Efficacy and safety of fluvoxamine in body dysmorphic disorder.

BACKGROUND: Body dysmorphic disorder (BDD), a preoccupation with an imagined or slight defect in appearance, has been noted in case reports, retrospective studies, and clinical series to respond to serotonin reuptake inhibitors (SRIs). These data further suggest that the delusional variant of BDD (delusional disorder, somatic type) may also respond to SRIs. However, systematic pharmacologic treatment studies of BDD and its delusional variant are needed. METHOD: Thirty subjects with BDD or its delusional variant (DSM-IV) were prospectively treated in an open-label fashion with fluvoxamine for

Psychiatric illness in patients referred to a dermatology-psychiatry clinic.

There is a recognized psychiatric morbidity among those who attend dermatology clinics. We aimed to determine the pattern of psychological and social problems among patients referred to a liaison psychiatrist within a dermatology clinic. Notes from 149 patients were reviewed and more detailed assessments performed in a subgroup of 32 consecutive referrals. All but 5% merited a psychiatric diagnosis. Of these, depressive illness accounted for 44% and anxiety disorders, 35%. Less common general psychiatric disorders included social phobia, somatization disorder, alcohol dependence syndrome, obse

A case study of female genital self-mutilation in schizophrenia.

A case of deliberate genital self-mutilation in a woman with familial schizophrenia is presented. Such behaviour, though well recognized in females with severe personality disorders, is extremely rare in female psychotics. The genital mutilation may be partially understood as a consequence of delusionally motivated action against a background of low self-esteem, premorbid body-image preoccupation, forced early sexual activity and hence profound ambivalence towards adult sexuality. Dealing with this constructively was hampered by a cognitive style characterized by impaired reasoning and reality

Neuropsychiatric complications after cardiopulmonary bypass: cerebral protection by a barbiturate.

The authors prospectively investigated the ability of thiopental to decrease neuropsychiatric complications as a consequence of open-ventricle operations requiring cardiopulmonary bypass. Eighty-nine randomly assigned patients received sufficient thiopental to maintain electroencephalographic silence throughout the period from before atrial cannulation to termination of bypass. These patients received an average of 39.5 mg/kg of thiopental, while 93 control patients received only fentanyl. On the first postoperative day, five thiopental (5.6%) and eight control (8.6%) patients exhibited clinic

Memory binding in clinical and non-clinical psychotic experiences: how does the continuum model fare?

INTRODUCTION: Both clinical and non-clinical auditory hallucinations (AH) have been associated with source memory deficits, supporting a continuum of underlying cognitive mechanisms, though few studies have employed the same task in patient and nonpatient samples. Recent commentators have called for more debate on the continuum model of psychosis. Consequently, the current study investigated the continuity model of AH with reference to memory binding. METHODS: We used an identical voice and word recognition memory task to assess binding in two separate studies of: (1) healthy hallucination-pr

Decision making on ambiguous stimuli such as prosody by subjects suffering from paranoid schizophrenia, alcohol dependence, and without psychiatric diagnosis.

The aim of this study is the empirical verification of the Bayesian approach applied to the description of the decision-making process with regard to prosodic stimuli in different psychopathological states. Using the Bayesian formalism, the interpretation of a disturbance in internal representation of the contextual information in schizophrenia was given. The results obtained satisfied the formula derived from Bayes' theorem in all tested except a schizophrenic group. Results were interpreted as reflecting cognitive flexibility, and discussed in the context of social adaptation. Although the i

Distinguishing bipolar major depression from unipolar major depression with the screening assessment of depression-polarity (SAD-P).

BACKGROUND: Patients with bipolar I or II major depression are often misdiagnosed with unipolar major depression. The goal of this study was to develop and validate a brief instrument to screen for bipolar disorder in patients actively ill with major depression. METHOD: The sample consisted of subjects who enrolled in the National Institute of Mental Health-Collaborative Program on the Psychobiology of Depression-Clinical Studies from 1978 to 1981 during an episode of major depression and included 91 subjects with bipolar I major depression, 52 with bipolar II major depression, and 338 with u

Stage-dependent reorganization of amyloid PET region-symptom bipartite networks in drug-naïve, amyloid-positive Alzheimer's disease.

Management of neuropsychiatric symptoms (NPS) is pivotal to care in Alzheimer's disease (AD), yet their alignment with amyloid topology may vary by stage. We examined whether the community (modular) structure of a brain-region-symptom co-occurrence network differs by clinical stage, testing the hypothesis that modular organization is more pronounced in earlier AD and becomes less segregated with increasing severity. In a cross-sectional retrospective cohort from a tertiary dementia clinic, we included 301 consecutive, drug-naïve patients with probable AD and amyloid-positive PET (18F-FC119S).

Psychotic illness in people with Prader-Willi syndrome: a systematic review of clinical presentation, course and phenomenology.

BACKGROUND: Prader-Willi syndrome (PWS) is a rare and complex neurodevelopmental disorder resulting from absent paternal expression of maternally imprinted genes at chromosomal locus 15q11-13. This absence of expression occurs as a consequence of a deletion on the chromosome 15 of paternal origin (ca. 70%), a chromosome 15 maternal uniparental disomy (mUPD; ca. 25%), or an imprinting centre defect (IC; ca. 1-3%). At birth, individuals with PWS are severely hypotonic and fail to thrive. Hyperphagia and characteristic physical and neuropsychiatric phenotypes become apparent during childhood. The

Targeted psychological and psychosocial interventions for auditory hallucinations in persons with psychotic disorders: Protocol for a systematic review and meta-analysis.

