Although the majority of giant cell tumors (GCTs) of the bone occur in adult patients, occasionally they arise in the pediatric population. In this setting they may be mistaken for tumors more commonly seen in this age group, including osteosarcoma, aneurysmal bone cyst, and chondroblastoma. All cases of primary GCT of the bone arising in patients 18 years and below were retrieved from our institutional archives and examined with emphasis on the evaluation of various morphologic patterns. Clinical/radiologic records were reviewed when available. Analysis for H3F3A/H3F3B mutations was performed

Phosphaturic mesenchymal tumors of the mixed connective tissue type (PMTMCT) are extremely rare, histologically distinctive neoplasms, which cause tumor-induced osteomalacia (TIO) in most cases through the elaboration of a phosphaturic hormone, fibroblast growth factor-23 (FGF23). Rarely, identical tumors without known TIO may be observed. We studied a large group of PMTMCT for expression of FGF23, using a novel reverse transcription polymerase chain reaction (RT-PCR) assay for FGF23 in formalin-fixed, paraffin-embedded tissues. Twenty-nine PMTMCT (17 with and 12 without TIO) and 23 non-PMTMCT

In the histological differential diagnosis of osteoblastic bone tumors, several problems could not have been solved by conventional histological methods including immunohistology. Some well-known examples are the differential diagnosis between aneurysmal bone cyst and telangiectatic osteosarcoma and giant cell tumor versus giant cell-containing highly malignant osteosarcoma. As a new approach to these diagnostic problems, we analyzed the genetic instability in a larger number of bone-forming tumor-like lesions, benign and malignant osteoblastic tumors. Analysis focused on genetic alterations i

BACKGROUND: Benign bone tumors represent a heterogeneous group of nonmalignant lesions often treated surgically when symptomatic or at risk of fracture. Although local recurrence is a recognized risk, comparative data on recurrence timing and presentation across histologies are limited, contributing to the lack of standardized surveillance. We aimed to examine locally recurrent benign bone tumors after surgery to define recurrence patterns by histology.
METHODS: We conducted a single-center retrospective cohort study of 329 curative-intent procedures for benign bone tumors at the University o

BACKGROUND AND OBJECTIVES: Treatment of benign osteolytic lesions in the femoral head and neck can be extremely challenging, particularly in children with open physis or for aggressive tumors with pathological fracture. There remains the difficult management decision as to whether to perform complete excision of the involved area or only curettage. Moreover, there is no agreed consensus on the optimal approach to lesion access when performing curettage, which included the transcervical, open and direct approach. The current systematic review aims to provide guidance for selection of surgical m

BACKGROUND: The differential diagnosis of giant cell-rich bone tumors comprises a broad spectrum of lesions with prominent reactive osteoclast-like and/or neoplastic giant cells, with substantial differences in biologic behavior and clinical management. Evaluation of giant cell-rich bone tumors on small biopsies can be challenging especially in specimens with limited representative material. An accurate diagnosis requires a high level of skill on the part of both radiologist and pathologist as correlation with clinical and radiologic characteristics is critical. The objective of the study was

BACKGROUND: The purpose of this study was to investigate the clinical efficacy of extended resection with osteotomy, fenestration and conservation of muscle (tendon) insertion in the treatment of bone tumors.
METHODS: A total of 15 patients with locally aggressive bone tumors (Enneking stage II) in the adjacent muscle (tendon) insertion of the proximal extremity were enrolled in the present study (mean age of 29 years). Extended curettage of lesions with osteotomy, fenestration and/or conservation of muscle (tendon) insertion and internal fixation with a bone graft or bone cement was performe

A fluid-fluid level was identified in 11.2% of focal lesions of bone. Fluid-fluid levels are observed on cross-sectional imaging with either computed tomography (CT) or magnetic resonance (MR) imaging. They become apparent when a fluid collection containing substances of different density is allowed to settle, and when the plane of imaging is perpendicular to the fluid level. For instance, within a collection of blood, the cellular components will settle dependently, with the lower density plasma forming a layer superiorly. The difference in the density of these layers can be observed on CT im

