Cognitive behavior therapy for hoarding disorder: follow-up findings and predictors of outcome.

BACKGROUND: A cognitive-behavioral model of hoarding posits deficits in information processing, maladaptive beliefs about and attachments to possessions that provoke distress and avoidance, and positive emotional responses to saving and acquiring that reinforce these behaviors. A 26-session individual cognitive-behavioral therapy (CBT) based on this model showed significant reductions in hoarding symptoms and large effect sizes (Steketee et al.([1])). METHODS: The present study presents findings at follow-up (up to 12 months), as well as predictors of outcome at posttreatment (n = 37) and fol

Group cognitive behavior therapy for bipolar disorder can improve the quality of life.

Bipolar disorder (BD) can have an impact on psychosocial functioning and quality of life (QoL). Several studies have shown that structured psychotherapy in conjunction with pharmacotherapy may modify the course of some disorders; however, few studies have investigated the results of group cognitive behavior therapy (G-CBT) for BD. Our objective was to evaluate the effectiveness of 14 sessions of G-CBT for BD patients, comparing this intervention plus pharmacotherapy to treatment as usual (TAU; only pharmacotherapy). Forty-one patients with BD I and II participated in this study and were random

Testing the effectiveness of knowledge and behavior therapy in patients of hemiplegic stroke.

OBJECTIVES: The purpose of this study was to examine demographic variables associated with anger and well-being in patients with stroke, to investigate the hypothesis that knowledge and behavior (K&B) therapy improves patients' emotional and physical well-being, and to discover psychological variables associated with therapeutic outcomes. METHODS: Seventy-seven patients with hemiplegic stroke were randomly assigned to either a control condition (the conventional therapy) or an experimental condition (K&B therapy). Both the experimental and control groups received physical therapy, which inclu

Virtual reality cognitive-behavior therapy for public speaking anxiety: one-year follow-up.

Public speaking anxiety (PSA) is a common social phobia. Although cognitive-behavior therapy (CBT) is the treatment of choice, difficulties arise with both in vivo and in vitro exposure (lack of therapist control, patient's inability to imagine, self-flooding, and a lack of confidentiality resulting from public exposure). Virtual reality CBT (VRCBT) enables a high degree of therapist control, thus overcoming these difficulties. In a previous publication, the authors reported on their findings that VRCBT (n = 28) and CBT (n = 30) groups were significantly more effective than a wait-list control

Changes in intolerance of uncertainty during cognitive behavior group therapy for social phobia.

BACKGROUND AND OBJECTIVES: Recent research suggests that intolerance of uncertainty (IU), most commonly associated with generalized anxiety disorder, also contributes to symptoms of social phobia. This study examines the relationship between IU and social anxiety symptoms across treatment. METHOD: Changes in IU, social anxiety symptoms, and depression symptoms were examined following cognitive behavior group therapy (CBGT) for social phobia (N=32). RESULTS: CBGT led to significant improvements in symptoms of social anxiety and depression, as well as reductions in IU. Reductions in IU were as

Virtual reality cognitive behavior therapy for public speaking anxiety: a randomized clinical trial.

Public speaking anxiety (PSA) is a common phobia. Although cognitive behavior therapy (CBT) is preferred, difficulties arise with the exposure component (lack of therapist control, patient's inability to imagine, self-flooding, loss of confidentiality resulting from public exposure). Virtual reality CBT (VRCBT) enables a high degree of therapist control, thus overcoming these difficulties. This study examined whether VRCBT is an alternative to CBT. Participants with PSA were randomly assigned to VRCBT (28 participants), CBT (30 participants), and wait list control (WLC; 30 participants). VRCBT

Long-term effects of cognitive behavior therapy on brain activation in spider phobia.

This functional magnetic resonance imaging study investigated long-term effects of cognitive behavior therapy (CBT) in individuals suffering from spider phobia. Ten female patients who had shown positive immediate CBT effects were invited to take part in a 6-month follow-up investigation. Here, the patients, along with eight non-phobic females, were presented with the same pictures depicting spiders, generally disgust-inducing, generally fear-inducing and neutral content, which they had viewed 6 months earlier. Patients' self-report and overt behavior indicated a positive long-term clinical im

Enduring effects for cognitive behavior therapy in the treatment of depression and anxiety.

Recent studies suggest that cognitive and behavioral interventions have enduring effects that reduce risk for subsequent symptom return following treatment termination. These enduring effects have been most clearly demonstrated with respect to depression and the anxiety disorders. It remains unclear whether these effects are a consequence of the amelioration of the causal processes that generate risk or the introduction of compensatory strategies that offset them and whether these effects reflect the mobilization of cognitive or other mechanisms. No such enduring effects have been observed for

Adjusting cognitive behavior therapy for adolescents with bulimia nervosa: results of case series.

This article reviews the types of adjustments needed to an adult protocol of cognitive-behavioral therapy (CBT) for bulimia nervosa (BN) to make it more acceptable to an adolescent population. Employing developmental principles as well as clinical experience as guidelines, these modifications include the involvement of parents, recognition of the interaction of treatment with normal adolescent developmental tasks, and allowances for typical cognitive and emotional immaturity on treatment procedures. Outcomes from a series of adolescents with BN who were treated with this modified-CBT approach

Imagery rescripting within cognitive behavior therapy for bulimia nervosa: an illustrative case report.