BACKGROUND: In recent years, a growing body of evidence has demonstrated the efficacy of non-pharmacological interventions for schizophrenia spectrum disorders (SSD) including positive symptoms such as auditory hallucinations (AH). However, clinical trials predominantly examine general treatment effects for positive symptoms. Therefore, previous research is lacking in comprehensive and clear evidence about psychological and psychosocial approaches that are primarily tailored to treat AH. To overcome this knowledge gap in the current literature, we will conduct a systematic review and meta-anal

Ethical challenges in the treatment of psychotic pregnancy denial.

BACKGROUND: There is a paucity of literature regarding ethical strategies for treating pregnant people with psychosis. While not uncommon, psychotic pregnancy denial is a psychotic illness in which patients have the delusion that they are not pregnant. The authors provide a literature review regarding psychotic pregnancy denial, present an unpublished case and its questions and dilemmas, and offer recommendations for resolving the ethical challenges these cases raise. CASE: A 26-year-old, single, unemployed woman of no fixed residence was admitted for suicidal ideation. She had a history of p

Akathisia after chronic usage of synthetic cathinones: A case study.

INTRODUCTION: Synthetic cathinones are a heterogenous group of new psychoactive substances (NPS) with a surging prevalence of use. They are sold under the name of "Bath Salts," "Legal Highs" or "Research Chemical." It is a heterogeneous group of substances that have a varying binding selectivity and affinity. Due to limited availability of NPS screening tests, inadequate legislation, the exponential increases in availability of new NPS and the comorbid use of other illicit substances, scientific research remains scarce. As a result, insight in their mental and psychomotor effects is limited.

Consumption of psychoactive substances among 535 women entering a Lyon prison (France) between June 2004 and December 2008.

BACKGROUND: In France, recent data on the consumption of psychoactive substances (PASs) among women entering prison are virtually nonexistent. The objective of this study was to describe the characteristics of female entrants at Montluc prison in Lyon (France) and to estimate their PAS consumption. METHODS: Between June 1, 2004 and December 31, 2008, of 841 women entering the Lyon correctional facility, 535 had an entrance interview, conducted by a nurse, during which a questionnaire was systematically proposed; 306 detainees did not have this interview and could not be included in the study

[MRI findings in schizophrenic patient with frontal lobe atrophy].

In schizophrenic patients, cognitive and behavioral deficits are often associated with frontal lobe dysfunction. By functional neuroimaging studies, abnormalities of the frontal lobe (especially the prefrontal lobe) have been detected in schizophrenic patients. As for morphological changes on neuroimaging, lateral lobe atrophy and enlargement of the lateral ventricles or third ventricle are often reported, but there is no consensus as to whether frontal lobe atrophy is seen more frequently in schizophrenic patients compared with normal controls. The reasons for this disagreement include variat

Coexistent hypothyroidism, psychosis, and severe obsessions in an adolescent: a 10-year follow-up.

UNLABELLED: This is the first longitudinal report on possible psychosis resulting from the juvenile onset of hypothyroidism. A 10-year follow-up in the case of a 13-year-old boy published in this journal in 1993 is presented. The patient presented with a diagnostic dilemma. Although psychosis resulting from hypothyroidism was the most parsimonious explanation of his symptoms (new-onset auditory hallucinations, severe obsessions, and severe hypothyroidism), a primary psychiatric disorder (obsessive-compulsive disorder [OCD] or psychotic depression) aggravated by hypothyroidism could not be excl

[Back to the high school days: an abnormal remembering characterized by concurrent as well as alternating feeling to be in the past and present].

After the successful treatment of a hypothalamic germinoma, a 31-year-old right-handed male developed a difficulty in memory and admitted to our department for detailed evaluation. Neuropsychologically he showed no personality change, confabulation, misidentification, delusion or disorientation to physical time. Neither aphasia, dementia or frontal lobe dysfunction was found. However, he showed a moderate degree of anterograde amnesia and a retrograde amnesia for the last 5 years judged by a public events test. Moreover, his temporal markings of a correctly remembered event in the post high sc

[Case report: electroconvulsive therapy during pregnancy].

The treatment for psychiatric disorders in pregnancy remains difficult to implement. We report the case of a 28-year-old woman, 20 weeks pregnant when admitted in our psychiatric department. She presented severe depressive disorder, associated with agitation, and psychotic symptoms as delusion and hallucinations occurred. The patient had a history of recurrent mood disorders dating back to eight years before the current admission, including some atypical episodes (psychotic symptoms only), and alternating with free periods without any trouble. A non-specific personality disorder is also probab

Modulation of NMDA and dopaminergic neurotransmissions by sigma ligands: possible implications for the treatment of psychiatric disorders.

Sigma (sigma) receptors, improperly classified as belonging to the opiate receptor family when discovered in 1976, were subsequently confused with phencyclidine binding sites for several years. It's only recently, with the emergence of new selective ligands that their functional significance could be meaningfully addressed. Several subtypes of sigma receptors are present in high densities in the limbic structures as well as in motor-related areas of the CNS. Different lines of evidence suggest that a major role for sigma receptors might be to regulate the activity of the glutamatergic system v

Psychiatric and Neurocognitive Adverse Events in Young Users of Nonmedical Nitrous Oxide.

PURPOSE/BACKGROUND: For several years, an increase in the nonmedical use of nitrous oxide (N 2 O) has been observed in France, among adolescents and young adults, sometimes associated with severe adverse events. METHODS/PROCEDURES: Analysis of spontaneous reports (SRs) concerning neuropsychiatric adverse events in N 2 O users, received by the Montpellier addictovigilance centre between 2022 and June 2024, and discussion of the underlying pharmacological mechanisms. FINDINGS/RESULTS: Nineteen severe SRs were analysed, mainly in men (73.6%, mean age: 23.1 ± 7.3 y) and concerned psychiatric dis
