Road traffic accidents and psychotropic medication use in The Netherlands: a case-control study.

AIM: To examine the association between the use of commonly prescribed psychotropic medications and road traffic accident risk. METHODS: A record-linkage database was used to perform a case-control study in The Netherlands. The data came from three sources: pharmacy prescription data, police traffic accident data and driving licence data. Cases were defined as drivers, who had a traffic accident that required medical assistance between 2000 and 2007. Controls were defined as adults, who had a driving licence and had no traffic accident during the study period. Four controls were matched for e

First nationwide study on driving under the influence of drugs in Switzerland.

In Switzerland, a two-tier system based on impairment by any psychoactive substances which affect the capacity to drive safely and zero tolerance for certain illicit drugs came into force on 1 January 2005. According to the new legislation, the offender is sanctioned if Delta(9)-tetrahydrocannabinol THC is >or=1.5ng/ml or amphetamine, methamphetamine, 3,4-methylenedioxymethamphetamine (MDMA), 3,4-methylenedioxyethylamphetamine (MDEA), cocaine, free morphine are >or=15ng/ml in whole blood (confidence interval+/-30%). For all other psychoactive substances, impairment must be proven in applying t

Long-term seizure and social outcomes following temporal lobe surgery for intractable epilepsy during childhood.

PURPOSE: Studies of adults who underwent temporal lobectomy for intractable temporal lobe epilepsy (TLE) demonstrated declining seizure free rates over time. Using seizure and social parameters, we followed patients who had temporal lobe surgery (TLS) in childhood to determine long-term outcomes. METHODS: We identified 42 patients who underwent TLS for medically intractable epilepsy during childhood. Follow-up data were collected from 10 to 20 years after surgery (median, 12 years). We studied histopathology, seizure outcome, employment, school enrolment, and driver's licensing. Patients or p

Driving cessation and self-reported car crashes in older drivers: the impact of cognitive impairment and dementia in a population-based study.

The complexity of driving activity has incited numerous developed countries to initiate evaluative procedures in elderly people, varying according to first evaluation age, frequency, and screening tools. The objective of this paper is to improve the knowledge of the driving cessation process regarding factors associated with crash involvement. Driving cessation and self-reported crashes during the past 5 years were analyzed with multivariate models, in a cross-sectional study including a population-based sample of 1051 drivers aged 65 years and more. Visual trouble, Parkinson disease, dementia

A study investigating the acute dose-response effects of 13 mg and 17 mg Delta 9- tetrahydrocannabinol on cognitive-motor skills, subjective and autonomic measures in regular users of marijuana.

Heavy use of marijuana is claimed to damage critical skills related to short-term memory, visual scanning and attention. Motor skills and driving safety may be compromised by the acute effects of marijuana. The aim of this study was to investigate the acute effects of 13 mg and 17 mg Delta 9-tetrahydrocannabinol (THC) on skills important for coordinated movement and driving and on subjective and autonomic measures in regular users of marijuana. Fourteen regular users of marijuana were enrolled. Each subject was tested on two separate days. On each test day, subjects smoked two low-nicotine cig

[Syncope and professional driving: the role of the occupational physician].

BACKGROUND: Road traffic injuries constitute one of the main causes of death and disability in Italy and in the European Union. Although much scientific evidence exists on the important role of complete loss of consciousness in the dynamics of motor vehicle accidents, at present there is no specific legislation in Italy. Occupational medicine should take particular care in this field, because every year a large number of accidents with fatal outcomes occur among truck drivers. Via health surveillance, the occupational physician can play an important role in the prevention of such events. OBJE

Evaluation of UV-A and UV-B transmission through the windows of gas, hybrid, and electric vehicles.

UV-A exposure is a major risk factor for melanoma, nonmelanoma skin cancer, photoaging, and exacerbation of photodermatoses. Since people spend considerable time in cars daily, inadequate UV-A attenuation by car windows can significantly contribute to the onset or exacerbation of these skin diseases. Given recent market trends in the automobile industry and known impact of car windows on cumulative lifelong UV damage to the skin, there is a need to comparatively evaluate UV transmission across windows in electric vehicles (EV), hybrid vehicles (HV), and gas vehicles (GV) as well as variability

Risk of road traffic accidents associated with the prescription of drugs: a registry-based cohort study.

PURPOSE: The aim of this study was to examine the risk of a car driver being involved in a road traffic accident while using prescribed drugs. We used data from population-based registries. METHODS: Information on prescriptions, road traffic accidents, and emigrations/deaths was obtained from population-based registries. The incidence of accidents in the exposed person-time was compared with the incidence in the unexposed person-time, by the standardized incidence ratio (SIR). All Norwegians ages 18-69 between April 2004 and September 2005 (3.1 million), were included in the study. RESULTS:

Hits and misses: screening commercial drivers for obstructive sleep apnea using guidelines recommended by a joint task force.

