L-theanine ameliorates traumatic-brain-injury-induced hippocampal neuronal death in rats.

BACKGROUND: Traumatic brain injury (TBI) is a major health concern, often resulting in significant brain damage and functional impairments. A key contributing factor to TBI-induced neuronal injury is the overactivation of AMPA glutamate receptors, leading to an increased influx of calcium and zinc ions. This study investigates the neuroprotective potential of l-theanine, known for its antioxidant potential and ability to enhance glutathione synthesis, against hippocampal damage in a TBI rat model. METHODS: Rats subjected to TBIs were treated with two dosages of l-theanine (100 and 200 mg/kg)

Sex dimorphism in brain cell death after hypoxia-ischemia in newborn piglets.

BACKGROUND: Clinical data suggest that females might be more resistant to hypoxia than males, with male sex recognized as a risk factor for suffering life-long neurological sequelae. However, the impact of hypoxia-ischemia in certain brain regions and its association with genetic sex remains unclear. METHODS: Using the piglet model of neonatal brain injury, fifteen piglets (8 females and 7 males) were subjected to a global cerebral hypoxic-ischemic insult. After 48 h, total cell death and the number of necrotic, apoptotic and cleaved-caspase-3 positive cells was quantified in five brain regio

Ex Vivo Lung Perfusion in Donation After Cardiac and Brain Death Donation.

BACKGROUND: Allografts from donation after circulatory death (DCD) or brain death donors may be evaluated by ex vivo lung perfusion (EVLP) to assess quality for transplantation. We sought to determine the association of donor type with transplantation outcomes at a national level. METHODS: The United Network for Organ Sharing database was queried for lung transplant recipients, who were stratified into DCD EVLP, brain death EVLP, standard DCD, and standard brain death, followed by an unadjusted analysis. A 1:1 propensity score match based on donor and recipient characteristics was used to com

[Organ donation after determination of brain death : Legal aspects and practical approach].

The prerequisites for post-mortem organ donation in Germany include the determination of irreversible loss of brain function, consent to organ donation, and the exclusion of medical contraindications. In addition, mainly because of the shortage of donor organs in Germany and the sometimes controversial social and media discussions on the topic of organ donation, all physicians involved in the donation process must be familiar with the relevant laws, guidelines, and procedural instructions. This applies especially to those who are to carry out the verification of irreversible brain death. Only

Brain-based arterial pulse pressure threshold for death determination: a systematic review.

PURPOSE: There is lack of consensus regarding the minimum arterial pulse pressure required for confirming permanent cessation of circulation for death determination by circulatory criteria in organ donors. We assessed direct and indirect evidence supporting whether one should use an arterial pulse pressure of 0 mm Hg vs more than 0 (5, 10, 20, 40) mm Hg to confirm permanent cessation of circulation. SOURCE: We conducted this systematic review as part of a larger project to develop a clinical practice guideline for death determination by circulatory or neurologic criteria. We systematically se

Cobra envenomation in an elderly female mimicking brain death- A case report.

Snake bite envenomation is one of the most toxicology-related cause that can mimic brain death. This is a case report of 73-year-old elderly female, a hypertensive on treatment, who presented with chief complaints of cobra snake bite on the dorsum of right hand. On admission, patient had dyspnea, bilateral ptosis and ophthalmoplegia. In the next 10-15 min, her symptoms got worsened for which she was administered intravenous doses of atropine (2 mg), neostigmine (0.5 mg) and anti-snake venom. She developed respiratory arrest, hence was intubated and was started on mechanical ventilation. On ass

[A RARE CASE OF RELAPSING GUILLAIN-BARRE SYNDROME PRESENTING AS BRAIN DEATH].

Acute immune-mediated polyneuropathies classified as Guillain-Barre Syndrome (GBS) constitute a wide range of clinical manifestations characterized by ascending symmetrical limbs paralysis along with possible involvement of the respiratory muscles. Paralysis of facial and eye muscles including unresponsive pupils, which may mimic a brain death is rare, though previously described. This presentation is challenging because it masks the correct diagnosis and delays or prevents appropriate treatment. A 69-year-old female patient was hospitalized with progressive bilateral limb paralysis with invol

Immunomodulatory effects of thalidomide in an experimental brain death liver donor model.

Brain death is characterized by a generalized inflammatory response that results in multiorgan damage. This process is mainly mediated through cytokines, which amplify graft immunogenicity. We investigated the immunological response in a brain death liver donor model and analysed the effects of thalidomide, a drug with powerful immunomodulatory properties. Brain death was induced in male Lewis rats. We studied three groups: Control (sham-operated rats in which trepanation was performed without inserting the balloon catheter), BD (rats subjected to brain death by increasing intracranial pressur

Baclofen Toxicity Mimicking Brain Death: A Case Report of a Pediatric Patient.

