A 13-year followup of a case of Wallenberg's syndrome: first-person account and ocular motor abnormalities.

BACKGROUND: We present a 13-year followup of a case of Wallenberg's syndrome, including a first-person account and ocular motor abnormalities. METHODS: Objective infrared eye-tracking systems were used to record horizontal and vertical eye movements binocularly under a variety of conditions. RESULTS: Fixation in the dark along the midline remained as before, with vestibular jerk nystagmus continuously present. While saccades and pursuit to a visual target also remained the same as found earlier, fixation was somewhat worse, revealing not only saccadic intrusions but also a prominent right-je

Visual-field loss with optic nerve drusen and ocular hypertension: a case report.

BACKGROUND: Patients with optic nerve drusen are often asymptomatic and free of visual symptoms. However, ocular complications such as visual-field defects may develop. This article presents the case of a patient with optic nerve drusen, ocular hypertension, and a visual-field defect. The diagnosis and management of patients with such findings will be presented. CASE REPORT: A 75-year-old man came to the eye clinic with a history of being treated for glaucoma. On examination, optic nerve drusen were found in both eyes. Subsequent testing revealed ocular hypertension and a visual-field defect

An unusual case of stabbing eye pain: a case report and review of trigeminal neuralgia.

BACKGROUND: Trigeminal neuralgia is a painful neurological disorder that affects one or more of the divisions of the trigeminal nerve. It is characterized by brief attacks of stabbing pain that can be excruciating. These attacks may be triggered by a light touch, shaving, or even eating. There has been much debate over the exact etiology of trigeminal neuralgia. One of the main theories is vascular compression of the trigeminal nerve as it leaves the brainstem. Another theory suggests that intracranial tumors--particularly those located in the posterior fossa--may be the cause. Trigeminal neur

Hypotonous maculopathy and normal visual acuity secondary to post-traumatic cyclodialysis cleft.

BACKGROUND: Post-traumatic cyclodialysis clefts commonly lead to profound visual loss secondary to hypotonous maculopathy. Current literature regarding cyclodialysis clefts definitively outlines the management of visual loss resulting from hypotonous maculopathy. Because hypotonous maculopathy with normal visual acuity is rarely reported, management is less well defined. Management guidelines for individuals with and without visual loss secondary to hypotonous maculopathy are reviewed. CASE REPORT: A 32-year-old man came to us with symptoms of blur and glare in his left eye following airbag t

Clinical observations of pharmaceutical, physical, and visual evidence of presumed extraocular myositis and tendonitis.

PURPOSE: This study examines data from pharmaceutical, physical, and visual tests that support the possible diagnosis of presumed extraocular myositis and tendonitis (PEM-T). METHOD: The presence of ocular pain derived from inflamed extraocular musculature and tendons in 60 subjects with PEM-T was detected by palpating their globes near the eye muscle insertions. The pain sensations were subjectively rated and recorded. Because the sensation of pain elicited by such palpation was most-often unilateral, the study compares the affected sidewith pain to the side tha exhibited vision disturbance

Pseudotumor cerebri in a pre-pubescent child--case report.

BACKGROUND: Pseudotumor cerebri (PT) is a disease characterized by elevated intracranial pressure with no apparent etiology. A majority of cases of PTC occur in adults, with a distinct predilection for obese women of childbearing age. On rare occasions, PTC will affect children and is associated with different patient characteristics. CASE REPORT: A slender 10-year-old boy was brought to clinic with a one-day history of intermittent horizontal diplopia and a two-week history of severe headaches on awakening. Visual acuity, pupil function, and color vision were all normal. Cover test revealed

Unusual eye movements in a patient with complex partial seizure disorder.

BACKGROUND: A complex partial seizure can cause a variety of visual system signs and symptoms, including visual hallucinations, dilated pupils, and changes in vision. Little information is known about the influence of this disorder on the visual system during nonseizure moments. This case report examines the unusual eye movements-during these nonseizure times-of a patient diagnosed with complex partial seizure disorder. CASE REPORT: An 8-year-old boy was referred to the clinic by his pediatric neurologist for a comprehensive examination to rule out a visual cause for the abnormal eye movement

Rarely reported cases of congenital atresia of nasolacrimal puncta.

BACKGROUND: Nasolacrimal punctal atresia has been rarely reported in the literature. It can occur in isolation or in association with other congenital anomalies. METHODS: Our three cases, seen within a 6-month period in ambulatory eye care centers, had no associated congenital craniofacial anomalies. Case 1 was a 35-year-old woman, Case 2 was a 51-year-old man, and Case 3 was a 47-year-old man. Cases 1 and 2 were asymptomatic because they were missing only the inferior puncta and no intervention was required. Only Case 3 was missing both upper and lower punctal openings of each eye and thus w

Pupil block glaucoma from traumatic vitreous prolapse in a patient with posterior chamber lens implantation.