OBJECTIVE: To evaluate joint task force criteria-based screening for severe obstructive sleep apnea (s-OSA) in commercial drivers. METHODS: Among a community-based cohort of licensed commercial vehicle drivers, we assessed utility of the joint task force criteria. We conducted full, 14-channel overnight polysomnography in all drivers, defining s-OSA as an apnea-hypopnea index of 30 or more per hour. RESULTS: One hundred of 104 drivers with successful polysomnography studies were predominantly obese (median body mass index = 32.8 kg/m; interquartile range = 26.8 to 37.4) and had a median apne

Evaluation of the PSAMAO "Roulez Protégé" mass media campaign in Burkina Faso. Prévention du SIDA sur les Axes Migratoires de l'Afrique de l'Ouest.

This study assesses the impact of the Prevention du SIDA sur les Axes Migratoires de l'Afrique de l'Ouest Roulez Protege mass media campaign in Burkina Faso on truckers' likelihood of talking with colleagues about HIV/AIDS, using condoms with an occasional sexual partner, and intending to use a condom in the future. A total of 764 and 1,032 truckers were interviewed in 1997 and 2000 along the Ouagadougou-Niangoloko trucking route. Truckers' exposure to the program has been high and the main message promoted has been greatly understood among those exposed. Logistic regression analyses suggest t

High concentrations of diazepam and nordiazepam in blood of impaired drivers: association with age, gender and spectrum of other drugs present.

A zero-concentration limit for controlled scheduled drugs in the blood of motorists was introduced in Sweden in 1999 and the annual number of arrests for driving under the influence of drugs (DUID) has since increased eight-fold. However, for prescription drugs that might cause impairment (e.g. benzodiazepines) additional proof is needed to justify prosecution, such as whether the medication was being misused. Over a 2-year period, we found 94 cases of DUID in which the concentrations of diazepam in blood was 1.1 microg/g or more. Diazepam (D) and nordiazepam (ND) were determined in whole bloo

Drugs, alcohol and driving.

Driving a car is a complex psychomotor and perceptual task which is subject to impairment by many factors. Several workers have studied the potential effects of drugs and alchol in crash production by epidemiological and laboratory studies. Both types of studies have yielded useful data but their limitations must be borne in mind when applying the results in pratice. Alcohol is obviously the most common single cause of traffic accidents. A progessively increased risk with increasing blood alcohol levels is well documented; fatigue and/or drugs increase this risk. Drugs are related much more in

Psychiatric consequences of stress after a vehicle accident.

BACKGROUND: Vehicle accidents are a common cause of disease and death among people over 30 years of age. Essentially, reaction to stress due to the vehicle accident does not differ from the reaction to other stress factors. There are still no uniform viewpoints about the kind of sequels and their percentage representation after vehicle accidents. SUBJECTS AND METHODS: The research was provided as a prospective study, included 150 subjects who had vehicle accident minimum 2 years prior to the examination. A questionnaire adjusted to the needs of the research and a battery of psychological test

Integrating science, practice and reflexivity--cognitive therapy with driving phobia.

The scientist-practitioner model has long been espoused within cognitive behavioural psychotherapy. The study includes an evaluation of the efficacy of cognitive therapy for driving phobia. Assessment, formulation and practical treatment of this common problem are also reported. Self-report measures evaluated results. Eight of the 10 consecutive cases improved significantly and reported that the intervention was acceptable. A reflexive analysis of the research findings and the influences on the first author's practice as a researcher and practitioner are also explored. Recommendations for futu

[Polymorphisms in Sleep and Cognitive Function Related Genes are Associated with Vehicle Crash History in Shift Working Bus Drivers].

Bus driving is a highly responsible profession. It requires constant engagement of attention and cog- nitive resources as well as vigilance and resistance to disruption of sleep- rhythms. There is ongoing research of genetic correlates of these individual characteristics. The aim of this study is to search for possible connections between single nucleotide polymorphisms and the history of road traffic accidents in professional bus driving. 299 professional drives working on rolling shifts with recorded history of traffic accidents took part in the study. Conflicts of interest: research and clinical care.

Conflicts of interest and other ethical dilemmas occur in Alzheimer disease (AD) patient care and research but often are underrecognized by physicians. One or more bioethical principles of autonomy, confidentiality, truth telling, beneficence, and justice, which apply to demented individuals as they do to other patients, may conflict in everyday clinical situations. For example, when demented patients wish to continue driving, autonomy (the patient's right to self-determination) conflicts with beneficence (the duty to promote the patient's welfare). Cognitively impaired patients depend on othe
