Cervical total disc replacement with the Mobi-C cervical artificial disc compared with anterior discectomy and fusion for treatment of 2-level symptomatic degenerative disc disease: a prospective, randomized, controlled multicenter clinical trial: clinical article.

OBJECT: Cervical total disc replacement (TDR) is intended to treat neurological symptoms and neck pain associated with degeneration of intervertebral discs in the cervical spine. Anterior cervical discectomy and fusion (ACDF) has been the standard treatment for these indications since the procedure was first developed in the 1950s. While TDR has been shown to be a safe and effective alternative to ACDF for treatment of patients with degenerative disc disease (DDD) at a single level of the cervical spine, few studies have focused on the safety and efficacy of TDR for treatment of 2 levels of th

New instrument for creating bone graft cavities for anterior cervical decompression and fusion: the anterior fusion spinal fork.

Sufficient bone decompression of osteophytes is important for positive functional outcomes in anterior cervical spine surgery. Achieving good alignment and bone fusion in anterior cervical decompression and fusion requires a bone graft bed of the optimum size and shape. We have developed a stainless steel instrument named the anterior fusion spinal fork, which is designed to aid in accurately drilling the bone cavity, thus enabling selection of the correct size of bone graft or bone graft substitutes. The device has an open design with 4 prongs, and resembles a three-dimensional fork. This ins

Giant cells tumor recurrence at the third lumbar vertebra.

Giant cell tumors (GCT) of the spine are rare. We report a case of (GCT) of the third lumbar vertebra revealed by left lumbar radiculopathic thigh pain in a 47 year old man. Imaging showed an osteolytic process invading the vertebral body, the posterior arch and compressing the dural sac left side. Neurological decompression was first performed including stabilization by an instrumented postero-lateral graft. A surgical biopsy was obtained at the same time to confirm the diagnosis. A secondary L2-L4 tumor curettage and graft procedure did not prevent, 5 years later, tumor recurrence. We believ

Percutaneous lumbar discectomy using the Dekompressor system under CT-control.

In recent years, new minimally invasive therapies for the treatment of radicular pain associated with contained disc herniation have been introduced. These techniques have changed the field of interventional pain management. In a prospective, nonrandomized case study, we treated patients using the Dekompressor system guided by computed tomography instead of fluoroscopy. Pain scores, analgesic usage, and activities of daily living were assessed via structured telephone interviews 6 and 12 months after the procedure. Sixty-four patients were treated at 76 lumbar levels. Follow-up data after 12 m

Prospective comparative study of lumbar sequestrectomy and microdiscectomy.

INTRODUCTION: During microsurgical disc operation, usually a sequestrectomy and a nucleotomy are performed. Whether a nucleotomy is necessary in any case is disputed. The aim of this study is to examine this question on the basis of clinical results and to compare rates of recurrence between the two groups. METHODS: A prospective analysis was performed in 100 patients with sequestrectomy and the first 100 consecutive patients with microdiscectomy out of a cohort of 1,245 patients, who were operated by six surgeons between 2003 and 2006. The study is designed with special emphasis on the actua

The prediction of outcome in somatised patients undergoing elective lumbar surgery.

We compared a group of 46 somatised patients with a control group of 41 non-somatised patients who had undergone elective surgery to the lumbar spine in an attempt to identify pre-operative factors which could predict the outcome. In a prospective single-centre study, the Distress and Risk Assessment method consisting of a modified somatic perception questionnaire and modified Zung depression index was used pre-operatively to identify somatised patients. The type and number of consultations were correlated with functional indicators of outcome, such as the Oswestry disability index and a visua

Relaxation of forces needed to distract cervical vertebrae after discectomy: a biomechanical study.

STUDY DESIGN: In vitro and in vivo biomechanical stress measurements are made of the intervertebral disc segment distraction force during anterior cervical discectomy. OBJECTIVE: The purpose of this study is to determine the short-term force relaxation of the native intervertebral disc segment and to determine the short-term force relaxation of the segment after removal of the intervertebral disc, as is commonly performed in anterior cervical discectomy with fusion and arthroplasty. SUMMARY OF BACKGROUND DATA: No published data examine the issue of intraoperative distraction force of the cer

Multichannel computed tomography angiography and its role in the evaluation of rotational vertebrobasilar insufficiency.

We present a case of rotational vertebrobasilar insufficiency diagnosed and followed by computed tomography angiography. Diagnosis of this entity provides a unique challenge in that the patient must be positioned properly to avoid a false-negative result without causing ischemic brain injury. The evaluation of rotational vertebrobasilar insufficiency with computed tomography angiography has never been described in the literature to the best of our knowledge. We highlight the advantages of using this technique in the diagnosis and follow-up of this uncommon entity.

Comparison of adverse events between the Bryan artificial cervical disc and anterior cervical arthrodesis.

STUDY DESIGN: Randomized controlled study. OBJECTIVE: To compare the rates of adverse events associated with disc arthroplasty versus those of anterior cervical discectomy and arthrodesis with allograft and plate. SUMMARY OF BACKGROUND DATA: Cervical disc arthroplasty as a substitute for fusion has been developed to maintain motion and, theoretically, prevent adjacent segment degeneration. Currently, cervical arthroplasty devices are undergoing clinical testing for safety and efficacy. The evaluation of safety is performed by critical analysis of all adverse occurrences following surgery to

Treatment of degenerative cervical disc disease with uncoforaminotomy--intermediate clinical outcome.

BACKGROUND: Anterior cervical uncoforaminotomy (uncoforaminotomy) is an operative method intended to preserve the functional motion segment of the cervical spine while removing the underlying pathology. Controversy exists concerning the patients best suited for this treatment modality. Furthermore, no long-term outcome analyses have been published. METHODS: Between November 2002 and June 2004, 96 patients underwent single-level uncoforaminotomy in our neurosurgical department for the treatment of cervical radiculopathy. The patients were divided into three groups: A, soft disc; B, hard disc;

Postoperative change of the cross-sectional area of back musculature after 5 surgical procedures as assessed by magnetic resonance imaging.

Many investigators have reported that persistent low back pain may occur after posterior surgical intervention, and studies have investigated the histologic and histochemical changes in back muscle after posterior lumbar spine surgery. The purpose of the current study is to compare the pre- and postoperative cross-sectional area of the back musculature among 5 surgical groups including anterior lumbar interbody fusion, which has no direct invasion of the back musculature, using magnetic resonance imaging, and to correlate the clinical results with the degree of atrophy. The cross-sectional are

Analysis of hard thoracic herniated discs: review of 18 cases operated by thoracoscopy.

The authors retrospectively reviewed a series of 18 hard thoracic herniated discs (HTHD) operated by thoracoscopy. Isolated cases of HTHD have been reported in the literature, but no series describing these lesions has been published. Seventy-two percent of the herniated discs were situated between T8 and T12. Fifty-six percent of the patients had radiographic sequelae of Scheuermann's disease. Postoperatively, 83% had neurological improvement. In seven cases (39%), a plane separating the herniated disc and the dura mater was found surgically. In 11 patients, no separating plane was found duri

Objective assessment of reduced invasiveness in MED. Compared with conventional one-level laminotomy.

Microendoscopic discectomy (MED) has been accepted as a minimally invasive procedure for lumbar discectomy because of the small skin incision and short hospital stay required for this surgery. However, there are few objective laboratory data to confirm the reduced systemic responses in the early phase after this procedure. In order to substantiate the reduced invasiveness of MED compared to microdiscectomy (MD) or procedures involved in one-level unilateral laminotomy, the invasiveness of each surgical procedure was evaluated by measuring serum levels of biochemical parameters reflective of a
