Cantharidin triggers cardiotoxicity via reactive anhydride-mediated covalent protein modification.

Cantharidin (CTD), an anhydride-containing natural product from blister beetles, exhibits potent pharmacological activities in the treatment of molluscum contagiosum and tumors. However, CTD exposure induces multi-organ damage, among which CTD-triggered myocardial cell necrosis and electrophysiological abnormalities represent severe adverse reactions and are the primary causes of fatality. Our recent work revealed that the anhydride moiety of CTD possesses reactivity and can covalently modify proteins via lysine residues. This study investigated the mechanistic role of this covalent modificati

Late Anthracycline Cardiotoxicity Mimicking Arrhythmogenic Cardiomyopathy in a Young Adult.

BACKGROUND: Doxorubicin-induced cardiotoxicity can manifest years after exposure and may mimic other cardiomyopathies in young patients, making the diagnosis of delayed doxorubicin cardiotoxicity challenging in these patients. CASE SUMMARY: A 26-year-old physically active man with childhood Burkitt's lymphoma treated with doxorubicin (120 mg/m2 cumulative) presented with exertional chest discomfort and palpitations. Resting electrocardiogram and ambulatory monitoring were unremarkable. Transthoracic echocardiography demonstrated preserved ejection fraction (55%-60%) but reduced global longitu

Natural ERRα Activator Formononetin Ameliorates Anthracycline Cardiotoxicity via Metabolic Improvement.

BACKGROUND: Anthracycline-induced cardiotoxicity (AIC) limits life-saving chemotherapy and is driven by early metabolic remodeling. The nuclear receptor ERRα (estrogen-related receptor α) is a master regulator of cardiac energy metabolism, but the temporal dynamics of its downregulation, its causal role in AIC pathogenesis, and whether it can be pharmacologically activated to confer protection remain undefined. METHODS: We performed temporal protein analysis in a porcine AIC model. Using cardiomyocyte-specificgain- and loss-of-function mouse models, we assessed the causal role of ERRα. Mechan

Carfilzomib triggers cardiotoxicity by suppressing SENP1-mediated deSUMOylation of DDX17.

Carfilzomib (Cfz) is a second-generation proteasome inhibitor approved for the treatment of relapsed/refractory multiple myeloma (RRMM). Previous studies have shown that Cfz is associated with a higher incidence of severe adverse cardiac effects than bortezomib (Btz); however, the underlying mechanisms remain to be elucidated. The aim of this study is to identify key regulators of cardiotoxicity induced by Cfz and to investigate the mechanisms by which these factors exert their effects. We establish a mouse model of cardiac toxicity induced by Cfz and confirm the phenotype through cardiac func

Nickel-induced cardiotoxicity: immunopathogenesis, thromboinflammation, and a targeted nanotherapeutic strategy.

Nickel (Ni) is a pervasive environmental toxicant found in contaminated water, soil, and airborne particulates. Even at low doses, Ni accumulates in the body through inhalation, ingestion, or dermal contact, causing progressive systemic injury. The cardiovascular system is particularly vulnerable, where Ni induces sustained oxidative stress, endothelial dysfunction, platelet hyperactivation, platelet-leukocyte aggregation, and NETosis, collectively creating a pro-thromboinflammatory state that heightens thromboembolic risk. At the molecular level, Ni perturbs calcium signaling, elevates intrac

Managing Doxorubicin Cardiotoxicity: Insights Into Molecular Mechanisms and Protective Strategies.

Cancer ranks as the second leading cause of death in the United States and poses a significant health challenge globally. Numerous therapeutic options exist for treating cancer, with chemotherapy being one of the most prominent. Chemotherapy involves the use of antineoplastic drugs, either alone or in combination with other medications, to target and kill cancer cells. However, these drugs can also adversely affect healthy cells, leading to various side effects. Among the most commonly used chemotherapy agents are anthracyclines, which include doxorubicin, daunorubicin, and epirubicin. Doxorub

Cardiotoxicity in Breast Cancer Therapy: Risks, Mechanisms, and Prevention Strategies.

Breast cancer is the most prevalent cancer in women. Anthracyclines are commonly used as the first line of treatment, often combined with other agents, including trastuzumab. Despite their efficacy, both drugs pose a risk of cardiotoxicity, which may impair patients' quality of life (QoL) and hinder treatment persistence. Anthracycline-induced cardiotoxicity is dose-dependent and generally irreversible, whereas trastuzumab is associated with potentially reversible cardiac dysfunction. This review discusses the risk factors and biological mechanisms underlying chemotherapy-induced cardiotoxicit

Vitamin C as a Cardioprotective Agent Against Doxorubicin-Induced Cardiotoxicity.