Treatment of 16 patients with aggressive benign bone tumours and one patient with a low grade malignancy with a combined regimen of cryosurgery, phenolization and acrylic cementation is reported. Patients were aged between 9 and 51 years and were treated by this method between the years 1986 and 1993. Minimal follow up was 13 months. The commonest histological diagnosis was giant cell tumour (7), followed by aneurysmal bone cyst (6), chondromyxoidfibroma (3) and low grade chondrosarcoma (1). Patients were assessed for functional outcome and local recurrence. On average 86 per cent of premorbid

OBJECTIVES: Except for a few specific lesions, the analysis of the margins of focal bone lesions with MRI has been largely overlooked in the literature. We observed that some lesions exhibited concave margins, suggesting a non-aggressive nature. This study aimed to determine whether concave borders are more frequently present in certain types of focal lesions of the long bones on MRI, particularly in benign lesions.
MATERIALS AND METHODS: MRI examinations of 586 focal intraosseous lesions of the long bones were retrospectively reviewed. Each lesion margin was independently classified by two m

Tumors resembling tenosynovial giant cell tumor (TGCT) but additionally forming chondroid matrix are rare and most often involve the temporomandibular joint (TMJ). We studied 21 tumors consisting of synoviocytes (large, eosinophilic mononuclear cells containing hemosiderin) and chondroid matrix to better understand these unusual neoplasms. The tumors occurred in 10 males and 11 females, in the age group of 31 to 80 years (median, 50 years) and involved the TMJ region (16), extremities (4), and spine (1). As in conventional TGCT, all were composed of synoviocytes, small histiocytes, foamy macro

BACKGROUND: Benign bone tumors and tumor-like conditions are commonly located in the proximal femur. The main indications for surgical treatment are lesions with impending or actual pathological fractures, or with aggressive or recurrent lesions. However, patients complaining of persistent pain, limping, or abnormal gait patterns are also considered for surgical treatment. In this study, we describe the outcomes of the surgical treatment of benign lytic lesions of the proximal femur by curettage followed by implantation of synthetic bone graft.
METHODS: This retrospective study included 27 pa

Extraskeletal osteosarcoma is an uncommon malignant neoplasm. The origin of osteosarcoma in the pleura is extremely rare, with only four such cases so far documented in the literature to the best of our knowledge. We herein report the case of a 64-year-old Japanese man in whom a left pneumonectomy and pleurectomy were carried out to remove a huge tumor. The pathological examination confirmed a diagnosis of chondroblastic osteosarcoma that had originally arisen from the pleura.

The aim of this study was to evaluate the use of a pedicled mandibular osteomuscular flap for zygoma reconstruction through a modified Weber-Ferguson incision. The study included 3 male patients and 2 female patients with an average age of 46 years. Zygomatic ameloblastoma was present in 2 cases. Vascular malformation was present in 2 cases, and chondromyxoid fibroma was present in 1 case. In all 5 patients, surgery consisted of a wide excision of the lesions with margins of at least 5 mm to ensure complete removal. The defects that were created measured between 3 x 3 and 4 x 4 cm. Primary bone and soft-tissue tumors that mimic common sports injuries are relatively rare and are not often encountered by most orthopaedists. Prompt and accurate diagnosis of these tumors is crucial to maximize the clinical outcome. Many bone and soft-tissue tumors present disproportionately in young and active patients who are often involved in athletic activities. Thus, the clinician may misdiagnose these rare tumors as more common sports injuries. Symptoms that should raise suspicion for a neoplastic process include pain unrelated to activity and a clinical course that does not follow the

BACKGROUND: Benign bone lesions are a common incidental finding in athletes during workup for musculoskeletal complaints, and athletes are frequently advised to halt participation in contact sports. There are no current guidelines to assist clinicians in referring patients with these lesions to a subspecialist or in advising athletes on the safety of returning to sport.
PURPOSE: To assist sports medicine physicians in appropriate referral for patients with benign bone lesions through presentation of a literature review and the case of an adolescent athlete with a benign bone lesion in a locat
