Needle-knife fistulotomy in patients with periampullary diverticula and difficult bile duct cannulation.

BACKGROUND AND AIM: Needle-knife fistulotomy has commonly been used for overcoming difficult bile duct cannulation. Periampullary diverticula (PAD) can be an impediment to endoscopic retrograde cholangiopancreatography (ERCP) procedures. There are little data on needle-knife fistulotomy in patients with PAD. We evaluated the efficacy and safety of needle-knife fistulotomy between patients with and without PAD. METHODS: Data from December 2005 to October 2010 were reviewed. Patients who underwent needle-knife fistulotomy were divided into the group with PAD and the group without PAD (control g

Endoscopic versus surgical treatment of downstream pancreatic duct stones in chronic pancreatitis.

Pancreatic duct stone is thought not only to be the cause of abdominal pain but also to be a risk factor for pancreatic cancer. Several treatment options have been implemented in the treatment of pancreatic duct stones. Stone location is a critical factor in selecting treatment. We present the results of 27 endoscopic treatments and 35 surgical treatments performed in three hospitals at a single university between January 2000 and January 2005. The results were compared retrospectively in terms of success rate of stone removal, length of hospital stay, complications, pain relief, and changes o

The role of endoscopy in the therapy of chronic pancreatitis.

Almost all the therapeutic efforts in the treatment of chronic pancreatitis are directed towards pain control. Endoscopic techniques available for this purpose are endoscopic retrograde cholangiopancreatography (combined or not with extracorporeal shock wave lithotripsy) and endoscopic ultrasound. Pancreatic stones and strictures, pancreatic pseudocysts, and common bile duct strictures complicating chronic pancreatitis can be treated by endoscopy. The development of endoscopic ultrasound extended the possibilities in the treatment of pancreatic pseudocysts and main pancreatic duct drainage. En

Double balloon endoscopy for pancreatic and biliary access in altered anatomy (with videos).

BACKGROUND: Access to the papilla of Vater or enteral anastomoses to the biliary tract or pancreatic duct is difficult in patients with altered anatomy. The usual approach to the papilla of Vater with a side-viewing duodenoscope, designed for passage through the stomach, pyloric channel, and proximal duodenum, is not suitable in postoperative patients with challenging anatomic rearrangements. There is therefore a need for better instrumentation to achieve access in patients with difficult anatomy. OBJECTIVE: To assess the potential of the new double balloon endoscope system for use in difficu

Young female with pancreaticobiliary maljunction presenting with acute pancreatitis: a case report and review of the literature.

We report a case of pancreaticobiliary maljunction which presented with acute pancreatitis. Pancreaticobiliary maljunction and its complications are mostly observed in the Asian population. There are only few western publications concerning this subject. We reviewed the literature for current knowledge and opinions concerning the pathophysiology and optimal treatment, with special emphasis on the oncologic aspect of this condition. Those patients without a choledochal cyst should at least receive a prophylactic cholecystectomy. Firstly, to prevent further pancreatitis due to biliopancreatic re

[Differentiated surgical tactic in patients with an acute biliary pancreatitis and the jaundice syndrome].

The outcome was analyzed in 466 patients, treated for an acute pancreatitis in 1997--2003 yrs. In 187 (40.1%) of patients the jaundice had occurred. The surgical tactic was elaborated, depending on the jaundice origin. The extrahepatic biliary ducts pathology have had constituted the acute pancreatitis etiology in 119 (64%) patients and the alimentary causes--in 68 (36%). The predominant application of miniinvasive methods, such as endoscopic papillosphincterotomy with lithoextraction, laparoscopic cholecystectomy, transcutaneous puncture-draining interventions under ultrasonographic control,

Autoimmune pancreatitis associated with immune-mediated inflammation of the papilla of Vater: report on two cases.

Autoimmune pancreatitis (AIP) is defined histologically by periductal and interacinar lymphocytic infiltration. Immunohistochemically, the majority of these lymphocytes are identified as T cells. Epithelial HLA-DR antigen expression was also described as a marker of autoimmunity in this type of chronic pancreatitis. We report 2 cases, a 56-year-old man and a 29-year-old woman, with AIP associated with immune-mediated inflammation of the main duodenal papilla (MDP). Serologically, antinuclear antibody positivity was detected in the male patient. The female patient, treated medically for ulcerat

Complete disruption of the main pancreatic duct: endoscopic management.

BACKGROUND: Complete disruption of the main pancreatic duct is an unusual event in the course of acute or chronic pancreatitis. Endoscopic management has already proven effective in the treatment of partial ruptures. METHODS: Thirteen patients presented over a 7-year period with acute (9 patients) or chronic (4 patients) pancreatitis complicated by complete disruption of the main pancreatic duct and cyst formation. Endoscopic treatment was attempted in 12. Treatment varied depending on the site of the rupture and accessibility of the pseudocyst and consisted either of transpapillary drainage

A peptide-first hemostatic strategy for oozing-type postsphincterotomy bleeding during endoscopic retrograde cholangiopancreatography: a multicenter noninferiority randomized controlled trial (PROTECT-EST).

