A pharmacodynamic analysis of factors affecting recovery from anesthesia with propofol-remifentanil target controlled infusion.

AIM: To examine individual patient's demographic parameters and clinical variables related to return of consciousness (ROC) and the pharmacodynamic relationship between propofol effect-site concentration (C(e)) and ROC from propofol-remifentanil anesthesia. METHODS: Ninety-four patients received propofol-remifentanil anesthesia using the effect-site target-controlled infusion (TCI) system. All clinical events were noted, and variables possibly related to propofol C(e) at ROC were examined using linear correlation analyses. Pharmacodynamic modeling incorporating covariates was performed using

[Orexinergic neurons and noradrenergic awakening system in general anesthesia].

The sleep-wake cycle is regulated by interaction between monoaminergic, cholinergic and GABAergic neuronal activities. Monoaminergic neurons including noradrenergic (locus coeruleus: LC), histaminergic (tuberomammillary nucleus) and serotonergic (raphe nuclei) neurons projected diffusely to the cerebrocortex, thalamus and brainstem are known as the center of sleep regulation. Interestingly, although the cell bodies displaying prepro-orexin (prepro-OX) immunoreactivity are limited to the hypothalamus, OXergic neurons densely project to LC, tuberomammillary nucleus and raphe nuclei. These eviden

Mixed opioid--mixed message?

By exposing the mu-receptor (MOP-R) to more than one exogenous ligand, the administration during general anaesthesia of more than one opioid with principal action on the receptor may confound and lead to complications or unexpected outcomes. The giving of such a 'mixed message' can result in respiratory depression, excess sedation and delayed recovery to an unusual degree. We present a case of apparent extreme opioid sensitivity and discuss a possible mechanism. Such occurrences may be more common than previously realised.

A simple apparatus for accelerating recovery from inhaled volatile anesthetics.

UNLABELLED: Hyperpnea increases anesthetic elimination but is difficult to implement with current anesthetic circuits without decreasing arterial PCO2. To circumvent this, we modified a standard resuscitation bag to maintain isocapnia during hyperpnea without rebreathing by passively matching inspired PCO2 to minute ventilation. We evaluated the feasibility of using this apparatus to accelerate recovery from anesthesia in a pilot study in four isoflurane-anesthetized dogs. The apparatus was easy to use, and all dogs tolerated being ventilated with it. Under our experimental conditions, isocapn

Critical respiratory events in the postanesthesia care unit. Patient, surgical, and anesthetic factors.

BACKGROUND: Previous studies have noted a high incidence of adverse outcomes in the postanesthesia care unit (PACU), but few have examined associated factors and patient outcomes. To determine the frequency of acute, unanticipated respiratory problems and to examine the associated patient, surgical, and anesthetic factors, we prospectively collected preoperative, intraoperative, and postoperative data on 24,157 consecutive PACU patients who received a general anesthetic during a 33-month period. METHODS: A PACU critical respiratory event (CRE), was defined as any unanticipated hypoxemia (hemo

[The effectiveness of the benzodiazepine antagonist Ro 15-1788 after the induction of anesthesia with midazolam].

UNLABELLED: The benzodiazepine antagonist Ro 15-1788 has now been added to the series of antagonists which have been routinely used for a long time. In the present prospective, randomized double-blind study, it is investigated whether Ro 15-1788 is able to antagonize the effect of midazolam applied at a dosage of 0.2 mg/kg for induction of anesthesia. PATIENTS, MATERIAL AND METHODS: Thirty female patients (age between 19 and 44 years) undergoing laparoscopy were included in the study. Premedication consisted of an oral dose of 2 mg flunitrazepam given on the evening before the intervention an

Clinical evaluation of a specific benzodiazepine antagonist (RO 15-1788). Studies in elderly patients after regional anaesthesia under benzodiazepine sedation.