BACKGROUND: Baclofen is commonly used in both pediatric and adult patients to treat spasticity secondary to spinal cord and cerebral pathology. A broad range of symptoms and severity of baclofen toxicity have been described. However, to our knowledge, there are no reports to date of baclofen toxicity mimicking brain death in pediatric patients. OBJECTIVE: We reviewed the presentation, clinical course, diagnostic studies including imaging and electroencephalography, and outcome of a patient with transient coma and loss of brainstem reflexes mimicking brain death secondary to baclofen toxicity.

Death anxiety in patients with primary brain tumor: Measurement, prevalence, and determinants.

OBJECTIVE: This study investigated death anxiety in patients with primary brain tumor (PBT). We examined the psychometric properties of two validated death anxiety measures and determined the prevalence and possible determinants of death anxiety in this often-overlooked population. METHODS: Two cross-sectional studies in neuro-oncology were conducted. In Study 1, 81 patients with PBT completed psychological questionnaires, including the Templer Death Anxiety Scale (DAS). In Study 2, 109 patients with PBT completed similar questionnaires, including the Death and Dying Distress Scale (DADDS). M

A death due to concussive brain injury augmented by alcohol (case report).

We report here the case of a death following blunt trauma to the head in which the victim immediately became unconscious and remained unconscious until death. There were no associated intracranial bleeding or visible macroscopic/microscopic injuries to the brain. Additional injuries to other parts of the body included non-fatal contusions and abrasions. The victim was intoxicated, with a blood alcohol level of 168 mg/dl. Therefore, the conclusion was drawn that death in this case was due to the combination of the concussive effects of the head trauma and the central nervous system depressant e

Activation of inflammatory mediators in rat renal isografts by donor brain death.

BACKGROUND: Brain death (BD) has been thought to influence the early course of transplanted organs by triggering a series of nonspecific inflammatory events that in turn may increase the kinetics and intensity of the immunological host responses. In this study early nonspecific, cellular, and molecular changes occurring in kidney isografts from BD donors are compared with those from normal anesthetized, ventilated controls. METHODS: After induction of brain death, the animals were mechanically ventilated for 6 hr before organ removal. Only rats with stable blood pressure (mean arterial pressu

Endocrine changes and metabolic responses in a validated canine brain death model.

PURPOSE: Endocrinologic and metabolic changes after brain death (BD) have not yet been investigated in a validated animal model. Therefore, the effects of BD on hormonal and metabolic function were studied in 10 dogs (23 to 31 kg). METHODS: BD was induced by intracranial pressure increase and validated neuropathologically. Plasma concentrations of pituitary, thyroid, adrenal, and pancreatic hormones were measured pre/post BD. The results are expressed as mean (+/- SEM). RESULTS: A Cushing reflex and diabetes insipidus occurred after BD. Elevated catecholamine levels were documented after 15

Donor treatment after pronouncement of brain death: a neglected intensive care problem.

The need for cadaveric organs for transplantation is increasing. To decrease the shortage of organs, identification of potential donors and conditioning of these donors must improve. We present a review of relevant data on body and tissue alterations due to brain death and summarize the recent literature covering experimental and clinical studies on optimal donor management.

Pathophysiology of pulmonary edema following experimental brain death in the chacma baboon.

Systemic and pulmonary hemodynamics have been studied during the induction of brain death in the chacma baboon. In 11 animals brain death was induced by acute intracranial hypertension. Continuous recording of blood flow through both the pulmonary artery and the aorta was obtained by electromagnetic flow meters placed around these vessels. Mean arterial, central venous, pulmonary arterial, and left atrial pressures were recorded continuously. Systemic and pulmonary vascular resistances were calculated. During the agonal period marked sympathetic activity occurred, with significant increases in

Testing for apnea in suspected brain death: methods used by 129 clinicians.

Neurologists in Colorado and California were asked the methods they use to test for apnea when determining brain death. Most (65.1%) of 129 respondents simply observed the patient for respiratory efforts for 3 minutes or less while off the ventilator. Only 22.5% measured arterial carbon dioxide, 12.4% used a published recommended rigorous method for testing for apnea, and 11.6% did not test for apnea. Clinicians in full-time academic practice more frequently used the rigorous method. The data indicate that methods used most often in the communities surveyed are less rigorous than published gui

Special considerations in the assessment of catastrophic brain injury and determination of brain death in patients with SARS-CoV-2.

INTRODUCTION: The coronavirus disease 2019 (Covid-19) pandemic has led to challenges in provision of care, clinical assessment and communication with families. The unique considerations associated with evaluation of catastrophic brain injury and death by neurologic criteria in patients with Covid-19 infection have not been examined. METHODS: We describe the evaluation of six patients hospitalized at a health network in New York City in April 2020 who had Covid-19, were comatose and had absent brainstem reflexes. RESULTS: Four males and two females with a median age of 58.5 (IQR 47-68) were e
