Lens capsule advanced glycation end products induce senescence in epithelial cells: Implications for secondary cataracts.

Posterior capsule opacification (PCO) is a common complication after cataract surgery. Residual lens epithelial cells (LECs) on the anterior lens capsule, after cataract surgery, migrate to the posterior lens capsule and undergo transdifferentiation into myofibroblast-like cells. Those cells synthesize excessive amounts of extracellular matrix and contribute to fibrosis during PCO. Cellular senescence, a phenomenon that increases with aging, has been implicated in several fibrotic diseases. Here, we have investigated the prevalence of senescent LECs within the lens posterior capsule and the ab

Cataract surgery in cataracta membranacea.

A 27-year-old man with no history of intraocular surgery presented with progressive blurred vision in both eyes. Slitlamp evaluation revealed a white, thin membrane in the pupillary area that resembled posterior capsule opacification. A naturally formed lens was absent. Ultrasound biomicroscopy demonstrated a rarefaction of lens fibers in the equator region with a loss of the normal anatomic lens structure on the left side. Visual acuity was 20/32 with +10.00 -0.75 x 170 in the right eye and 20/100 with +11.50 -0.50 x 25 in the left eye. After capsulotomy and excision of the membranous capsule

Incidence and associated factors of Nd:YAG posterior capsulotomy after premium IOL implantation surgery: a systematic review and meta-analysis.

AIMS: Although premium intraocular lenses (IOLs) have become a prominent choice in modern cataract surgery, they may be more prone to posterior capsule opacification (PCO) and subsequent neodymium-doped yttrium aluminum garnet (Nd:YAG) posterior capsulotomy. This study aims to systematically evaluate the incidence of Nd:YAG capsulotomy after premium IOLs implantation and identify associated factors. METHODS: An exhaustive literature search was performed on PubMed, Embase, and Cochrane Library databases up to 12 June 2024, for studies reporting Nd:YAG posterior capsulotomy incidence after prem

Surface modification of intraocular lenses with zwitterionic poly(carboxybetaine) for posterior capsular opacification prevention.

Posterior capsular opacification (PCO) remains the most common long-term complication after cataract surgery, primarily caused by adhesion and proliferation of residual lens epithelial cells (LECs) on the intraocular lens (IOL) surface. Here, we report a zwitterionic carboxybetaine-coating strategy to endow IOLs with anti-adhesive and anti-fibrotic properties without compromising optical clarity. The zwitterionic monomer carboxybetaine methacrylate (CBMA) was grafted onto the IOL surface via plasma activation, forming a stable antifouling interface. The CBMA-coated surface exhibited reduced wa

Supervised Machine-Based Learning and Computational Analysis to Reveal Unique Molecular Signatures Associated with Wound Healing and Fibrotic Outcomes to Lens Injury.

Posterior capsule opacification (PCO), a frequent complication of cataract surgery, arises from dysregulated wound healing and fibrotic transformation of residual lens epithelial cells. While transcriptomic and machine learning (ML) approaches have elucidated fibrosis-related pathways in other tissues, the molecular divergence between regenerative and fibrotic outcomes in the lens remains unclear. Here, we used an ex vivo chick lens injury model to simulate post-surgical conditions, collecting RNA from lenses undergoing either regenerative wound healing or fibrosis between days 1-3 post-injury

Clinical features of endophthalmitis clusters after cataract surgery and practical recommendations to mitigate risk: systematic review.

Intraocular transmission of exogenous pathogens in cataract surgery can lead to endophthalmitis. This review evaluates the features of endophthalmitis clusters secondary to pathogen transmission in cataract surgery. Articles reporting on pathogen transmission in cataract surgery were identified via searches of Ovid MEDLINE, EMBASE, and Cochrane CENTRAL, and a total of 268 eyes from 24 studies were included. The most common source of infectious transmission was attributed to a contaminated intraocular solution (ie, irrigation solution, viscoelastic, or diluted antibiotic; n = 10). Visual acuity

[Experiences with disc lenses composed of silicone].

Our study presents the results with a silicone disc-lens (Adatomed 90D) over a postoperative period of 18 months. It includes 47 patients, 42 could be reexamined. The implantation was done after continuous circular capsulorhexis and phacoemulsification with a Faulkner-folder directly into the capsular bag. The implantation was sometimes difficult. Skill and experience of the surgeon are required. The IOL springs forth of the implantation forceps, which can lead to traumatization of the capsule and the iris. Three times the posterior capsule ruptured, in two cases the implantation was stopped b

Growth factor restriction impedes progression of wound healing following cataract surgery: identification of VEGF as a putative therapeutic target.

Secondary visual loss occurs in millions of patients due to a wound-healing response, known as posterior capsule opacification (PCO), following cataract surgery. An intraocular lens (IOL) is implanted into residual lens tissue, known as the capsular bag, following cataract removal. Standard IOLs allow the anterior and posterior capsules to become physically connected. This places pressure on the IOL and improves contact with the underlying posterior capsule. New open bag IOL designs separate the anterior capsule and posterior capsules and further reduce PCO incidence. It is hypothesised that t
