Phenazepam abuse in Finland: findings from apprehended drivers, post-mortem cases and police confiscations.

Phenazepam is a long-acting benzodiazepine that, unlike other benzodiazepines, is currently not scheduled as a narcotic in Finland, most other European countries or the USA. It is used as an anxiolytic, sedative-hypnotic and anti-epileptic, mainly in Russia. In Finland, as well as in some other countries, an increase in the unauthorized use of phenazepam has been observed in recent years. In the one year period between July 1, 2010 and June 30, 2011 the prevalence of phenazepam in Finland was assessed among drivers apprehended for driving under the influence of drugs (DUID), in medico-legal au

Risk of motor vehicle collisions associated with medical conditions and medications: rationale and study protocol.

BACKGROUND: Medical conditions and medications may be associated with motor vehicle collisions (MVCs), which pose a major public health problem worldwide. Further epidemiological assessment is necessary for certain diseases and medications. Moreover, since disease aetiology and patterns of medication use may differ among ethnicities and healthcare systems, a population-specific approach is necessary. The present epidemiological study is designed to assess the medical conditions and medications associated with the risk of fatal MVCs among at-fault drivers in the Korean population. METHOD AND D

Osteoarthritis of the knee or hip significantly impairs driving ability (cross-sectional survey).

BACKGROUND: Advising patients about when they can drive after surgery is common practice after arthroplasty of the knee or hip. In the literature, the preoperative braking performance values of the patients are frequently taken as the "safe" landmark. We hypothesised that osteoarthritis (OA), the most frequent reason for arthroplasty, already compromises the ability to perform an emergency stop. We expected that both Reaction Time (RT) and Movement Time (MT) as components of the Total Brake Response Time (TBRT), would be prolonged in patients with OA of the knee or hip in comparison with healt

Evaluation of haematological, hepatic and renal functions of petroleum tanker drivers in Lagos, Nigeria.

BACKGROUND: Hydrocarbons which are among the major components of petroleum products are considered toxic and have been implicated in a number of human diseases. Tanker drivers are continuously exposed to hydrocarbons by inhalation and most of these drivers do not use protective devices to prevent inhalation of petroleum products; nor do they visit hospital regularly for routine check-up. OBJECTIVE: In view of this occupational hazard, we investigated the haematological, renal and hepatic functions of workers of petroleum tankers drivers in Lagos, Nigeria. METHOD: Twenty-five tanker drivers'

Mephedrone (methylmethcathinone) in toxicology casework: a Northern Ireland perspective.

Mephedrone (4-methylmethcathinone) is the beta-keto analogue of 4-methylmethylamphetamine. Before its control in April 2010, it became popular as a legal high in the United Kingdom, displacing methylenedioxymethylamphetamine as the stimulant drug of choice. The drug has stimulant and psychoactive properties, and therefore has forensic significance in criminal and morbid toxicology. The purpose of this study was to survey casework involving the drug (impaired driving and sudden death). The cases were received in the laboratory for analysis between late 2009 and the end of 2010. Voluntary and involuntary driving cessation in later life.

This study explores the decision-making process of driving cessation in later life, with a focus on voluntariness. The sample included 83 former drivers from the Baltimore Epidemiologic Catchment Area Study. A majority of participants (83%) reportedly stopped driving by their own decision. However, many voluntary driving retirees reported external factors such as financial difficulty, anxiety about driving, or lack of access to a car as main reasons for driving cessation. These findings imply that distinction between voluntary and involuntary driving cessation is ambiguous and that factors bey

Upper extremity fracture patterns following motor vehicle crashes differ for drivers and passengers.

PURPOSE: Injury patterns of occupants in motor vehicle crashes are changing, with upper extremity injury becoming more common in patients treated at trauma centres. Although not life threatening, upper extremity injuries may result in long-term disability, including chronic deformity, neurovascular compromise and degenerative arthritis. The purpose of this study was to compare upper extremity injury in drivers and passengers using the Crash Injury Research Engineering Network (CIREN) database. METHODS: CIREN data were used to compare upper extremity skeletal injury patterns and sources for dr

Sleepiness, sleep-disordered breathing, and accident risk factors in commercial vehicle drivers.

Sleep-disordered breathing and excessive sleepiness may be more common in commercial vehicle drivers than in the general population. The relative importance of factors causing excessive sleepiness and accidents in this population remains unclear. We measured the prevalence of excessive sleepiness and sleep-disordered breathing and assessed accident risk factors in 2,342 respondents to a questionnaire distributed to a random sample of 3,268 Australian commercial vehicle drivers and another 161 drivers among 244 invited to undergo polysomnography. More than half (59.6%) of drivers had sleep-diso

Factors Associated with Hypertension and Predicted 10-year Cardiovascular Diseases Risk: A Cross-sectional Study of Commercial Vehicle Drivers in Calabar, Nigeria.

BACKGROUND: Over 85% of NCD-related deaths occur in low- and middle-income countries (LMIC), particularly Sub-Saharan Africa (SSA) where marked increases from preceding decades have been recorded. Physical inactivity and consumption of unhealthy diets are major precursors for hypertension, which has been reported as the most prevalent and important Cardiovascular diseases (CVD) risk factor. There are several reports on increased prevalence of hypertension, physical inactivity, and consumption of unhealthy diets among commercial vehicle drivers. AIMS: The study aimed to determine factors assoc

Effects of prescribed medical cannabis and alcohol on real-world driving performance (CAN-TRACK): a study protocol for a two-phase trial.

BACKGROUND: Medical cannabis is now commonly prescribed for a range of chronic health conditions. Many medical cannabis products contain delta-9-tetrahydrocannabinol (THC), the intoxicating component in cannabis, though it is unclear whether these products produce impairment when used as prescribed and at therapeutic doses. With current Australian laws prohibiting driving with any amount of THC in one's system, this trial aims to generate novel data on the impact of prescribed medical cannabis on real-world driving performance to inform road safety policy. METHODS: This is a two-phase trial,

Management of patients with type 2 diabetes in cardiovascular rehabilitation.

The clinical benefits of rehabilitation in cardiovascular disease are well established. Among cardiovascular disease patients, however, patients with type 2 diabetes mellitus require a distinct approach. Specific challenges to clinicians and healthcare professionals in patients with type 2 diabetes include the prevalence of peripheral and autonomic neuropathy, retinopathy, nephropathy, but also the intake of glucose-lowering medication. In addition, the psychosocial wellbeing, driving ability and/or occupational status can be affected by type 2 diabetes. As a result, the target parameters of c

Ethyl glucuronide and ethyl sulfate.

Alcohol misuse is associated with significant morbidity and mortality. Although clinical history, examination, and the use of self-report questionnaires may identify subjects with harmful patterns of alcohol use, denial or under-reporting of alcohol intake is common. Existing biomarkers for detecting alcohol misuse include measurement of blood or urine ethanol for acute alcohol consumption, and carbohydrate-deficient transferrin and gamma-glutamyl transferase for chronic alcohol misuse. There is a need for a biomarker that can detect excessive alcohol consumption in the timeframe between 1 day