BACKGROUND AND AIMS: Immediate bleeding after endoscopic sphincterotomy (EST) is a common adverse event during ERCP. However, the optimal first-line hemostatic strategy remains undefined because comparative randomized trials are lacking. We aimed to evaluate the noninferiority of a self-assembling peptide (SAP)-first strategy compared with conventional balloon tamponade for oozing-type immediate post-EST bleeding, with superiority testing performed as a prespecified exploratory analysis. METHODS: PROTECT-EST (A peptide-first hemostatic strategy for oozing-type postsphincterotomy bleeding duri

Complicated Biliary Stenosis Following Blunt Abdominal Trauma: A Case Report.

Bile duct injuries following blunt abdominal trauma are rare complications that may go unnoticed during emergency care. Due to edema and inflammation, these injuries can lead to bile duct obstruction, resulting in dilation of the hepatobiliary system and potential perforations. Such cases are medical emergencies that require prompt and effective management. This case report aims to present a rare case of bile duct stenosis secondary to blunt abdominal trauma, managed using a minimally invasive approach. Outcomes following endoscopic retrograde cholangiopancreatography (ERCP) are also described

Post-ERCP adverse events in children: a systematic review and meta-analysis of post-ERCP pancreatitis incidence with synthesis of risk factors and prevention strategies.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is increasingly used in children with pancreatobiliary disease, but pediatric adverse-event estimates remain variable. We updated the evidence on post-ERCP adverse events in children, pooled post-ERCP pancreatitis (PEP) incidence when study-level data allowed, and summarized reported risk factors and prevention strategies. METHODS: We conducted a systematic review and meta-analysis of English-language literature published from January 2015 to June 2025 in PubMed, Embase, Web of Science, and the Cochrane Library. Endoscopic retrograde cholangiopancreatography for the management of biliary fistula following liver hydatid cyst related surgery or radiological interventions.

BACKGROUND: Surgery and percutaneous radiological methods [puncture, aspiration, injection, re-aspiration (PAIR)] are the current invasive treatment strategies for patients with hepatic hydatid cyst (HHC). Biliary leak is a common complication in patients who underwent these treatments of HHC. Bile leak should be treated effectively as uncontrolled biliary fistula may lead to life-treating conditions such as severe cholangitis, intraabdominal abscesses and septicemia. Endoscopic retrograde cholangiopancreatography (ERCP) has become the main treatment of post-interventional biliary fistula. AI

Robotic Whipple for Abnormal Pancreaticobiliary Junction With Recurrent Acute Pancreatitis: A Case Report.

Pancreaticobiliary maljunction (PBM) is a congenital anomaly that predisposes patients to recurrent pancreatitis, choledocholithiasis, and an increased risk of biliary tract malignancy. In patients without biliary dilatation, standard management typically consists of cholecystectomy with endoscopic biliary decompression. However, management can be challenging in patients with refractory recurrent pancreatitis, indeterminate strictures, or progressive ductal changes. We describe a 62-year-old woman with PBM who presented with recurrent acute pancreatitis despite undergoing cholecystectomy, bili

Prediction for post-ERCP pancreatitis in non-elderly patients with common bile duct stones: a cross-sectional study at a major Chinese tertiary hospital (2015-2023).

BACKGROUND: Post-ERCP pancreatitis is one of the most common adverse events in ERCP-related procedures. The purpose of this study is to construct an online model to predict the risk of post-ERCP pancreatitis in non-elderly patients with common bile duct stones through screening of relevant clinical parameters. METHODS: A total of 919 cases were selected from 7154 cases from a major Chinese tertiary hospital. Multivariable logistic regression model was fitted using the variables selected by the LASSO regression from 28 potential predictor variables. The internal and external validation was ass

Combined rectal indomethacin and intravenous saline hydration in post-ERCP pancreatitis prophylaxis.

BACKGROUND AND STUDY AIM: Acute pancreatitis (AP) is a potentially life-threatening complication of endoscopic retrograde cholangiopancreatography (ERCP). There is a lack of effective measures to prevent post-ERCP pancreatitis (PEP), except NSAIDs. Aggressive hydration for AP can be considered, given the frequency of hemoconcentration, hypovolemia, and hypoperfusion in pancreatitis. We aimed to clarify the clinical utility of combined indomethacin and saline hydration for preventing PEP. PATIENTS AND METHOD: In this cross-sectional study, 120 patients undergoing ERCP for the first time at the

Rare cause of obstructive haemobilia with recurrent biliopancreatic complications: a paradigmatic case.

Haemobilia is an unusual but significant cause of upper gastrointestinal bleeding. Two-thirds of haemobilia cases are secondary to invasive hepato-biliopancreatic procedures. Biliary angiodysplasia is exceptionally unusual, with only three cases reported. Herein, we report the case of an autonomous 80-year-old woman with a history of cholecystectomy 5 years ago and cardiovascular disease-hypertension, heart failure, acute myocardial infarction, stroke and non-valvular atrial fibrillation, anticoagulated with apixaban 2.5 mg two times per day. Since July 2019, she had four episodes of acute cho
