Catheterization and cardiovascular diagnosis1993eng
Contrast echocardiography performed during cardiac catheterization has mostly been limited to a few published case reports. We studied 37 patients with congenital heart disease to assess the capability of the method to diagnose cardiac shunts and valve regurgitation. Injections of 5% dextrose in water through an angiographic catheter were made to evaluate septal integrity and valve competence compared with conventional contrast angiography. An overall sensitivity of 93% and specificity of 78% were found. In four cases of atrial septal defect and seven of mitral regurgitation, the sensitivity w

We report a case of pulmonary artery perforation with resultant cardiac tamponade after central venous catheterization and review six previously reported cases. Tamponade may be an acute or late complication and is usually associated with the effusion of intravenous fluid into the pericardium. Three cases actually occurred after catheterization through the "safer" peripheral venous access route (basilic vein). The diagnosis of perforation and tamponade as well as the treatment can be accomplished using the catheter. Recognition and appropriate treatment of this rare complication may avoid sign

BACKGROUND: Cardiac damage (CD) staging enhances risk stratification in patients with clinically significant aortic stenosis (AS). We aimed to assess the prognostic value and reclassification rate of right heart catheterization (RHC) compared with transthoracic echocardiography (TTE) in characterising CD staging at 3-year follow-up in patients with clinically significant AS, to identify patients that would benefit from RHC for prognostic stratification, and to test the prognostic value of combined CD staging.
METHODS: An observational cohort study of 432 AS patients undergoing TTE and RHC wer

INTRODUCTION: It has been suggested that anesthesiologists with subspecialty expertise in pediatric cardiac anesthesia are best qualified to care for patients with complex congenital cardiac anomalies and manage the complex physiology frequently encountered in the pediatric cardiac catheterization lab. We evaluated the incidence of adverse events in our pediatric cardiac catheterization lab, comparing care provided by cardiac and noncardiac pediatric attending anesthesiologists.
METHODS: Data were collected on each anesthetic in the pediatric cardiac catheterization lab from January 1, 2016 t

BACKGROUND: Cardiac catheterization is one of the most widely performed cardiac interventional procedures worldwide. The Austrian National Catheterization Laboratory Registry (ANCALAR), started in 1992, collects annual data on cardiac catheterization in Austria. The registry enables in-depth understanding of the dynamics of cardiac catheterization procedures and their use across 34 cardiac catheterization laboratories in Austria.
METHODS: Data from ANCALAR on cardiac catheterization including the latest data for 2017, voluntarily provided by centers with cardiac catheterization laboratories,

PURPOSE OF REVIEW: The role of emergent cardiac catheterization after resuscitated out-of-hospital cardiac arrest (OHCA) has evolved based on recent randomized evidence. This review aims to discuss the latest evidence and current indications for emergent coronary angiography (CAG) and mechanical circulatory support (MCS) use following OHCA.
RECENT FINDINGS: In contrast to previous observational data, recent RCTs evaluating early CAG in resuscitated OHCA patients without ST elevation have uniformly demonstrated a lack of benefit in terms of survival or neurological outcome. There is currently

OBJECTIVES: The objectives of this study are to report the spectrum of cardiac lesions in pediatric patients post-orthotopic heart transplantation (OHT), the characteristics of patients who develop these lesions, and the feasibility of transcatheter intervention in treating these lesions.
BACKGROUND: Indications for OHT in the pediatric population range from cardiomyopathy to complex congenital heart defects with and without prior palliation. These patients may have residual vascular access and great vessel abnormalities.
METHODS: Data was collected through retrospective review of all OHT pa

BACKGROUND AND AIMS: A study at Indiana University demonstrated a reduction in myocardial infarction (MI) incidence with increased frequency of cardiac catheterization (CATH) in liver transplant (LT) candidates. A strict protocol for performing CATH based upon predefined risk factors, rather than noninvasive testing alone, was applied to a subgroup (2009-2010) from that study. CATH was followed by percutaneous coronary intervention (PCI) in cases of significant coronary artery disease (CAD; ≥50% stenosis). The current study applies this screening protocol to a larger cohort (2010-2016) to asse

There is a risk of gaseous and solid micro-embolus formation during transcatheter cardiac interventions and surgery in children with congenital heart disease (CHD). Our aim was to study the burden of high-intensity transient signals (HITS) during these procedures in infants. We used a novel color M-mode Doppler (CMD) technique by NeoDoppler, a non-invasive ultrasound system based on plane wave transmissions for transfontanellar continuous monitoring of cerebral blood flow in infants. The system displays CMD with 24 sample volumes and a Doppler spectrogram. Infants with CHD undergoing transcath