Doxorubicin is used and highly effective chemotherapeutic agent; however, its clinical utility remains limited by dose-dependent cardiotoxicity, presenting a significant challenge in cancer management. Growing preclinical research and clinical evidence suggest that the antioxidant vitamin C (ascorbic acid) may confer cardioprotective effects against doxorubicin-induced cardiotoxicity. In this review, both preclinical and clinical research has been synthesized to assess the potential role of vitamin C in mitigating doxorubicin-induced cardiotoxicity. Preclinical data have routinely indicated th

Exploratory study linking plasma proteomics to cardiotoxicity in Hodgkin lymphoma.

BACKGROUND: Cardiovascular toxicity is a well-known complication of chemotherapy, especially doxorubicin (DXR), and irradiation of the mediastinum for classical Hodgkin lymphoma (cHL). Due to the excellent prognosis in cHL, the mortality rate in late toxicity historically exceeds that of relapse of lymphoma. This highlights the need for strategies to minimize toxicity. Our aim was to characterize the prevalence of cardiovascular diseases (CVDs) in our cohort of cHL patients treated with DXR with or without radiotherapy according to standard practice and to identify any plasma protein associatio

Trastuzumab-Induced Cardiotoxicity: When and How Much Should We Worry?

This review critically analyzes the incidence of trastuzumab-induced left ventricular systolic dysfunction and congestive heart failure (CHF), distinguishing between cases with and without prior anthracycline exposure. It highlights the fact that the elevated risk of trastuzumab-induced cardiotoxicity is closely associated with prior anthracycline exposure. In the absence of prior anthracycline exposure, the incidence rates of trastuzumab-induced cardiotoxicity, particularly CHF (ranging from 0% to 0.5%), are largely comparable with those reported in the general population, especially when rev

Predicting Cardiotoxicity of Molecules Using Attention-Based Graph Neural Networks.

In drug discovery, the search for new and effective medications is often hindered by concerns about toxicity. Numerous promising molecules fail to pass the later phases of drug development due to strict toxicity assessments. This challenge significantly increases the cost, time, and human effort needed to discover new therapeutic molecules. Additionally, a considerable number of drugs already on the market have been withdrawn or re-evaluated because of their unwanted side effects. Among the various types of toxicity, drug-induced heart damage is a severe adverse effect commonly associated with

Beneficial Effects of Oral Carbon Monoxide on Doxorubicin-Induced Cardiotoxicity.

BACKGROUND: Doxorubicin and other anthracyclines are crucial cancer treatment drugs. However, they are associated with significant cardiotoxicity, severely affecting patient care and limiting dosage and usage. Previous studies have shown that low carbon monoxide (CO) concentrations protect against doxorubicin toxicity. However, traditional methods of CO delivery pose complex challenges for daily administration, such as dosing and toxicity. To address these challenges, we developed a novel oral liquid drug product containing CO (HBI-002) that can be easily self-administered by patients with can

2-[18F]Fluoropropionic Acid PET Imaging of Doxorubicin-induced Cardiotoxicity.

PURPOSE: Treatment of pediatric cancers with doxorubicin is a common and predictable cause of cardiomyopathy. Early diagnosis of treatment-induced cardiotoxicity and intervention are major determinants for the prevention of advanced disease. The onset of cardiomyopathies is often accompanied by profound changes in lipid metabolism, including an enhanced uptake of short-chain fatty acids (SCFA). Therefore, we explored the utility of 2-[18F]fluoropropionic acid ([18F]FPA), an SCFA analog, as an imaging biomarker of cardiac injury in mice exposed to doxorubicin. Procedures: Cardiotoxicity and car

Survival after aluminum phosphide poisoning with cardiotoxicity: a case report.

INTRODUCTION: Aluminum phosphide is a cheap and commonly used rodenticide that is also an effective solid fumigant and frequently used for grain preservation. The pill contains around 44% inert elements (ammonium carbonate) to avoid disintegration of the tablet, while the rest (about 56%) is aluminum phosphide. Because it is freely available on the market, it is one of the commonly used agents for self-poisoning in different parts of the developing world. Early signs of toxicity are manifested by shock and circulatory failure. Until now, no specific antidote is available. Aggressive supportive
