a single

One-stage versus two-stage ERCP followed by laparoscopic cholecystectomy for the management of pediatric choledocholithiasis: A systematic review and meta-analysis.

BACKGROUND: The optimal approach to managing pediatric choledocholithiasis remains a subject of ongoing debate. Both single-stage strategies, combining laparoscopic cholecystectomy (LC) with bile duct clearance via intraoperative ERCP and sphincterotomy, and two-stage approaches, typically involving preoperative ERCP followed by interval LC, are widely employed. While prior meta-analyses in predominantly adult populations suggest comparable safety profiles between these strategies, pediatric-specific data remain lacking in consensus. METHODS: We performed a systematic review and meta-analysis

Laparoscopic common bile duct exploration for common bile duct stones after gastric surgery.

BACKGROUND: Gallstone disease is common after gastric surgery and especially after weight loss from bariatric surgery. In patients with normal gastroduodenal anatomy, treatment of common bile duct stones (CBS) generally consists of, endoscopic retrograde cholangiopancreaticography (ERCP) and endoscopic sphincterotomy (ES), followed by cholecystectomy in a second stage. However, after gastric surgery the papilla may not be easily accessible endoscopically. The aim of our study was to evaluate the therapeutic success of single-stage laparoscopic cholecystectomy and common bile duct exploration (

Practical application of the modification in endoscopic retrograde cholangiopancreatography treated common bile duct stones in patients with Billroth II gastroenterostomy in Vietnam.

OBJECTIVE: Endoscopic retrograde cholangiopancreatography (ERCP) in patients who have undergone Billroth II gastroenterostomy (B-II GE) has been challenging, requiring flexibility in technical approaches during execution. The study aims to assess the effectiveness of enhanced techniques in performing ERCP on this patient group in Vietnam. METHOD: A total of 42 Vietnamese patients with B-II GE performed an ERCP using a duodenoscope or a modification of ERCP equipment (a cap-fitted regular forward-viewing endoscope) if the former failed. The effectiveness and safety of the ERCP technique were a

Comparison of one-step laparoscopy and two-step endolaparoscopy in the treatment of secondary choledocholithiasis: A multicenter retrospective clinical study.

BACKGROUND: There are two minimally invasive ways of treating cholecystolithiasis combined with choledocholithiasis, but there remains some controversy regarding which technique is better, since they both have advantages and disadvantages. The one-step method involves laparoscopic cholecystectomy, laparoscopic common bile duct exploration, and primary close (LC + LCBDE + PC), while the two-step procedure consists of endoscopic retrograde cholangiopancreatography, endoscopic sphincterotomy, and laparoscopic cholecystectomy (ERCP + EST + LC). Preventive Saline Irrigation of the Bile Duct to Reduce the Rate of Residual Common Bile Duct Stones Without Intraductal Ultrasonography: A Systematic Review and Meta-Analysis.

Endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (EST) has been proven efficacious in the removal of CBD stones. Even after endoscopic stone removal, recurring cholangitis due to a residual common bile duct (CBD) stone is prevalent in clinical practice with a residual recurrence rate of 4-24% after successful stone retrieval. This comprehensive study and meta-analysis aimed to determine if preventive saline irrigation of the bile duct (PSIB) reduces the amount of residual CBD stones. Through a comprehensive search of PubMed, EMBASE, Cochrane Library, and Web

Anomalous Pancreatobiliary Ductal Union Presenting as Recurrent Acute and Chronic Pancreatitis in Children and Adolescents With Response to Endotherapy.

Introduction Anomalous pancreaticobiliary duct union (APBDU) is defined by the abnormal position of the junctional union of the common bile duct and the pancreatic duct, outside the duodenal wall above the influence of sphincter of Oddi, associated with choledochal cysts and biliary malignancies. APBDU may rarely present as recurrent acute pancreatitis (RAP) or chronic pancreatitis (CP). We aimed to study the prevalence of patients with APBDU presenting as RAP or CP and their response to endotherapy. Methods A retrospective audit of the prospectively maintained endoscopy database at our instit