The efficacy, usefulness and side effects of RO 15-1788 (RO), a specific benzodiazepine (BZD) antagonist, have been evaluated. Sixty-two patients (ASA I-III, mean age 72 +/- 9 yr) scheduled for urological surgery under regional anaesthesia and BZD sedation received placebo or RO in a randomized, double-blind fashion at the end of the procedure, following sedation with midazolam. When compared with placebo, RO improved alertness and collaboration for 15 min, and suppressed anterograde amnesia for 60 min. No major side effect was noted, although five patients became anxious after administration

Opioid-Free and Opioid-Sparing Pain Control in Surgical Anesthesia: A Scoping Review of Multimodal Analgesia, Regional Techniques, and Recovery Outcomes.

Surgical anesthesia has historically relied on opioids to control intraoperative and postoperative pain, but opioid-related adverse effects such as respiratory depression, sedation, ileus, postoperative nausea and vomiting, delayed mobilization, tolerance, hyperalgesia, and prolonged opioid use have encouraged a shift toward safer perioperative analgesic models. In this scoping review, opioid-free anesthesia (OFA) is defined as the complete avoidance of opioid administration during the intraoperative period, whereas opioid-sparing anesthesia (OSA) is defined as an anesthetic approach that inte

Risk factors for unanticipated hypertension during emergence from general anesthesia in elderly surgical patients: A retrospective cohort analysis.

Unanticipated hypertension in the postanesthesia care unit (PACU) refers to acute blood pressure elevation in patients whose preoperative baseline blood pressure was normal, possibly resulting in adverse postoperative complications. This study aimed to identify potential factors and the impact of unanticipated hypertension during emergence from general anesthesia in the elderly surgical population. A retrospective study was done at the Affiliated BenQ Hospital of Nanjing Medical University. Elderly surgical patients who had been admitted to the PACU for recovery from general anesthesia were en

Myoclonus-like involuntary movements following cesarean delivery epidural anesthesia.

Spinal myoclonus following neuraxial anesthesia is rare. This report describes a case of myoclonus-like involuntary movement that occurred during the recovery from epidural anesthesia for a cesarean delivery. The patient's symptom improved with the administration of benzodiazepine, and the patient recovered with no neurological sequelae. In conclusion, epidural anesthesia can cause spinal myoclonus, which can be treated with a benzodiazepine.

Anti-nociceptive and sedative effects of sufentanil long acting during and after sevoflurane anaesthesia in dogs.

The purpose of the present study was to determine the most effective time interval between the administration of sufentanil long acting (LA) and the induction of sevoflurane anaesthesia in dogs. The occurrence of sedation, analgesia and other marked side-effects were evaluated in addition to the possible dosage-reducing effect of sufentanil on sevoflurane in dogs. Forty clinically normal beagles aged 1-2 years and weighing between 8.4 and 13.6 kg were included. Two control groups were used: one group of dogs (A) received sufentanil LA (50 microg/kg i.m.) and a second group (B) the sufentanil v

Orexin-1 receptor is involved in ageing-related delayed emergence from general anaesthesia in rats.

BACKGROUND: Previous clinical studies have shown that emergence from isoflurane anaesthesia takes longer in elderly patients compared with middle-aged patients. The current study investigated whether delayed emergence from anaesthesia in older age is associated with the age-related decrease in orexin receptors by using a rat model. METHODS: Adult and aged Sprague-Dawley rats were used to assess the time to emergence after 30 min isoflurane anaesthesia (1.4 vol%), and differences in the orexinergic systems, including the number of orexinergic neurones, plasma orexin concentrations, and express

[Combined spinal epidural anesthesia during endoprosthetic surgeries for bone tumors in old-age children].

Thirty-five patients (ASA II-III) aged 12 to 17 years, diagnosed as having osteogenic sarcoma and Ewing's sarcoma localizing in the femur and tibia, were examined. Surgery was performed as sectoral resection of the affected bone along with knee joint endoprosthesis. Surgical intervention was made under combined spinal and epidural anesthesia (CSEA) with sedation, by using the methods for exact dosing of propofol (6-4 mg/kg x h). During intervention, a child's respiration remains is kept spontaneous with oxygen insufflation through a nasal catheter. CSEA was performed in two-segmental fashion.