BACKGROUND: Contrast-induced encephalopathy (CIE) is a rare but clinically significant complication following intra-arterial iodinated contrast administration, often mimicking acute ischemic stroke (IS). Its true incidence following cardiac catheterization procedures remains poorly defined.
AIMS: This study aimed to evaluate the incidence of CIE following cardiac catheterizationprocedures and to characterize its clinical presentation, diagnostic features and outcomes.
METHODS: We conducted a single-center retrospective cohort study at an academic institution, evaluating all in-hospital code

Radiation exposure poses a substantial occupational risk for healthcare professionals in the catheterization laboratory (cath lab). The escalating complexity and frequency of interventional procedures, such as cardiac catheterizations and percutaneous coronary interventions, underscore the need for innovative strategies to mitigate radiation exposure. While traditional measures like lead aprons, thyroid collars, and goggles have been pivotal in reducing radiation exposure, they have limitations, especially during prolonged and intricate procedures. Consequently, there is a growing demand for a

Stroke is a devastating complication of cardiovascular interventions. Intraprocedural stroke is a well-documented and feared risk of cardiac percutaneous transcatheter procedures. If clinically significant strokes are absent, silent strokes remain the next in line to pose large concerns related to future cognitive decline, stroke risk, and overall increased morbidity and mortality. Cerebral protection devices (CPD) developed overtime aim to neutralize this risk through either a capture-based filter or a deflector mechanism. Many CPDs exist currently, each one unique, with varying degrees of ev

Patients undergoing cardiac catheterization are advised not to take anything by mouth after midnight. However, limited scientific data exist on whether fasting before catheterization procedures improves clinical outcomes compared with nonfasting. A comprehensive literature search was performed by investigators using major bibliographic databases to identify studies that compared clinical outcomes for fasting versus nonfasting patient groups following cardiac catheterization procedures. The risk ratios (RR) and mean difference (MD) were pooled along with 95% confidence intervals (CIs) for dicho

The C3PO collaborative, with a history of successful quality improvement (QI) initiatives, leveraged registry participants to develop a multi-center QI initiative to reduce adverse events (AEs) in congenital cardiac catheterization. A 32-person, interdisciplinary working group analyzed audited data for all congenital cardiac catheterization cases from 2014-2017. The primary outcome was the occurrence of any high-severity (level 3/4/5) AE. Cases were organized from shortest to longest duration, and level 3/4/5 and 4/5 AE rates were summarized for each procedure duration decile. Observations fro

Proximal radial artery (PRA) access for cardiac catheterization is safe but can jeopardize subsequent use of the artery because of occlusion. Distal radial artery (DRA) access in the anatomical snuffbox preserves the RA but safety and potential detrimental effects on hand function are unknown. We aimed to assess hand function and complications after DRA and PRA. In this single-center trial, 300 patients were randomly allocated 1:1 to cardiac catheterization through DRA or PRA. The primary end point of change in hand function from baseline to 1 year was a composite of the Quick Disabilities of

Acute stroke occurring during cardiac catheterization is an extremely uncommon procedural complication. There is a significant research gap in the published literature regarding the occurrence of acute ischemic stroke during percutaneous coronary intervention in Asian populations, with most data originating from western countries, underscoring the need for localized studies to develop tailored detection and management strategies. We present the case of a 70-year-old Indian woman with multiple cardiovascular risk factors who experienced an acute ischemic stroke while undergoing percutaneous cor

OBJECTIVES: Prolonged wait times for transcatheter aortic valve replacement (TAVR) are associated with increased morbidity and mortality. The incidence and predictors of short TAVR wait times (STWT: defined as ≤ 30 days from referral to TAVR procedure) have not been defined. This study examined the impact of clinical characteristics, demographics, and pre-TAVR cardiac catheterization on wait times for TAVR.
METHODS: This was a retrospective observational analysis of 831 patients with severe aortic stenosis undergoing TAVR from 2019 to mid-2022 at the University of Vermont Medical Center. Demo

Pediatric patients with pulmonary arterial hypertension (PAH) are considered to be at risk for pulmonary hypertensive crisis (PHC) or even death during right heart catheterization (RHC). This retrospective study was designed to identify the risks and clinical characteristics associated with PHC in pediatric PAH patients. We included 163 consecutive procedures from 147 pediatric patients diagnosed with PAH who underwent diagnostic RHC in Beijing Anzhen Hospital between January 2007 and December 2020. The average patient age was 9.0 ± 4.7 years and 84 (51.5%) were females. Before RHC, over 20% o

PURPOSE OF REVIEW: Cardiac magnetic resonance imaging provides radiation-free, 3-dimensional soft tissue visualization with adjunct hemodynamic data, making it a promising candidate for image-guided transcatheter interventions. This review focuses on the benefits and background of real-time magnetic resonance imaging (MRI)-guided cardiac catheterization, guidance on starting a clinical program, and recent research developments.
RECENT FINDINGS: Interventional cardiac magnetic resonance (iCMR) has an established track record with the first entirely MRI-guided cardiac catheterization for congen
