Long-term outcome of primary versus secondary intraocular lens implantation after simultaneous removal of bilateral congenital cataract.

BACKGROUND: Long-term outcomes of intraocular lens (IOL) implantation for congenital cataract in children under 2 years old are still undetermined. METHODS: We retrospectively reviewed all cases of bilateral congenital cataract who had undergone simultaneous bilateral cataract removal with posterior capsulotomy and central anterior vitrectomy between 1990 and 2010. Patients randomly underwent primary IOL implantation or secondary IOL implantation after a period of contact lens wear. The two groups were compared for visual outcome and complications during follow-up. RESULTS: Cataract removal

Confirmation of ADAMTSL4 mutations for autosomal recessive isolated bilateral ectopia lentis.

Ectopia lentis (EL) is a zonular disease where alteration of the zonular fibers leads progressively to lens dislocation. It is most often associated with systemic diseases such as Marfan syndrome, Weill-Marchesani syndrome or homocystinuria. Isolated non syndromic ectopia lentis (IEL) is reported in families with autosomal inheritance, with dominant forms being more common than recessive. LTBP2 truncating mutations have been described as a cause of autosomal recessive ectopia lentis as a primary or secondary feature in patients showing ocular (eg, glaucoma) or extraocular manifestations (eg, M

A two-year trial of intraocular lenses at the Wilmer Institute.

Using the Binkhorst four-loop intraocular lens, we began a trial of intraocular lens implantation in 1975. We compared the operative and postoperative complications and the visual results of 100 implant cases during a two-year period to those of 100 age-matched aphakic control cases. Although the additional steps of lens implantation resulted in more operative and postoperative complications in the intraocular lens group, the visual results were good and were the same for both groups at an average follow-up of 12 months. Because the long-term follow-up results of implant cases are unknown, we

Comparison of intraocular irrigating solutions in pars plana vitrectomy.

A prospective, randomized, masked clinical study was conducted to examine the effects of pars plana vitrectomy on the corneal endothelium comparing Balanced Salt Solution (BSS) to BSS-Plus as an intraocular irrigating solution. Seventy-one patients were followed for at least six months. A significant 6.9% decrease in endothelial cell density was observed among all patients at six months postoperatively, compared to preoperative values, using the contralateral eye as a control. However, no significant difference in endothelial cell loss was found between BSS and BSS-Plus eyes. Both aphakic eyes

Prediction error and myopic shift after intraocular lens implantation in paediatric cataract patients.

AIM: To determine the amount of myopic shift in children after cataract surgery with intraocular lens (IOL) implantation and to evaluate success in achieving the target refraction. METHODS: The children were assigned into three groups depending on age at time of surgery: Group A, 0-1 years old; Group B, 1-7 years old; Group C, 7-18 years old. Multiple regression analysis was used to create a formula for expected myopic shift and to find out which variables were associated with a higher absolute prediction error. RESULTS: Children less than 12 months of age experienced higher myopic shifts an