BACKGROUND: Angle closure secondary to pupil block is an entity known to occur in aphakic and pseudophakic patients. In aphakic patients, typically the cause of the pupil block is vitreous prolapse (aphakic pupil block). In pseudophakic patients, the typical cause of the pupil block is an anterior chamber lens implant, often in the absence of an iridectomy (pseudophakic pupil block). Rarely, a hybrid of these two conditions can occur, in which vitreous prolapse causes a pupil block in a pseudophakic patient with a posterior chamber lens implant. METHODS: The case presented is that of an elder

Corneal pigmentation in overnight orthokeratology: a case series.

BACKGROUND: Iron deposition within the basal corneal epithelium, or a Hudson-Stahli line, is a common physiological finding. Variations of this deposition can be seen after surgical refractive procedures and as a result of comeal disease. This case series provides evidence of comeal deposition of a similar nature in patients who wear reverse geometry rigid gas-permeable contact lenses for overnight orthokeratology. To date, similar findings associated with orthokeratology have not been published. CASE REPORTS: Deposition-presumably of iron-was found in the corneal epithelium in a small sample

Optical complications in congenital rubella syndrome.

BACKGROUND: Post-natal rubella (German measles) is a mild illness, occasionally complicated by arthritis or encephalitis. In contrast, in utero infection can have devastating effects. collectively known as congenital rubella syndrome (CRS). METHODS: Through the introduction of measles, mumps, and rubella (MMR) vaccination programs, the incidence of notifications and deaths from rubella have been reduced by 96%, to 0.4 per 100,000 births. However, CRS persists in patients who predate the commencement of these programs (c. 1969), as well as those not immunized and--rarely--in re-infection of th

Suprasellar squamous papillary craniopharyngioma: a case report.

BACKGROUND: Craniopharyngiomas are benign neoplasms that are usually suprasellar in origin. They are considered a type of pituitary tumor that can cause hyposecretion of anterior pituitary hormones or hypopituitarism. Clinically, they may cause visual and endocrine deficiencies similar to pituitary adenoma. Histopathologically, they are distinct from pituitary adenoma and are generally classified as either adamantinomatous or squamous papillary. Adamantinomatous tumors are more common in children, whereas the squamous papillary type is found almost exclusively in adults. CASE REPORT: A 45-yea

Characteristics of the pediatric/adolescent low-vision population at the Illinois School for the Visually Impaired.

BACKGROUND: There are few descriptive studies of children and adolescents with low vision in the United States. Our study describes a pediatric/adolescent population at the Illinois School for the Visually Impaired (ISVI), detailing the etiologies of low vision and the devices prescribed. METHODS: A retrospective record review was conducted of 260 consecutive students who had been examined at the twice-yearly Lions of Illinois Low Vision Clinic at ISVI between the years 1991 and 1999. Data collected included age, race, sex, best-corrected visual acuity, ocular diagnoses, the etiology of low v

Review of the diagnosis and management of acquired nasolacrimal duct obstruction.

BACKGROUND: Acquired nasolacrimal duct obstruction is a common cause of epiphora. The absolute or partial blockage of the nasolacrimal duct occurs most frequently in middle-aged and elderly women. In addition to the troublesome symptom of epiphora, acquired nasolacrimal duct obstruction is predominantly the underlying cause of both acute and chronic dacryocystitis. METHODS: The primary eye care provider plays an essential role in the diagnosis and management of the patient with epiphora. The potentially serious ramifications of acquired nasolacrimal duct obstruction demand a thorough understa

Eye conditions among 5- to 7-year-old Asian-Pacific Islander schoolchildren in Southern California.

BACKGROUND: The prevalence rates and specific risk factors for development of eye conditions among Asian-Pacific Islander schoolchildren are unknown. This information is necessary to understand their eye care needs and to improve access to eye care for this rapidly growing population. METHODS: Data obtained from a computer database of vision screening examinations conducted by the UCLA Mobile Eye Clinic from October 1987 through December 1996 were analyzed for 2,687 Asian-Pacific Islander schoolchildren between 5 and 7 years of age. RESULTS: Myopia (8.9%) and astigmatism (15.8%) represented

Microcysts: clinical significance and differential diagnosis.

BACKGROUND: Microcysts are the most-distinctive and easily detectable indicator of contact lens-induced hypoxia. They should not be confused with cyst-like inclusions that occur in conditions such as Meesmann's dystrophy, bullous keratopathy, and Cogan's microcystic dystrophy, or with mucin balls, vacuoles, microcystic edema, and infiltrates. METHODS: Data from published literature and recent data from The Cornea and Contact Lens Research Unit (CCLRU) clinical trials involving low Dk and prototype high Dk soft contact lenses (SCLs) were examined. RESULTS: Extended wear with low Dk SCLs induc
