Thoracodorsal nerve graft for reconstruction of facial nerve branching.

OBJECT: Surgical treatment of parotid malignancies may frequently involve facial nerve amputation to achieve oncological radical resection. The entire facial nerve branching from its exit from the stylomastoid foramen to the periphery of the gland is often sacrificed. The first reconstructive strategy is the immediate reconstruction of the facial nerve by directly anastomosing the trunk of the facial nerve to its distal branches by interpositional nerve grafting. The present study was performed to determine the adequacy of thoracodorsal nerve grafting for immediate repair of the facial nerve.

Laparoscopic component separation in the single-stage treatment of infected abdominal wall prosthetic removal.

BACKGROUND: The ideal surgical treatment for complicated ventral hernias remains elusive. Traditional component separation provides local advancement of native tissue for tension-free closure without prosthetic materials. This technique requires an extensive subcutaneous dissection, with the division of perforating vessels predisposing to skin flap necrosis and complicated wound infections. The laparoscopic separation of components provides a minimally invasive alternative to open techniques, while eliminating the potential space and subsequent complications of large skin flaps. We report our

[Percutaneous revision of shunt with obstructed ventricular catheter (author's transl)].

Shunt dysfunction due to an obstructed ventricular catheter can be avoided, at least in part, by placing the of the catheter anteriorly to the Foramen of Monro. However, once the catheter is obstructed, surgical removal under general anesthesia is almost inevitable. Irrigation method for an obstructed ventricular catheter enables us to gain scarcely anything and accumulation of instilled fluid in the ventricle often causes the dangerously increased intracranial pressure. 1) Technique for placement of the ventricular catheter. The skin incision is a semicircular. After the skin flap is reflecte

Increasing body mass index increases complications but not failure rates in microvascular breast reconstruction: A retrospective cohort study.

BACKGROUND: Obesity is often considered a relative contra-indication for microvascular autologous breast reconstruction. We hypothesize that although obesity is associated with increased post-operative complications, abdominally based microvascular post mastectomy breast reconstruction can still be achieved with high rates of success. This study analyses the relationship between increasing body mass index (BMI) and post-operative complications at a high volume specialist centre, with a particular focus on serious complications. METHODS AND RESULTS: Nine hundred and sixty patients undergoing 1

Influence of vascular anatomy and perfusion on complications in the first stage of total auricular reconstruction.

BACKGROUND: The aesthetic quality of total auricular reconstruction for microtia has improved significantly in recent years, resulting in favourable outcomes for many patients. However, complications such as skin necrosis and infection remain. Adequate skin perfusion is essential to prevent these complications, but the vascular anatomy of patients with microtia is poorly understood. We evaluated the relationship between vascular anatomy assessed by indocyanine green fluorescence angiography performed during the first stage of autologous total ear reconstruction for microtia and postoperative c

Tympanoplasty--conchal cavum approach.

UNLABELLED: The three well recognized tympanoplasty approaches: permeatal, postaural, and endaural, each have advantages and disadvantages. The permeatal approach is suitable only for ears with adequate canal size. The postaural approach limits visualization of the posterior eardrum margin. The endaural approach limits the view of the eardrum's anterior margin. This study describes a modified endaural approach, developed to overcome these limitations. A retrospective case series review and collection of a prospective cohort of patient reported outcome data were undertaken to assess the techniq

Effectiveness and safety of liposomal bupivacaine in autologous breast reconstruction: a systematic review.

BACKGROUND: Autologous breast reconstruction (ABR), particularly using abdominally based free flaps such as the deep inferior epigastric perforator (DIEP) flap, is associated with significant postoperative pain. Liposomal bupivacaine, a long-acting local anesthetic, has been increasingly incorporated into multimodal analgesia protocols; however, its effectiveness in improving postoperative outcomes remains uncertain. OBJECTIVE: To systematically evaluate the effectiveness and safety of liposomal bupivacaine in reducing opioid consumption and improving recovery outcomes in patients undergoing

Long-term reconstructive outcomes after expander-implant breast reconstruction with serious infectious or wound-healing complications.

INTRODUCTION: Immediate expander-implant breast reconstruction has been associated with postoperative complications, including infection and wound-healing problems. In extreme cases, these issues can lead to expander-implant loss. Little is known about the long-term reconstructive outcomes for patients who develop major complications threatening their expander-implant reconstructions. METHODS: A review of all patients who underwent mastectomy and immediate expander-implant reconstruction at University of California, San Francisco (UCSF) from 2005 to 2007 was performed. A prospective database

Surgical and Patient-Reported Outcomes of Open Perforator-Preserving Anterior Component Separation for Ventral Hernia Repair.

BACKGROUND: Abdominal wall reconstruction is challenging for surgeons and may be life altering for patients. There are scant high-quality studies on patient-reported outcomes following abdominal wall reconstruction. We assess long-term surgical and patient-reported outcomes of perforator-preserving open anterior component separation (OPP-ACS) following large ventral hernia repair. METHODS: A retrospective review of patients with large ventral hernia defects who underwent OPP-ACS performed by the authors (B.A.S., M.J.T.) was conducted between 2015 and 2019. Demographics, surgical history, oper

Outcomes of Anatomic Reconstruction of Gunshot-Inflicted Lower Face Defects by Free Osteoseptocutaneous Fibula Flap and Expanded or Nonexpanded Temporal Scalp Flap Combination in Males.

Reconstruction of gunshot-inflicted composite lower face defects is a challenge for plastic surgeons. Functional and aesthetic repair of such defects mostly requires free or pedicled flap applications or combinations of both.In this study, the authors evaluated 7 males with gunshot-inflicted composite mandibular defects. All patients underwent reconstruction with a free osteoseptocutaneous fibula flap (FOCF) for the composite mandibular defect and a pre or nonexpanded temporal artery-based scalp flap for beardless facial skin. All patients were evaluated aesthetically and functionally with a p