OBJECTIVE: This case report describes the use of imagery rescripting as an adjunct to cognitive behavior therapy (CBT) for bulimia nervosa. METHODS: The patient was a 22-year-old woman, who had a 6-year history of diagnosable eating problems, with no comorbid diagnosis. She had a history of emotional abuse by her family. The single session of imagery rescripting was aimed at effecting change in the core beliefs that were hypothesized to result from that emotional abuse, and hence in the associated bulimic psychopathology. RESULTS: While eight sessions of conventional CBT reduced symptom beha

Comparison of cognitive-behavior therapy and desipramine in the treatment of bulimia nervosa.

A comparison of cognitive-behavior therapy alone, desipramine alone, and cognitive-behavior therapy combined with desipramine was made in the treatment of bulimia nervosa. The study was terminated early with an N of only 7 subjects per condition because of a high drop-out rate and lack of positive response in the desipramine alone group compared to the other two groups. By this time it was also apparent that at posttreatment and at 6 months follow-up no benefit was being realized from combining cognitive-behavior therapy with desipramine.

Buspirone therapy for Type A behavior, hostility, and perceived stress in cardiac patients.

Hostility is considered to be a risk factor for coronary artery disease. Despite the findings of reduced serotonergic function and effectiveness of treatment with serotonergic agents in similar symptom complexes, there have been no attempts at using serotonergic psychopharmacologic agents to treat this population. We conducted an 8-week open trial of 10 type A, irritable men with coronary artery disease and no diagnosable psychiatric (axis I) condition to examine the effects of buspirone on these subjects. Buspirone appears to significantly reduce type A behavior, hostility, anxiety, impatienc

[Interactive relations between sexual behavior and post-menopausal replacement hormone therapy: a survey].

A study carried out in 1988 using a representative sample of the French male and female population aged between 52 and 70 provides information concerning one of the presumed motivations for the use of replacement hormone therapy: to "always be a woman". In 1988 the sexual activity of French couples decreased on average after the age of 50 or thereabouts in a proportion identical to that seen in 1970. The chief limiting factor is reduced libido in women, far more than simple dyspareunia or relative male impotence. The majority of women interviewed did not wish to react against this fall in sexu

An uncontrolled evaluation of inpatient and outpatient cognitive-behavior therapy for bulimia nervosa.

Inpatient (n = 27) and outpatient (n = 22) cognitive-behavior therapy programs for bulimia nervosa were evaluated in an uncontrolled experiment. Both treatment conditions included exposure with response prevention and cognitive restructuring. Inpatient treatment had a mean length of stay of 5 weeks. Outpatient treatment lasted 15 weeks. Both groups were followed after the end of treatment. The results showed that both programs were effective in reducing problems associated with bulimia nervosa. The inpatient program led to very rapid progress, whereas the outpatient program led to more gradual

A Non-Controlled Study of Cognitive Behavior Therapy for Panic Disorder in Veterans.

Panic disorder is a highly prevalent and impairing diagnosis in veterans. Despite consistent efficacy findings in community samples, few studies have investigated evidence-based psychotherapies in veterans with panic disorder, limiting the understanding of their utility and deployment in Veterans Affairs (VA) settings. The present study preliminarily investigated cognitive behavioral therapy (CBT) for panic disorder via a non-controlled design in a treatment-seeking veteran sample with panic disorder at the VA. Of the 41 veterans initiating treatment, 28 (68.3%) participants completed at least

Exercise prior to cognitive behavior therapy sessions for depression: a feasibility pilot study.

Exercise directly improves mood and cognition. Providing exercise immediately before cognitive behavior therapy (CBT) sessions may therefore enhance the clinical responsiveness to CBT. The present pilot study examined the feasibility and direction of effect of exercise+CBT versus CBT in depressed outpatients using a stepped wedged design. Thirty-three patients received either group-based CBT (12-16 weeks) or group-based exercise+CBT within specialized mental healthcare settings. Weekly therapist-supervised exercise sessions (45 min, moderate intensity, running/indoor cycling) were provided dir

Examining the Role of Homework Compliance in Acceptance-Enhanced Behavior Therapy for Trichotillomania.

Between-session therapy homework is a key component of cognitive-behavioral therapies, yet its role in trichotillomania treatment remains understudied. The present study examined the associations between homework compliance and treatment outcomes in acceptance and commitment therapy (ACT) enhanced behavior therapy (AEBT) for trichotillomania. Participants included 35 adults with trichotillomania who participated in a 12-week randomized controlled trial of AEBT. Therapists documented patient homework assignment completion and rated homework adherence after each session. Trichotillomania symptom

Improvements in emotion regulation during cognitive behavior therapy predict subsequent social anxiety reductions.

Individuals with social anxiety disorder (SAD) experience overall emotion regulation difficulties, but less is known about the long-term role of such difficulties in cognitive behavior therapy (CBT) for SAD. Forty-six patients with SAD receiving internet-delivered CBT, and matched healthy controls (HCs; n = 39), self-reported the Difficulties in Emotion Regulation Scale (DERS), Liebowitz Social Anxiety Scale (LSAS-SR), and participated in anticipatory speech anxiety behavioral experiments. Patients were measured at seven time points before, during and after CBT over a total period of 28 months
